Affecting Variables on Subjective Memory Impairment in the Elderly with Benign Prostatic Hyperplasia (전립선비대증 노인의 주관적 기억장애 영향요인)

  • This study aimed to investigate variables affecting subjective memory impairment in the elderly with benign prostatic hyperplasia. We used 179 survey data collected from the urology department of two metropolitan general hospitals. The subjects were patients with benign prostatic hyperplasia. T-test, one-way analysis of variance, and $Scheff{\acute{e}}^{\prime}s$ test were performed to identify differences among variables. Pearson's correlation-coefficient and stepwise multiple regression were used to identify variables that affect subjective memory impairment. This study revealed that lower urinary tract symptom and activities of daily living were statistically significant variables, with an explanatory power of 30.0%. The lower urinary tract symptom was due to the benign prostatic hyperplasia increasing the risk of subjective memory impairment. Treatment of the lower urinary tract symptom was delayed because as the elderly got older, they increasingly recognized benign prostatic hyperplasia as a reasonable symptom. Timely intervention and early screening are necessary to prevent lower urinary tract symptom. Finally, programs to reduce the incidence of lower urinary tract symptom and educate nurses to assess, evaluate, perform intervention for subjective memory impairment should be developed.

The effect of disability and disability type on the depression of the elderly (장애여부, 장애유형이 노인의 우울에 미치는 영향)

  • This descriptive study investigated depression among 4,904 elderly people aged 60 to 90 years using the Korean Retirement and Income Study. The depressive group and non-depressive group were classified by the depression self-diagnostic scale, CES-D. The effects of general characteristics, disability, disability type, IADL, and social support on depression were examined. A t-test, chi-squared test, and logistic regression analysis were used to identify risk factors affecting depression. The average age of the depressive group was 75.10 years, which was higher than the average age of the non-depressive group (73.44 years), indicating that depression increased with age. Elderly who were female, elderly with no spouse, elderly with lower educational attainment and with a lower household income were in the depressed group. The score to perform IADL of disabled persons was higher than that of non-disabled persons, and the social support was lower. Logistic regression analysis performed to determine the risk factors affecting depression revealed that it was affected by disability and type of disability. Although this study has some limitations, the results presented herein can be used as a basis for understanding elderly with disabilities and developing future healthcare programs for them.

The Effects of the Cognition Improvement Activities of the Elderly in the Welfare Center on Hand Dexterity, Cognition, Instrumental Activity of Daily Living and Depression (복지관 노인의 인지증진활동이 손의 기민성 정도와 인지능력, 도구적 일상 생활 및 우울에 미치는 영향)

  • Objective : The purpose of this study is to investigate the effects of hand dexterity, cognition skill, instrumental activities of daily living, and depression after cognition improvement activities in elderly welfare centers. Method : The cognition improvement activities were performed for 10 ristimes on 20 elderly people. This study assessed hand dexterity, cognition skills, IADL, and depression using grooved pegboard, MOCA-K, K-IADL, and K-GDS before and after cognition improvement activities. Result : After cognition improvement activities, hand dexterity improves positively and cognition skills and depression improves significantly. However, instrument activities of daily living was not affected by cognition improvement activities. Conclusion : The cognition improvement activities affect positively on hand dexterity, cognition skills and depression. The elderly people of welfare center were highly satisfied with cognition improvement activities. However, this study were held in one welfare center in K city, so further study required to recruit more sample size and develop regional programs to generalize the result.

