Graphite material has been recognized as a very competitive candidate for reflector, moderator, and structural material for very high temperature reactor (VHTR). Since VHTR is operated up to $900-950^{\circ}C$, small amount of impurity may accelerate the oxidation and degradation of carbon graphite, which results in increased porosity and lowered fracture toughness. In this study, ultrasonic wave propagation properties were investigated for both as-received and degradated material, and the feasibility of ultrasonic testing (UT) was estimated based on the result of ultrasonic property measurements. The ultrasonic properties of carbon graphite were half, more than 5 times, and 1/3 for velocity, attenuation, and signal-to-noise (S/N) ratio respectively. Degradation reduces the ultrasonic velocity slightly by 100 m/s, however the attenuation is about 2 times of as-receive state. The results of probability of detection (POD) estimation based on S/N ratio for side-drilled-hole (SDHs) of which depths were less than 100 mm were merely affected by oxidation and degradation. This result suggests that UT would be reliable method for nondestructive testing of carbon graphite material of which thickness is not over 100 mm. In accordance with the result produced by commercial automated ultrasonic testing (AUT) system, human error of ultrasonic testing is barely expected for the material of which thickness is not over 80 mm.

Recently, damage to waterside structures, such as bridges or retaining walls, is increasing due to typhoons, flooding, aging, etc. In such cases, the damage is not limited to the structures themselves, but can include effects on a wider scale, such as the suspension of and restriction of access to the facilities, human injury, economic loss, etc. To preclude such damage, recovery methods suitable for the particular field circumstances should be applied when damage occurs. By enforcing prompt repairs, the material and human damage and losses that can occur can be minimized. Since the impact of losses caused by damage and disaster increases with the elapse of time, emergency recovery is even more important. In the emergency recovery process, appropriate repair and reinforcement is crucial. In the present study, the derivation scenarios of the emergency recovery method were applied to some field recovery cases, and their applicability was verified by comparison with the recovery methods actually used. It is expected that the results of this study will be useful for practical application, by suggesting more appropriate recovery methods.

The railway signalling system plays an essential role in the safe and efficient train operation as serving control functions of train operation intervals and train routes. The reliability and safety of the system are very important because the failure of the railway signalling system can lead to train collision or derailment as well as train operation stop. Until now, in railway signalling system the conventional wayside signal mode has been used generally. There are, however, the risk of accidents such as human mistakes caused by that the driver identifies the signal lamp status and controls train speed with the naked eye. It is also necessary to refurbish the obsolete system. Thereby, It is being effective that the onboard signal mode has been recently introduced and applied in order to transmit the speed control information to train by using the computer and communication equipment. It is necessary to switch over the system in a way while providing passengers with an operation service to replace the obsolete signal system. In this paper, we verify the cases through trial assessment which are solved by the way of adding specific functionalities in the problems of interference among the procedure of switch-over processes and a serial of processes for system verification while a train is operated in the new system in parallel to the existing system.

