Purpose : To describe the clinical, MR imaging, and pathologic findings of pediatric meningiomas. Materials and Methods : The authors retrospectively reviewed the medical records and MR images of 16 pediatric patients with pathologically proven meningioma. Mean patient age at diagnosis was 14 years (range, 3-18). MR images were reviewed for details of lesion sizes, locations, signal intensity (SI), marginal characteristics, internal architectures, enhancements, and dural and parenchymal changes. The findings of other imaging modalities and of pathological examinations were also analyzed. Results : Mean tumor size was 5.24 cm (range, 1.3-18.1 cm) and locations were supratentorial in 12 and infratentorial in 4. SI of masses were variable, that is, high in 9, iso in 4, and low in 3 on T2 weighted images (T2WI), and low (n=11), iso (n=4), or high (n=1) on T1WI images. All lesions were visualized as well-demarcated enhancing masses. Five of the tumors were heterogeneous with cystic or necrotic components. Dural attachment was observed in 11 patients and adjacent brain edema in 10. Tumors exhibited hyperdense (n=6) or isodense (n=4) on non-enhanced CT scans, and 3 of the 7 angiograms demonstrated blood supply from the internal carotid artery. Pathologic examinations revealed the following subtypes; transitional cell (n=4), meningotheliomatous (n=4), chordoid (n=2), fibrous (n=2), clear cell (n=1), hyalinized (n=1), rhabdoid papillary (n=1), and atypical (n=1). Conclusion : Pediatric meningiomas occur usually in teenagers, have diverse pathological types, and may produce atypical imaging findings, such as, a heterogeneous internal content or findings suggestive of intraaxial tumors.
The statistical study was done on 74 cases of foreign bodies in the air passages, who visited the department of otolaryngology of SNUH during recent 5 years from 1975 to 1980. The result was as follows, 1) Of the total 74 cases, 55 cases were male and 18 cases were female. The ratio between male and female was 3 : 1. 41.8% of all-cases were under 2 years of age, 75.5% were under 3 years, and 83.6% were under 5 years. 2) Major symptoms on visit were dyspnea, cough, cyanosis. No significant symptoms was noted in 6.7%. Initial physical findings were coarse breathing sound, decreased breathing sound, and stridor. No abnormal physical findings were noted in 10.8%. 3) 48.6% of total cases visited hospital within 24 hours. There was one case who visited hospital over 2 years later. 4) Initial simple chest film showed atelectasis in 25.0%, emphysema in 32.8%, definite foreign body shadow in 17.2% and within normal limit in 17.2%. 5) 60.8% of total cases were vegetables, 15.5% were metals, 16.2% were fish bone, and 2.7 % were others. 6) Vegetables were most frequent foreign bodies in the cases under 5 years of age. 7) 40.5% of total cases were lodged in the right main bronchus, 31.0% were in the left main bronchus, 9.8% were in trachea and 6.7% were in multiple sites, 8) Foreign bodies were removed by ventilating bronchoscopy in 82.3%, by tracheostomy only in 2.7%, by tracheal suction in 2.7%, by spontaneous removal in 1.3%, by thorachotomy in 5.4% and 5.4% of total cases were expired.

Background: Although there have been several studies regarding the clinical value of an automated TB-PCR study using sputum, bronchial washing, and other body fluid samples for the detection of pulmonary tuberculosis, there are only a few reports on the use of fresh tissue samples. Materials and methods: The acid-fast bacilli stain(AFB), tuberculosis culture, automated TB-PCR study, and histopathology examination were performed in 42 fresh tissue samples. Results: Among the 42 cases, 18 cases were diagnosed with tuberculosis based on the clinical findings. Sixteen of the 18 cases were TB-PCR positive and of these 16 cases, only 2 cases were positive in the AFB stain or culture study. However, all 18 cases showed the histopathology findings of chronic granulomatous inflammation that was compatible with tuberculosis. Based on the clinical findings, the sensitivity, specificity, positive predictability, and negative predictability of the automated TB-PCR study were 88.9%, 100%, 100%, and 92.3% respectively. Conclusion: An automated TB-PCR assay is an important diagnostic tool for diagnosing tuberculosis in fresh tissue samples.
