  • Dental implant have been in use for a long time to restore at missing tooth. But, To place dental implant at good position very is difficult. Improperly positioned dental implants make some problems to have a good function of dental implant. so, To place dental implant at accurate position is the most important step throughout the whole process. Digital guided system of dental implant is very useful to have a accurate position of dental implant and it makes upper restoration more esthetic and funcional.

  • 치과용 임플란트(Implant)란 상실된 자연치아를 대신하여 골 내에 매식하는 인공치근을 말한다. 임플란트는 인접 자연치아의 보호, 심미적 안정 등의 장점으로 인해 그 수요가 늘어나고 있으며, 단일치아 임플란트의 경우, 부분 무치악 환자들에게 있어서 우수한 치의학적 해법이 되어왔다. 대부분의 임플란트는 두 개 이상의 구성요소로 이루어져 있으며, 각각의 구성요소는 나사에 의해 결합되어 있다. 많은 연구결과를 통해, 임플란트의 나사 풀림 현상(Screw loosening)은 임플란트와 관련하여 가장 흔한 문제로 나타나고 있다.(중략)

  • Implant fracture is rare, but one of the most serious problem in implantation. Treatment of implant fracture can be different according to the extent of the fracture and on the state of the surrounding prosthetic restoration. Maintaining or submerging implant after treatment of peri-implantitis can be useful options for cases of tear-like fracture on the coronal area of an implant.

  • This paper is the first paper among two papers which constitute the paper about the structural design of a dental implant. This paper surveyed types and characteristics of dental implants. Merits and shortcomings of clinically used dental implants were analysed through this survey. A new structural model of dental implant was proposed on the basis of survey results. Especially, a comparative structural analysis of the new dental implant and the well known dental implants has been performed using the finite element analysis method. The analysis results confirm that the newly proposed dental implant has a good structural performance. The well known dental implants used for the comparative structural analysis are 3i implant and Sargon implant. These implants are imported dental implants and we could know that most implants clinically used are endosteal implants and these two implants(3i implant and Sargon implant) are also endosteal implants.

A 15-year clinical retrospective study of Br${\aa}$nemark implants (Br${\aa}$nemark 임플란트의 15년 임상적 후향 연구)

  • Purpose: This study was to compare the cumulative survival rate (CSR) of Br${\aa}$nemark machined surface implants and TiUnite$^{TM}$ imlants and to analyze association between risk factors and the CSR of the implants. Materials and methods: A retrospective study design was used to collect long-term follow-up clinical data from dental records of 156 patients treated with 541 Br${\aa}$nemark machined and TiUnite$^{TM}$ implants at Korea University Guro hospital in South Korea from 1993 through 2008. Machined implant and TiUnite$^{TM}$ implant were compared by CSR. Exposure variables such as gender, systemic disease, location, implant length, diameter, prosthesis type, opposing occlusion type, date of implant placement, type of edentulous space, abutment type, existence of splinting with natural teeth, and existence of cantilever were collected. Life table analysis was undertaken to examine the CSR. Cox regression method was conducted to assess the association between potential risk factors and overall CSR (${\alpha}$=.05). Results: Patient ages ranged from 16 to 75 years old (mean age, 51 years old). Implants were more frequently placed in men than women (94 men versus 63 women). Since 1993, 264 Br${\aa}$nemark machined implants were inserted in 79 patients and since 2001, 277 TiUnite$^{TM}$ implants were inserted in 77 patients. A total survival rate of 86.07% was observed in Br${\aa}$nemark and Nobel Biocare TiUnite$^{TM}$ during 15 years. A survival rate of machined implant during 15 years was 82.89% and that of TiUnite$^{TM}$ implant during 5 years was 98.74%. The implant CSR revealed lower rates association with several risk factors such as, systemic disease, other accompanied surgery, implant location, and Kennedy classification. Conclusion: Clinical performance of Br${\aa}$nemark machined and TiUnite$^{TM}$ implant demonstrated a high level of predictability. In this study, TiUnite$^{TM}$ implant was more successful than machined implant. The implant CSR was associated with several risk factors.

