고압산소치료를 받은 급성 일산화탄소 중독 환자들에서 발생한 지연성 신경정신과학적 후유증 발생과 관계된 인자 (Factors Associated with Delayed Neuropsychological Sequelae in Acute Carbon Monoxide Poisoning Patients Treated by Hyperbaric Oxygen)

  • Purpose: Delayed neuropsychological sequelae (DNS) commonly occurs after recovery from acute carbon monoxide poisoning. The aim of this article is to identify the factors associated with DNS development. Methods: We retrospectively evaluated patients, admitted to the medical center emergency department from June 2005 to March 2011, who were suffering from acute carbon monoxide (CO) poisoning. We categorized the patients into two groups - those with DNS, and those without DNS. Multiple regression analysis was performed to identify the factors related to manifestation of DNS. Results: Of the total one hundred fifty seven patients (157) recruited for the study, twenty two (22) developed DNS. Longer CO exposure times and lower GCS scores were positively associated with development of DNS symptoms. Conclusion: Our study identified two potential factors which are predictive of DNS development in CO intoxication, however, more studies are needed. Adequate follow-up after hospital discharge to monitor for and accurately identify manifestation of DNS, is also important.

일산화탄소 중독이 뇌내 아미노산 신경전달물질 함량변화에 미치는 영향 (The Effect of Carbon Monoxide Intoxication on the Changes in Contents of Amino Acid Neurotransmitter of Rat Brain)

  • To study influence of carbonmonoxide (CO) poisoning on the content of amino acid neurotransmitter in brain, male rat was exposed to CO 5000 ppm for 30 minutes (60-75% HbCO). Aspartic acid and glutamic acid level in the cerebral cortex and aspartic acid level in the striatum were significantly decreased. GABA level in the cerebral cortex was significantly increased after the 30 and 60 minutes of CO intoxication. Taurine level in both the cerebral cortex and the striatum was increased although nonsignificant. Consequently, the CO-induced hypoxia brain showed lower level of excitatory neurotransmitter, aspartic acid and glutamic acid and higher level of inhibitory neurotransmitter, GABA and taurine. These results suggest that the change in content of amino acid neurotransmitter in the rat brain may be concerned with several CO poisoning symptoms.

급성 일산화탄소 중독 환자에서 발생한 양하지 말초 운동신경병증 1례 (Motor Peripheral Neuropathy Involved Bilateral Lower Extremities Following Acute Carbon Monoxide Poisoning: A Case Report)

  • Carbon monoxide (CO) intoxication is a leading cause of severe neuropsychological impairments. Peripheral nerve injury has rarely been reported. Following are brief statements describing the motor peripheral neuropathy involved bilateral lower extremities of a patient who recovered following acute carbon monoxide poisoning. After inhalation of smoke from a fire, a 60-year-old woman experienced bilateral leg weakness without edema or injury. Neurological examination showed diplegia and deep tendon areflexia in lower limbs. There was no sensory deficit in lower extremities, and no cognitive disturbances were detected. Creatine kinase was normal. Electroneuromyogram patterns were compatible with the diagnosis of bilateral axonal injury. Clinical course after normobaric oxygen and rehabilitation therapy was marked by complete recovery of neurological disorders. Peripheral neuropathy is an unusual complication of CO intoxication. Motor peripheral neuropathy involvement of bilateral lower extremities is exceptional. Various mechanisms have been implicated, including nerve compression secondary to rhabdomyolysis, nerve ischemia due to hypoxia, and direct nerve toxicity of carbon monoxide. Prognosis is commonly excellent without sequelae. Emergency physicians should understand the possible-neurologic presentations of CO intoxication and make a proper decision regarding treatment.