Breast ductal carcinoma in situ(DCIS) refers to cases in which breast epithelial cells have become malignant but are still limited to normal ducts, and has been increasing rapidly in recent years. In this case, a two-year follow-up revealed findings on mammography and ultrasonography that indicated a small mass classified as BI-RADS Category 3, However far from typical malignant tumor these findings were. In the second year of follow-up, a hypoechoic mass with an indistinct boundary of the right breast in the 6 o'clock direction, on mammography appeared to be about 2.1×1.3 cm in size, and biopsy diagnosed it as ductal carcinoma. Since ductal endothelial cancer has no characteristic clinical findings and can show positive clinical and imaging findings in the early stages, regular follow-up is considered important for early diagnosis, and detection of ductal endothelial cancer through mammography and ultrasound is important for improving the prognosis of all breast cancer patients. During the initial examination conducted four years ago, we reported cases of intra ductal cancer in which asymmetric shading, microcalcification, and branched mass, indicative of intra ductal cancer, were observed during follow-up. It is advisable to concurrently explore methods for reducing X-ray dosage to mitigate potential side effects of contrast medium.
Purpose : Acute disseminated encephalomyelitis (ADEM) is a demyelinating disease of the central nervous system and mostly develops after viral illness or vaccinations. We investigated the clinical differences and neurologic outcomes according to the distribution of the lesions on brain MRI. Methods : The study group was composed of 21 patients from January 1995 to August 2003 in Kyunghee University hospital. We grouped the patients according to the MRI findings as follows. Group I (14 cases): Multi- or unifocal lesions only in the cerebral white matter. Group II (7 cases): lesions in the gray matter with or without white matter involvement. Results : 1. Preceding events were as follows: no defined prodrome (38.1%), upper respiratory tract infection (28.6%), nonspecific febrile illness (19.0%), gastointestinal disturbance and vaccination. 2. Presenting symptoms were as follows: seizures (76.2%), headache/vomiting (47.6%), altered consciousness (38.1%), hemiparesis, cerebellar ataxia, visual disturbance and facial nerve palsy. 3. Laboratory findings were as follows: CSF pleocytosis (76.2%), leucocytosis (38.1%) and elevated CSF protein (28.6%). 4. Fifteen patients were recovered completely without neurological sequelae. Three patients in group I and 1 patient in group II had intractable seizures. Two patients in group I and 2 patients in group II had motor disturbance. Conclusion : There were no statistically significant differences in preceding events, presenting symptoms, and neurological outcomes according to the distribution of the lesions on brain MRI. However, the ADEM have quite diverse clinical manifestations and neuroimage findings. MRI plays an important role in making diagnosis of the patients who are suspected of ADEM.

Purpose : Rotavirus is the main cause of infantile diarrheal disease worldwide, especially in patients 3-24 months of age. Infants younger than 3 months of age are relatively protected by transplacental antibody. So the purpose of this study is to evaluate the clinical features and severity of neonatal rotaviral gastroenteritis less than 1 month of age. Methods : A retrospective chart review was established of 62 neonates less than 1 month of age and with a diagnosis of rotaviral gastroenteritis who had been admitted to Pochon CHA University between June 2002 through July 2004. The rotavirus was examined by stool latex agglutination. Results : During 2 years, the total number of admitted patients for rotaviral gastroenteritis was 688 and among these, less than 1 month of age accounted for 9%(62). The occurrence was generally even distribution from January to July($7.14{\pm}1.0$) but since then decreased($2.4{\pm}1.8$). The most common chief complaint was mild fever(46%) when admitted which subsided within 1 hospital day in most patients. 4 patients had seizure and cyanosis with no typical symptoms of rotaviral gastroenteritis. During admission, all the patients had diarrhea. 17% of the patients had leukocytosis and positive C-reactive protein. In one patient, stool occult blood test was positive but there was no necrotizing gastroenteritis evidence. The mean period of hospital day was $5.8{\pm}2.5$ and breast-milk feeding was 62.9%. Conclusion : Neonatal rotaviral gastroenteritis is not a rare disease. Most patients have fever and diarrhea and improve through conservative therapy but a few patients may have severe complications so we must be more cautious about the hygiene for prevention.
