임상간호사의 간호전문직관, 직무 스트레스, 간호근무환경이 인간중심간호에 미치는 영향 (Effects of Nursing Professionalism, Job Stress and Nursing Work Environment of Clinical Nurses on Person-centered Care)

  • Purpose: This study attempted to examine the degree of nursing professionalism, job stress, nursing work environment, and person-centered care of clinical nurses, and also to identify the relationship between the variables and the factors affecting person-centered care. Methods: Participants included 162 nurses with a clinical experience of six months or more, who have worked at hospitals with more than 500 beds in Gangwon Province. Results: Person-centered care showed a statistically significant positive correlation with nursing professionalism (r=.43, p<.001) and nursing work environment (r=.34, p<.001). The adjusted R2 was 0.266, indicating that the measured variables explained 26.6% of the variance in person-centered care. Nursing professionalism had the greatest impact on person-centered care among clinical nurses (β=.37, p<.001), followed by job stress (β=.21, p=.005), nursing work environment (β=.19, p=.007), and master's or higher degree (β=.15, p=.036). Conclusion: The findings show that four factors affect person-centered care: nursing professionalism, job stress, nursing work environment, and education level. For clinical nurses to provide high-quality person-centered care, it is necessary to strengthen nursing professionalism, address job stress and improve nursing work environment.

전문간호사를 위한 임상 전문실습교육프로그램 요구 (Needs of Training Programs for Practice Courses in Advanced Practice Nurse Education)

  • Purpose: This study aimed to assess needs of training programs for APN practice courses and continuing education programs for APNs to provide baseline data to improve APN education and practice. Methods: A total of 98 questionnaires were distributed to professors in 38 nursing institutions and 64 (65.3%) were returned. Data collection was performed in November and December, 2007 through email. Results: Professors in charge of geriatric and home care nurse practitioner programs constituted 43.7% of the study participants. Forty-one percent answered that they were satisfied with the practice programs they currently have operated, while 36.5% responded to have difficulty in appointment of institutions for practice programs and 56.3% had problems in assignment of preceptorship. In case of developing training programs for APN practice courses, 58.7% answered that they are willing to participate, and 90.6% felt needs for development of continuing education programs for APNs. Regarding liability insurance for APN students, only 11.5% provided insurance policy. Conclusion: The study results indicated that standardized training programs are needed to assure quality of APN education, and continuing education programs are required to provide opportunities for APNs to improve competencies.

병원 간호사의 그릿과 긍정심리자본이 직무열의에 미치는 영향 (The Influences of Hospital Nurses' Grit and Positive Psychological Capital on Job Engagement)

  • 본 연구는 간호사의 직무열의에 그릿과 긍정심리자본이 미치는 영향을 파악하기 위한 조사연구이다. 연구 대상자는 종합병원의 간호사 159명이며 2020년 4월 9일부터 4월 21까지 설문 조사를 하였다. 자료의 분석은 t-test, 분산분석, Pearson's 상관계수, 위계적 다중회귀분석을 시행하였으며, SPSS version 24.0을 이용하였다. 연구 결과 직무열의는 그릿 중 꾸준한 노력과 긍정심리자본의 모든 하위영역과 정적 상관관계가 있었다. 직무열의에 영향을 미치는 요인은 긍정심리자본의 하위영역인 낙관주의(β=.28, p=.001)와 자기효능감(β=.23, p=.029)으로 모형의 설명력은 41.8%이었다(F=12.34, p<.001). 추후 직무열의를 증진시키기 위하여 긍정심리자본 향상 프로그램을 개발하고 적용하며 그 효과를 파악하는 후속연구가 필요하다고 사료된다.

