ITI - Octa system을 이용한 fixed removable bridge의 제작

  • 일반적으로 악골의 흡수가 심한 환자에게 총의치 보철물은 적은 유지력 때문에 매우 불리하다. 특히, 하악골의 흡수가 심한 환자의 경우 2$\sim$4 정도의 임플란트를 식립후에 제작되는 stud type의 over-denture나 bar-over denture 또는 magnetic over-denture 등이 있고, 5$\sim$6개 정도의 임플란트 식립으로 제작되는 fixed removable bridge가 환자에게 높은 만족감을 줄 수 있어 총의치 보철 치료시 고려해야 할 보철 치료 방법중의 하나이다. fixed removable bridge를 제작하는데 있어 먼저 placement guide를 이용하여 모델의 임플란트의 위치와 구강내의 임플란트의 위치를 반드시 확인해야 하고, 구조물을 만들기 전에 wax denture를 제작하여 환자의 simile line 이나 악간 관계를 확인 후에 구조물 제작을 해야 한다.

  • 치과용 임플란트(Implant)란 상실된 자연치아를 대신하여 골 내에 매식하는 인공치근을 말한다. 임플란트는 인접 자연치아의 보호, 심미적 안정 등의 장점으로 인해 그 수요가 늘어나고 있으며, 단일치아 임플란트의 경우, 부분 무치악 환자들에게 있어서 우수한 치의학적 해법이 되어왔다. 대부분의 임플란트는 두 개 이상의 구성요소로 이루어져 있으며, 각각의 구성요소는 나사에 의해 결합되어 있다. 많은 연구결과를 통해, 임플란트의 나사 풀림 현상(Screw loosening)은 임플란트와 관련하여 가장 흔한 문제로 나타나고 있다.(중략)

  • Unlike class I patients, skeletal class II patients have unstable occlusion thus leading to instability of mandibular complete denture. Therefore, mandibular implant overdenture has been the standard of care due to its advantages in stability and retention. The types of attachments can be divided into two categories: solitary and bar type. The indications vary between two categories. In this clinical report, digital technology was utilized from the implant planning to the choice of appropriate attachment. Implants were placed at the desired location as previously planned in terms of angle and depth. Maxillary removable partial denture and mandibular implant overdenture are expected to have fair prognosis.

  • When restoration for partially edentulous patients, abutments are not always in favorable positions for making removable partial dentures. Because of these situations, patients are sometimes unsatisfied with the stability and support of their removable partial prostheses. In this regard, removable partial denture using a few implant surveyed crown prostheses can be a good alternative. It can be expected to increase stability and support of removable partial dentures by strategically placing a small number of implants and restoring with implant-supported surveyed crowns. In these cases, the patients who had unilateral residual teeth on mandible were treated with two implant surveyed crowns in the tactical place to have bilateral distribution. After definitive removable partial prosthesis, the patients showed satisfaction with the masticatory function and comfort of using removable dentures.

  • This study subject is to provide appropriately basic information about annually increased implant treatment by their lost teeth base on increasing old people and also analyze factors influencing implant treatment decisions for patient. For about 3 month from July on the 2010, This survey was carried out at 13 dental clinic as target of 217 patients on categorized into implant treatment-group and non-treatment-group in Seoul and Gyeonggi-do. Implant treatment-group showed highest portion at 50(30.3%), non- Implant treatment-group showed highest portion at 20's(35.7%), And The patients of implant treatment-group are good oral condition, and they showed a positive attitude. Result of analysis by multiple logistic analysis(regression), Factors of influencing implant treatment decision are age, gender, period of education, monthly income, condition of periodontal tissue, regular checkup, counseling attitude, trust in hospital. Therefore, proactive management about the necessity of regular dental check-up is needed the education and promotion. And medical team should have efforts to Improving technique of medical consultations and Improving the trust of the hospital's for implant treatment decision. Also, The patient's subjective determine rather than it is important to preparing to facilitate decision of specific clinical basis. And The government's financial support and policy alternatives are certainly required in order to be implant treatment without cost burden.

A prospective study of patient satisfaction after implant supported mandibular overdenture treatment in fully edentulous patients (완전 무치악 환자에서 하악 임플란트 지지 피개의치 사용 후의 환자만족도에 관한 전향적 연구)

  • Purpose: The purpose of this prospective study was to compare the satisfaction levels of the patients worn implant supported mandibular overdentures and conventional complete dentures. Materials and methods: Full edentulous patients (n=40) who were planned to do implant supported mandibular overdentures, aged 51 to 82 years, were enrolled in this study. All subjects rated their satisfaction levels (mastication, pronunciation, esthetics, pain, and general satisfaction) of their original conventional dentures prior to treatment and their new overdentures 1 week and 12 weeks after the delivery on 10 cm visual analogue scales. Repeated-measures ANOVA was performed to compare the satisfaction level of each three period (P<.05). Results: The mean satisfaction levels of the implant supported overdentures after 1 week and 12 weeks of delivery were significantly higher than those of the conventional complete dentures in all scoring parameters (P<.05). After 12 weeks of overdenture delivery, the increase of the satisfaction levels in mastication, pronunciation, and pain and the decrease in esthetics and general satisfaction were observed in comparison with those after 1 week of delivery, however, no significant difference was found between the satisfaction levels of 1 week and 12 weeks after delivery. Conclusion: An implant supported overdenture might be one of the effective ways to give more satisfaction to patients who were not gratified with a conventional denture and eventually to improve their quality of life.

