Making profits of Health checkup by Marketing Activation (마케팅 활성화를 통한 건진 수익증대)

  • 문제: 2008년 수입예산의 편성을 위해 2개년도의 수입 구조를 비교 했을 때 검사공간의 부족외에 여러 내부적요인과 동일 권역에 경쟁 건진센터의 건립 등 외부적요인이 복합적으로 작용하여 수입의 감소가 예견되고 있는 상황이었다. 목적: 해결 가능한 내부적 요인을 찾아 시정하고 외부적 요인에 대해서도 적극적으로 대처하여 수입을 증가 시킬 방안을 실행한다. 의료기관: 서울에 소재한 대학병원의 건강증진센터 질 향상 활동: 수입을 감소 시키는 원인을 내부와 외부로 나누어 분석하고 개선 할 수 있는 방안을 찾아내고 실현가능 한 것을 시행하였다. 개선효과: 개선 활동 후 건강검진 수입이 전년대비 27%, 수입이 가장 높았던 해와 비교 할 때 17%가 증가 한 것으로 나타났다. 교훈: 수입감소를 유발 하는 요인들을 여러경로를 통해 문제점을 수집 및 분석하고 고객이 요구하는 것을 파악하는 것이 최우선이었다. 그 결과 장비 신규 구입과 건진센터 규모의 확대와 같은 시설과 장비의 개선, 해외의료홍보, 텔레마케터의 양성과 교육 등과 같이 여러부서에 걸쳐 협조가 필요한 사항과, 새로운 건진프로그램의 개발 및 영업력의 강화 등 부서 차원의 프로세스 개선과 교육을 통해 고객에게 한걸음 더 다가 가는 것이 가장 중요 한 요소로 나타났다.

Effects of job satisfaction and patients satisfaction on medical profit at public hospitals (공공병원 직원의 직무만족도 및 환자 만족도가 의료수익에 미치는 영향)

  • Purpose: This study was conducted to examine the factors associated with medical profit per bed related job satisfaction and patients satisfaction in public hospitals. Method: A cross-sectional survey was conducted through staff and patients in 39 hospitals (34 local province hospitals, and 5 red-cross hospitals) from September to October in 2011. A questionnaire was designed to collect information on job satisfaction, inpatients and outpatients satisfaction. The data of medical profit per bed was collected in 2011 annual financial reports from each 39 hospitals. Finally, data from 5,521 staff, 1,730 in-patients and 1,730 out-patients were analyzed. Multiple logistic regression analysis was performed to determine factors associated with medical profit per bed related job satisfaction and patients satisfaction in public hospitals. Results: The medical profit was positively correlated with job satisfaction of staff. As the results of multiple logistic regression, the significant variables of medical profit per bed, were communication satisfaction of staff(OR=0.48, 95% CI=0.25-0.91), hospital environment satisfaction of in-patients(OR=1.53, 95% CI=1.05-2.22). Conclusion: The satisfaction of patient and staff were significantly effect to medical profit in public hospitals. The Government and administrators have make an effort to improve satisfaction of staff and patients in public hospitals.

The health and medical statistics survey in Medical Records Offices required by the outside institutions (의무기록부서의 외부기관 통계지원 업무에 관한 연구)

  • This study was conducted in order to examine the quantity of health and medical statistics survey in Medical Records Offices which was required by outside institutions and whether it is profitable for the hospitals or not. The thirteen outside institutions required health and medical statistics of the 24 types of the survey to Medical Records Offices. 16.7% of health and medical statistics of the survey was paid to medical records offices such as the National Cancer Registration Survey, Patient Survey, Tuberculosis Patient Survey, and Hospital Discharge Patients Injury Survey. Medical Records Offices' total length of time for the health and medical statistics survey was over 200 hours per year like the National Cancer Registration Survey, Healthcare Accreditation System and Hospital Discharge Patients Injury Survey. The Medical Record Administrators in the hospitals with fewer than 500 beds work full time from 1 to 3. It is indicated in the study that it is necessary to improve the health and medical statistics survey system in Medical Records Offices required by the outside institutions and to employ additional Medical Record Administrator for more accurate Health and Medical Statistics Survey.

