The Correlation of Oral Stereognosis, Cognition, Instrumental Activities of Daily Living, and Quality of Life in the Elderly : A Pilot Study (노인의 구강 입체인지와 인지, 수단적 일상생활, 삶의 질과의 관계 : 예비연구)

  • The study seeks to conduct an oral stereognosis assessment of the elderly, identify characteristics and confirm the association with cognition, Instrumental Activities of Daily Living(IADL), Quality of Life(QOL). Oral stereognosis(OS) was evaluated on 20 senior citizens aged 75 or older living in Gyeonggi-do. Cognition was used as MoCA-K(Montreal Cognitive Assessment-Korean), IADL as K-IADL(Korean Instrumental Activities of Daily Living), and QOL as GQOL(Geriatric Quality of Life scale). OS decreases accuracy with age, unaffected by the level of education. Shapes with clear edges and broad sides were found to be easy to recognize. OS is related to cognition, IADL and QOL. Through this study, the OS of the elderly could predict the functional level and QOL, including cognition. Therefore, it can be used as a basic research for the physical and mental health management of the elderly through oral lectures, and the development of oral stereognosis tools for the elderly through large scale subjects should be made.

Factor analysis about the measurement of dental impact on daily living in workers (근로자들의 일상생활 구강건강인지(DIDL) 도구 요인분석)

  • The purpose of this study was to measure of dental impact on daily lining among workers. The number of subjects were 1.457 workes. To predict the oral health of workers, we used the measurement of dental impact on daily living. Using methodology was self-questionnaire which included the perceived oral health status, and performance, appearance, pain, eating restriction and discomfort. The associations between the performance factors and perceived oral health status, pain, eating restriction and discomfort, the results were as follows: 1. There were consisted of 48.3% of female. 63.1% of smokers. 92.3% of lower than 30years and 99.4% of graduating high school. 2. Measurement of daily impact on daily living were classified into the following 6 categories. Facter analysis for validity was carried out for corroboration. 3. Association between performance factors and other factors(pain, eating restriction and discomfort) were evaluated by multiple regression was significant(44%, p<0.01).

Comparing Physiological Changes in Breathing Conditions during Cognitive Tasks (인지부하 환경에서 호흡방식이 생체신호의 변화에 미치는 영향)

  • With external air pollution forcing many people indoors, new methods of facilitating healthier indoor life are necessary. This study, therefore, investigates the effects of indoor oxygen concentration and respiration methods on biosignals and cognitive ability. The study included twenty healthy subjects who inhaled air through a mask from a gas delivery system. All subjects were asked to perform three types of breathing (nasal, oral, and oral breathing with high oxygenation) and respond to cognitive stimuli (rest close eye, rest open eye, 1-back and 2-back working memory tasks). The changes in cognitive load according to respiration were analyzed by measuring response time, accuracy, and biosignals to stimuli. The result showed that, in all three respirations, heart rate significantly increased with the increase in cognitive load. Also, in oral respiration, the airway respiration rate significantly increased according to the increase in cognitive load. The change appeared to compensate for insufficient oxygen supply in oral respiration during cognitive activity. Conversely, there was no significant change in airway respiration rate during oral respiration with a high concentration oxygen supply as in nasal respiration. This result suggests that a high concentration oxygen supply might play a role in compensating for insufficient oxygen concentration or inefficient oxygen inhalation, such as oral respiration. Based on the results of this study, a follow-up study is necessary to determine the impact of changes in the autonomic nervous system, such as stress and emotions, to find out more precise and comprehensive effects of oxygen concentration and breathing type.

The Change of Oral Function Before and After Practicing Program for Oral Function Improving (구강기능향상을 위한 프로그램시행 전·후 구강기능의 변화)

  • The purpose of this study was to examine the effect of a program geared toward improving elderly people's oral function. After a program was provided to the selected elderly people free of charge for three months, they were asked to rate their own oral function to see whether they underwent any changes after their participation in the program, and their oral function was tested to obtain objective data. The collected data were analyzed by the statistical package SPSS WIN 18.0. The findings of the study were as follows: As for changes in their self-rated indicators of oral function, they faced less troubles in most of the oral function items after they participated in the program, and there were statistically significant differences in some of the items. As a result of making an objective evaluation of their oral function, they underwent a little change in salivary flow rate from 1.19 to 1.30, though the change was not statistically significant. In terms of opening, they showed a statistically significant rise of scores from 4.22 to 4.53, and they also showed a statistically significant rise of scores in pronunciation from 30.52 to 38.88. Regarding satisfaction with the program, they gave 4.48 to the program, which implied that they were greatly satisfied with the program. The abovementioned findings suggest that oral health experts and program providers should try to encourage elderly people to keep on taking oral health programs with interest. Currently, oral exercise programs are conducted in some public health centers and in the field of clinical dentistry, and it's required to offer more oral exercise programs as community exercise programs for the elderly.