Purpose: Neonatal hepatitis is the major cause of neonatal cholestasis and may be divided into infectious, metabolic, genetic, and idiopathic neonatal hepatitis. Non-familial, non-metabolic, and non-A, B, C viral neonatal hepatitis is known to have made satisfactory progress, but little is known about its chronic clinical features. Methods: Clinical and histological assessments were carried out in 34 cases with chronic neonatal hepatitis [elevated serum alanine aminotrasferase (ALT) level for more than 6 months] except for A, B, C viral hepatitis, metabolic, or genetic neonatal hepatitis, who were admitted to the Department of Pediatrics, Pusan National University Hospital, from January 1998 to January 2004. Results: Males were more common (70%). Jaundice (100%) and hepatomegaly (44%) were frequent manifestations. Peak serum ALT levels were most commonly below 300 IU/L in 41.2% of patients and peak serum direct bilirubin levels were most commonly between 1.0~5.0 mg/dL in 50% of patients. Ten cases (34%) of 29 patients had positive serum cytomegalovirus (CMV) IgM or urine CMV polymerase chain reaction. Serum ALT level was normalized within 1 year in 11 (37.9%) of 29 cases, and within 2 years in 9 (69.2%) of 13 cases. Serum ALT level was elevated persistently over 2 years in four (30.7%) of 13 cases. Histologic findings such as portal or periportal activity, lobular necrosis, portal or periportal fibrosis were more severe in patients with persistent ALT elevation over 2 years than in those showing normalization of ALT within 2 years (p>0.05). Conclusion: When the elevation of ALT level sustains over 1 year in non-familiar, non-metabolic, non-A, B, C viral neonatal hepatitis, an assessment of the severity of liver injury and a careful monitoring about chronic liver disease may be required.
목 적 : 측두엽 간질 환자의 발작간 뇌 스캔에서 관찰되는 측두엽 혈류 감소와 같은 쪽 시상 혈류 감소 소견의 빈도를 알아보고 이러한 소견이 간질 병소 국소화에 미치는 유용성에 대해 알아보고자 한다. 대상 및 방법 : Tc-99m-ECD를 이용하여 발작간 뇌 스캔을 시행한 67명의 측두엽 간질 환자에서 편측 측두엽과 같은 쪽 시상에 혈류 감소를 보인 12명의 환자를 대상으로 하였다. 간질 병소는 표면 뇌파 검사, 발작시 뇌파 검사, 심부 뇌파 검사, 자기 공명 영상 그리고 임상적 소견을 종합하여 국소화 하였다. 결 과 : 편측 측두엽과 같은 쪽 시상에 혈류 감소는 18%의 환자에서 관찰되었다. 7명의 환자는 왼쪽 측두엽과 시상에 혈류 감소를 보였다. 이들 7명중 4명은 자기공명영상 소견상 왼쪽 내측 측두엽 경화 소견을 보였다. 5명의 환자는 오른쪽 측두엽과 시상에 혈류 감소를 보였다. 이들 5명중 4 명은 자기공명영상 소견상 오른쪽 내측 측두엽 경화 소견을 보였다. 결 론 : 발작간 뇌 스캔상 관찰되는 편측 측두엽과 같은 쪽 시상의 혈류 감소 소견은 측두엽과 시상사이의 상호 연결에 의한 해리 현상으로 부분 발작의 병태 생리와 밀접한 연관이 있으리라 생각한다. 또한 이러한 소견은 간질 병소 국소화에 도움이 되리라 생각한다.
