Effects of Individualized Diet Education Program on the Dietary Knowledge and Self-Care Compliance among Hemodialysis Patients (개별식이교육 프로그램이 혈액투석 환자의 식이요법에 대한 지식과 식이자가간호 이행에 미치는 효과)

  • Purpose: The purpose of this study was to explore the effect of individualized diet education program on dietary knowledge and self-care compliance among hemodialysis patients. Methods: The design of the study was quasi-experimental nonequivalent control group pre-post test design. Fifty patients were assigned to an experimental group and fifty patients were assigned to a control group. Individualized diet education program was applied to the experimental group once every two weeks for twelve weeks. Results: Dietary knowledge increased significantly in the experimental group when compared to the control group. Dietary self-care compliance improved significantly in the post-test of the experimental group, however, the difference was not significant. Conclusion: The individualized dietary education program was effective in improving dietary knowledge. The individualized diet education program needs to be incorporated as an important nursing intervention. Nurses should provide individualized diet education program for hemodialysis patients.

The Effects of a BSE(Breast Self-Examination) Education Program on Knowledge, Self-Efficacy and Performance Level in Female Nursing Students (간호대학생에게 제공한 유방자가검진 교육이 지식, 자기효능감 및 수행에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effect of BSE education and practice on knowledge, self efficacy and performance in female nursing students. Method: The subjects consisted of 40 students from 2 nursing colleges. They responded to questionnaires that included knowledge, self-efficacy and performance of BSE within a 3-month interval. The experimental group was subjected to a 90 minute-educational session. Their knowledge of BSE was measured using Choi's tool and self-efficacy was measured using Champion and Scott. Result: Self-efficacy and frequency of BSE performance in the experimental group were significantly higher than those of the control group while BSE knowledge was not significantly different between the two groups. Conclusion: Nursing students don't put their knowledge into practice. This fact suggests that changing a behavior needs something more than knowledge. In this study, the BSE education had an effect on self-efficacy and performance. Therefore, practical education needs to be reinforced for nursing students to perform BSE for their own health and to be able to demonstrate it for clients.

The Effect of an Internet Community on Knowledge, Self-efficacy and Self Care Behavior in Workers with Hypertension (인터넷 커뮤니티가 고혈압 근로자의 지식, 자기효능감, 자가간호행위에 미치는 효과)

  • Purpose: This study was aimed to evaluate the effect of an internet community on knowledge, self-efficacy, and self-care behavior in workers with hypertension. Method: The research design used was a nonequivalent control group pre-test and post-test design. Sixty hypertensive workers participated in this study(Exp.=29, Cont.=31). The data was collected from the 3rd of November 2003 to the 27th of February 2004 using an interview with questionnaires. Information related to hypertension was provided on the internet community weekly and an e-mail newsletter was sent in order to increase participation in the internet community. We used a tool developed by Park Young-Im(1994) that measured knowledge related to hypertension and self-efficacy. A tool developed by Jung Mi-Young(2001) was used for self-care behavior. The collected data was analyzed with an $x^2-test$ and t-test using the SPSS WIN 10.0 program. Result: The internet community helped workers with hypertension to increase their knowledge related to hypertension, as well as increasing their self-efficacy and self-care behavior. Conclusion: An internet community could be applied as hypertensive nursing intervention.

Nursing Interventions In Korea (한국의 간호사들이 행하는 간호중재)

  • 본 연구의 목적은 한국의 간호사들이 행하는 간호중재를 규명하는 것이다. 연구대상은 한국의 2 병원에서 일하는 수 간호사와 평 간호사를 포함한 443면의 간호사이며, 연구도구는 Iowa Intervention Project Team 이 개발한 336 문항의 5점 척도로 이루어진 "간호중재"설문지를 사용하였다. 5점 척도는 다음과 같다 : (a) 거의 행하지 않는다 : (b) 거의 한달에 한번 행한다 : (c) 거의 일주일에 한 번 행한다 : (d) 거의 하루에 한 번 행한다 : (e) 하루에 여러 번 행한다. 한국자료를 위해 설문지는 본 연구자에 의해 한글로 번역되었고, Back-translation을 이용하여 한글로 번역한 설문지는 다시 bilingual nurse에 의해 영어로 옮겨졌고, 마지막으로 영어로 옮겨진 설문지를 원래의 영문으로 된 설문지와 비교한 후에(Equivalence Test) 수정${\cdot}$보완하여 사용하였다. 한국 간호사들은 336 간호중재 모두를 행하고 있으며, 21개의 간호중재가 적어도 하루에 한 번 정도는 이루어지고 있었다. 간호사들은 "생리적 : 복합영역"에 속하는 간호중재를 가장 빈번하게 행하였고, 이 중에서 "비경구적 투약"은 일주일에 거의 11번 정도로 가장 많이 행하여졌다. 가장 적게 행하는 간호중재는 "동물이용요법"이었다. 간호사들은 교육을 많이 받을수록, 임상경험이 많을수록 더 많은 간호중재를 빈번하게 행했다. 특히, 석사학위를 받은 간호사는 "행위 영역"에 속한 간호중재를 더 많이 행했는 데, 예를 들면 "적극적 경청", "적응 증진", "의사결정지지"등이다. 또한 4년제 대학을 졸업한 간호사들은 "생리적 : 복합영역"에 속한 간호중재들을 전문대학을 졸업한 간호사보다 더 많이 행했는 데, 예를 들면, "뇌부종 관리", "반사 장애 관리", "기구를 사용한 호흡"등이다. 10년 이상의 경험을 가진 간호사들은 "행위 영역"에 속한 간호중재를 임상 경험이 1년 미만이거나 또는 1-5년 사이의 경험을 가진 간호사보다 더 많이 행했는 데, 예를 들면, "죄책감 극복 증진", "진실 말하기"등이다. 또한 6-10년 사이의 임상경험을 가진 간호사들은 "자가 간호영역"에 속한 "옷 입히기", "두발 간호", "구강건강 유지" 등을 임상경험이 1년 미만인 간호사들보다 더 빈번하게 행했다. 이 연구는 간호중재 규명이 간호원가 산출을 위한 연구와, 간호의 질 향상을 위한 연구, 간호지식 탐구를 위한 연구에 공헌할 것을 기대한다.

