Factors Influencing the Turnover Intention of COVID-19 Ward and General Ward Nurses in Public Hospitals (경기도 공공병원의 COVID-19병동 간호사와 일반병동 간호사의 직무 스트레스가 이직의도에 미치는 영향)

  • Purpose: This study aimed to determine the factors influencing the turnover intention of nurses in coronavirus disease (COVID-19) and general wards. Methods: Data were collected through a survey of 340 nurses at seven public hospitals in Gyeonggi Province from October 12 to October 30 in 2020. The data were analyzed using descriptive statistics, t-tests, ANOVA, Pearson correlation coefficient and multiple regression. Results: The job demand stress of nurses in COVID-19 wards was 63.99±8.05 points, which was lower than 65.77±7.35 points of nurses in general wards. The turnover intention of nurses in COVID-19 wards was 3.24±0.74 points, which was higher than the 3.06±0.67 points of those in general wards. The multiple regression analysis indicates that the factors affecting the turnover intention of the two groups (COVID-19 and general wards) were type of wards (β=-.15, p<.001), age (COVID-19 wards: β=-.37, p<.001, General wards: β=-.40, p<.001), job resource stress (COVID-19 wards: β=.52, p<.001, general indicates: β=.60, p<.001), and clinical experience in present wards (COVID-19: β=.13, p=.021). Conclusion: The results indicate that nurses who care for COVID-19 patients have higher turnover intentions than general nurses and suggests that a decrease in the job resource stress is vital for reducing nurse turnover intentions.

Optimal Nursing Workforce and Financial Cost to Provide Comprehensive Nursing Service in the National Health Insurance System (국민건강보험 간호·간병통합서비스의 전면 도입을 위한 간호인력 및 재정비용 추계)

  • This study estimated the optimal nursing workforce and financial costs of providing comprehensive nursing services at hospitals under the national health insurance system. Data on registered nurses, nursing aids, medical institutions, and number of patients were obtained from the Health Insurance Review and Assessment Service. The optimal size of the nursing workforce was calculated using the workload model. A bottom-up approach was used to estimate the annual total financial cost of comprehensive nursing services. The number of registered nurses and nursing aids would need to be increased by 81.75% and 83.23%, respectively, in order to fully apply comprehensive nursing care on a national scale. The additional financial costs for comprehensive nursing services at all hospitals was estimated to be as much as 110.39% of the current cost. For the comprehensive nursing service, nurses with a career and newcomers need to be retained at their hospitals, and the validity of the nurse-patient ratio should be continuously checked. The financial shock to the national health insurance system could be minimized by gradually extending the system to all hospitals.

Influence of Leader-Member Exchange Quality of Head Nurses and Clinical Nurses on Organizational Commitment and Job Satisfaction in Clinical Nurses (수간호사와 일반간호사의 교환관계의 질이 일반간호사의 직무만족과 조직몰입에 미치는 영향)

  • Purpose: The purpose was to identify the influence the quality of head and clinical nurses' LMX (Leader-Member Exchange) on job satisfaction and organizational commitment. Methods: The participants were 42 head nurses and 202 clinical nurses who worked in 7 hospitals with more than 300 beds in I-city. The data were collected from March 10 to April 10, 2013 using a self-report questionnaire. Data analysis was performed using hierarchical regression with the SPSS/WIN 20.0 program. Results: The mean score for head nurses' LMX quality was 3.66 and for clinical nurses, 3.51. Clinical nurses' LMX quality and age had a positive impact on job satisfaction (F=8.00, p<.001). Clinical nurses' LMX quality and marriage (not single) had a positive impact on organizational commitment (F=6.76, p<.001). Conclusion: The LMX quality of head nurse was higher than that of clinical nurses, but did not positively affect clinical nurses' job satisfaction or organizational commitment indicating that the LMX quality of clinical nurses is more important than that of head nurse. Thus head nurses should make efforts to lead their units or teams in a positive and friendly way. This positive recognition will promote greater job satisfaction and organizational commitment of clinical nurses.

Changes in Nurse Staffing Grades in General Wards and Adult and Neonatal Intensive Care Units (의료기관의 일반병동, 성인 중환자실, 신생아 중환자실의 간호등급 변화)

  • Purpose: This study aimed to explore the distributions of nurse staffing grades and to report changes in staffing grades in general wards and adult and neonatal intensive care units(ICUs) by hospital type and location. Methods: Data collected from the Health Insurance Review and Assessment Service were analyzed. Nurse staffing was categorized from grades 1 to 6 or 7 for general wards, 1 to 9 for adult ICUs, and 1 to 4 for neonatal ICUs based on the nurse-to-bed ratio. Results: The staffing grade for the general wards improved during 2008-2016 in 69.8% of the tertiary hospitals, 58.5% of the general hospitals, and 31.7% of the non-general hospitals. The adult ICUs at tertiary hospitals exhibited a greater improvement in staffing grades (48.8%) than did those of general hospitals (44.2%) during 2008-2015. Tertiary hospitals in non-capital regions showed a greater improvement than those in the capital region. The majority of neonatal ICUs (67.1%) had no change in the staffing grade during 2008-2015. Conclusion: Improvements in nurse staffing differed by hospital type and location. Government policies to improve nurse staffing in non-tertiary hospitals and those in non-capital regions are required to reduce variations in nurse staffing.

