THE EVALUATION OF MICROLEAKAGE OF PIT AND FISSURE SEALANT BY USAGE OF SELF-ETCHING ADHESIVE SYSTEM (자가 산부식 접착제를 이용한 치면열구전색의 미세누출 평가)

  • The purpose of this study was to evaluate the effectiveness of fissurotomy and double application time of acidic primer of self-etching adhesive system of acid treatment on enamel surfaces for prevention of microleakage of pit and fissure sealants. The microleakage of pit and fissure sealants was evaluated by measuring the penetration depth of methylene blue solution. Specimens were divided by 4 groups according to the method of treatment. Group I: 35% phosphoric acid etching, 20 seconds. Group II: Priming with self-etching primer, 20 seconds. Group III: Priming with self-etching primer, 40 seconds. Group IV: Priming with self-etching primer, 20 seconds, after fissurotomy. The etched pattern produced on enamel was observed using a scanning electron microscope. Obtained data were analysed statistically using Kruscal-Wallis test followed by Mann-Whitney test for comparison of groups. The results are as follows: 1. Microleakage scores of group IV priming with self-etching primer 20 seconds after fissurotomy was the lowest. 2. Microleakage scores between group II and group III were not shown significant difference. 3. Enamel for 20 seconds etching with 35% phosphoric acid was observed the most effective etching pattern. And the etching pattern on the fissure enamel with self-etching adhesive 20 seconds after fissurotomy was more prominent than group II, III only using self-etching primer.

Priming time and etching effect on shear bond strength of self-etching adhesive (프라이머 처리시간과 산부식이 자가산부식 접착제의 접착강도에 미치는 영향)

  • 자가산부식 접착제 (self-etching primer)는 법랑질과 상아질을 동시에 산부식과 프라이머 처리를 함으로 임상 시술 시간을 단축시킬 뿐만 아니라, 임상과정 중 발생할 수 있는 술자의 실수 및 타액등에 의한 오염의 가능성을 줄일 수 있는 장점을 가지고 있다. 그러나 약산을 이용한 산부식법이 법랑질에 대해 탈회효과 및 접착강도에 있어 논란이 되고 있다. 이에 본 연구에서는 인산을 이용한 추가적인 산부식 및 자가산부식 접착제의 적용시간을 달리할 경우 법랑질에 대한 접착강도의 변화를 알아보고자 하였다. 135개의 발거한 우치의 법랑질 면을 #600사포로 연마한 후 9개의 군으로 분류하였다. 각 군은 부가적인 32% 인산을 처리하지 않은 군 (1~3군), 15초간 처리한 군 (4~6군), 60초간 처리한 군 (7~9군)으로 나누고, 이를 다시 Clearfil SE Bond의 프라이머로 5초 (1, 4, 7군), 20초 (2, 5, 8군), 60초 (3, 6, 9군)간 처리한 군으로 분류하였다. 접착제 처리한 면에 Clearfil AP-X 복합레진을 접착하고 24시간 경과 후 전단응력 강도를 측정하였다. 결과는 One-way ANOVA 처리후 Duncan's multiple range test로 사후검증하였다. 동일한 자가산부식 프라이머 처리 시간을 가진 군에서 산 부식을 한 경우가 그렇지 않은 군에 비해 높은 접착강도를 나타내었으며 인산부식 시간의 차이에 의한 영향은 없었다.(1 < 4, 7군/ 2 < 5, 8군/ 3 < 6, 9군). 자가산부식제의 프라이머 적용 시간에 의한 효과는 1군(5초 적용)을 제외한 나머지 군의 경우 동일한 인산 처리군에서는 차이가 없는 것으로 나타났다.(1군 < 2, 3군/ 4=5=6군/ 7=8=9군). Clearfil SE Bond 접착제의 법랑질에 대한 접착강도는 부가적인 산부식을 통하여 증가시킬 수 있으며 프라이머의 적용 시간에 의한 효과는 제조자의 지시에 의한 시간 이상 적용할 경우 차이가 없는 것으로 나타났다.

THE EFFECT OF CPP-ACP PRE-TREATMENT ON BOND STRENGTH OF A SEALANT (CPP-ACP 적용이 치면열구전색제의 결합강도에 미치는 영향)

  • Recently the effect of CPP-ACP on early caries remineralization and prevention has hashbeen extensively researched. However, there has been a lack of research on micro-shear bond strength of a sealant applied on a CPP-ACP treated surface. Therefore, the purpose of this study was to evaluate the effect of CPP-ACP on the micro-shear bond strength of a sealant bonded with 3 different bonding systems. From the sound human 3rd molars, the 150 specimens were prepared and randomly assigned to six groups. Group I: Immersion in artificial saliva for 2 weeks + 35% phosphoric acid Group II: Immersion in artificial saliva for 2 weeks + 35% phosphoric acid + dentin bonding agent Group III: Immersion in artificial saliva for 2 weeks + self-etching adhesive Group IV: CPP-ACP application for 2 weeks + 35% phosphoric acid Group V: CPP-ACP application for 2 weeks + 35% phosphoric acid + dentin bonding agent Group VI: CPP-ACP application for 2 weeks + self-etching adhesive Sealant was applied and the micro-shear bond strength was measured. From the result of this study, it can be assumed that the CPP-ACP pretreatment can weaken the bond strength of a sealant if the enamel surface is conditioned with self-etch adhesive.

