Mineral Contents in Scalp Hair in Patients with Non-insulin Dependent Diabetes Mellitus (두발중(頭髮中) 일부(一部) 금속(金屬) 농도(濃度)와 인슐린비의존형(非依存形) 당뇨병(糖尿病)과의 관련성(關聯性))

  • To estimate association of minerals relating to non-insulin dependent diabetes mellitus(NIDDM) the contents of essential minerals and toxic heavy metals in the scalp hair were measured during the period from March 1993 to June 1993. The cases were selected 69 (32 male, 37 female) from the patients who visited Kyungpook University Hospital during the period from March 1993 to June 1993 and 74 controls (47 male 27 female) from normal population. zinc, copper, manganese, lead, cadmium and aluminum were analyzed by atomic absorption spectrophotometer(IL. 551). The results were summarized as follows. In NIDDM group, mean contents of zinc and copper were $132.99{\pm}19.66$ ppm, $8.58{\pm}5.07$ ppm respectively; where as $152.51{\pm}46.15$ ppm, $12.48{\pm}6.56$ ppm in control group. The mean of above two mineral content showed significant difference statistically (p<0.01). But, in manganese the content of NIDDM group; $0.44{\pm}0.27$ ppm were significantly higher than the control group; $0.34{\pm}0.18$ ppm(p<0.05). A thesis submitted to the Council of the Graduate School of Kyungpook National University in partial fulfillment of the require ements for the degree of Ph.D. in Medical Science in June 1994. The mean contents of toxic metals - lead, cadmium, aluminum - in NIDDM group were $10.04{\pm}4.60$ ppm, $0.57{\pm}0.12$ ppm, $11.31{\pm}3.98$ ppm; whereas $8.10{\pm}2.45$ ppm, $0.41{\pm}0.23$ ppm, $7.61{\pm}4.16$ ppm in control group. The mean of above three mineral showed significant difference statistically(p<0.01). Cadmium, zinc and copper were selected statistically significant variables with presence of the NIDDM by multiple logistic regression analysis; copper, zinc and cadmium may be associated with NIDDM.

  • The purpose of this study was to test the effect of a diabetic education program using the self-efficacy theory on the self-care behavior and glucose metabolism. The subjects of the study consisted of 25 NIDDM patients who had participated with a diabetic education program from June 23 to July 14. 1999. The research design was one-group pretest-posttest design. The pretest included measuring. self-care behavior and 2PPBS. the diabetic education program was conducted to the group for 4weeks. After the diabetic education program. the posttest included remeasuring of self-care behavior and 2PPBS. Wilcoxon signed rank test was used for the significence of the differences between values before and after the diabetic education program. The results are as follows. The diabetic education program was effective in increasing the score of the self-care behavior in exercise but was not effective in increasing the score of the self-care behavior in diet, test and medication. The diabetic education program was not effective in decreasing the levels of glucose metabolism.

The Effects of Exercise Therapy Applied in an Efficacy Expectation Promoting Program on Self-Efficacy and Metabolism.- in NIDDM(Non-Insulin Dependent Diabetes Mellitus) Patients - (효능기대증진프로그램을 적용한 운동요법이 자기효능과 대사에 미치는 영향-인슐린 비의존성 당뇨병 환자를 중심으로-)