Breastfeeding Knowledge, Attitude, and Nursing Practice of Nurses in Neonatal Intensive Care Units (신생아집중치료실 간호사의 모유수유에 대한 지식, 태도, 간호활동)

  • Purpose: The purpose of this study was to describe breastfeeding knowledge, attitude and nursing practice of neonatal intensive care unit (NICU) nurses. Methods: Survey methodology using a self administered questionnaire was employed as the research design. A total of 153 questionnaires from 7 university hospital NICUs in Seoul, Daejeon, and Daegu were used for data analysis. Results: The percentage of correct answers on the breastfeeding knowledge questions for the NICU nurses was 59.3%. The nurses in NICU reported a somewhat positive attitude to breastfeeding, mean per item $3.70{\pm}0.41$ on a scale of 5 points. Their breastfeeding nursing practice was at a moderate level, mean per item, $3.26{\pm}0.74$. Breastfeeding knowledge had significant correlations with breastfeeding attitude (r=.554, p<.001) and nursing practice (r=.203, p=.002). Also, There was a significant correlation between breastfeeding attitude and nursing practice (r=.243, p=.002). Conclusion: These results suggest that breastfeeding educational programs and strategies for NICU nurses should be developed to increase their breastfeeding knowledge, attitude, and nursing practice.

Impact of Health Literacy on Disease-related Knowledge and Adherence to Self-care in Patients with Hypertension (고혈압 환자의 건강정보 활용능력이 질병관련 지식 및 자가간호 이행에 미치는 영향)

  • Purpose: The purposes of this study were to identify the level of health literacy and to determine its impact on disease-related knowledge and adherence to self-care in adults with hypertension. Methods: This was a cross-sectional, descriptive study in which patients with hypertension were recruited from outpatient hypertension clinics in Seoul, Korea from December, 2009 to February, 2010. A total 186 patients completed series of questionnaires to assess health literacy, disease-related knowledge, and adherence to self-care. Hierarchical linear regression was used to determine whether health literacy was associated with disease-related knowledge and adherence to self-care. Results: Seventy-one (38.2%) and seventy-two (38.7%) patients had inadequate and marginal health literacy, respectively. In hierarchical linear regression, health literacy independently predicted disease-related knowledge (${\beta}$= .43, p < .001) and adherence to self-care (${\beta}$= .37, p < .001) after controlling for age, education level, having a job or not, and body mass index. Conclusion: These findings show that health care providers need to pay attention to patients with hypertension who have inadequate health literacy. Further, it is recommended to develop and implement new strategies for assessing health literacy in clinical practices. Interventions to improve health literacy could promote disease-related knowledge and adherence to self-care in patients with hypertension.

Effects of Health Literacy and Knowledge on Diabetic Self-care in the Elderly with DM Living Alone (독거노인 당뇨병 환자의 건강정보이해능력과 당뇨지식이 자가간호 이행에 미치는 영향)

  • Purpose: The aim of this study was to evaluate the level of self-care adherence in the elderly with diabetes mellitus (DM) who have lived alone, and to investigate the association between health literacy, diabetic knowledge and self-care adherence. Methods: Descriptive research using the cross-sectional approach was conducted. Data was collected by using a convenience sampling of 201 participants who aged sixty-five years old or more and have lived alone. Statistical analysis was conducted by using an independent t-test, one-way ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficient and stepwise multiple regression analysis. Results: Mean scores of health literacy, diabetic knowledge and adherence of self-care were $4.41{\pm}3.54$ points out of a possible $12, 6.22{\pm}2.55$ points out of a possible $15, 58.56{\pm}17.28$ points out of a possible 112, respectively. Adherence of self-care showed a statistical association with health literacy (r=.26, p<.001) and diabetic knowledge (r=.30, p<.001). Statistically significant factors in the regression model were monthly income, diabetic knowledge and exercise. Conclusion: The level of self-care adherence in the elderly with diabetes living alone was relatively low. Diabetic knowledge was an important factor to improve the level of self-care adherence for the elderly with diabetes living alone.