Oral Health-Related Quality of Life of the Elderly under Visiting Health Care (방문건강관리사업 대상 노인의 구강건강 관련 삶의 질과 관련 요인)

  • The aim of this study was to investigate the relationship between the oral health status of elderly under visiting health care and their oral health-related quality of life. The subjects were 300 elderly under visiting health care program over 65 years old in Asan city and they were interviewed. The result showed that average Geriatric Oral Health Assessment Index (GOHAI) score was 37.36 and the lowest scored question was 'never satisfied with the oral condition'. In GOHAI related to oral health behaviors, unmet dental treatment experience was statistically significant (p<0.001). In overall GOHAI, number of remaining teeth (p<0.001) and the age had statistical significance and men had higher score in pain and discomfort (p<0.05), under age of 74 limited function factors score was high (p<0.01). The number of remaining teeth had statistical significances in most factors except pain and discomfort (p<0.001). The factors affecting GOHAI were age (p<0.05), number of remaining teeth (p<0.001) and perceived health state (p<0.001). In order to improve oral health-related quality of life of elderly receiving visiting oral health care services, it is necessary to increase denture support by securing budgets and developing oral health promotion programs at the local government level.

Perceived oral health awareness in dementia and dementia-suspected depending on KMME (일부 치매 및 치매의심환자들의 인지기능에 따른 구강보건인식 조사)

  • Objectives: The purpose of this study is to investigate cognitive function, performance of activities of daily living, and recognition on oral health with the cognitive function testto dementia or dementia-suspected patients in the outpatients. Methods: The subjects were 94 dementia or dementia-suspected patients visiting C University hospital for the dementia test. Study instruments included Korea Mini-Mental State Examination KMMS, The Bayer-Activities of Daily Living Scale; B-ADL, Seoul-Instrumental Activities of Daily Living; S-IADL, Global Deterioration Scale; GDS, Korean Dementia Screening Questionnaire; KDSQ, and underlying diseases. Results: Dementia or dementia-suspected patients were 42 by KMMSE test, 25 patients had impaired functioning of daily living by B-ADL test, 27 patients showed the presence of depression by GDS test, and 45 patients showed impaired functioning of daily living. There was a statistically significant difference in the subjective recognition on oral health conditions. There was a statistically significant difference in the subjective recognition on oral health conditions by ADL. There was a positive correlation between the cognitive function and ADL performance. Higher cognitive function is proportional to ADL performance. Conclusions: The cognitive function was closely associated with ADL and subjective oral health conditions.

A Study on the Oral Health and Oral Health Related Quality of Life of Stroke Patients in a Rehabilitation Ward: A Pilot Study (재활병동에 입원한 뇌졸중 환자의 구강건강 및 구강건강관련 삶의 질에 관한 연구: 예비연구)

  • The aim of this study was to examine the geriatric status, oral health, and oral health related quality of life of stroke patients hospitalized at a rehabilitatio ward and assess the relationships among these factors. A total of 78 stroke patients participated in this cross-sectional study. The following oral examinations were performed by a dentist and dental hygienist: Decayed, Missing, Filled Teeth, gingival, debris, calculus, and community periodontal index (CPI). The patients' sociodemographic and geriatric status were evaluated using the Barthel index and a mini-mental state examine (MMSE) according to their medical record and a questionnaire survey. Oral health related quality of life was assessed using the Oral Health Impact Profile (OHIP)-14 questionnaire. Except for three participants, most stroke patients had a gingival index of 2 or more, which means probing results in bleeding gums. Additionally, 53.8% of participants had a CPI code of 3 or 4, which indicates a probing depth of 4 mm or more. CPI index was significantly related to debris, calculus, and gingival index. The OHIP-14 score significantly related to the Barthel index and MMSE (p<0.05). The oral health of stroke patients in the rehabilitation ward was relatively poor and related to poor oral hygiene. Oral health related quality of life was related not to oral health but to geriatric status significantly.

