Fixed Prosthodontic Restorations after Forced Eruption of Traumatised Anterior Teeth (치아 정출술을 이용한 고정성 보철 수복)

  • A subgingival crown-root fracture presents a restorative problem to the clinician because restoration is complicated by the need to maintain the sound tooth structures. Forced eruption offers a method of treatment of teeth fractured close to the alveolar crest. Extrusion of such teeth allows elevating the fracture line above the epitherial attachment and so the proper finishing margins can be prepared. The purpose of this case is to report successful tooth restoration using forced eruption in case of crown-root fractures.

  • Statement of problem: As the number of elders is growing with the advancement of medicine, partially or fully edentulous patients have increased. Medically compromised conditions are common in the older population so that it should be taken into account in prosthetic treatment planning as well as their economic conditions. In the older patients, removable prosthesis has been preferred to implant prosthesis. However, cast metal based removable partial dentures also has several limitations. Purpose: In this report, we present several cases of Valplast$^{(R)}$ flexible denture which were fabricated in patients who had medically compromised conditions or whose remaining teeth showed a relatively poor prognosis. Results & Conclusion: This article describes an alternative treatment for a partially edentulous patient with mouth opening limitation, after cancer surgery, compromised general condition and questionable remaining teeth. In these patients, Valplast$^{(R)}$ flexible denture was used because of its unique characteristics and the results were all satisfactory. Patients had 1-2 check-up and there were no postoperative pain or fracture of denture up to now.

  • It often happens that a removable partial denture needs to be repaired due to tissue changes in the remaining alveolar ridge, fracture of the denture, or fracture of the abutment tooth. There are several advantages to retrofitting a customized surveyed crown under the existing RPD. Retrofitting a crown to the RPD decreases the economic burden to the patient and avoids the need for several appointments to fabricate a new RPD. It is difficult for artificial teeth used to repair dentures due to fractured natural teeth to have a shape similar to that of natural teeth, and to repair aesthetic artificial teeth, it is necessary to manufacture customized artificial teeth similar to the shape of each patient's teeth. Recently, CAD/CAM technology has been used to fabricate customized prosthetics on existing RPD to achieve high retention and fitness accuracy, and by manufacturing customized artificial teeth, more aesthetic and harmonious artificial tooth repair is possible. This is a case in which a denture was repaired using a digital method to fabricate a customized prosthesis on an existing partial denture and customized artificial teeth that mirrored the adjacent dentition, saving time and cost, simplifying the process, and achieving aesthetically and functionally satisfactory results.

Implant-retained overdentures with pre-fabricated bar attachment system in edentulous patients (무치악 환자에서 기성 조립식 bar를 이용한 임플란트 피개의치 증례)

  • Edentulous patients with severe alveolar bone resorption have trouble with using traditional complete denture. In order to overcome these problems, implant-retained overdenture was developed. SFI-bar$^{(R)}$ system can save time and cost compared to other existing bar systems which need complicated laboratory procedures because it can be adjusted directly in a patient's mouth. A 55-year-old male, who had experienced a fractured lower old implant-retained overdenture, wanted a durable and painless denture. The fractured Locator$^{(R)}$ attachments were removed and edentulous mandible was restored with SFI-bar$^{(R)}$. A 77-year-old female with a medical history of the Parkinson's disease and severely absorbed alveolar bone of mandible, wanted to wear a retentive mandibular denture without pain. After placing two implants in front of mental foramen, two adaptors were connected to two implants and a tube bar was connected to the adaptors. A female part fitted to the bar was attached to the new denture. These clinical reports describe two-implant-retained overdenture using the SFI-bar$^{(R)}$ system in mandibular edentulous patients. Since the patients were satisfied esthetically and functionally during 2 years' observation, we would like to report cases.

  • Periodontal and prosthodontic maintenance of implant overdenture is a very important factor for the long term success of the prosthesis and implants. Failure of maintenance can lead to prosthetic failure due to peri-implant bone loss and fracture and wear of the prosthesis or components. In this case, the existing gold milled bars were reconstructed with cobalt chrome milled bar in a manner that does not interfere with the external shape for the retreatment of fractured implant overdenture by maintenance failure. Two implants of mandible were selected strategically and the CM LOC attachments were connected to the two implants, and implant overdentures were fabricated. As a result, prosthesis with a functional and esthetic design that facilitates good hygiene management of the patient was delivered, which is advantageous for long term maintenance, and regular check-ups were scheduled for proper maintenance.

