A Convergence Study on the Factor Influencing to Healthcare-Associated Infection Control Guidelines of Nursing Students (간호학생의 의료관련감염 관리지침 준수 영향요인에 관한 융합연구)

  • In this preliminary study exploring the development of an convergent educational program, we identified the factors that affect adherence to healthcare-associated infection (HAI) control guidelines. The data were collected using a self-reported questionnaire of 183 nursing students. The collected data were analyzed using AMOS 21.0 and SPSS 21.0 program. The model fit was ${\chi}^2=52.06$ (df=9, p<.01), GFI=.93, RMSEA=.16, NFI=.85, and CFI=.90, and the explanatory power was 26.2%. As a result in this study, it was found that the theory of planned behavior(TPB) was appropriate for explaining the intention about healthcare-associated infection control guidelines. It is necessary to develop an education program that can reinforce the concepts of TPB, and iterate research to verify its effectiveness.

  • 본 연구의 목적은 한국의 간호사들이 행하는 간호중재를 규명하는 것이다. 연구대상은 한국의 2 병원에서 일하는 수 간호사와 평 간호사를 포함한 443면의 간호사이며, 연구도구는 Iowa Intervention Project Team 이 개발한 336 문항의 5점 척도로 이루어진 "간호중재"설문지를 사용하였다. 5점 척도는 다음과 같다 : (a) 거의 행하지 않는다 : (b) 거의 한달에 한번 행한다 : (c) 거의 일주일에 한 번 행한다 : (d) 거의 하루에 한 번 행한다 : (e) 하루에 여러 번 행한다. 한국자료를 위해 설문지는 본 연구자에 의해 한글로 번역되었고, Back-translation을 이용하여 한글로 번역한 설문지는 다시 bilingual nurse에 의해 영어로 옮겨졌고, 마지막으로 영어로 옮겨진 설문지를 원래의 영문으로 된 설문지와 비교한 후에(Equivalence Test) 수정${\cdot}$보완하여 사용하였다. 한국 간호사들은 336 간호중재 모두를 행하고 있으며, 21개의 간호중재가 적어도 하루에 한 번 정도는 이루어지고 있었다. 간호사들은 "생리적 : 복합영역"에 속하는 간호중재를 가장 빈번하게 행하였고, 이 중에서 "비경구적 투약"은 일주일에 거의 11번 정도로 가장 많이 행하여졌다. 가장 적게 행하는 간호중재는 "동물이용요법"이었다. 간호사들은 교육을 많이 받을수록, 임상경험이 많을수록 더 많은 간호중재를 빈번하게 행했다. 특히, 석사학위를 받은 간호사는 "행위 영역"에 속한 간호중재를 더 많이 행했는 데, 예를 들면 "적극적 경청", "적응 증진", "의사결정지지"등이다. 또한 4년제 대학을 졸업한 간호사들은 "생리적 : 복합영역"에 속한 간호중재들을 전문대학을 졸업한 간호사보다 더 많이 행했는 데, 예를 들면, "뇌부종 관리", "반사 장애 관리", "기구를 사용한 호흡"등이다. 10년 이상의 경험을 가진 간호사들은 "행위 영역"에 속한 간호중재를 임상 경험이 1년 미만이거나 또는 1-5년 사이의 경험을 가진 간호사보다 더 많이 행했는 데, 예를 들면, "죄책감 극복 증진", "진실 말하기"등이다. 또한 6-10년 사이의 임상경험을 가진 간호사들은 "자가 간호영역"에 속한 "옷 입히기", "두발 간호", "구강건강 유지" 등을 임상경험이 1년 미만인 간호사들보다 더 빈번하게 행했다. 이 연구는 간호중재 규명이 간호원가 산출을 위한 연구와, 간호의 질 향상을 위한 연구, 간호지식 탐구를 위한 연구에 공헌할 것을 기대한다.

