Accuracy of CAD-CAM RPD framework according to manufacturing method: A literature review (국소의치 구조물(framework)의 CAD-CAM 제조방식에 따른 정확도: 문헌고찰)

  • Purpose. The purpose of this study was to evaluate the currently published literatures investigating the accuracy of computer-aided design and computer-aided manufacturing removable partial denture (CAD-CAM RPD) framework with different manufacturing techniques and methods. Materials and methods. A comprehensive search for literatures was conducted in PubMed database using specific keywords with the patient, intervention, comparison, and outcome (PICO) question, "Is there a difference in accuracy of RPD frameworks manufactured using digital workflow according to the manufacturing process and methods?" Results. A total of 7 articles were selected. Two studies compared intraoral scanning and laboratory scanning for RPD frameworks and had heterogenous results. In the studies using different manufacturing process, RPD frameworks had clinically acceptable accuracy in both subtractive and additive manufacturing. Polyetheretherketone (PEEK)-milled RPD frameworks showed higher fit accuracy than traditionally casted or 3D printed RPDs. Direct milling method showed a higher accuracy than indirect milling method. However, in rapid prototyping, indirect method showed higher accuracy than direct method. Conclusion. The RPD frameworks fabricated using CAD-CAM technology showed a clinically acceptable level of accuracy regardless of manufacturing process or techniques. Consistent results have not been reported regarding the digital impression methods, which were intra oral scanning or laboratory scanning, and further studies are needed.

  • The tongue is one of the most common sites of oral cancer. Glossectomy is known as the gold standard for tongue cancer treatment. However, surgical removal can lead to reduced mobility of the tongue and the patients may have difficulty performing normal oral functions like swallowing and pronunciation. Therefore, additional prosthetic consideration to supplement the function of the impaired tongue is needed for oral rehabilitation of such patients. Palatal augmentation prosthesis helps the tongue to reach the palate by lowering the position of the palatal polished surface. The oral functions of the patients with limited tongue mobility can be improved by the prosthesis. In this case, palatal augmented maxillary denture and conventional mandibular denture were fabricated for the completely edentulous patient with reduced tongue mobility after glossectomy due to tongue cancer. As a result, the oral functions of the patient were improved with the prosthesis.

  • Prosthodontic decision-making process is done through analysis of clinical information and mutual communication between patient and dentist. When opposing teeth are extruded due to missing tooth or tooth defects, selection of appropriate treatment plan and predictory prognosis can be complicated in functional rehabilitation. In case of severely disharmonious occlusal plane result from extruded teeth, re-establishment of the occlusal plane is required, if necessary via prudent evaluation. In this case, class III patient had unesthetic appearance and mastication discomfort caused by disharmonious occlusal plane due to severe extrusion. Through a structured diagnostic process, appropriate treatment plan was selected. Esthetic and functional results were obtained through from full mouth rehabilitation with re-establishment of the occlusal plane.

  • If the maxillomandibular relationship is unstable or requires treatment for improved function and esthetics, the first treatment objective is to provide stable occlusal contacts. The patient has moderate mental retardation. Due to inadequate vertical overjet, horizontal arch malrelation and excessive tooth loss, full mouth rehabilitation was required. The purpose of treatment planning is to establish physiologic maxillomandibular relation. Using cephalometrics for occlusal analysis is an aid to make a decision. The amount of horizontal bone reduction was calculated with evaluating soft tissue profile. After provisional denture placement, healing of the surgical site and newly adapted arch relationship was evaluated. The patient satisfied with esthetics and showed stable holding contacts. Periodic examination is needed to exclude occlusal disharmony.

