The effects of labeling gap and susceptibility artifacts in pCASL perfusion MRI (pCASL 관류 영상에서 표지 간격과 자화감수성 인공물이 영상에 미치는 영향)

  • To report problems found in a patient who has implemented stent implantation and then conducted a perfusion MRI using ASL(Arterial Spin Labeling), in order to suggest a solution to them. The perfusion MRI was conducted, using pCASL among ASL methods. Data from pCASL(Pseudo Continuous Arterial Spin Labeling) was acquired together with the structural image simply by changing position(labeling gap 15 mm, 170 mm) of the labeling pulse to avoid stent. Data was processed through the ASLtbx. When perfusion MRI was acquired using pCASL, it showed that the position of the conventional labeling pulse (labeling gap 24 mm) was overlapped with that of stent, which made signal intensity in right brain tissue appear as if it were void. When the labeling pulse was positioned (labeling gap 15 mm) to avoid stent, high signal intensity images were acquired. In labeling pulse (labeling gap 170 mm), the signal intensity was more reduced due to relaxation before labeled blood arrived at the imaging slice. pCASL can be stably repeated measurements because it does not use a contrast agent. And it should be selected with the appropriate image acquisition parameters for the high quality image.

  • In this study, the performance of the technological innovation of small and medium enterprises is divided into short-term aspects of technological innovation and long-term aspects of ambidextrous innovation, and the effects of dynamic capabilities affecting these achievements are explored. For the purpose of conducting empirical analysis, the survey conducted a survey of 219 small businesses belonging to various industries, and dynamic capabilities performed a parallel multi-parameter analysis to demonstrate the effectiveness of technological innovation performance, including exploratory innovation and exploitative innovation. Through this study, it aims to understand the dynamic capabilities of the small and midsize enterprises and provide suggestions on how to manage of the company, thereby improving the understanding of dynamic capabilities and helping to improve the management of small and medium-sized enterprises.

  • Cardiac arrest is a series of conditions that occur when the heart is stopped, regardless of the cause. Cardiac arrest due to heart disease is included in the cause of death in korea every year and is unpredictable. One of the only ways to save a patient's life when a cardiac arrest is cardiopulmonary resuscitation is to maintain circulation through this procedure. The visual information is transmitted to the brain through the optic nerve. Among them, the mittor can see its movement and its shape, and it can be used for correction and analysis. Therefore compared the quality of chest compressions with visual information using mirrors. There was a significant difference in the mean depth of chest compressions($48.93{\pm}6.76$, $53.86{\pm}4.56$, <0.001), and there was also a difference in compression to relaxation ratio($0.87{\pm}0.13$, $0.96{\pm}0.10$, <0.002). There was also a significant difference in attitude awareness($4.93{\pm}0.85$, $8.14{\pm}1.38$, <0.001).

A Study on Quality Improvement of Cardiopulmonary Resuscitation Using Pad Attachment Glove (패드부착장갑을 이용한 심폐소생술 질 향상 연구)

  • This study was conducted to present effective CPR by identifying the usefulness of Pad Attachment Glove. The study subjects and data collection were conducted from 1 April 2019 to 30 April on Paramedic students of G university located in G-wide city and analyzed using the SPSS/Win 23.0. The study found that the chest compressions after wearing pad attachment gloves were higher in the areas of Chest Compressions total score, Number of chest compressions, chest recoil rate, hand position accuracy, CPR velocity than before wearing them. It was understood that pad attachment gloves improved the quality of chest compressions more than Hands-Only CPR. This will be used as a basic material for cardiopulmonary resuscitation education in the future, and is believed to contribute to devising a practical program.

  • Recurrence is the most common complication after shoulder instability surgery, and the main causes of the postoperative recurrence of instability are trauma, misdiagnosis, and technical errors. The risk factors of recurrence may be classified as patient related, anatomical or technical. Causes of failure should be thoroughly evaluated by meticulous history taking, physical examination, and imaging studies, and followed by proper treatment of pathologic lesions. Nonoperative treatment should be considered initially in cases of recurred instability after shoulder instability surgery, but if this fails, repeated recurrence is prevented by performing appropriate anatomical reconstruction of ruptured Bankart lesions, capsular laxities, glenoid deficiencies and humeral head bone defects.

A Study on Facial Expression Acting in Genre Drama - with Focus on K-Drama Voice2 - (장르 드라마에서의 표정연기연구 - 드라마 '보이스2'를 중심으로 -)

  • For the actors on video, facial expression acting can easily become 'forced facial expression' or 'over-acting'. Also, if self-restraint is emphasized too much, then it becomes 'flat acting' with insufficient emotions. By bringing forth questions in regard to such facial expression acting methods, this study analyzed the facial expression acting of the actors in genre dramas with strong commercial aspects. In conclusion, the facial expression acting methods of the actors in genre dramas were being conducted in a typical way. This means that in visual conventions of video acting, the aesthetic standard has become the important standard in the facial expression acting of the actors. In genre dramas, the emotions of the characters are often revealed in close-up shots. Within the close-up shot, the most important expressive medium in a 'zoomed-in face' is the 'pupil of the eye', and emotions are mostly expressed through the movements of the eye and muscles around it. The second most important expressive medium is the 'mouth'. The differences in the degree of opening and closing the mouth convey diverse emotions along with the expression of the 'eye'. In addition, tensions in the facial muscles greatly hinder the expression of emotions, and the movement of facial muscles must be minimized to prevent excessive wrinkles from forming on the surface of the face. Facial expressions are not completed just with the movement of the muscles. Ultimately, the movement of the muscle is the result of emotions. Facial expression acting takes place after having emotional feelings. For this, the actor needs to go through the process of 'personalization' of a character, such as 'emotional memory', 'concentration' and 'relaxation' which are psychological acting techniques of Stanislavsky. Also, the characteristics of close-up shots that visually reveal the 'inner world' should be recognized. In addition, it was discovered that the facial expression acting is the reaction acting that provides the important point in the unfolding of narratives, and that the method of facial expression and the size of the shots required for the actors are different depending on the roles of main and supporting characters.