Retrospective study on marginal bone resorption around immediately loaded implants (즉시 하중 임플란트에서 변연골 흡수량에 관한 후향적 연구)

  • Purpose: Patients who treated implant immediate loading within a week after implant placement at Wonkwang University Dental Hospital Implant Center were evaluated marginal bone resorption. These retrospective analyses are intended to reinforce the clinical evidence for the implant immediate loading. Materials and methods: Medical history and radiographic data were investigated, which were the patients' who treated implant immediate loading and restoration with provisional prostheses between January 2005 and June 2016, at Wonkwang University Dental Hospital Implant Center. Total number of implants was 70, marginal bone resorption was measured according to implant length, diameter and connection type. To measure marginal bone resorption, periapical radiographs were taken when the implants were placed and after 6 month. Statistical analysis was done in Mann-whitney U test and Kruskal-wallis test with SPSS 22.0 software (P<.05). Results: Mean marginal bone resorption around immediately loaded implants according to implant connection type was shown $1.24{\pm}0.72mm$ in internal hexagon connection type and $1.73{\pm}1.27mm$ in external hexagon connection type. There was no statically significant difference in marginal bone resorption with implant length and diameter. Conclusion: Implants with immediated loading in internal hexagon connection type showed less marginal bone resorption significantly than in external hexagon connection type.

Three-Dimensional Finite Element Analysis for Comparison between Titanium Implant Abutment and Zirconia Implant Abutment (지르코니아 임플란트 지대주와 티타늄 임플란트 지대주의 삼차원적 유한요소응력분석)

  • Recently, restoring implants in the esthetically demanding region, zirconia-based materials are widely used due to their superior mechanical properties, accuracies, and esthetics. The purpose of this study was to investigate the load transfer and mechanical stability of zirconia and titanium implant abutments by using the three-dimensional finite element analysis model. The internal conical joint type and external butt joint type implant system was selected as an experimental model. Finite element models of bone/implant/prosthesis complex were constructed. An load of 250N was applied vertically beside 3mm of implant axis. Stress distribution of zirconia and titanium implant abutment is similar. The maximum equivalent stress of titanium implant abutment is lower than zirconia implant abutment about 15%. Howevere considering a high mechanical strength that exceed those of titanium implant abutment, zirconia implant abutment had similar mechanical stability of titanium implant abutment clinically.

A 5-year prospective clinical study of Neobiotech implants for partially edentulous patients (부분 무치악환자에서 Neobiotech 임플란트의 5년 전향적 임상연구)

  • Purpose: The aim of the present prospective clinical study was to assess the cumulative survival rate (CSR) of Neobiotech implants restored with fixed partial prosthesis in relation to its potential risk factors. Materials and methods: Thirty six partially edentulous patients received Neobiotech implants and implant supported fixed partial prosthesis at Korea University Guro Hospital Dental Center from November 2009 until November 2011. The observation period was set from the implant placement and the last clinical visit until December 2015. Implant survival rate was determined using the Kaplan-Meier method. The relationship between implant survival rate and the potential risk factors were analysed using the multi Cox proportional analysis (P<.05). Results: A total of 69 implants were placed in 36 patients after a mean observation period of 45.9 months. Two out of 69 implants failed before loading, yielding a 5-year cumulative survival rate of 97.1%. The maxillary implants have a lesser CSR than the mandibular implants based on log rank test analysis (maxilla=91.3%; mandible=100% P<.05). However, the multi Cox proportional analysis showed that implant location has no significant correlation with implant failure (P>.05). Conclusion: Neobiotech implants showed predictable results with a 5 year cumulative survival rate of 97.1%.

An assessment of accuracy of half-guided implant surgery using implant surgical guide: A case report (임플란트 수술용 가이드를 사용한 부분 유도 임플란트 수술의 정확도 평가: 증례보고)

  • Nowadays, dental implant is one of the widely used treatment options for edentulous patients. Recently, a method of improving the accuracy of implant surgery using an implant surgical guide has been introduced and widely used in order to accurately place the implant in a proper position. Full-guided and half-guided implant surgery can be distinguished according to the level of surgical guide application during the implant surgery. It is true that full-guided implant surgery exhibits higher accuracy, but half- guided implant surgery is often performed in a clinical situation due to the factors such as the circumstances of the operation. A partially edentulous patient who lost teeth due to tooth fracture and periodontal disease was treated using implant and fixed prosthesis. Half-guided implant surgery was performed using an implant surgical guide during implant surgery, the accuracy of implant placement was analyzed.

Full mouth rehabilitation in a patient with peri-implantitis: A case report (Peri-implantitis 환자에서의 전악 재수복 증례)

  • Peri-implantitis appears in almost 20% of patients who received implant treatment, and increase in its number is inevitable as time goes by. Although it can be treated by both non-surgical and surgical procedures, in cases which include severe bone loss, explantation and rehabilitation may be necessary. Careful treatment planning and considerations to prevent recurrent peri-implantitis should be taken into account. In the following case presented, a patient with chronic periodontitis and peri-implantitis was successfully rehabilitated after removal of several implants. Extraction and explantation of multiple teeth and implants were followed by full mouth reconstruction with fixed implant prostheses on the mandible and implant retained overdenture on the maxilla. Surgical and prosthetic measures to prevent recurrent peri-implantitis were taken into consideration.