  • This study was conducted to investigate whether exercise therapy applied in an efficacy expectation promoting program based on the self-efficacy theory of Bandura(1977) would increase self-efficacy and metabolism in NIDDM patients. The study design was a nonequivalent control group pre-test post-test quasi-experimental desist The exercise therapy applied in the efficacy expectation promoting program was composed of a staged exercise program, a small booklet relating personal experience with diabetes mellitus and a telephone coaching program on performance accomplishment. vicarious experience and verbal persuasion, which are all induction modes of efficacy expectation. The subjects of the study were twenty eight NIDDM patients who received follow-up care regularly through the out-patient department of endocrine medicine in one general hospital which had a diabetic clinic. Fourteen were assigned to the experimental group and fourteen to the control group. The experimental group participated in the exercise therapy applied in the efficacy expectation promoting program from three to five times per week for four weeks and the control group did not have the program. The collected data were analyzed using the X²-test, t-test, paired t-test, and Cronbach's Alpha using SPSS /PC/sup +/. The results are summarized as follows 1. Experimental group had higher efficacy score than control group(t=5.98, p=.00). And. There was a significant different in the efficacy score before exercise therapy applied in the efficacy expectation promoting program and after in experimental group(t=-6.42, p=.00). 2. Experimental group did not have lower level of glucose metabolism than control group(FBS : t=.32, p=.75, HbAlC : t=.60, p=.55, pc 2hrs. glucose : t=-.29, p=.78). But, There was a significant different in the aunt of glucose metabolism before exercise therapy applied in the efficacy expectation promoting program and after in experimental group(FBS : t=3.63, p=.003, HbAlC t=4.20, p=.00, pc 2hrs. glucose : t=1.93, p=.001). 3. Levels of lipid metabolism were partly a significant different between Experimental group and control group(triglyceride t=-1.87, p=.07, HDL cholesterol : t=-.29, p=.77. body weight : t=1.78, p=.09, Total cholesterol : t=-2.17, p=.04). And, There was partly a significant different in the amount of lipid metabolism before exercise therapy applied in the efficacy expectation promoting program and after in experimental group(triglyceride : t=2.50, p=.03, HDL cholesterol : t=-.43, p=.67, body weight : t=5.34. p=.00, Total cholesterol : t=2.26, p=.04). In conclusion, it was found that exercise therapy applied in an efficacy expectation promoting program was an effective nursing intervention for increasing self-efficacy and metabolism.

Effect of Different Kind of Plant Oil Sources on Serum and Hepatic Lipid levels of Streptozotocin-induced Diabetic Mice (Streptozotocin으로 유발한 당뇨 마우스에서 식물성 지방 급원에 따른 혈액 및 간조직의 지질 조성)

  • This study was carried out to compare the effects of different plant oil sources on lipid metabolism of insulin dependent diabetic mice. Streptozotocin-induced (65 mg/kg B.W.) diabetic mice were fed four kinds of experimental diets with 20% lipid, which composed of 10% lard, and 10% soy bean oil (SBO), 10% rice germ oil (RGO), 10% rice brab oil (RBO) and 10% sesame oil (SSO) respectively, for 7 weeks. Diet intake, body weight, organs weights and lipids levels of serum, liver and feces were measured. There was no significant difference in diet intake and body weight among experimental groups. But the concentrations of serum triglyceride of RGO and RBO groups, and of serum total cholesterol of RGO and SSO groups were lower than those of the others. The hepatic total cholesterol level was the lowest in RGO group. The contents of total lipid and total cholesterol excreted in feces of RGO and RBO groups were higher than those of SBO and SSO groups. These results suggested that rice germ oil can effectively reduce serum triglyceride level and hepatic total cholesterol concentration of insulin dependent diabetic mice than rice bran oil, soy bean oil and sesame oil, and hypolipidemic effect of rice germ oil be due to increasing fecal lipid excretion. But we need to investigate the more detailed factors on lipid reducing effect in rice germ oil and rice bran oil.

THE ROLE OF TYPE 2 DIABETES AS A PREDISPOSING RISK FACTOR ON THE PULPO-PERIAPICAL PATHOGENESIS: REVIEW ARTICLE (치수 치근단 병소의 전구 위험요인으로서의 제 2 형 당뇨의 역할에 관한 소고)