전문간호사의 전문직업성, 임상적 의사결정능력 및 간호업무성과의 관계 (The Relationship between Nursing Professionalism, Clinical Decision Making Abilities, and Job Performance in Advanced Practice Nurses)

  • Purpose: To identify the extents of nursing professionalism, clinical decision making abilities and job performance of advanced practice nurses and investigate the relationship among the variables Methods: Participants, selected by a convenience sampling method, were 135 advanced practice nurses working in 4 tertiary care general hospitals in 'B' Metropolitan City and in 1 tertiary care general hospital in 'J' city. Data collection was done from January 15 to February 28, 2013 using self-reporting questionnaires. Results: The average score for nursing professionalism of study participants was $81.19{\pm}7.56$. for clinical decision making abilities, $140.42{\pm}9.62$ and for job performance, $102.54{\pm}10.30$. These averages are relatively high. The relationship between the extent of nursing professionalism and the extent of clinical decision making abilities showed an intermediate level positive correlation (r=.45, p<.001). The relationship between the extent of nursing professionalism and the extent of job performance was also an intermediate level positive correlation (r=.42, p<.001). The extent of clinical decision making abilities and the extent of job performance was an intermediate level positive correlation (r=.41, p<.001). Conclusion: Developing a program, which can improve nursing professionalism and clinical decision making abilities of nurse, is required to enhance their job performance.

중환자실 간호사의 환자-가족 중심 간호에 대한 인식이 돌봄 행위 수행에 미치는 영향 (The Effect of Perceived Patient- and Family-Centered Care on Nurses' Caring Behavior in Intensive Care Units)

  • Purpose: The purpose of this study was to identify the factors influencing nurses' caring behavior. Methods: The descriptive study included 178 intensive care unit (ICU) nurses who completed a self-report structured questionnaire. The data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, and Pearson's correlation and multiple regression analysis using the IBM SPSS/WIN 23.0 program. Results: Significant correlation existed between perception of Patient- and Family-Centered Care (PFCC) and nurses' caring behaviors (r=.36, p<.001). Multiple regression analysis showed that perception of support (β=.36, p<.001), total career length (β=.33, p<.001), and familiarity with PFCC (β=.15, p=.018) affected the caring behavior of ICU nurses. These variables explained 30.0% of the variance in caring behavior. Conclusion: Effective strategies are needed to improve perceptions of support as well as to promote PFCC for increasig the frequency of caring behavior among ICU nurses.

일 상급종합병원의 전문간호사에 대한 환자만족도 (Patient Satisfaction with Advanced Practice Nurse in a Tertiary Hospital)

  • Purpose: The purpose of this study was to identify patient satisfaction about nursing care provided by Advanced Practice Nurses (APN) using the La Monica-Oberst Patient Satisfaction Scale (LOPSS). Methods: The investigation was conducted including 263 patients who experienced care provided by 41 APNs at a tertiary hospital in Korea. Patient satisfaction with nursing care was measured using 41-item LOPSS. We analyzed the data using descriptive statistics, independent t-test and one-way ANOVA. Results: Overall patient satisfaction with APN care was high. The LOPSS scores for APNs were 52.26 for good impression, 61.75 for interpersonal support, 81.32 for dissatisfaction and 195.93 for total score. "APNs talk down to me" was a reverse coding item which demonstrated the highest score for patient satisfaction. Among patients' characteristics, age and type of main caregiver were independent factors related to patients's satisfaction. Among APN's characteristics, work experience was significantly related to patients' satisfaction. Conclusion: This study identified patient satisfaction regarding nursing care provided by APNs. High patient satisfaction in APN services demonstrated APNs' contribution to quality improvement in health care services. It is recommended that various outcome research about APN service needs to be performed.

암 전문병원 간호사의 간호역할 수행도와 중요도 (Nurse's Role Performance and Perception of the Importance of Role among Nurses Practicing in a Cancer Care Facility)

  • Purpose: The purpose of this study is to examine the nurse's role performance and the perception of the importance of role among nurses practicing in a cancer care facility. Methods: A descriptive study design was applied with convenient sampling of 175 nurses working at a cancer care facility in Daegu, Korea. Data was collected using a structured questionnaire from July 1, 2013 to July 5, 2013. Results: Cancer care facility nurse's role performance score was measured on average $3.23{\pm}0.64.$ Oral medication was the most frequently performed role, followed by encouragement, care related to medication, and pain management. The perception of the importance of nurse's role was measured on average $3.31{\pm}0.35$. Care related to medication was regarded as the most important, followed by transfusion, oral medication, and intravenous & intramuscular medication. Nurse's role performance was different according to nurse's age (F=3.21, p=.024) and current practice area (F=3.73, p=.012). The perception of the importance of nurse's role was different only in relation to current practice area (F=6.82, p<.001). Conclusion: Nurses practicing in oncology setting frequently involve encouraging patients and pain management. Training programs designed to facilitate frequently performed and highly regarded nurse's roles are required for nurses practicing in oncology setting.