일산화탄소 중독 지연성 뇌손상 소양인 환자 치험 1례 (Case Study of Soyangin Patient with Delayed Encephalopathy after Carbon Monoxide Intoxication)

  • Objective This case report is to show the effectiveness of Sasang Constitutional Medicine treatment on the patient with delayed encephalopathy after acute carbon monoxide intoxication. Methods A 40-year-old female patient diagnosed with delayed encephalopathy after carbon monoxide poisoning 2 weeks ago before visiting our hospital. The patient suffered gait disturbance and cognitive disorder. She was identified as Soyangin and treated with Sasang constitutional Herbal Medicine(Yangkyuksanwha-tang and Hyungbangjihwang-tang). Improvement of symptoms was evaluated through Korean Version of Mini Mental Status Examination score(MMSE-K), Modified Barthel Index(MBI), Manual Muscle Testing(MMT), Range of Motion(ROM) and Hand Function Test. Results After 7 weeks of treatment, total score of MMSE-K increased from 0 to 26, the patient could walk by herself and had no trouble carrying out her daily life. Conclusions This case showed the effectiveness of Sasang constitutional medicine treatment for delayed encephalopathy after acute carbon monoxide intoxication.

인삼 사포닌이 일산화탄소중독 및 노화과정에서 흰쥐의 신경전달물질 함량 변화에 미치는 영향 (Effect of Ginseng Saponins on the Amount of Catecholamine Neurotransmitters in Carbon Monoxide-intoxicated Rats and Aged Rats)

  • After rats were exposed to 5,000 ppm carbon monoxide for 30 minutes, the amounts of catecholamine neurotransmitters in stratum were measured using high performance liquid chromatograph equipped with electrochemical detector. The concentration of dopamine in stratum was significantly decreased after carbon monoxide intoxification, but those of dihydroxyphenylacetic acid, norepinephrine, and epinephrine was not changed. However the pretreatments of Ginseng total saponin and panaxatriol saponin increased the concentrations of dopamine and its acidic metabolites (DOPAC and HVA). Ginseng total saponin also increased the concentrations of norepinephrine and epinephrine. Similar results were obtained from aged rats.

  • Delayed anoxic encephalopathy after carbon monoxide (CO) poisoning is characterized by neurological deterioration that occurs after recovery from acute CO intoxication. There has been no established therapy. We report a patient recovered from acute CO intoxication developed various neurological symptoms. After the administration of high dose prednisolone and anticholinesterase inhibitor, the therapeutic effect was remarkable and confirmed by quantitative analysis of diffusion-tensor imaging (DTI). DTI could be used to evaluate the therapeutic effect for delayed anoxic encephalopathy after CO poisoning.

건축물내 일산화탄소 경보기 설치기준에 관한 연구 (A Study on the Standard for Installation of Carbon Monoxide Detector in a Building)

  • 최근 5년간 국내 가스사고 중 CO중독사고는 사망 36명, 부상 79명이 발생하였으며, 전체 가스 사고에 비해 인명피해율이 8배나 높게 나타나고 있다. 이러한 CO중독사고의 대부분이 노후 가스보일러 및 다중 가스 사용시설의 배기통 불량에 의한 가스누출사고로 조사되고 있다. 본 연구에서는 실험을 통해 건축물 내 보일러실의 배기통 불량에 의한 CO가스의 누출확산 메커니즘을 해석하였다. 배기통에서 누출된 CO 가스는 건축물 내부의 천정부근에서 가장 높은 농도분포를 보인다. 또한 CO경보기 설치 실험을 통해서 천정부근의 CO경보기가 가장먼저 작동하고, $30{\sim}40$분 경과한 후에 바닥 및 중간에 설치된 경보기가 작동하는 것을 확인하였다. 본 연구를 통해 CO경보기의 적정 설치위치를 확인하였으며, 이 결과를 토대로 건출물내 CO경보기 설치위치를 제안하였다.

과산화수소 관장이 급성 일산화탄소중독의 회복에 미치는 영향 (Effect of Hydrogen Peroxide Enema on Recovery of Carbon Monoxide Poisoning)

