일산화탄소 중독시 식초산이 혈액 반응에 미치는 영향 (The Hemato-Chemical Effect of Acetic Acid Treatment on Carbon Monooxide Intoxication)

  • CO-intoxication is a serious problem in public health since the coal briquette has been used as one of fuels from 1950's. It has been discussed that the treatment with acetic acid vapor may be effective for CO-intoxication. This study was undertaken to investigate the action of acetic acid therapy, comparing with the spontaneous air treatment The acetic acid vapor was introduced to the blood combined with CO (in vivo and in vitro). The dissociation of COHb, the production of COHb, the levels of Hb and adrenaline and nor-adrenaline were measured. The effect of acetic acid vapor on dissociation of COHb was about 7-9% more effective than the spontaneous air treatment. The acetic acid vapor treatment for the dissociation of COHb was similar effect to the spontaneous air treatment. In an experiment of the combining CO gas with blood, the acetic acid vapor treatment was less effective in the production of COHb than that of spontaneous air treatment. Treatment with the acetic acid vapor to rabbit intoxicated with CO gas induced a little amount of Hb in blood comparing with the spontaneous air treatment. But, it is not a significant increment statistically. By the acetic acid vapor treatment after CO gas intoxication the adrenaline was increased and noradrenaline was decreased. With these results, it is assumed that the effect of acetic acid therapy on CO-gas intoxication would be caused by inductions of Hb and adrenaline and to be reduction of nor-adrenaline.

급성 일산화탄소 중독 후 해마에서 Parvalbumin 양성 중간뉴론의 변화에 대한 N-acetylcystein의 효과 (Effects of N-acetylcystein on changes in parvalbumin-positive interneurons in the hippocampus after carbon monoxide poisoning)

  • Purpose: The purpose of this study was to investigate effect of N-acetylcysteine (NAC) on the injury of putative parvalbumin positive interneurons defined by molecular marker and hippocampal long-term potentiation (LTP), a marker of neural plasticity following acute carbon monoxide (CO) poisoning. Methods: Adult Sprague-Dawley rats were exposed to 1100 ppm CO for 40 minutes followed by 3000 ppm CO for 20 minutes. Animals received daily intraperitoneal injection of NAC (150 mg/kg) for 5 days after CO exposure. Changes in learning and spatial memory were evaluated by Y-maze test 5 days after the poisoning. In vivo LTP in hippocampal CA1 area was evaluated by using extracellular electrophysiological technique. Immunohistochemical staining were adopted to observe expressional damages of parvalbumin (PV) immunoreactive interneurons in the hippocampus following the poisoning. Results: Acute CO intoxication resulted in no changes in memory performance at Y-maze test but a significant reduction of LTP in the in hippocampal CA1 area. There was also a significant reduction of PV (+) interneurons in the hippocampal CA1 area 5 days after CO poisoning. Daily treatment of NAC significantly improved hippocampal LTP impairment and reduced immunoreactivity for PV in the hippocampus following the acute CO poisoning. Conclusion: The results of this study suggest that reduction of hippocampal LTP and PV (+) interneurons in the hippocampus is sensitive indicator for brain injury and daily NAC injections can be the alternative therapeutics for the injury induced by acute CO poisoning.

급성 일산화탄소 중독 환자들에서 흡연과 지연성 신경정신과학적 후유증 발생의 상관관계 (Association between Smoking and Delayed Neuropsychological Sequelae in Acute Carbon Monoxide Poisoning)

  • Purpose: This study examined the association between smoking and delayed neuropsychological sequelae (DNS) in acute carbon monoxide (CO) poisoning. Methods: Patients admitted to the medical center emergency department from March 2016 to March 2017 because of CO poisoning were examined retrospectively. The patients were divided into two groups: DNS and Non-DNS group. Multiple factors were analyzed to explain DNS, which was assessed by motor disturbances, cognitive impairment, dysphagia, Parkinson-like syndromes, epilepsy, and emotional lability in CO poisoning. Results: A total of 120 patients were included. The factors related to DNS were smoking (pack-years) (p=0.002) and initial carbon monoxide-hemoglobin level (p=0.015). On the other hand, after multivariate logistic regression analysis, smoking (Odds ratio 1.07; 95% CI, 1.02-1.13; p=0.004) was the only factor associated with DNS. Conclusion: Smoking is a very reliable factor for predicting the occurrence of DNS. A history of smoking in patients who suffer from CO intoxication is important. If a patient smokes, treatment should be started actively and as soon as possible.