Revision of Performance Appraisal Tool and Verification of Reliability and Validity (일반간호사 인사고과도구 수정 및 신뢰도, 타당도 검증)

  • Purpose: This study was done to develop performance appraisal guidelines based on the standardized appraisal tool developed in a previous study. Methods: A preliminary investigation was done to draw up a draft guidelines through a literature review. To verify the content validity of the guidelines, 10 nurse managers reviewed the standardized appraisal tool and draft guidelines. Through this process, the performance appraisal guidelines were revised and supplemented. In order to examine the reliability and validity of the guidelines, 13 nurse managers conducted performance appraisal for 178 nurses. Results: As a result, the items of pre-existing tool was reduced from 63 to 30 for usefulness, and guidelines for each items were developed. Conclusion: The revised performance appraisal tool and guidelines are expected to be useful in objectively evaluating nursing performance objectively.

Effect of Role Conflict on Job Satisfaction by Comprehensive Nursing Care Service Ward Nurse: Moderating and Mediating Effect of Resilience (간호·간병통합서비스 병동간호사의 역할갈등이 직무만족에 미치는 영향: 회복탄력성의 조절 및 매개효과)

  • Purpose : This study aimed to identify the moderating and mediating effects of resilience on the relationship between role conflict and job satisfaction among nurses working in a comprehensive nursing care service ward. Methods : Participants were 130 comprehensive nursing care service ward nurses from four hospitals. Data were collected using structured questionnaires and analyzed by t-test, ANOVA, Scheffe test, Pearson correlation coefficients, and hierarchical multiple regression analysis, using SPSS 25.0. The mediating effect of resilience on the relationship between role conflict and job satisfaction was analyzed using multiple regression analysis according to Baron and Kenny's procedure. Results : Statistically significant negative correlations were found between role conflict and resilience (r=-.25, p<.007) and between role conflict and job satisfaction (r=-.38, p<.001). A significant positive correlation was found between resilience and job satisfaction (r=.54, p<.001). A partial mediating effect of resilience was found between role conflict and job satisfaction (Z=-2.53, p=.012) but no moderating effect was found. Conclusion : To improve the job satisfaction of comprehensive nursing care service ward nurses, interventions for alleviating role conflict and improving resilience as well as evaluations of the two variables are necessary.

Triage Nurses' Work Experiences in Emergency Department: A Qualitative Research (응급실 중증도 분류 간호사의 업무 경험: 질적 연구)

  • Purpose: The purpose of the study was to describe work experiences of triage nurses working at emergency departments in Korea. Methods: A qualitative descriptive research method was used. Twenty-one nurses of emergency department participated in the first interview and 16 of them participated in the second interview. Data were collected through in-depth interviews and field notes. Qualitative content analysis was utilized to analyze the data. Results: Forty-eight codes, 12 categories, and six themes emerged from the data analysis: caring on the frontline, burden as an initial evaluator, handling things alone, lack of others' understanding, becoming an expert, and needs for improvement. Participating triage nurses faced many challenges and difficult situations in emergency departments. They felt burden and loneliness as initial evaluator. Also, they felt pride in their work and struggled to become a professional. Conclusion: This study is the first qualitative study that has focused on exploring the experiences of triage nurses in emergency departments in Korea. The results provide description and in-depth understanding of working experiences of triage nurses. The results provide valuable fundamental data for regulation, clinical practice, education, and research for triage nurses in Korean emergency departments.

Perceptions of patient-centered care and patient-centeredness experiences of patients and nurses in comprehensive nursing care units at general hospitals (종합병원 간호간병통합서비스 병동 입원 환자와 간호사의 환자중심간호에 대한 인식과 환자중심성 경험)

  • Purposes: This study aimed to compare the perception between nurses and patients about comprehensive care services and to evaluate patient-centeredness experiences at comprehensive nursing care units. Methodology: We enrolled 267 nurses and 184 patients from comprehensive nursing care units of seven general hospitals. We performed data collection and analysis using structured questionnaires and SPSS/PC 23.0 program, respectively, with frequency, percentage, mean, standard deviation, and ${\chi}^2$. test. Findings: We observed a significant difference in perception about comprehensive nursing care services between nurses and patients(p < .001). While the patient-centeredness experience score was the highest in the nursing service, it was the lowest in patient right assurance. Regarding patients' right assurance, "easy-to-express complaints" and "opportunity to participate in decision making" exhibited the lowest score. Practical Implication: This study suggests that it is imperative to assess the above-mentioned problems comprehensively to enhance patient centeredness at comprehensive nursing care units.

