  • Fracture causes serious problems in many instance of prosthetic failures. But it is hard to find the definite causes when fractures occur. Fractography encompasses the examination of fracture surfaces that contain features resulting from the interaction of the advancing crack with the microstructure of the material and the stress fields. All fractured specimens(implant retaining screw) retrieved from Gangneung-Wonju national university dental hospital for 3 years(from 2007 to 2009). After pretreatment of samples, the scanning electon microscope were used for surface examination and fracture analysis. In case of most of the fractured specimens, fracture took place by fatigue fracture and fractured surface represents fatigue striation. Fatigue striation indicate the progression of the crack front under cyclic loading, are characteristic of stage 2 crack growth. The site of crack initiation and stage 1 crack growth were not easily identified in any of the failure, presumably because of the complex microstructural features of the polycrystalline sample. In case of fractured by overload, dimpled or cleavage surface were observed. Using the interpretation of characteristic markings(ratchet mark, fatigue striation, dimple, cleavage et al) in fracture surfaces, failure events containing the crack origin, crack propagation, material deficiency could be understand. Using the interpretation of characteristic markings in fracture surfaces, cause and mechanism of fractures could be analyzed.

  • The aim of this study was to assess change of the oral health related quality of life after fixed prosthetic treatment using implant therapy. One hundreds and twenty patients from 3 dental clinics in Seoul, Incheon and Daegu were recruited after verbal consent. The oral health related quality of life by OHIP-14, the treatment satisfaction by North Texas Periodontal Associates and the oral health interest w+ere measured before fixed prosthetic treatment using implant and one months after prosthetics. Total OHIP-14 changed significantly between pre- and post- treatment (p<0.001). Although gender(p=0.01), economic status (p=0.04) and education status (p=0.01) affect to OHIP-14 significantly before treatment, these factors didn't have effects on OHIP-14 after treatment. The subjective satisfaction in masticatory function, social function and psychological function increased significantly after treatment (p<0.001). Besides, the patients' oral health interests increased significantly after treatment (p<0.001). The fixed prosthetic treatment using implant therapy can improve the oral health related quality of life, subjective satisfaction of mastication, social function and psychological function, and oral health interest of dental patients.

  • As implant can be covered by National Health Insurance Service (NHIS), it was increased the interest in the removable partial denture (RPD) with the surveyed fixed prosthesis supported by implant fixture. To achieve predictable result, it needs the prudent implant planning by basic principles of RPD and patient's residual ridge. This 67 years old age male had a few unilateral remaining teeth, and hoped the treatment covered by NHIS. As using CAD software, the position of implant is planned with regarding to occlusal table of provisional denture, basic principles of RPD, and resorbed residual ridge. The definitive prostheses can ensure the stability and retention of removable prosthesis. When planning implant fixed prostheses, the digital technique was utilized to consider basic principle of RPD and resorbed residual ridge. As a result, it provided satisfactory prostheses.

  • This study was aimed to provide basic data which are fairly useful to improve satisfaction level of patients by analyzing factors related to each functional satisfactions of dental implant treatment. The subjects of this study were 120 patients who received implant treatment and analyzed with t-test, one-way ANOVA and pearson's correlation. The results of this study were as follows. 1. Differences of each functional satisfaction according to general characteristics was not reveal statistically significant. 2. A group to got periodic scaling and tooth brushed four or more times a day showed the highest satisfaction of social and esthetic function(p < .05). 3. A group to implantation in premolar area showed the highest satisfaction of masticatory function and a group to implantation in incisor to be high in satisfaction of social and esthetic function, but the differences were not statistically significant 4. A group to got implant treatment at a dental hospital showed the highest satisfaction of both social and esthetic function, and at a general hospital were found lowest(p < .05). 5. Masticatory function, social function, esthetic function, and overall satisfaction were found to be in a positive correlations, and especially, the correlation coefficient between the overall satisfaction and social function was found to be highest(p < .01).

  • Tooth impaction is defined as a failure of tooth eruption resulting from the physical obstacles in the eruption path or the abnormal position of the tooth germ. Impaction of mandibular second molar is relatively rare, and it may incite pathologic conditions such as dental caries, periodontitis, or root resorption of adjacent first molar. Thus, early diagnosis and treatment is recommended. In the first case, a 10-year-old male patient, was treated by brass wire to separate the bilaterally impacted mandibular second molars. In the second case, a 12-year-old female patient, was treated with Humphrey appliance for impacted mandibular left second molar, which was detected during a periodical dental examination. In the third case, a 17-year-old female patient with impacted mandibular right second molar, was treated by uprighting spring with mini-implant anchorage around premolar area. In the last case, an 18-year-old male patient, was treated with mini-implant placed on the ramus of mandible connected to the orthodontic buttons bonded on impacted mandibular left second molar by elastic thread.

  • When attempting to restore the oral function of a partially edentulous patient, there are a number of prosthetic treatment options available, depending on the structure of remaining teeth. For example, when only one set of maxillary and mandibular teeth are diagonally in place across from each other, it is difficult to gain stable occlusion. In this case, implants can be put in place at the corresponding edentulous area to achieve balance. By doing so, a stable occlusion can be achieved. For this case report, a patient with crossed occlusion after extraction was treated with maxillary RDP (removable dental prosthesis) and mandibular implant-supported RDP (removable dental prosthesis). Moreover, an implant fixture was placed under the posterior molar of the distal extension base diagonally across from the remaining maxillary teeth. Then, magnetic attachment was implemented. According to the patient who received the treatment, the result was functionally and aesthetically satisfactory.

  • Without proper treatment on the multiple tooth missing area, the lack of posterior support and the supra-eruption of the teeth cause many severe complications of occlusion, vertical dimension and masticatory function. This report is a case of full-mouth rehabilitation of a patient with loss of posterior support and collapsed occlusion due to missing teeth area left untreated for a long time. The patient who is 68-year old male patient had some teeth fallen out while removing his old maxillary denture and was complaining about pain in the region of anterior teeth due to traumatic contact. The vertical dimension was corrected by 4 mm from the top cervical point of the canine through various evaluations and the edentulous area was treated with the implant fixed prostheses through computer guided implant surgery based on the diagnosis and treatment plan for definitive prostheses supported by computed tomography (CT) data analysis and CAD-CAM (Computer-aided design/computer-aided manufacturing) technique. After full mouth rehabilitation, the patient was very satisfied with remarkable improvements in mastication, function, and aesthetics.

  • Orofacial dystonia is a neuromotor disorder that causes irregular or repetitive movements of the face, lips, tongue, and jaw involuntarily, also called tic disorder. Edentulous patients with these symptoms experience functional and aesthetic problems, including difficulty using complete dentures, speech and swallowing difficulties, and orofacial pain. In this case, for a patient with orofacial dystonia who experienced complete edentulism at a relatively young age, restorative treatment was performed with a maxillary complete denture with bilateral posterior zirconia occlusal surfaces and a mandibular implant-supported fixed prosthesis, and continuous smile training was performed. The aim was to improve the aesthetics of facial muscles. As a result of the treatment, the patient was very satisfied with not only improved chewing function and aesthetics, but also regained psychological stability and was able to lead a normal daily life, so we would like to report this.