The safety of tunnels is quantified by quantitative risk assessment when planning the disaster prevention facilities of railway tunnels, and it is decided whether they are appropriate. The purpose of this study is to estimate the probability of the train stopping in the tunnels at train fire, which has a significant effect on the results of quantitative risk assessment for tunnel fires. For this purpose, a model was developed to calculate the coasting distance of the train considering the coefficient of train running resistance. The probability of stopping in case of train fire in the tunnel is predicted by the Monte Carlo simulation method with the coasting distance and the emergency braking distance as parameters of the tunnel lengths and slopes, train initial driving speeds. The kinetic equations for predicting the coasting distance were analyzed by reflecting the coefficient train running resistance of KTX II. In the case of KTX II trains, the coasting distance is reduced as the slope increases in a tunnel with an upward slope, but it is possible to continue driving without stopping in a slope downward. The probability of the train stopping in the case of train fire in tunnel decreases as the train speed increases and the slope of the tunnel decreases. If human error is not taken into account, the probability that a high-speed train traveling at a speed of 250 km/h or above will stop in a tunnel due to a fire is 0% when the slope of the tunnel is 0.5% or less, and the probability of stopping increases rapidly as the tunnel slope increases and the tunnel length increases.
As Korean government and safety-related organizations make continuous efforts to reduce the number of industrial accidents, accident rate has steadily declined since 2010, thereby recording 0.48% in 2017. However, the number of fatalities due to industrial accidents was 1,987 in 2017, which means that more efforts should be made to reduce the number of industrial accidents. As an essential activity for enhancing the system safety, accident analysis can be effectively used for reducing the number of industrial accidents. Accident analysis aims to understand the process of an accident scenario and to identify the plausible causes of the accident. Accident analysis offers useful information for developing measures for preventing the recurrence of an accident or its similar accidents. However, it seems that the current practice of accident analysis in Korean manufacturing companies takes a simplistic accident model, which is based on a linear and deterministic cause-effect relation. Considering the actual complexities underlying accidents, this would be problematic; it could be more significant in the case of human error-related accidents. Accordingly, it is necessary to use a more elaborated accident model for addressing the complexity and nature of human-error related accidents more systematically. Regarding this, HFACS(Human Factors Analysis and Classification System) can be a viable accident analysis method. It is based on the Swiss cheese model and offers a range of causal factors of a human error-related accident, some of which can be judged as the plausible causes of an accident. HFACS has been widely used in several work domains(e.g. aviation and rail industry) and can be effectively used in Korean industries. However, as HFACS was originally developed in aviation industry, the taxonomy of causal factors may not be easily applied to accidents in Korean industries, particularly manufacturing companies. In addition, the typical characteristics of Korean industries need to be reflected as well. With this issue in mind, we developed HFACS-K as a method for analyzing accidents happening in Korean industries. This paper reports the process of developing HFACS-K, the structure and contents of HFACS-K, and a case study for demonstrating its usefulness.
The purpose of this paper is to present the effect of image stimulation on body stability as a conceptual model and to investigate the effect of image stimulus(2D, VR) on body stability(hand stability) through experiments Recently, stereoscopic images such as virtual and augmented reality are combined with smart phones and exercise equipments, and the diffusion is becoming active. The possibility of a safety accident or human error is also increasing as it temporarily affects the balance of the body and hand stability after the image stimulus is removed. The conceptual model is presented based on the results of previous studies. Based on the experimental results, the conceptual model has been explained in combination with the human information processing process and cognitive resource models that take place in the brain. Twenty subjects were exposed to 2D and VR stimuli, and display fatigue was measured by cybersickness questionnaire and hand stability by hand steadiness tester. Experimental results show that VR images induce higher display fatigue and lower hand stability than 2D. In this study, it is meaningful that hand stability according to image type and display fatigue level which have not been tried yet is revealed through conceptual model and experiment.

Enhanced machine reliability has dramatically reduced the rate and number of railway accidents but for further reduction human error should be considered together that accounts for about 20% of the accidents. Therefore, the objective of this study was to suggest a new taxonomy of performance shaping factors (PSFs) that could be utilized to identify the causes of a human error associated with railway accidents. Four categories of human factor, task factor, environment factor, and organization factor and 14 sub-categories of physical state, psychological state, knowledge/experience/ability, information/communication, regulation/procedure, specific character of task, infrastructure, device/MMI, working environment, external environment, education, direction/management, system/atmosphere, and welfare/opportunity along with 131 specific factors was suggested by carefully reviewing 8 representative published taxonomy of Casualty Analysis Methodology for Maritime Operations (CASMET), Cognitive Reliability and Error Analysis Method (CREAM), Human Factors Analysis and Classification System (HFACS), Integrated Safety Investigation Methodology (ISIM), Korea-Human Performance Enhancement System (K-HPES), Rail safety and Standards Board (RSSB), $TapRoot^{(R)}$, and Technique for Retrospective and Predictive Analysis of Cognitive Errors (TRACEr). Then these were applied to the case of the railway accident occurred between Komo and Kyungsan stations in 2003 for verification. Both cause decision chart and why-because tree were developed and modified to aid the analyst to find causal factors from the suggested taxonomy. The taxonomy was well suited so that eight causes were found to explain the driver's error in the accident. The taxonomy of PSFs suggested in this study could cover from latent factors to direct causes of human errors related with railway accidents with systematic categorization.