The effect of Zirconium Nitride coating on shear bond strength with denture base resin in Co-Cr alloy and titanium alloy (질화 지르코늄 코팅이 코발트 크롬 합금과 타이타늄 합금에서 의치상 레진과의 전단결합강도에 미치는 영향)

  • Purpose: The purpose of this study was to evaluate of Zirconium Nitride (ZrN) coating on shear bond strength with denture base resin in Co-Cr and Ti-6Al-4V alloy. Materials and Methods: Co-Cr and Ti-6Al-4V alloy disks (10 mm in diameter, 2.5 mm in thickness; each other: n = 14) were prepared and divided with 2 groups each other by ZrN coating. After primer was applied to disks surface, denture base resin with diameter 6 mm, height 5 mm was bonded on metal disk surface. After surface roughness was measured by Profiler, shear bond strength was determined with Universal testing machine and analyzed with two-way ANOVA. The specimen surfaces and failure mode were examined using a scanning electron microscope. Results: ZrN coated groups showed significantly higher rough surface than non-coated groups (P < 0.05). Irrespective of alloy materials, shear bond strength of ZrN coated groups were lower than non-coated groups (P < 0.001). The scanning electron microscope (SEM) of ZrN coated groups showed mixed and adhesive fractures. Conclusion: ZrN coating weakened bonding strength between denture base resin and Co-Cr, Ti-6Al-4V alloy.

Clinical evaluation of the removable partial dentures with implant fixed prostheses (임플란트 고정성 보철물을 이용한 가철성 국소의치의 합병증에 관한 임상적 평가)

  • Purpose: The purpose of this study was to identify clinical complications in removable partial denture (RPD) with implant-supported surveyed prostheses, and to analyze the factors associated with the complications such as location of the implant, splinting adjacent prostheses, the type of retentive clasps, Kennedy classification, and opposing dentition. Materials and Methods: A retrospective clinical study was carried out for 11 patients (7 male, 4 female), mean age of 67.5, who received RPD with Implant-supported surveyed prostheses between 2000 and 2016. The mechanical complications of 11 RPDs and 37 supporting implant prostheses and the state of natural teeth and peripheral soft tissue were examined. Then the factors associated with the complications were analyzed. Results: The average of 3.4 implant-supported prostheses were used for each RPD. Complications found during the follow-up period of an average of 42.1 months were in order of dislodgement of temporary cement-retained prostheses, opposing tooth fracture/mobility, screw fracture/loosening, clasp loosening, veneer porcelain fracture, marginal bone resorption and mobility of implant, artificial tooth fracture. Complications occurred more frequently in anterior region compared to posterior region, non-splinted prostheses compared to splinted prostheses, surveyed prostheses applied by wrought wire clasp compared to other clasps, and natural dentition compared to other removable prostheses as opposing dentition. There were no significant differences in complications according to the Kennedy classification. Conclusion: All implant-assisted RPD functioned successfully throughout the follow-up. However, further clinical studies are necessary because the clinical evidences are still not enough to guarantee the satisfactory prognosis of implant-assisted RPD for long-term result.

Causes of failures of long-term used double crown denture and new rehabilitation with dental implant and tooth combined denture using remaining teeth and implants (장기간 사용한 이중관 의치의 실패원인 및 자연치와 임플란트를 이용한 이중관 의치 수복증례)

  • Hybrid telescopic double crown have ever been good treatment option for patient with periodontally unfavorable few remaining teeth for successful prognosis. Tooth and implant combined telescopic double crown can be used for improving retention and support for denture with strategically placed implants on edentulous ridge. In this case, an 55-year-old female patient had chronic periodontitis with few remaining teeth on maxilla and fixed partial prosthodontics on mandible. Treatment of hybrid telescopic double crown with friction pin lasted 9.5 years only with natural teeth. After 9.5 years, additional implants was installed due to fracture of cast-posted abutment teeth. After implantation, tooth and implant combined double crown had fabricated. Through re-treatment, no complications of new denture has been found during 2 years follow up. This case presents fair prognosis of tooth and implant combined double crown denture in periodontally unfavorable condition.

The Fabrication of Accurate Removable Partial Denture by Analyzing the Failed Cases (실패 증례 분석을 통한 국소의치 제작 시 주의사항)

  • Removable partial denture for oral function recovery of partial edentulous patients is a complicated treatment because the dentist should consider both hard tissue and soft tissue. Removable partial denture produced without consideration of precise design for each component and rotation of denture may cause fracture, deformation or loss of abutment teeth. In case of failure to achieve a proper denture, the reason of failure should be analyzed and the result must be applied to later RPD cases. Finding the solutions through the failed denture cases will help patients use their dentures more comfortably and keep their sound residual teeth and tissue longer.