Correlation Between Selective Motor Control Test and Functional Performance Evaluation in Children With Spastic Cerebral Palsy (선택적 운동 조절 척도와 기능적 수행도 평가 간의 상관: 경직형 뇌성마비 아동을 대상으로)

  • The purpose of this study was to investigate the relationship among functional evaluation systems, the Selective Motor Control Scale (SMC scale), the Gross Motor Function Classification System (GMFCS), the Gross Motor Function Measure (GMFM) and Activities of daily living in children with spastic cerebral palsy and to provide the foundation data about SMC scle for evaluation system of abilities of selective motor control in children with spastic cerebral palsy. For this, sixty eight children with spastic cerebral palsy were participated in this study. The children were evaluated by using the SMC scale for their selective motor control ability and by using the GMFCS and GMFM for their gross motor function. The activities of daily living were assessed by using the Functional Independence Measure of Children (WeeFIM). There were a significant correlation between the SMC scale and the GMFCS (r = -.485, p < .05). The good correlation between the SMC scale and GMFM was found (r = .482, p < .05). The activities of daily living were not a significant correlation with SMC scale (r = .019, p > .05). The SMC scale in practice will provide usefulness for assessment of abilities of selective motor control in children with spastic cerebral palsy.

Effects of a Computerized Cognitive Training on Cognitive Function, Depression, Self-esteem, and Activities of Daily Living among Older Adults with Mild Cognitive Impairment (컴퓨터 인지강화 프로그램이 경도인지장애 노인의 인지기능, 우울, 자아존중감 및 일상생활 수행능력에 미치는 효과)

  • Purpose: The aim of this study was to evaluate the effects of a computerized cognitive training on older adults affected by mild cognitive impairment (MCI) in terms of cognitive function, depression, self-esteem, and activities of daily living (ADL). Methods: This study used a non-equivalent control group non-synchronized design. A total of 53 older adults who reside in long-term care facilities were recruited, 26 subjects for an experimental group and 27 subjects for a control group. A computerized cognitive training was performed for 20~40 minutes/day, three days/week for ten weeks. Data were analyzed using SPSS/WIN 21.0 with $x^2$ test, Fisher's exact test, and t-test. Results: Scores of MMSE (t=3.30, p=.002), depression (t=-2.15, p=.036), and self-esteem (t=2.76, p=.008) were significantly better in the experimental group than the control group. However, the difference in ADL (t=-1.01, p=.316) was not significant between the two groups. Conclusion: These findings suggest that the computerized cognitive training can be used as an effective nursing intervention to improve cognitive function and self-esteem and lower depression among older adults with MCI.

The Effect of Gradually Observation-Reduction Action Observation Training on Upper Extremity Function and Activities of Daily Living in Patients with Chronic stroke: a Pilot Study (점진적 관찰감소 동작 관찰훈련이 만성기 뇌졸중 환자의 상지 기능과 일상생활 활동에 미치는 영향 : 예비 연구)

  • This study was conducted to investigate the effect of gradually decreasing action observation training on the upper extremity function and Activities of daily living in chronic stroke patients. For patients with chronic stroke, the groups were divided into experimental group undergoing gradually decreasing action observation training(n=4), control group receiving existing action observation training(n=3) and the study was conducted 6 times a week, 30 minutes per session for a total of 2 weeks. The results of the comparison between the groups before and after intervention and the comparison between the two groups did not show statistically significant differences in BBT, FMA, K-MBI, and MAL, but the experimental group showed a greater difference in terms of average score than the control group. As a result, it was confirmed that gradually decreasing action observation training can have a more positive effect than the existing action observation training.

Estimation of Elderly Needing Long-term Care in S-city Gyeonggi-do according to ADL and IADL (경기도 S시 노인의 신체적 및 도구적 일상생활수행능력에 근거한 장기요양시설 보호대상자 추계)

  • Objectives: The purpose of this study is to estimate the needs of long-term care in S-city Gyeonggi-do according to ADL and IADL. Methods: We surveyed to investigate the needs of 397 study samples which had been selected by stratified randomized sampling, 2.7% in each Dong of S-city. We estimated the disability of elderly by using standards based on ADL and IADL of OECD and Korea Institute for Health and Social Affairs. Results and Conclusions: According to standards of OECD, the elderly with severe disability were 4,712(31.2%). According to standards of Korea Institute for Health and Social Affairs, the severe disabled elderly with helper were 3,776(24.9%) and the severe disabled elderly without helper 2,130(14.1%) So We concluded that the long-term care beds estimated by need were from 2,130 to 4,712. To estimate the long-term care beds, we have to consider together physical, mental, nutrition, and cognition status, existence of helper, helper characteristics, and emotional relationship between elderly and helper. But we considered only disability grades of physical states and existence of helper in this study. So it will be useful to conduct study to consider these other factors in future. At the same time this study is of value to estimate objectively long-term care needs based on physical needs in one region.