  • Diabetes Mellitus (DM) is a syndrome accompanied with the abnormal secretion or function of insulin, a hormone that plays a vital role in controlling the blood glucose level (BGL). Type land 2 DM are most common form and the prevalence of the latter is recently increasing, The aim of this article was to assess whet her Type 2 DM could act as a predisposing risk factor on the pulpo-periapical pathogenesis. Previous literature on the pathologic changes of blood vessels in DM was thoroughly reviewed. Furthermore, a histopathologic analysis of artificially-induced periapical specimens obtained from Type 2 diabetic and DM-resistant rats was compared. Histopathologic results demonstrate that the size of periapical bone destruction w as larger and the degree of pulpal inflammation was more severe in diabetic rats, indicating that Type 2 D M itself can be a predisposing risk factor that makes the host more susceptible to pulpal infection. The possible reasons may be that in diabetic state the lumen of pulpal blood vessels are thickened by atheromatous deposits, and microcirculation is hindered, The function of polymorphonuclear leukocyte is also impair ed and the migration of immune cells is blocked, leading to increased chance of pulpal infection. Also, lack of collateral circulation of pulpal blood vessels makes the pulp more susceptible to infection. These decrease the regeneration capacity of pulpal cells or tissues, delaying the healing process, Therefore, when restorative treatment is needed in Type 2 DM patients, dentists should minimize irritation to the pulpal tissue un der control of BGL.

The Riboflavin Status in Insulin-Dependent Diabetes Mellitus During Growing Period (성장기 인슐린 의존성 당뇨병 환자의 리보플라빈 영양상태)

  • Riboflavin status of 17 insulin-dependent diabetic mellitus(IDDM) patients in growing period was evaluated as a function of energy intake and expenditure, biochemical nutritional status and diabetic control indicators. Compared with recommended dietary allowances for Koreans(RDA, 1989), only 35.3% of subjects was at good levels of all nutrients intakes and 52.9% of subjects was below normal level of height and weight. Nutrients consumed below RDA levels were energy(=88.5% of subjects), niacin(64.7%), iron(52.9%) and protein(23.5%) respectively. The riboflavin status was within normal range by urinary riboflavin excretion but 17.6% of subjects was evaluated as showed riboflavin deficiency by erythrocyte glutathione reductase activity coefficient(EGRAC). Correlation between riboflavin intake, urinary riboflavin excretion, EGRA level and diabetic duration were not statistically significant. Correlation analyses indicated that EGRA level was inversely correlated with thiamin, niacin and cabohydrate intake. No significant correlations were found between the EGRA and glycosylated hemoglobin A1(HbA1) (r=-0.464, p=0.129). From this study, it is suggested that IDDM subjects need to maintain balanced diet containing nutrients above RDA for individual activity during growing period. It needs more study whether the current recommended riboflavin allowance is adequate for diabetic patients.

Evaluation of Nutrition Education for Insulin Dependent Diabetic Students (인슐린 의존성 당뇨병 학생을 대상으로 실시한 영양교육의 효과)

  • This study was conducted to evaluate changes in nutritional knowledge, attitudes, behavior intention and behavior in a sample of 27 insulin dependent diabetic students participating in diabetics' camp. Nutritional knowledge related to diabetes, attitudes toward diabetes treatment and behavior intention about dietary changes were tested before and after nutrition education. Six months after nutrition education, an open ended questionnaire about their actual behavior changes was mailed to all participants and 17 of them responded. Pre-and post-testing showed that nutrition education was effective in significant changes in knowledge and in promoting positive attitudes and behavior intention. Increases of knowledge were consistently the same regardless of sex, level of education, regularity of diet, and duration of disease. Compared to male students, female students showed more positive change in knowledge, attitudes and behavior intention. It also appeared that middle school students showed more positive improvement in knowledge, attitudes and behavior than elementary and high school students. A follow-up test showed that their actual behavior changes were not squared with their behavior intention. They pointed out difficulties in having proper amount of meals and snack and the conflict with school time schedule as the major reason for discordance. These findings suggest that nutrition education for diabetics can be effective to improve their knowledge, attitudes and behavior intention and understanding about barriers to behavior change is important for better compliance to the disease.

  • The objective of this study was to evaluate the bifidogenic effect and a-glucosidase inhibitory effect of artificially cultured Phellinus linteus. The water extract of P. linteus promoted the growth of Bifidobacterium breve as well as the decrease of final pH in the media culturing intestinal bacteria. The growth of lactic acid bacteria inhibited effectively the bacterial enzymes, ${\beta}-glucosidase,\;{\beta}-glucuronidase$ and tryptophanase, of intestinal bactetria. The water extract of P. linteus inhibited maltase, sucrase and ${\alpha}-amylase$ of rat intestine.