An Effect of a Self-Efficacy Information Resources Program on Perceived Self-Efficacy and Self-Care in Patients with Coronary Artery Disease (자기효능 정보자원 프로그램이 관상동맥 질환 환자의 지각된 자기효능감과 자가간호행위에 미치는 효과)

  • Purpose: This study was conducted to investigate whether a self-efficacy information resources program based on the self-efficacy theory of Bandura (1986) would increase self-efficacy and self-care in patients with CAD. Method: The study was a one group, pretest-posttest pre-experimental design research. The Self-Efficacy Information Resources Program was composed of a video tape program and a telephone coaching program and based on vicarious experiences and verbal persuation. The Program was used as an experimental treatment from February 27. to May 27, 1999. Twenty patients participated from a university hospital located in Seoul. Three days later after a pretest the telephone coaching program was given. It was repeated weekly during a 3 week period. At the end of this period post-test data were collected by mail. Data were analyzed with a random permutation test using SAS program. Result: 1) Scores on self-efficacy after the Self-Efficacy Information Resources Program were significantly higher than before the program (p=0.06). 2) Scores on self-care after the Program were significantly higher (subjective self-care, p=0.0005; objective self-care, p= 0.0002) than before the program. Conclusion: The Program was found to be an effective nursing intervention for increasing self-efficacy and self-care. When nurses who care for CAD patients plan to increase their patients' self care ability, they should always consider the concept of self-efficacy.

Impact of Health Literacy on Disease-related Knowledge and Adherence to Self-care in Patients with Hypertension (고혈압 환자의 건강정보 활용능력이 질병관련 지식 및 자가간호 이행에 미치는 영향)

  • Purpose: The purposes of this study were to identify the level of health literacy and to determine its impact on disease-related knowledge and adherence to self-care in adults with hypertension. Methods: This was a cross-sectional, descriptive study in which patients with hypertension were recruited from outpatient hypertension clinics in Seoul, Korea from December, 2009 to February, 2010. A total 186 patients completed series of questionnaires to assess health literacy, disease-related knowledge, and adherence to self-care. Hierarchical linear regression was used to determine whether health literacy was associated with disease-related knowledge and adherence to self-care. Results: Seventy-one (38.2%) and seventy-two (38.7%) patients had inadequate and marginal health literacy, respectively. In hierarchical linear regression, health literacy independently predicted disease-related knowledge (${\beta}$= .43, p < .001) and adherence to self-care (${\beta}$= .37, p < .001) after controlling for age, education level, having a job or not, and body mass index. Conclusion: These findings show that health care providers need to pay attention to patients with hypertension who have inadequate health literacy. Further, it is recommended to develop and implement new strategies for assessing health literacy in clinical practices. Interventions to improve health literacy could promote disease-related knowledge and adherence to self-care in patients with hypertension.

The Effects of Visiting Nursing Intervention on Self Care Behavior and Blood Pressure in Persons with High Risk of Cerebrovascular Diseases (방문간호 중재가 뇌혈관질환 고위험자의 자가간호행위 및 혈압에 미치는 효과)

  • Purpose: This study was to evaluate the effects of visiting nursing intervention on self care behavior and blood pressure with high risk of cerebrovascular diseases. Method: A one-group pretest-posttest was designed. Data collection was done in 150 participants from March. 2003 to November, 2003 at a public health center. Self care behavior was assessed by the scores of smoking, alcohol, diet, exercise, stress and medication compliance. The scores of knowledge related cerebrovascular diseases, blood pressure, blood glucose were estimated. The Participants were intervened in with basic assessment, emotional support, education. This program took a period ranging from 3 months to 7 months depending on the cerebrovascular risk level. The mean number of visiting times was 1.67 per month. Result: 1. Knowledge level was improved(t=-2.542, p= .012). 2. Systolic and diastolic pressure were lower(t=5.439, p<.001; t=4.966, p<.001) 3. Self care level was higher (t=-12.981, p=.001) after the intervention. Conclusion: The visiting nursing intervention was found to have an effect on the scores of knowledge, self care behavior and blood pressure with high risk of cerebrovascular diseases. The results provided evidences for the importance of visiting nursing intervention in the high risk factor of cerebrovascular disease for self care.

  • The purpose of this study attempted to investigate the degree and relationship of health knowledge, health beliefs, and preventive health behavior of public health center officials and provide basic data to develop measures to promote preventive health behavior of public health center officials. Factors affecting health behavior were identified as sub-factors of health beliefs: benefit (β = . 298), severity (β = . 193), and trigger for action (β = . 183). The explanatory power of preventive health behaviors between these variables was found to be 33.9%. This study was attempted in the absence of research on COVID-19 knowledge, health beliefs, and preventive health behaviors among public health center officials, and objective observation or qualitative research that could supplement the limitations of self-reported questionnaires was conducted to 19 Research exploring the meaning of preventive health behaviors is needed.