The nuclear power industry in the world has recognized the importance of integrating non-technical and team skills training with the technical training given to its control room operators to reduce human errors since the Three Mile Island and Chernobyl accidents. The Nuclear power plant (NPP) industry in Korea has been also making efforts to reduce the human errors which largely have contributed to 120 nuclear reactor trips from the year 2001 to 2006. The Crew Resource Management (CRM) training was one of the efforts to reduce the human errors in the nuclear power industry. The CRM was developed as a response to new insights into the causes of aircraft accidents which followed from the introduction of flight recorders and cockpit voice recorders into modern jet aircraft. The CRM first became widely used in the commercial airline industry, but military aviation, shipboard crews, medical and surgical teams, offshore oil crews, and other high-consequence, high-risk, time-critical industry teams soon followed. This study aims to develop a CRM training program that helps to improve plant performance by reducing the number of reactor trips caused by the operators' errors in Korean NPP. The program is; firstly, based on the work we conducted to develop a human factors training from the applications to the Nuclear Power Plant; secondly, based on a number of guidelines from the current practicable literature; thirdly, focused on team skills, such as leadership, situational awareness, teamwork, and communication, which have been widely known to be critical for improving the operational performance and reducing human errors in Korean NPPs; lastly, similar to the event-based training approach that many researchers have applied in other domains: aircraft, medical operations, railroads, and offshore oilrigs. We conducted an experiment to test effectiveness of the CRM training program in a condition of simulated control room also. We found that the program made the operators' attitudes and behaviors be improved positively from the experimental results. The more implications of the finding were discussed further in detail.

Accidents associated with hazardous materials, including oil and HNS, in maritime transportation show increasing trend. Therefore, preventive countermeasures for such accidents should be provided. The purpose of this study is to analyze level of risk on human loss and potential damage to environment, using data on domestic marine accidents carrying hazardous materials (2002~2014), and identify high-risk accident types for urgent risk management which needs findings of accident causes and proper mitigation measures. High-risks on human loss are explosion and suffocation, occurred in process of ship maintenance and tank cleaning. On the other hand, high-risk on environmental damage is spill caused by ship accidents (collision, grounding and etc.). Especially, spill occurs during loading operation of oil and HNS onboard a ship. Strict operation supervision/management and safety education/training on a regular basis could prevent accidents, because human factors such as not wearing safety gear and careless cargo handling cause most of the marine accidents.

Though, fishing vessel accidents account for 70 % of all maritime accidents in Korean waters, most research has focused on identifying causes and developing mitigation policies in an attempt to reduce this rate. However, predicting and evaluating accident risk needs to be done before the implementation of such reduction measures. For this reasons, we havve performed a risk analysis to calculate the risk of accidents and propose a risk criteria matrix with 4 quadrants, within one of which forecasted risk is plotted for the relative comparison of risks. For this research, we considered 9 types of fishing vessel accidents as reported by Korea Maritime Safety Tribunal (KMST). Given that no risk evaluation criteria have been established in Korea, we established a two-dimensional frequency-consequence grid consisting of four quadrants into which paired frequency and consequence for each type of accident are presented. With the simple structure of the evaluation model, one can easily verify the effect of frequency and consequence on the resulting risk within each quadrant. Consequently, these risk evaluation results will help a decision maker employ more realistic risk mitigation measures for accident types situated in different quadrants. As an application of the risk evaluation matrix, accident types were further analyzed using accident causes including human error (factor) and appropriate risk reduction options may be established by comparing the relative frequency and consequence of each accident cause.