  • 본 실험실에서는 혈당강하효과를 갖는 chiroinositol을 다량 함유하고 있는 식품이나 식용물질을 탐색하여 선발하고, 선택된 식품이나 식용물질에서 chiroinositol을 lab-scale로 분리 정제하여 제조된 chiroinositol의 추출물로 동물실험을 실시하여 혈당강하효과를 증명하고 그 기작을 연구하고 있다. 약 300여종의 식품 및 식용물질을 GC-MS 및 HPLC로 분석한 결과 식품으로 안전한 탈지대두와 두부 순물에서 chiroinositol의 함량이 각각 6.45mg/g, 20mg/g으로 조사되어 선택되었고, chiroinositol의 순도가 40-60%(w/w)인 chiroinositol의 추출물을 이용하여 동물실험을 통한 혈당강하효과를 조사한 결과, 1) steptozotocin(STZ)으로 유발된 고혈당쥐에 경구투여시, 농도 의존적으로 혈당을 감소시키는 탁월한 기능을 나타내었고, 2) 정상쥐의 혈당제거율 검사(glucose tolerance test)에서강한 제거율 및 6시간의 지속시간을 나타내었으며, 3) STZ로 유발된 고혈당쥐의 혈당제거율 검사에서 강한 혈당강하 효과(약 40%)를 나타내었으며 지속시간은 약 12시간이었다. 4) 극심한 고혈당(450 내지 500mg/d)의 경우, 경미한 고혈당(300내지 350mg/d)의 경우보다 혈당 감소율이 20% 가량 높았으며, 지속시간도 1시간 이상 긴 것으로 나타났으며, 5) 인슐린과 복합처리 했을 때 상승작용(synergy)을 나타내었으며, 저혈당증으로 전혀 발전되지 않았다.

Administration of Triticum aestivum Sprout Water Extracts Reduce the Level of Blood Glucose and Cholesterol in Leptin Deficient ob/ob Mice (Leptin 결핍 ob/ob 마우스에서 소맥엽 추출물의 혈당 강하 및 혈중 콜레스테롤에 미치는 효과)

  • Type 2 diabetes mellitus (NIDDM) is a metabolic disorder that is characterized by high blood glucose in the context of insulin resistance and relative insulin deficiency. In order to control the type 2 diabetes mellitus, anti-hyperglycemic effect of Triticum aestivum L. water extracts (TAWE) was investigated in 7 week old male diabetic C57BL6/J-ob/ob mice. For the experiments, the diabetic animal model ob/ob mice and non-diabetic animal model lean mice were divided into 3 groups: non-treatment control group (Control), and two experimental groups orally treated with 25 or 100 mg/kg/day dose of TAWE (TAWE-25 and TAWE-100, respectively). The lean mice were used as the non-diabetic normal control. TAWE was orally administrated for 6 weeks and the diabetic clinical markers, including blood glucose level, body weight, organs weight and insulin level were determined. The oral administration of TAWE-100 in ob/ob diabetic mice significantly decreased blood glucose level (78.4%) and body weight (11.9%) compared with diabetic control group. The weights of organs, including spleen, liver, kidneys, heart and lung were not different among groups, while the treatments of TAWE-100 in ob/ob diabetic mice significantly reduced blood total cholesterol (24.35%) and triglyceride (23.97%) levels compared with the diabetic control group. The levels of serum insulin and glucose tolerance were improved after TAWE-100 treatment in ob/ob diabetic mice. Moreover, the immunohistochemical staining for insulin detection in pancreatic islet $\beta$-cells expressed high level of insulin in TAWE-100 treated ob/ob mice. From the above results, the intake of TAWE may be effective in anti-hyperglycemia by the attenuation of glucose and lipid levels. TAWE-containing diets or drugs may be beneficial for controlling diabetes mellitus type 2 in human.