500병상 이상 의료기관에서의 전문지원인력 운영실태 (The Current Status of Professional Medical Support Staffs in Medical Institutions with over 500 Beds)

  • Purpose: This study was to investigate the nationwide operational status of the professional medical support staffs (PMSS) who practice the expanded roles in the hospital setting. Methods: The data were obtained through survey from 36 hospitals with over 500 beds from 25th May to 12th July 2016. Data from 1,666 PMSS were analyzed. Results: Since the job titles varied, we classified them into 5 groups according to their roles; advanced practice nurse, clinical nurse expert, PA (physician assistant), coordinator, and others. There were differences in the operation status of PMSSs depending on the region, nurse staffing grade and number of hospital beds. Qualification criteria varied from hospital to hospital, and almost half of the hospitals didn't have any qualification standards for them. There were differences in age, educational level, clinical careers, rewards, and job satisfaction in 5 groups. Especially PA group had low salary, poorer working conditions, more difficulties in performing their work, and lower job satisfaction than other groups. Most PMSS (99.5%) were using a delegated prescription authority, however only 68.3% had job description and 19.9% had documented delegated role. Conclusion: Adequate training curriculum, documented delegated roles, and the protocols for legal protection and efficient medical services are needed.

  • Purpose: This study aimed to investigate the current statistics of professional medical support staffs(PMSSs) working in general hospitals with less than 500 beds. Methods: This study was conducted on 35 general hospitals with less than 500 beds from September 11th to October 27th, 2017. Results: Four hundred fifty one PMSSs were currently providing medical support. The number of clinical nurse experts was the highest among the roles, followed by Physician Assistants(PA) and Advanced Practice Nurses. The mean job satisfaction score was 3.07 out of 5. In the case of PA group, most of the delegated prescriptions were performed, however the delegated roles were not much documented in written format. The paucity of documentation requires a development of a committee for PMSSs, including a development of selection criteria and a scope of practice in each institution. Conclusion: The results of this study suggested the composition of a committee for PMSSs in the medical institutions and renaming the specified titles of PMSSs.

상급종합병원 간호사의 근거기반실무에 대한 지식과 태도 및 수행 (Registered Nurses' Knowledge, Attitudes, and Practice about Evidence-Based Practice at General Hospitals in Korea)

  • Purpose: Evidence-based practice (EBP) has emerged as a marker for healthcare quality and a global issue in nursing care. This study was conducted to identify Korean registered nurses' knowledge, attitudes, and practice about EBP in Korea. Methods: With a cross-sectional design, the EBP questionnaire (EBPQ) was administered to a convenience sample of nurses (N=453) recruited from 44 general hospitals. To increase the response rate, the directors of nursing were individually contacted by the researchers. The final response rate was 95.7%. Results: The majority of participants were females (99.3%), staff nurses (57.8%), and with bachelors degree (40.6%). Most of them (314, 73.4%) had ever heard about EBP. The overall mean score for the EBPQ was $4.72{\pm}0.69$ out of maximum score of 7. The mean score for the knowledge of EBP, attitudes towards EBP, and practice/use of EBP were $4.61{\pm}0.75$, $4.85{\pm}0.94$, and $4.90{\pm}0.88$, respectively. Educational level, age, and exposure to EBP were statistically significant predictors of Korean nurses' EBP perception. Conclusion: The findings showed that nurses with higher education are more likely to use and value the best available evidence for practice. This study suggests needs not only for further exploration but also for developing educational plans provided by each institute to assist nurses with the process of EBP and the associated skills.