양성 종양(benign tumor)을 언급하기에 앞서 과오종(hamartoma) 및 과다 형성(hyperplasia)과의 차이를 구분할 필요가 있다. 양성 종양은 기원조직과 유사한 조직이 이상 증식하는 것으로 서서히 성장하지만 일반적으로 치료하기 전까지 이상 증식을 지속하는 진성 신생물을 일컫는다. 이에 비해 과오종은 정상 조직이 무질서하게 과증식하는 것으로 일정기간 후에는 성장을 멈추기 때문에 진성 신생물로 간주하지 않는다. 그러나 일부 과오종이 양성 종양에 포함되기도 하는데, 예를 들어 치아종은 정상적인 치성 조직의 성장이 완료되는 시기와 거의 동일한 시기에 성장을 멈추지만 양성 종양으로 분류된다. 과다 형성은 조직의 세포가 정상적인 배열 양상을 보이면서 세포의 수가 증가하는 것으로 지속적인 성장 양상을 보이지만 그 성장이 제한적이므로 양성 종양과는 구별된다. 양성 종양은 일반적으로 무통성으로 서서히 성장하기 때문에 종양의 크기가 증가하여 안면 종창이나 동통 등을 유발하는 경우에 발견될 수 있으며, 방사선검사에서 우연히 발견되기도 한다. 방사선검사는 병소의 위치, 범위, 특징 및 병소와 인접 해부학적 구조와의 관계 등 많은 방사선학적 진단 정보를 제공한다. 일부 종양은 방사선사진에서 매우 특징적인 소견을 나타내기 때문에 방사선학적 소견으로 예비 진단을 할 수 있을 정도의 진단정보를 제공하기도 하는 반면 어떤 종양들은 방사선사진에서 관찰되는 소견이 매우 유사하여 진단에 어려움을 주기도 한다. 따라서 종양의 확진을 위해서는 생검이 필수적이며, 방사선검사는 반드시 생검에 앞서 진행되어야만 정확한 방사선학적 진단을 할 수 있다. 양성 종양은 각각의 특징적인 방사선학적인 소견을 나타내지만 일반적으로 관찰되는 양성 종양의 특징이 존재하므로 이러한 일반적인 특징을 관찰하여 병소가 양성인지 악성인지를 감별할 수 있다. 첫째, 양성 종양은 대개 호발하는 부위가 있으므로 종양의 발생부위는 감별 진단을 하는 데 매우 중요하다. 일반적으로 치성 병소는 치아가 형성되는 하악관 상방의 치조돌기에서, 혈관성 및 신경성 병소는 하악관 내에서, 연골성 종양은 하악과두와 같이 연골세포가 잔존되어 있는 부위에서 발생하는 경우가 많다. 둘째, 양성 종양은 대체로 명확한 경계와 피질골성 변연을 보이며, 종종 병소가 결체조직으로 둘러싸여 있어 병소 주위에 방사선투과성 띠가 관찰되기도 한다. 셋째, 양성 종양은 일반적으로 인접 주위 조직에 압력을 가하면서 서서히 성장하기 때문에 인접 치아의 변위 또는 흡수, 피질골의 비박, 팽융 등의 소견을 보이며 피질골의 천공은 드물다. 방사선학적으로 양성 종양의 병소 내부는 방사선투과상으로 관찰되거나, 방사선불투과상으로 관찰되거나, 방사선투과상과 방사선불투과상이 혼재된 상으로 관찰된다. 저자는 이 지면에서 이러한 방사선학적 특징을 기초로 하여 구강악안면영역에 발생하는 양성 종양을 분류하고 각각의 특징에 대해 살펴보고자 한다.
Radiographs were made of the feet of one hundred and twenty four normal children who were ranged in age from 1 to 13 years. On lateral radiographs that were made with the feet in standing, the talo-first metatarsal, calcaneal pitch and calcaneo-fifth metatarsal angles and arch ratio were measured. Most of the children in this study revealed flattened feet in earlier ages and the mean values were changed with age up to normal range later in the early school ages. These data might provide one of the standard for assessment of the flatfoot in children. However, the diagnosis and treatment of the flatfoot should be based on clinical assessment and radiographic values should not determine clinical management even if the values are beyond the normal range.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