IATROGENIC CHEMICAL BURN ON FACIAL SKIN BY 37% PHOSPHORIC ACID ETCHANT (37% 인산 부식제에 의해 발생한 안면피부의 화학 화상)

  • When we use the total-etch dentin adhesive system for composite resin restorations, gel or liquid acid etchant such as 37% phosphoric acid is commonly used. Thirty seven percentage phosphoric acid is very powerful erosive agent, and can cause severe harmful effects when it contacts with an oral mucosa and facial skin. This case describes iatrogenic chemical burn on facial skin caused by phosphoric acid which was happened during composite resin restorative procedure. Chemical burn by acid etchant can be evoked by careless handling of remnant and syringe. In order to prevent these iatrogenic injuries, we should check the complete removal of the etching agent both in intra and extra-oral environments after etching and rinsing procedure and it is necessary to use of the rubber dam or isolation instruments. If accidental burn were occurred. immediate wash with copious water. And bring the patient to the dermatologist as soon as possible.

Alternative Pretreatment Methods for Resin Infiltration in Primary Anterior Teeth (유전치 Resin Infiltration을 위한 표면처리 방법의 대안)

  • Resin infiltration has been used as a treatment option for the management of early caries lesions recently. However, the etching procedure with hydrochloric acid might be somewhat stressful for the clinicians due to safety problem especially for young children, leading to less utility. This study aims at searching for some alternative surface pretreatment methods of resin infiltration for the early caries lesions in primary anterior teeth by comparing penetration depth of various methods. No significant difference was found in penetration ratio between etched surface with 15% hydrochloric acid and 35% phosphoric acid. However, the penetration ratio was significantly higher in groups pretreated either with dental pumice or abrasive metal strip (p < .05). By the result of this study, etching with phosphoric acid as an alternative of hydrochloric acid was thought clinically acceptable as a pretreatment method for resin infiltration in early caries lesions for primary anterior teeth. It was notable that surface conditioning with dental pumice or metal strip before etching was effective in increasing the penetration. This procedural modification might be much more correspondent with minimally invasive concept and hopefully contribute to increased safety and utility in pediatric dentistry.

Influence of Application Method on Shear Bond Strength and Microleakage of Newly Developed 8th Generation Adhesive in Primary Teeth (새로 개발된 8세대 접착제의 적용 방법에 따른 유치에서의 전단결합강도와 미세누출)

  • The purpose of this study was to evaluate the effect of application time and phosphoric acid etching of 8th generation adhesives containing functional monomer on adhesive performance in primary teeth. 80 extracted non-carious human primary teeth were selected and divided into 8 groups based on 3 factors: (1) adhesive: G-Premio bond and Single bond universal; (2) application time: shortened time and manufacture's instruction; (3) acid etching mode: self-etching and total-etching. Shear bond strength was measured using a universal testing machine, and fractured surface were observed under scanning electron microscope. Microleakage was evaluated by dye penetration depth. G-Premio bond were not significant different in shear bond strength and microleakage depending on application time of adhesive and acid etching mode. In Single bond universal, shear bond strength of short application time was significantly lower than that of long adhesive application time (p = 0.014). Clinically applicable shear bond strength values (> 17 MPa) were identified in all groups. These results suggested that G-Premio bond be used clinically for a short application time without phosphoric acid etching.

The study of shear bond strength of a self-adhesive resin luting cement to dentin (상아질에 대한 자가 접착 레진 시멘트의 전단결합강도에 관한 연구)