Comparison of chewing ability and quality of life before and after the dental implantation (임플란트 시술환자의 시술 전.후의 저작능력과 삶의 질 비교)

  • Statement of problem: Recently the populations of patients receiving implant surgery are greatly increase for maintaining oral health. Purpose: This study was conducted for implanted patients to assess the chewing ability patient satisfaction level and changes in quality of life before and after the implant surgery. Material and methods: The current study subjected 109 adult patients, older than 20 years of age, who received implant surgery from December, 2006 to October, 2007 at the 6 dental clinics located at Daegu and Ulsan metropolitan cities. Twice of surveys were conducted for the patients before and after receiving the dental implant surgery. Results: As the motivation of receiving implant surgery, 45.9% of the patients selected the surgery for the chance o "f chewing function recovery", and " failure of treatment and complications" was found to be the most worrisome at the time of surgery by recording 38.5%.The satisfaction level before the implant surgery scored 30.37, while the score was increased to 45.01 after the surgery by showing a significant difference before and after receiving the surgery(P<.001). Regard on the surgery, 91.8% of the patients responded as "Satisfy", and 89% of the study subjects responded that they have willingness to recommend the surgery to their families and friends. The chewing ability score measured by using the surveys on edible foods, the score before the surgery was 15.24, while the score was increased to 19.11 after the surgery by showing a significant difference before and after receiving the surgery(P<.001). The quality of life score was also found to be increased to 11.17 after the surgery from 9.99 before the surgery by showing a significant difference(P<.001). Conclusion: In a future, the studies on the numbers of implanted loss teeth and the location of tooth loss are necessary, more long-term follow study are needed, and it is thought to be necessary to enlarge the sample size of subjects in conducting the studies.

Mandibular implant overdenture using milled bar attachment on implants with unfavorable fixture angulation: A case report (불량한 식립 방향을 갖는 임플란트를 이용한 하악 Milled Bar 임플란트 피개의치수복 증례)

  • Implant overdenture exhibits higher retention, masticatory function, and patient's satisfaction compared to conventional dentures. Particularly, in treatment of severe alveolar bone resorption, implant overdenture can be considered as the first treatment option. The types of attachments used for implant overdenture can be classified into solitary type, which implants are not connected to each other, and bar type in which implants are connected. In the case of solitary type commonly used in clinical practice, parallel relationship is important. When it is not established, there is a higher risk of attachment wear, retention loss, and frequent visits for maintenance. In this case, satisfactory results have been obtained with implant overdentures using milled bar on two unparallel implants placed on the mandible.

Implant assisted removable partial denture with implant surveyed crown: A 20-month follow-up case report (단일 임플란트 서베이드 크라운을 이용한 하악 임플란트 융합 국소의치: 20 개월 경과 관찰 증례 보고)

  • Traditional options for posterior edentulous treatment include removable partial dentures and implant fixed prostheses. Recently, the concept of implant assisted removable partial denture, in which two treatments are fused, has been introduced in consideration of systemic health and patient's needs, costs, residual alveolar bone status and so on. Implant assisted removable partial denture has the advantage of increasing the retention and stability of the denture and improving its esthetics in cases of large bone defects or biomechanical disadvantages. In addition, it is possible to strategically place the implants in a site where the alveolar bone is relatively sufficient, thereby overcome the limit of the conventional removable partial denture design as well as reducing the burden on a wide range of implant surgery. Cost reduction is also expected. In this case, the patient was treated by placing the implant in both premolar sites of the mandible and fabricating the distal extension removable partial denture with the implant fixed prosthesis as an abutment. After delivering the definitive prosthesis, the patient showed satisfaction with the masticatory function and esthetics. and has been regularly followed-up for more than one year. The following 20-months follow-up case report describes the design of an implant-assisted-removable partial denture (IARPD) in which two cementretained implant crowns used to provide support and stability.

A 5-year prospective clinical study of Neobiotech implants for partially edentulous patients (부분 무치악환자에서 Neobiotech 임플란트의 5년 전향적 임상연구)

  • Purpose: The aim of the present prospective clinical study was to assess the cumulative survival rate (CSR) of Neobiotech implants restored with fixed partial prosthesis in relation to its potential risk factors. Materials and methods: Thirty six partially edentulous patients received Neobiotech implants and implant supported fixed partial prosthesis at Korea University Guro Hospital Dental Center from November 2009 until November 2011. The observation period was set from the implant placement and the last clinical visit until December 2015. Implant survival rate was determined using the Kaplan-Meier method. The relationship between implant survival rate and the potential risk factors were analysed using the multi Cox proportional analysis (P<.05). Results: A total of 69 implants were placed in 36 patients after a mean observation period of 45.9 months. Two out of 69 implants failed before loading, yielding a 5-year cumulative survival rate of 97.1%. The maxillary implants have a lesser CSR than the mandibular implants based on log rank test analysis (maxilla=91.3%; mandible=100% P<.05). However, the multi Cox proportional analysis showed that implant location has no significant correlation with implant failure (P>.05). Conclusion: Neobiotech implants showed predictable results with a 5 year cumulative survival rate of 97.1%.