  • Chemotherapy and radiotherapy used on pediatric oncology patients often causes dentofacial anomalies. Defects noted include tooth and root agenesis, root thinning, root shortening, localized enamel defect and maxillofacial underdevelopment. The effect of radiotherapy usually is confined to the radiation site but the effect of chemotherapy may be more wide spread becuase of its systemic distribution. Many pediatric cancers are treated with a combination of radiation and multiagent chemotherapy. Dental treatment affected by chemotherapy and radiation therapy damage to developing teeth and maxilloface includes retention of teeth, space maintenance, prosthetic considerations, requirements for oral hygiene.   • The purpose of this study, which was processed from March $22^{nd}$ to April $9^{th}$ of 2010, was to figure out recognition, well-formed instructions, and Practical application about dental hygiene devices based on the dental health characteristics of 350 adults who dwell in Busan and Gyeongnam regions. Also, this study recommended the adults to use dental hygiene devices and provided such devices to help individuals take care of their dental health at home. The collected data was statistically processed with a statistics SPSS (Statistical Packages for Social Science 14.0. SPSS Inc. USA) program. First of all, in order to demonstrate the characteristics of the sample population statistics and that of the descriptive data, frequency analysis was performed and to find out the relationship between variations of the dental health, Chi-squared test through Crosstabs was operated. According to the study, recognition and Practical application of dental hygiene devices were very low. Therefore, public announcement about necessity and effectiveness of such devices should be reinforced nationwide and instructions on choosing the right device and using of the dental hygiene devices based on patients' conditions by dental hygienic human efforts gathered from dental health organization should be carried out.

  • Prosthetic treatment using implants in fully edentulous patients includes implant-supported fixed prosthesis, implant hybrid prosthesis, implant retained- or supported-over-denture and implant supported fixed prosthesis has advantages such as psychological stability, pronunciation. If an implant supported fixed prosthesis is planned, the implants should be placed in consideration of pronunciation, esthetics, and oral hygiene. For this, clinical and radiological diagnosis is indispensable. When placing the prosthetic driven implant at the site determined from the diagnosis, a sufficient amount of alveolar bone and soft tissue support are required. If these requirements found to be insufficient, a wide range of bone grafting should be performed in advance. In this case, a fully edentulous patient with severe alveolar bone resorption due to periodontal disease was treated with a full mouth rehabilitation using implant-supported fixed prosthesis preceding maxillary sinus graft and alveolar bone augmentation. We report this patient were satisfied with esthetic and function.

Elderly Convalescence Facility Workers' Knowledge About Oral Health and Subjective Oral Health Care is the Degree of the Elderly (노인요양시설 종사자들의 구강보건 지식정도와 노인구강건강관리에 대한 인지정도 연구)

  • According to the result of examining the recognition on subjective oral health knowledge and elderly oral health management with a self-administered questionnaire survey conducted to 233 workers of elderly care facilities located in Gyeonggi-do, it was shown that their general oral health knowledge level was high while their elderly oral health knowledge was lower than general oral health knowledge. And it was found that respondents answering they had keen interest in the level of oral health knowledge according to the interest in elderly oral health showed a higher level of oral health knowledge than those answering they had no interest in it, which indicated statistically significant difference (p<.001). Regarding the study subjects' elderly oral health management and educational demand, quite a high percentage of 83,7% responded the education is needed. A response that it is appropriate for dental hygienists to give elderly oral health management and the education showed 57.9%, which was the most. Also, it was shown that in the level of oral health knowledge according to the recognition of elderly oral health managers and educators, subjects recognizing that it is needed to be dental hygienists indicated a high level of oral health knowledge, which showed statically significant difference (p<.05).

  • Soft tissue collapse around prepared teeth and pontic is inevitable after removal of the provisional restoration during the impression taking procedures. When inserting gingival retraction cord, soft tissue is displaced to an undesired contour. Viscosity of impression material also causes gingival displacement. Therefore, the consideration to transfer the prosthetically contoured soft tissue to master cast is required, especially in the esthetic area. In this report, the methods to maintain the soft tissue contour and transfer to the mastercast will be introduced. Harmonious contour of the soft tissue can be achieved with provisional restoration and be transferred to the master cast with two different techniques mentioned in this case report.

  • Decrease of occlusal vertical dimension (OVD) due to loss of teeth structure and destruction of the occlusal plane by severely worn dentition may cause cross bite or temporomandibular joint disorder by following change of facial feature or the loss of anterior guidance. Full mouth rehabilitation via an increase of the OVD can be considered to avoid this problem and proper evaluation of patient's OVD is essential. An 80 year old male visited for overall prosthodontic treatment, cross bite due to continuous wear and following decrease of the OVD were observed. We analyzed the existing occlusal relationship using the diagnostic cast, the radiographic evaluation and clinical test, and then proper increase of OVD was selected. The new OVD on diagnostic wax up was placed by the temporary restoration. After 3 months of observation period, final restoration with fixed partial dentures and implant overdenture were made. Throughout the follow-up period of 8 months, the aesthetic and functional improvement can be obtained.