Comparison for Hepatitis B Knowledge, Self Care Practice and Quality of Life according to the Disease Activity among Patients with the Hepatitis B Virus (B형간염 바이러스 보유 환자의 질환 활성도에 따른 B형간염 지식, 자가간호수행 및 삶의 질 비교)

  • Purpose: The hepatitis B virus is a major cause of chronic liver disease. The clinical guidelines recommend that inactive chronic hepatitis (ICH) patients also check their liver function every 6 to 12 months and manage the potential risks. This study compared the hepatitis B knowledge, self-care practice, and quality of life in patients with HBV according to the disease activity. Methods: This study was conducted in a university hospital and surveyed on 65 ICH patients and 68 progressive chronic liver disease (PCLD) patients from November in 2012 to September in 2013. Results: The knowledge of hepatitis B was lower in the group of a lately perceived HBV infection and ICH. Self-care practice was lower in the male and the patients group with a perceived HBV infection within 5 years. The "taking regular liver function test" score was lower in the ICH. Eight out of 12 Liver Disease Quality of Life instrument (LDQOL) subscales were lower in PCLD. Conclusion: The hepatitis B knowledge and self-care practice are relatively lacking in ICH and the patients group with a perceived HBV infection within 5 years. More effective education programs will be necessary to enhance the hepatitis B knowledge and self-care for patients with HBV and even for ICH.

The Effect of the Experience of Diabetes Education on Knowledge, Self-Care Behavior and Glycosylated Hemoglobin in Type 2 Diabetic Patients (당뇨교육경험이 제2형 당뇨환자의 지식, 자가간호행위, 당화혈색소에 미치는 영향)

  • Purpose: This study was to identify the effect of the experience of diabetes education on knowledge, self-care behavior and glycosylated hemoglobin (HbA1C). Further, this study was held to examine about patient's preferred methods of education and re-education frequency. Methods: 166 type 2 diabetes patients from two hospitals in Incheon participated in this study. Data were analyzed by using descriptive analysis, t-test, ANOVA, Scheffe's test and multiple regression analysis. Results: 72.3% patients needed re-education and the average interval of re-education was 8.53 months. Patients preferred education methods were lectures, practical training, and studying from pamphlet. Depending on the frequency of diabetes education, there were significant differences in the level of diabetes knowledge (F=10.88, p<.001) and self-care behaviors (F=4.59, p=.012), but there was not significant difference with HbA1C (F=1.53, p=.220). As to how much the diabetes education helped managing diabetes, there was a significant difference in the level of self-care behaviors (t=2.01, p=.049), but there were not significant differences in level of knowledge (t=1.10, p=.275) and HbA1C (t=-.33, p=.746). The experience of diabetes education was a significant factor which influenced patient's knowledge (t=3.93, p<.001) and self-care behaviors (t=2.21, p<.001). But HbA1C was not influenced by the experience of diabetes education (t=-1.68, p=.096). Conclusion: It is necessary to provide diabetes education with appropriate interval and methods and subjects that reflect the needs of patient through the study results.

Relationship of Knowledge of Cardiac Rehabilitation, Educational Needs, and Self Care in Inpatients with Coronary Artery Disease in China (중국 관상동맥질환자의 심장재활지식, 교육요구도 및 자가간호행위)

  • Purpose: The purpose of this study was to investigate the knowledge of cardiac rehabilitation, educational needs, and self care in inpatients with coronary artery disease in China and to provide basic data for development of cardiac rehabilitation programs. Method: From Jan. 16th to Feb. 26th, 2009, 104 patients with coronary artery disease were hospitalized in general hospitals in K and H city in China. They were interviewed with a structured questionnaire. The data was analyzed using descriptive statistics, t-test, ANOVA, and Pearson's correlation analysis. Results: In inpatients with coronary artery disease in China, the score of knowledge of cardiac rehabilitation was average ($66{\pm}.18$), and the score of educational needs was high ($4.27{\pm}.49$). The score of self care was average ($3.12{\pm}.58$). The regular exercise group (p= .005) had a high knowledge of cardiac rehabilitation, the nonsmoking group (p= .022) and regular exercise group (p= .037) did well by themselves. Educational needs also affected the overall knowledge of inpatients with coronary artery disease (r= .288, p= .003). Conclusion: This study suggests that knowledge of cardiac rehabilitation and educational needs should be considered in enhancing cardiac rehabilitation programs designed for inpatients with coronary artery disease in China.