Comparative Medium Term Results of Arthroscopic ACL Reconstruction with Quadrupled Semitendinosus Tendon versus BPB tendon (4겹의 반건양건과 골-슬개건-골을 이용한 관절경적 전방 십자 인대 재건술의 중기적 치료 결과 비교)

  • Purpose : The purpose of this study was to compare the postoperative success and stability of arthroscopically assisted anterior cruciate ligament(ACL) reconstructions using central one third bone-patellar-tendon bone(BPB) autograft versus quadrupled semitendinos-us(ST) autograft as the medium term review. Materials and Methods : Eighty patients(40 BPBs,40 STs) with isolated ACL injury were available for a mean follow up of 49.4 months(BPB) and 48.8 months(ST). There was no significant difference between the two groups with respect to age and sex. We compare the final results between two groups with respect to subjective Lysholm score, objective laxity including anterior drawer test, Lachman test, pivot shift test, KT-2000 measurements and International Knee Documentation Committee(IKDC) evaluation system. Results : Postoperatively, positive anterior drawer test was found in $22.5\%\;and\;27.5\%$, positive Lachman test was found in $30.0\%$ and $25.0\%$ and positive pivot shift test was found in $15\%\;and\;20\%$ of the ST and BPB group, respectively(p>0.05). Mean side to side difference of KT-2000 at 20 lbs was 2.2 mm for the ST group and 2.1 mm for the BPB group. There was no significant difference between the two groups about Lysholm score(>0.05). Anterior knee pain was knee common in the BPB group. Eighty-three percent of the patients were nearly normal according to the IKDC grade in both groups. Conclusion : We consider that autogenous semitendinosus tendon is a good alternative subsititute in ACL reconstruction together with the bone-patellar tendon-bone autograft.

Internal Fixation of Proximal Humerus Fracture with Polyaxial Angular Stable Locking Compression Plate in Patients Older Than 65 Years (65세 이상의 상완골 근위부 골절 환자에서 다축 각안정 잠김 압박 금속판을 이용한 내고정술)

  • Purpose: The clinical and radiographic outcomes of the internal fixation, which were executed on patients over the age of 65 with proximal humerus fracture by using a polyaxial angular stable locking compression plate (Non-Contact-Bridging proximal humerus plate, Zimmer, Switzerland, NCB), were evaluated. Materials and Methods: Thirty two patients over the age of 65 among the proximal humerus fracture treated with NCB plate, between August 2007 and January 2011, were chosen as the subjects. The average age of patients was 71 years, and the average postoperative follow-up period was 11.5 months. The fractures included 14 two-part and 18 three-part fractures. The clinical results were evaluated, using the visual analog scale (VAS) score and the Constant score. The radiological results were evaluated by time to union and Paavolainen method, which measures the neck shaft angle. Results: At the last follow-up examination, the mean VAS score was 3 points and the mean Constant score was 64.5 points, with bone union achieved after the average of 16.2 weeks following the surgery in all the cases. The mean neck shaft angle was 125.9 and 24 cases had good results, while 8 cases had fair results by Paavolainen method, at the last follow-up. There were 1 case of delayed union and cerclage wire failure, and 3 cases of subacromial impingement. There were no complications, such as loss of reduction, nonunion, screw loosening, or avascular necrosis of the humeral head. Conclusion: Internal fixation, using a NCB plate, was considered to be an effective surgical method in treating proximal humerus fracture in the elderly patients, on whom the fixation of the fracture and maintenance of reduction are difficult.

Comparison of Clinical Results and Second-Look Arthroscopy after Anterior Cruciate Ligament Reconstruction using Hamstring Tendon Autograft, Mixed graft and Tibialis Tendon Allograft (자가슬괵건, 혼합건 및 동종 경골건을 이용하여 실시한 전방십자인대 재건술후 임상결과 및 이차관절경 검사 비교)

  • Purpose: This study is to compare the clinical results of ACL reconstruction between three groups using hamstring tendon autograft, mixed and tibialis tendon allograft. Materials and Methods: Between August 2003 and August 2008, we analyzed 169 cases of ACL reconstruction, 66 cases used hamstring tendon autograft, 42 cases used mixed graft and 61 cases used tibialis tendon allograft, with a minimum follow-up of 12 months. For the clinical evaluation, we evaluated the Lysholm score, Telos stress test device and IKDC score. Results: The average side to side difference in Telos stress test decreased from $7.5{\pm}1.0$ mm to $1.6{\pm}1.0$ mm in autograft group, from $7.6{\pm}1.1$ mm to $1.4{\pm}1.1$ mm in mixed graft group and from $7.4{\pm}1.3$ mm to $2.5{\pm}1.3$ mm in allograft group. The average Lysholm knee score improved from 58.6 to 92.3 in autograft group, from 60.6 to 92.6 in mixed graft group and from 55.3 to 91.5 in allograft group. There was no significant difference between three groups in clinical results. At second look arthroscopy, tension of ligament and synovial coverage were good result in autograft and mixed graft than allograft group. Conclusion: All hamstring tendon autograft, mixed graft and tibialis tendon allograft groups showed satisfactory clinical results, with no significant difference in outcomes between the groups. Both hamstring tendon autograft and mixed graft showed good synovial coverage in second look arthroscopy. So mixed graft will be considered as good alternative in case of shorter or thin harvested hamstring tendon.