  • 과산화수소$(H_2O_2)$가 급성 일산화탄소(CO)중독의 회복에 미치는 영향을 알아보기 위하여 가토를 1% Corktm에 30분간 노출시킨후 자연회복군, 100%산소 흡입군 및 $H_2O_2$관장군(10ml/kg의 0.5% $H_2O_2$용액을 2ml내외의 사람혈액과 함께 관장)으로 나누어, 회복기 15,30,60 및 90분에 동맥혈의 $pH,\;PCO_2,\;CO_2$ 및 HbCO 포화도를 측정하여 다음과 같은 결론을 얻었다. pH는 급성 CO중독시 3개군 모두에서 감소하였고, 회복기에는 서서히 증가하여 자연회복군과 100%산소 흡입군은 회복기 90분에 거의 회복되나, $H_2O_2$관장군에서는 pH의 회복이 다른군보다 늦었다. $PaCO_2$는 급성 Co중독시 3개군 모두에서 감소하였고, 회복기에는 서서히 증가하였으나, 자연회복군의 $PaCO_2$는 회복기 90분에 거의 회복하는데 반하여 100%산소흡입군과 $H_2O_2$관장군의 $PaCO_2$는 회복이 늦었고 회복기 90분에도 완전히 회복되지 못하였다. $PaO_2$는 급성 Co중독시 약간 감소하였다가 회복기에는 회복기 15분부터 급격히 증가하였고 회복기 90분까지 대조치보다 높은 $PaO_2$를 유지하였다. 회복기동안 $H_2O_2$관장군의 $PaO_2$$102{\sim}107mmhg$로 자연회복군보다 약 10 mmhg 높은 수준을 보였다. HbCO 포화도는 급성 CO중독시 $54{\sim}72%$까지 증가하였다. 회복기에는 $H_2O_2$관장군의 HbCO 포화도의 회복이 자연회복군이나 100%산소 흡입군보다 빨랐으며, 100%산소 흡입군은 회복기 30분에서 60분사이에 자연회복군보다 더 빠른 회복을 보였다. 이상의 결과에서 0.5% $H_2O_2$관장은 CO중독시 혈액의 산소분압을 어느정도 증가시킬 뿐 아니라 혈색소와 결합된 CO의 해리를 촉진시켜, 단독요법 또는 산소요법과 병행하여 사용할 때 급성CO중독에 효과적인 치료법이 될 수 있을 것으로 사료된다.

  • The authors collected epidemiological and clinical informations on 1,307 CO patients from the questionairefilled by attendants of patients and from the clinical records kept in the hospital. All patients were treated by hyperbaric oxygenation at the Hyperbaric Chamber Unit of Seoul National University Hospital during 5 years, from January, 1969 to December, 1973. The following findings were obtained. 1. Female showed higher incidence rate than male. The highest incidence rate was observed in the age group of 15-29 years in both sex. 2. The most important variable influencing the recovery of the patients was the starting time of treatment. It was evident that earlier the starting time, shorter the recovery time. 3. Duration of admission was within 5 days in 57.1 percent of the admitted cases and 76.1 percent of them was discharged within 8 days. 4. As the complications, pulmonary edema, bronchopneumonia and trophic changes were observed. Also the neurological disorders were found. 5. The fatality rate followed by the hyperbaric oxygenation was 1.07 percent.

일산화탄소중독(一酸化炭素中毒)이 가토(家兎) 폐포표면활성물질(肺胞表面活性物質)에 미치는 영향(影響) (Effect of CO Intoxication on the Pulmonary Surfactant in Rabbits)

  • In order to observe a possible effect of CO intoxication on the activities of surfactant, the rabbit war exposed either to 0.1, 0.2 and 0.4% CO gas for two to six hours or 0. 1% CO gas for two hours daily for 1, 3 and 5 days, and the lung extract was prepared. The tension-area diagram of the lung extract was recorded automatically using the modified Langmuir-wilhelmy balance with a synchronized recording system, and the results were compared with control (i. e. : non-CO gas exposed normal rabbits). The results obtained are summarized as follows: 1. The maximal and minimal surface tensions, width of the tension-area diagram at the surface area of 40% in lung extract, and stability index of the normal rabbit lung extracts were $31.6{\pm}3.11\;dynes/cm,\;8.2{\pm}0.56\;dynes/cm,\;21.4{\pm}4.40dynes/cm\;and\;1.12{\pm}0.22$, respectively. 2. The activities of surfactant in the CO gas exposed group did not show any significant change from the control. 3. The above results suggest that CO gas produced no noticeable effect on the surfactant system of the lung.