코로나바이러스감염증-19 대유행 이후 응급실로 내원한 급성 일산화탄소 중독 환자의 특성 변화 (Changes in the characteristics of acute carbon monoxide poisoning patients who visited the emergency department during the COVID-19 pandemic)

  • Purpose: This study investigated the differences between patients with acute carbon monoxide (CO) poisoning who visited the emergency department (ED) before and during the coronavirus disease 2019 (COVID-19) pandemic. Methods: This was a single-center retrospective observational study. Patients with CO poisoning who visited the ED during the period from February 2020 to January 2023 were classified as the COVID-19 pandemic group, and those from February 2019 to January 2020 were classified as the non-pandemic group. Patients' medical records were reviewed, their demographic and clinical characteristics were compared, and the length of stay in ED was checked. The time from admission to the ED to the start of hyperbaric oxygenation (HBO) was defined as the door-to-HBO time, and this parameter was compared between both groups. Results: In total, 672 patients were included in this study. The proportion of intentional poisoning was significantly higher in the COVID-19 pandemic group than in the non-pandemic group (p=0.028). The proportion of intentional poisoning significantly increased in the 20- to 29-year-old age group during the COVID-19 pandemic (p<0.001). In addition, it took longer to initiate HBO in the COVID-19 pandemic group than in the non-pandemic group (p=0.001). Conclusion: These findings suggest that pandemics of infectious diseases, such as COVID-19, increase the proportion of intentional CO poisoning, and it may take longer to initiate HBO after visiting the ED. Efforts will be needed to decrease intentional CO poisoning and length of stay in ED.

급성일산화탄소 중독 후 발생한 중독정신병환자 1례에 대한 임상적 고찰 (A Psychotherapy of Oriental Medicine -Focus on Psychoanalysis-)

  • Objectives : In order to make a new psychotherapeutic model in the theory of oriental medicine by means of psychoanalysis method. Methods : The research was done by comparing the contents of the Tong-Ui Po-Kam, techniques of Yang Saeng(養生術), with the psychoanalysis Results : 1. There's no difference in the mind state which a doctor has to have between the oriental doctor and the occidental doctor. You should make effort not to project yourself on a patient. A conception of 'doctor' in oriental medicine is one who trains one's mind rather than just healer. 2. Heart(心) in oriental medical conception is revealed by circulation structure of Jeong(精), Gi(氣), and Sin(神). A healthy mind comes from harmony and smooth transport of Jeong(精), Gi(氣), and Sin(神). 3. The conception similar to unconsciousness doesn't exist in oriental medicine, but the contents about the attitudes or the methods of training one’s mind correspond to unconsciousness. 4. The psychotherapy of oriental medicine emphasized mutual dynamics between emotions at present, and presented a way of treatment by describing emotions from standpoint of 'Gi(氣)'. 5. The special feature of psychological treatment in oriental medicine is that treatment is done by understanding emotions of present time as Gi (氣), and using dynamics of Gi (氣)(; 相生相剋). Also the balanced state can be kept even after treatment and prevention can be done by observing one's own unconsciousness(; training one's mind helps circulation of Jeong(精), Gi(氣), and Sin(神)) I think that we need a new realistic therapy model to develop these psychotherapy methods in oriental neuropsychiatry, through studying emotions in the theory of oriental medicine, especially Korean's emotions and spiritual training.