Health Status and Use of Health Care Services of the Elderly Utilizing Senior citizen Centers (경로당 노인의 건강상태와 건강관리서비스 이용 관련요인 분석)

  • For this study a sample of 205 people, 66 males and 139 females, over 65 years of age, residing in C-gu of S-si and utilizing senior centers, were selected, The objective of the study was to provide basic data for health promotion program development provided by health centers. A questionnaire was used to collect date on general characteristics, health status, social health status and utilization rate for health services. The instruments used in this study were the Lawton scale, to measure daily routine function, the MMSE-K developed by Folstein and modified to fit the Korea situation, for mental health status, and the CES-Dtool developed by Radloff, for emotional health status. the SPSS Window program was used to calculate percentages. Tests of significance were done using t-test and ANOVA. Multiple regression analysis was used to identify variables influencing the use of health services. The results are as follows : Of those utilizing senior citizen centers, 40.9% of males and 17.3% of the female thought they were healthy. The average score for IADL was 7.4. The daily routine of female respondents consisted of buying household articles and drugs, and other IADLs such as riding the bus or subway alone. These resulted in a higher score compared to males. For emotional health, 7.6% of the males reported depression compared to 21.6% of the females. For mental health, 48.5% of the males and 28.8% of the females were found to be in the group suspicious for dementia. On social health, 57.6% of the males and 62.6% of the females reported no intimate human relations. Of those older people who had close human relations, 52.5% of the males indicated a friend as the closest person and 53.8% of the females, their children. On use of health services, there was a significantly higher need for mobile medical care services treatment for those with lower education levels and status of window/widower. There was a significantly higher need for health exmination services for those with lower levels of exercise, greater satisfaction with sleep, higher levels of oral health care, and higher social contacts. In conclusion, there is a need to provide varied programs for the promotion of health, along with parallel resolution of social, psychological and economic issues. It is recommended that health services for elderly people provided by the health centers be implemented with full recognition of these characteristics and differences.

Comprehensive Geriatric Assessment for Community Living Elderly in a Rural Area (일부 농촌지역 거주 노인들에 대한 포괄적 노인평가)

  • The aim of this study was to analyse and conduct the comprehensive geriatric assessment for the elderly in rural area. The subjects were 388 older people aged 65 years or older living in the community. Data for comprehensive assessment such as physical, mental, functional, social and environmental conditions were collected from January to February, 2001 through a person-to-person interview. Of the total 388 olders, 169(43.6%) were men and 219(56.4%) were women. Mean ages of men and women were $73.5{\pm}6.4$ and $74.0{\pm}6.2$ years respectively. Three common diseases of the elderly were arthralgia(51.6%), chronic back pain(33.2%) and hypertension(18.6%), and higher in women than in men. Impairment rate of vision, hearing and bowel or bladder control was 59.0%, 20.1%, and 28.4% respectively. But that of lover extremities 3.4%. In terms of cognitive function, short term memory loss was found in 33.7% of males and 44.7% of females. The percentage of fully independent in the six ADL items was 72.2% in men and 58.9% in women. In the social supportive system, 49.5% of the elderly were living with spouse, and 22.9% living alone, 26.3% having care giver. These results will provide basic data for the development of community-based health program, which gives appropriate health service for the elderly living in the community.

  • Objective : Autistic Spectrum Disorders(ASD) is a developmental disorder characterized by atypical sensory adaptation, communication problem, stereotyped behavior, and feeding disorders. The reasons for ASD feeding disorders are oral sensory motor, cognitive, behavioral, and social problems. Major symptoms include picky eating, selective eating, food refusal, food neophobia, limited food variety, and food aversion. ASD feeding disorders could be accompanied by various problems such as health and nutrition intake problems, feeding development, eating-related sociability, and family and caregiver stress. Feeding problems and disorders in ASD can present from birth. However, ASD is diagnosed by the age of 3, and there might be an appropriate treatment gap. Usually, symptoms of feeding disorders tend to decrease with age. However, the symptoms often remain, so early evaluation, intervention, and periodic checking are necessary. In this study, the general information about the feeding disorder characteristics of ASD, influencing factors, and intervention were described through a literature review. Conclusion : Sensory-based therapy and behavior-based therapies are generally used for feeding disorders in ASD. Sensory-based therapy is effective for food sensitivity and behavior-based therapy for food selection. As the symptoms of feeding disorders in ASD are diverse, a comprehensive approach includes play and participation, oral motor exercise, diet, and daily life. However, appropriate evaluation, intervention protocol, and guidelines for the treatment of feeding disorders in ASD are limited. Therefore, a complex approach based on a more systematic understanding is needed. Feeding rehabilitation specialists, such as occupational therapists, should provide appropriate evaluation and intervention.