  • Purpose: The objective of this study was to compare the bonding characteristics of a new self-adhesive resin cement to dentin, which does not require bonding and conditioning procedure of the tooth surface, and conventional resin cement. The effect of phosphoric acid etching prior to application of self-adhesive resin cement on the shear bond strength was also evaluated. Material and methods: Fortyfive non-carious human adult molars extracted within 6 months were embedded in chemically cured acrylic resin. The teeth were ground with a series of SiC-papers ending with 800 grit until the flat dentin surfaces of the teeth were exposed. The teeth were randomly divided into 3 experimental groups. In group 1, self-adhesive resin cement, RelyX Unicem (3M ESPE, Seefeld, Germany) was bonded without any conditioning of teeth. In group 2, RelyX Unicem was bonded to teeth after phosphoric acid etching. For group 3, Syntac Primer (Ivoclar Vivadent AG, Schaan, Liechtenstein) was applied to the teeth before Syntac adhesive (Ivoclar Vivadent AG, Schaan, Liechtenstein) and Helibond (Ivoclar Vivadent AG, Schaan, Liechtenstein) followed by conventional resin cement, Variolink II (Ivoclar Vivadent AG, Schaan, Liechtenstein). To make a shear bond strength test model, a plastic tuble (3 mm diameter, 3 mm height) was applied to the dentin surfaces at a right angle and filled it with respective resin cement, and light-polymerized for 40 seconds. All the specimens were stored in distilled water at $37^{\circ}C$ for 24 hours before test. Universal Testing Machine (Z020, Zwick, Ulm, Germany) at a cross head speed of 1 mm/min was used to evaluate the shear bond strength. The failure sites were inspected under a magnifier and Scanning Electron Microscope. The data was analyzed with One way ANOVA and Scheffe test at ${\alpha}$= 0.05. Results: (1) The shear bond strengths to dentin of RelyX Unicem was not significantly different from those of Variolink II/Syntac. (2) Phosphoric acid etching lowered the shear bond strength of RelyX Unicem significantly. (3) Most of RelyX Unicem and Variolink II showed mixed fractures, while all the specimens of RelyX Unicem with phosphoric acid etching demonstrated adhesive failure between dentin and resin cement. Conclusion: Shear bond strength to dentin of self-adhesive resin cement is not significantly different from conventional resin cement, and phosphoric acid etching decrease the shear bond strength to dentin of self-adhesive resin cement.

The effects of a sealant resin on enamel demineralization in orthodontic bracket bonding (브라켓 부착 시 실런트 레진이 법랑질 탈회에 미치는 영향)

  • The purpose of this study was to evaluate the effects of a sealant resin on enamel demineralization In orthodontic bracket bonding. The forty eight extracted sound bovine teeth were subdivided into four groups and treated with Phase II (Reliance, itasca. III) on the surface Group 1 was not treated. Group 2 was acid etched with 37% phosphoric acid for 30 seconds. Group 3 was applied with sealant after acid etching. Group 4 was applied with resin paste after acid etching and sealant application. Each group was demineralized in artificial caries solution. Demineralized enamel depth was measured by confocal laser scanning microscopy. The results were as follows: the mean demineralized enamel depth was $47.4{\mu}m$. (Group 1), $61.8{\mu}m$ (Group 2), $13.9{\mu}m$ (Group 3). $8.2{\mu}m$ (Group 4) the demineralized enamel depth was increased in Group 2 than in Group 1 (p<0.05); the demineralized enamel depth was reduced in Group 3 than in Group 1 and Group 2 with statistically significant differences (p<0.05): and demineralization in Group 4 was very little. The results of the present study indicate that sealant application is useful for reducing enamel demineralization in orthodontic bracket bonding.

THE EFFECTS OF SALIVARY CONTAMINATION OF ACID-ETCHED ENAMEL ON BRACKET BOND STRENGTH (산부식 후 타액오염이 교정용 접착제의 결합강도에 미치는 영향)

  • The purpose of this study was to determine the effect of salivary contamination of etched enamel on shear bond strength of a bracket adhered to etched enamel. Eighty extracted human permanent premolars were used in this study. These samples were divided into two groups. Buccal surface of samples were etched in vitro with 38% phosphoric acid for 15 seconds and 60 seconds. Each group was divided into four subgroups. Etched enamel surfaces were contaminated with saliva for 0, 1, 20, 60 seconds, washed and dried. Test surfaces were examined using scanning electron microscope(SEM). The shear bond strength of each sample was determined with a universal testing instrument(Instron Co. Model 4201). Results were as follows; 1. Salivary contamination for 1, 20, 60 seconds did not affect shear bond strength when compared with the uncontaminated enamel group. 2 There was no significant difference(P>.05) in shear bond strength between 15 sec. and 60 sec. etching in uncontaminated enamel groups. 3. When samples were examined using SEM, organic materials coated enamel surface masked the etched pattern partially.

RESIN TAG FORMATION OF SELF-ETCHING ADHESIVES (자가부식 접착제의 레진 Tag 형성)

  • The aims of the present study was to observe resin tag of the resin/enamel, dentin interface produced by self-etching adhesive systems and evaluate effect of additional acid etching on resin tag formation. Three self-etching primer(SE bond, AQ bond and L Pop) and an one bottle adhesive(Single bond) were used. Flat occlusal enamel and dentin disks were obtained from extracted human molars. A total of 20 surfaces were collected and divided into four groups of 5 samples. One-half of each specimen in each group was etched with 35% phosphoric acid prior to the application of each adhesive system, with the second half being kept unetched. Subsequently, resin composite was placed and polymerized. The samples were sliced and immersed into HCl and NaOCl solutions, followed by drying and sputter coating for examination with a SEM. The results were as follows; 1. Additional etching side of dentin displayed longer and thicker resin tag than unetched side in all self-etching adhesive groups. 2. In enamel, additional etching side displayed deeper and more distinct etching pattern than unetched side except L Pop. There is no difference between etched and unetched enamel in L Pop. The results obtained suggest the self-etching adhesive did not etch enamel and penetrate into dentinal tubule as deeply as did additional etching. Further research should include the evaluation of the relationship of boding strength, microleakage and resin tag morphology.