일산화탄소중독견(一酸化炭素中毒犬) 체내(體內)에서의 일산화탄소처리능(一酸化炭素處理能)에 관(關)하여 (Disposal of CO in CO-Poisoning Dogs)

  • The Present study attempted to analyze the fate of CO diffused into the circulating blood through the alveoli. Dogs were induced to CO poisoning by rebreathing CO gas mixture contained in Krog's spirometer, by closed circuit method, for 60 minutes. The spirometer was filled initially with 282 ml of CO and 20 liters of air and oxygen, so the composition of gases were arranged as 1.4% in CO and 50% in $O_2$ at the begining of the rebreathing. Oxygen was added corresponding to the utilization of $O_2$ by the animal in proceeding of the experiment. At 60th minutes of CO rebreathing, the concentration of CO in arterial blood and in mixed venous blood were analysed and compared with each other after the CO contents were corrected with the hematocrit measured in the arterial and mixed venous blood. The distribution of CO gas to other tissues was estimated by the analysis of CO diffused into the cystic bile and into the peritoneal gas pocket which was formed by injection of 300 ml air into the peritoneal cavity prior to the CO gas rebreathing. The blood volume was measured by dilution method using $^{51}Chromium$ tagged red cells. CO amount vanished in the animal body was calculated by subtraction of total CO content in blood stream and the CO remained in closed circuit breathing system from the CO amount given to the breathing system at the begining of the experiment. Results obtained are summarized as follows: 1. The content of CO corrected by the hematocrit value was slightly less in mixed venous blood than in arterial blood. The amount of CO diffused into the cystic bile and into the peritoneal cavity was averaged to 0.1% and 0.4% of the CO amount in 100 ml of blood, respectively. 2. For 60 minutes of CO rebreathing, CO-hemoglobin saturation reached about 77% at the 60th minutes, CO amount vanished in the experimental animal averaged 36.1 ml/dog/hr., or 21% of the total CO volume in the blood stream. The average vanishing rate of CO during 60 minutes of CO rebreathing per kg of body weight was 2.71 ml/hr. Production of CO measured in ten dogs under hypoxic condition averaged 0.023 ml/kg/hr. The major part of the CO vanished in the dogs seemed to be oxidized to $CO_2$ by various tissues of the animal. The conclusion might be delivered as such oxidation of CO to $CO_2$ by animal tissues can play a role in part of the process of recovery and protection of animal from CO-poisoning.

가스보일러 일산화탄소 누출사고 경감에 관한 연구 (A Study on Prevention of Accidents of Carbon Monoxide Leak from Gas Boilers)

  • 본 연구는 가스보일러 CO 누출사고 경감방안에 관한 연구로서, CO 경보기 및 콘덴싱 보일러 의무화 설치 및 지원금 방법을 제시하였다. CO 중독을 예방하기 위해 CO 경보기를 의무 설치하여야 하며 현재 국산 CO 경보기의 가격이 약 4만원이며, 기초수급생활자 및 일반가정 400만 가구 대상으로 3 년 간 1,000억원의 금액이 소요됨을 제시하였다. 기초생활수급자 20만원, 일반가정 10만원, 400만 가구를 지원해주면, 3 년 간 5,000억원의 금액이 소요되며, 재정마련은 연간 100만대 판매되는 보일러를 콘덴싱 보일러로 의무화 시 년 간 약 2,000억원의 에너지 절감이 되며, 콘덴싱 보일러의 수명이 10년으로 했을 때 총 2조원의 에너지 절감비용이 생긴다. 그 중 6,000억원의 비용은 CO 경보기 및 콘덴싱 보일러 설치 지원비로 사용하며, 10년간 국내가스사고 CO 누출사고 50% 이상의 사고를 경감할 수 있을 것으로 제시하였다.

산림청 지원사업에 따라 보급된 산업용 목재펠릿보일러에서 목재펠릿 연소 시 배출되는 일산화탄소와 질소산화물의 배출 특성 및 배출계수 분석 (Analysis of Emission Characteristics and Emission Factors of Carbon Monoxide and Nitrogen Oxide Emitted from Wood Pellet Combustion in Industrial Wood Pellet Boilers Supplied According to the Subsidy Program of Korea Forest Service)