Effect of Upper Extremity Function and Activities of Daily Living of Mirror Therapy on Patients Who Have a Stroke, with or without Unilateral Neglect (편측무시 유무에 따른 거울치료가 뇌졸중 환자의 상지기능과 일상생활활동에 미치는 효과 비교)

  • Objective : We tested the effect of mirror treatment on patients with and without unilateral neglect over a period of 4 weeks. We also wanted to know the effect of mirror treatment with regards to improving upper extremity function and dealing with activities of daily living. Methods : Subjects were 10 hemiplegia patients with unilateral neglect and 10 hemiplegia patients without unilateral neglect. They visited the department of occupational therapy. We performed mirror treatment for 30 minutes, 5 times a week for 4 weeks. We used Albert Test to compare our results from before and after the treatment, Fugl-Meyer motor function Assessment(FMA) and Box & Block Test(BBT) for upper extremity function, and Functional Independence Measure(FIM) for activities of daily living. Results : The results showed that mirror treatment had an effect on stroke patients with unilateral neglect. The treatment also had an effect on upper extremity function and activities of daily living regardless of unilateral neglect. Conclusion : The mirror treatment showed improvements on upper extremity function and activities of daily life as well as a reduction in the unilateral neglect after a stroke. In conclusion, the mirror treatment is useful for recovering function and improvement and for helping the patients complete activities of daily living.

Effects of the Dual-Task Training on Stroke Patients : A Systematic Review and Meta-analysis (이중과제 훈련이 뇌졸중 환자에게 미치는 영향 : 체계적 고찰 및 메타분석)

  • Objective : The purpose of this study was to analyze the effects of dual-task training on stroke patients. Methods : We searched the databases such as NDSL, RISS, PubMed, CoChrane and EMBASE for publications in the past decade. Finally, 10 papers were selected. Qualitative assessment was performed according to the traditional single-layer evidence model, and meta-analysis was performed using the Comprehensive Meta Analysis 3.0 program. Results : The quality level of each of the 10 selected papers all correspond to I and II in the traditional single-layer evidence model. The motor tasks that constitute dual-task training comprised walking or balancing tasks in 7 articles and the motor tasks related to upper extremity were selected in 3 studies. The effect sizes for ADL function and Cognitive function were 0.65 and 0.64 (medium size effect) respectively. Moreover, the effect sizes of Lower extremity and Upper extremity motor function were 0.34 and 0.22 (small size effect) respectively. The effect size of ADL function and Cognitive function were statistically significant p<0.05). Conclusion : This study confirmed that dual-ask training can be a useful intervention technique for recovering a stroke patient's ability to perform daily activities and cognitive functions. This could be used as a helpful data when selecting appropriate intervention for stroke patients in the clinical setting.

The Effect of Activity of Daily Living Exercise for Self-care Ability and Family Burden in Stroke Patients (일상생활동작 훈련이 뇌졸중 환자의 자가간호 수행능력과 가족의 부담감에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effect of ADL exercise in stroke patients and to define to strategy to promote their self-care ability, decrease to their family burden. Method: The experimental design was designed nonequivalent control group non-synchronized design. The study method had been done by investigating the experimental group and control group through the questionaire on 33 patients. ADL exercise was conducted by the researcher and was carried out experimental group once per day for 20 minutes for daily 28 days. Results: For the ADL exercise, self-care ability score was increased and family burden score was decreased significantly. Conclusion: The exercise increased the self-care ability and effect of family burden of stroke patients. Based upon these results, it is recommended that the nurses who take care of stroke patient carry out them the ADL exercise continuously.
