양측성 횡격막 마비는 드문 질환으로 외상, 심흉부 수술, 신경근육질병, 경부척추굳음증, 감염이 원인이 될 수 있다. 우폐상엽, 심낭, 상대정맥과 무명정맥을 침범한 흉선암의 적출술을 시행한 후 60세 남자 환자에서 양측성 횡격막 마비가 발생하였다. 심한 호흡곤란이 발생하였고, 인공호흡기 이탈이 불가능하였다. 양측 횡격막 주름성형술을 시행하였고, 인공호흡기 이탈 및 누운 자세에서 수면이 가능하게 되어 만족할 만한 결과를 얻었기에 문헌 보고하는 바이다.

Edwards Duromedics 기계판막은 잦은 판엽 이탈로 인해 1988년 5월부터 판매가 중지된 판막으로 국내에서는 현재까지 2예의 판엽 이탈이 보고되었다. 본원에서는 Edwards Duromedics 기계판막을 이용하여 승모판치환술을 받은 환자에서 발생한 판엽 이탈을 경험하였기에 증례 보고하는 바이다

The exposure of blood to foreign materials can cause the denaturation of plasma protein components such as immunoglobulins and complements. And those phenomena increase the morbidity and mortality after intracardiac operations through the cardiopulmonary bypass. From April, 1987 to September, 1987, we had observed the serial changes of plasma total protein IgG, IgA, IgM, complements[C3, C4] in bubble oxygenator group[n=5] and membrane oxygenator group[n=5]. Statistically significant difference between two groups were present in total protein and C3. We conclude that using membrane oxygenator in long extracorporeal circulation can reduce the activation of alternative pathway of complement system, and which can reduce post-perfusion complications of the lung though we can`t prove it in mass populations.

From 1982 to 1987, six patients underwent left ventricular aneurysmectomy with concomitant myocardial revascularization. Simultaneous repair of postinfarction ventricular septal defect was performed in 3 patients and left ventricular thrombectomy in two. There was no hospital mortality and late mortality during 220 months* follow up period-.[Mean 36.7 months, range 13 to 72 months] Their condition was improved in all. They live in physical condition of NYHA class I-II. We believe early surgical intervention can be life-saving, and can be done without undue surgical risk, even in cases of postinfarction VSD and LV aneurysm with failing heart.

심장 탈장은 심낭을 열고 전폐절제술을 시행 후 드물게 발생되는 합병증으로 수술이 끝날때나 수술직후 발생된, 44세 남자환자에서 심낭을 열고 좌측 폐를 전절제술 후 심낭 결손부위를 인공조직으로 봉합하였다. 수술이 종료될 때 심장탈장이 발생되어 개흉에 의한 응급 복원을 시행하였으나 심한 저 산소성 뇌 손상이 발생되었다.

본 증례는 66세(51.8kg)된 남자에서 수술전 심한 좌심실 기능 부전과 승모판 및 삼첨판 폐쇄부전이 있어 개심술 시행후 심한 저심박출증 발생으로 인공심폐기 이탈에서 실패하고 양심실 보조장치(Centrifugal Biopump)를 사용한 후 이로부터 성공적인 이탈과 퇴원이 보고하는 바이다.

양모의 주성분인 케라틴 단백질은 과거에는 주로 의복용 재료로서 이용되어 왔지만 최근에는 케라틴이 가지는 물리적, 화학적 특성 때문에 양모-케라틴 유도체를 기능성고분자 소재로 이용하려는 관심이 높아지고 있다. 양모케라틴 단백질은 원래 섬유 자체가 중금속 흡착성을 보유하고 있어 여러 용해 유도체의 흡착특성이 연구되고 있으며 우수한 생체적합성이 있어 헤파린과의 불용성 전해질 복합체를 필름으로 제조하여 인공혈관으로의 응용와 같은 의료분야에의 연구도 진행되고 있다. (1-3) (중략)

Although there have been diverse opinions, permanent anticoagulation is generally recommended in children with mechanical valves. We experienced the thrombotic dysfunction of mitral St. Jude Medical valve in a 4-year-old child who had been under control of anticoagulation with warfarin sodium. Besides the primary problems related to the prosthetic mechanism, various patient factors might have contributed to the valve thrombosis. We would emphasize the necessity of anticoagulation and the close observation for valve dysfunction in pediatric patients with St. Jude Medical valve in mitral position.