  • 산림청은 보조금 지원사업을 통해 2011년부터 2015년까지 76대의 산업용 목재펠릿보일러를 보급하였다. 보일러의 연소 시 발생하는 일산화탄소(CO) 및 질소산화물($NO_x$)는 각각 급성 중독 시 사망에까지 이르게 하는 물질이기 때문에 배출량을 줄이는 것이 매우 중요하다. 따라서 이들 보일러 중 열풍기와 초기에 보급된 일부 보일러를 제외한 63대의 보일러에서 배출된 CO 및 $NO_x$ 계측값을 분석하였다. 또한 측정된 배출가스 농도(배기가스 $O_2$ 농도 12% 기준)로부터 배출계수를 산출하였다. 산업용 목재펠릿보일러에서 배출된 CO의 평균값은 49 ppm이었으며, 해를 거듭함에 따라 CO의 농도가 줄어들고 있음이 확인되었다. 이때 CO의 배출계수는 0.73 g/kg였다. 산업용 목재펠릿보일러에서 배출된 $NO_x$의 평균값은 67 ppm였으며, $NO_x$의 배출계수는 1.63 g/kg이었다. CO와는 달리 설치년도에 따라 감소하는 경향은 나타나지 않았다. CO 및 $NO_x$ 계측값은 모두 환경부의 허용기준을 만족하였다. 이러한 $NO_x$ 배출계수를 저$NO_x$ 인증된 연소기에서 생성되는 $NO_x$ 배출계수와 비교하였다. 산업용 목재펠릿보일러의 $NO_x$ 배출계수는 저$NO_x$ 인증된 LNG 연소기의 $NO_x$ 배출계수에 비해 약 1.9배, 석탄 연소에 비해 약 0.92배였다.

지연성 일산화탄소중독후유증 환자에서 $^{99m}Tc-HMPAO$를 이용한 국소 뇌혈류량의 SPECT소견 (Studies on the Regional Cerebral Blood Flow in Delayed Carbon Monoxide sequelae using $^{99m}Tc-HMPAO$)

  • 8 patients of delayed CO sequelae were evaluated using Brain CT and $^{99m}Tc-HMPAO$ SPECT. The results were as follows; 1) CT findings of delayed CO sequleae were bilateral low density lesion in globus pallidus (l pt.), diffuse low density in white matter with bilateral low density in white matter (l pt.), diffuse low density in white matter with bilateral low density in globus pallidus (l pt.), diffuse low density in white matter with cortical atrophy (l pt.), bilateral low density in globus pallidus and diffuse low density in white matter with cortical atrophy (l pt.) and normal in 3 pts. 2) $^{99m}Tc-HMPAO$ Brain SPECT findings of delayed CO sequelae were decreased regional cerebral blood flow (rCBF) in frontal (1 among 8 pts.), frontal and basal ganglia (3 among 8 pts.), and diffuse patch decreased rCBF pattern (4 among 8 pts.) 3) $^{99m}Tc-HMPAO$ Brain SPECT study was well correlated with neurologic symptoms and signs in delayed CO sequelae. Our results may suggest that reduced cerebral blood flow contributes to the development of delayed CO sequelae.

인삼 사포닌이 일산화탄소중독 및 노화과정에서 생쥐의 뇌신경세포 분포에 미치는 영향 (Effect of Ginseng Saponins on the Distribution of Brain Nerve Cells in Carbon Monoxide-intoxicated Mice and Aged Mice)

  • The effects of ginseng saponins on the distribution of nerve cells in cerebral cortex of carbon monoxide (CO)-intoxicated mice were studied in the young ($5{\sim}8$ weeks) and aged ($43{\sim}52$ weeks) mice. Mice were exposed to 5000 ppm of CO for 40 minutes (72% HbCO). After that, nerve cells in motor(area 4), somatosensory(area 3) and visual(area 17) area of cerebral cortex was observed. In young mice, the number of nerve cells in each area was significantly decreased on 1st, 7th and 14th day after CO intoxication. In aged mice, that was also decreased after CO intoxication. Especially the number of the nerve cells in motor and somatosensory area was significantly decreased on 1st and 7th day, while that in visual area was decreased only on 1st day. The number of nerve cells in young mice pretreated with ginseng saponins were significantly decreased less on 7th and 14th day than that of untreated mice. The number of nerve cells in each area of normal aged mice was larger than that of normal young mice. The results suggest that CO exposure causes local degeneration or disturbance of nerve cells and delayed neurologic sequelae, while ginseng saponins might play a role of protective action on the nerve cells which were damaged by CO.