Evaluation of Radiation Exposure to Nurse on Nuclear Medicine Examination by Use Radioisotope (방사성 동위원소를 이용한 핵의학과 검사에서 병동 간호사의 방사선 피폭선량 평가)

  • Purpose Radiation exposure management has been strictly regulated for the radiation workers, but there are only a few studies on potential risk of radiation exposure to non-radiation workers, especially nurses in a general ward. The present study aimed to estimate the exact total exposure of the nurse in a general ward by close contact with the patient undergoing nuclear medicine examinations. Materials and Methods Radiation exposure rate was determined by using thermoluminescent dosimeter (TLD) and optical simulated luminescence (OSL) in 14 nurses in a general ward from October 2015 to June 2016. External radiation rate was measured immediately after injection and examination at skin surface, and 50 cm and 1 m distance from 50 patients (PET/CT 20 pts; Bone scan 20 pts; Myocardial SPECT 10 pts). After measurement, effective half-life, and total radiation exposure expected in nurses were calculated. Then, expected total exposure was compared with total exposures actually measured in nurses by TLD and OSL. Results Mean and maximum amount of radiation exposure of 14 nurses in a general ward were 0.01 and 0.02 mSv, respectively in each measuring period. External radiation rate after injection at skin surface, 0.5 m and 1 m distance from patients was as following; $376.0{\pm}25.2$, $88.1{\pm}8.2$ and $29.0{\pm}5.8{\mu}Sv/hr$, respectively in PET/CT; $206.7{\pm}56.6$, $23.1{\pm}4.4$ and $10.1{\pm}1.4{\mu}Sv/hr$, respectively in bone scan; $22.5{\pm}2.6$, $2.4{\pm}0.7$ and $0.9{\pm}0.2{\mu}Sv/hr$, respectively in myocardial SPECT. After examination, external radiation rate at skin surface, 0.5 m and 1 m distance from patients was decreased as following; $165.3{\pm}22.1$, $38.7{\pm}5.9$ and $12.4{\pm}2.5{\mu}Sv/hr$, respectively in PET/CT; $32.1{\pm}8.7$, $6.2{\pm}1.1$, $2.8{\pm}0.6$, respectively in bone scan; $14.0{\pm}1.2$, $2.1{\pm}0.3$, $0.8{\pm}0.2{\mu}Sv/hr$, respectively in myocardial SPECT. Based upon the results, an effective half-life was calculated, and at 30 minutes after examination the time to reach normal dose limit in 'Nuclear Safety Act' was calculated conservatively without considering a half-life. In oder of distance (at skin surface, 0.5 m and 1 m distance from patients), it was 7.9, 34.1 and 106.8 hr, respectively in PET/CT; 40.4, 199.5 and 451.1 hr, respectively in bone scan, 62.5, 519.3 and 1313.6 hr, respectively in myocardial SPECT. Conclusion Radiation exposure rate may differ slightly depending on the work process and the environment in a general ward. Exposure rate was measured at step in the general examination procedure and it made our results more reliable. Our results clearly showed that total amount of radiation exposure caused by residual radioactive isotope in the patient body was neglectable, even comparing with the natural radiation exposure. In conclusion, nurses in a general ward were much less exposed than the normal dose limit, and the effects of exposure by contacting patients undergoing nuclear medicine examination was ignorable.

  • 개인의 의료 정보는 개인의 프라이버시에 관련되므로 민감하게 취급되어야 하는 정보이다. 이러한 개인정보 유출은 유출된 정보의 해당 당사자의 사회적 고립과 정보의 질에 따라 당사자의 생명도 위협하게 되므로 철저한 판리가 필요하다. 따라서 의사, 간호사, 환자, 일반인 등의 사용자 식별을 통해 병원 기록의 접근 통제 및 사용 권한에 따른 정보의 암호화 수준과 해당 정보에 특화된 역할기반 접근제어(Role-Based Access Control)를 제정해야 한다. 환자 자신이 자신의 의료정보를 특정한 사람에게 접근 권한을 주어 확인할 수도 있게 하고 그 외의 다른 부분들도 제어 할 수 있게 권한을 부여 할 수 있어야 한다. 본 논문은 현재 의료 및 진찰 정보 관리를 위해 RBAC모델을 기반으로 의료정보보호를 위한 접근제어 방법을 분석하고 각 정보의 객체들과 사용자 간의 효율적인 역할 분담과 한계를 통해 의료 정보의 보호방안을 고찰한다.