The Effects of Diet Education Contents for the Customized Serious Game Development in Type II Diabetes (제2형 당뇨환자 기능성게임 개발을 위한 식이교육 콘텐츠 효과)

  • This study was carried out to investigate the effects of contents for the serious game development of the customized diet education in Type II Diabetes. Because the development of the existing serious game focuses on Type I Diabetes, this study was designed to carry out customized education through the interaction between Type II Diabetes and health manager. The period of data collection was from July 11 to December 26 in Y2015, the subjects were selected from a total of 26 people suitable for Type II diabetes who visited the public health center to apply for a diabetic diet education and completed the 8 continued education. After receiving customized diet prototype education, the self-care behaviors and HbA1C of the subjects were statistically significantly increased(p<.05)

Factors Related to COVID-19 Vaccination Intention Based on Theory of Planned Behavior in Convergence Era (융합 시대의 계획된 행위이론에 근거한 COVID-19 예방접종 행위의도 관련요인)

  • This study is to identify the factors influencing the attitudes, behavior controls, and subjective norms related to COVID-19 vaccination on the intention of preventive behavior in K provincial residents. A total of 232 people agreed to participate in the study. Data were analyzed using descriptive statistics, t-test, ANOVA, correlation, and Hierarchical analysis. As a result of this study, the factors that had a statistically significant effect on the intention to conduct COVID-19 vaccination were perceived behavioral control (β=.39, p<.001) and attitude toward COVID-19 preventive behavior (β=.24, p<.001), followed by subjective norm (β=.24, p<.001). The explanatory power for this was 56.8%. The COVID-19 vaccine is an important means of ending the COVID-19 pandemic and helps prevent infection or worsening of COVID-19 and protect those around you. In the post-COVID-19 situation, it is necessary to improve the intention of vaccination against COVID-19 and to take active measures and strategies. At this point in time when COVID-19 vaccination is becoming more prevalent across the country, we hope that the intent of the COVID-19 vaccination action will translate into a COVID-19 vaccination action.

Self Care Behavior of Hemodialysis Patients (혈액투석환자의 자가간호행위)

  • Purpose: The purpose of this study was two folds: first, to identify the level of self care behavior of the hemodialysis patients and second, to find the correlation between the self care behavior and the physiologic indices. Method: The subjects were 52 hemodialysis patients, male and female, who have regularly received hemodialysis dialysis at the Dialysis Room in a leading teaching hospital, Seoul. The patients responded to the self care behavior questionnaires including their socio-demographic characteristics. The respondents have regularly recorded the self care log book. The physiologic indices, clinical characteristics related to the disease and hemodialysis were collected by the chart review. Result: The mean score of the self care behavior was 3.46. The mean score of the self care behavior on categories demonstrated as follows: medication 4.29, fistula management 4.13, management of physical problem 3.71, diet 3.28, exercise and rest 3.22, blood pressure and body weight management 2.97 and social adjustment 2.05 in order. Thirty patients managed discomfort of their fistula. Eleven patients took exercise for 0.5-1 hr/week. Thirty patients measured their body weight daily and thirty two measured their blood pressure daily. The score of self care behavior was significantly correlated with the mean weight gains between the dialysis sessions(r=-.312, p=.05). The mean weight gains between dialysis sessions was found to be high as the level of serum phosphorus and potassium increased(r=-.316, p=.05, r=-.465, p=.01). Conclusion: The result suggests that nursing intervention to the hemodialysis patients to improve self care behavior should be encouraged and further developed.

The Effect of Self-efficacy Promotion Exercise Program for the Elderly Hypertensives (자기효능증진 운동프로그램이 고혈압 노인에게 미치는 영향)

  • This study was to examine the effect of the Self-efficacy Promotion Exercise Program to the elderly hypertensive with Stage 1. This research was the quasi-experimental study. The Self-efficacy Promotion Exercise Program combined to Aerobic exercise which is incrementally at a level of 40~60% HRmax and the source of the Self-efficacy Promotion Exercise. This research was carried out to experimental(33 patients) and control(32 patients), 60 minutes per day, 3 times a week for 12 weeks. The Self-efficacy Promotion Program was effect on enhancing the general(p<.001) and exercise self-efficacy(p<.001), the self-care behaviors(p<.001) and decreasing the systolic blood pressure(p<.001) to the elderly hypertensives. The 12 weeks Self-efficacy Promotion Exercise Program will be considering to useful for various care settings to the elderly hypertensives.