Regional difference between the distributions of dental revenues in metropolitan areas and rural areas: Empirical validation of the competition index (대도시와 농어촌에서 치과의료기관 의료수익 분포의 지역 간 차이 : 경쟁 지표에 대한 실증적 검증)

  • The increase rate of dentists' competition is very fast at metropolitan areas in South Korea. We compare metropolitan and rural parameters to investigate the relation between competition and revenue variation. The competition and revenue variables of 73 metropolitan and 75 rural areas were calculated from 2010 Census of Service Industry microdata which include non-insurance revenues of dental clinics. Independent sample t-test results showed that the level of competition among dental clinics in metropolitan areas is higher. The lowest and the low ranked revenues are higher in rural areas. The highest and the average revenues are higher in metropolitan areas. But, 25 percentile and median revenues has no significant difference between two areas. Simple log linear regression results showed that the number of clinics could explain the distribution of revenues in both areas better than the density of active dentists and Herfindahl-Hirschman index. In the areas with many clinics have high maximum and average revenues and low minimum revenues. The increasing rate of maximum revenues is higher in metropolitan areas though the decreasing rate of minimum revenues is higher in rural areas. Metropolitan areas have higher Gini coefficients than rural areas, but the increasing rate of Gini coefficients is lower than rural areas. Findings from this study are useful reference when the dentists select the opening areas. One is that the median revenues between metropolitan and rural areas have no significant difference. The other is that the rural areas ensure the more stable and uniform revenues. The results would help to relieve the consumptive competition among dentists and to achieve the distributional efficiency of dental human resources.

Review on Interchangeability between Efficiency Ranking and Profitability Ranking in Public Medical Centers (공공의료원의 효율성과 수익성 평가 지표의 대체 가능성 검토)

  • The public medical centers are required to make efficient and profitable operation. The efficiency is usually measured by DEA(data envelopment analysis), and profitability is measured by medical income rate. But DEA is measured by relative, not absolute value. So, profitability rate is used alternatively for DEA. This study want to analyze the interchangeability between DEA ranking and medical income rate ranking among public medical centers. The return on total assets is same ranking with DEA on bed number, employee number and total asset, but 60-70% relationship with DEA on bed number and employee number, as input resources. The operating margin is similar ranking with DEA on bed number and employee number, but 50-60% relationship with DEA on bed number, employee number and total asset, as input resources.

  • 무선 주파수 인식(RFID) 기술은 바코드 시스템과 마그네틱 카드 시스템이 우리 생활에 밀접하게 이용되고 있으나 생산 방식의 변화, 소비자 의식의 변화, 문화 및 기술의 진보, 바코드와 마그네틱 카드의 단점 해소 요구에 의해 개발된 시스템이다. 즉, 무선으로 사람, 물건, 동물 등을 인식, 추적, 식별할 수 있는 기술이다. RFID 카드는 다른 접촉식 카드와는 달리 이용자가 카드를 리더에 삽입하는 시간이 필요치 않으며 기계적인 접촉이 없기 때문에 마찰이나 손상이 없고 오염이나 환경의 영향이 적은 것이 특징이다. RFID 시스템은 공장 자동화, 교통 분야, 의료 분야, 레저나 시설에서도 많이 응용되어 새로운 수익을 창출할 수 있는 기술이 될 것으로 사료된다.

Medical Profit Planning Model and Innovative Strategies to Improve Profitability Management in Hospitals (병원의 수익성 관리개선을 위한 의료이익계획모형 및 혁신.합리화전략 개발연구)

  • This study discusses how to rationalize or innovate the managerial practice of the profitability-deteriorated hospitals in Korea, and attempts to suggest proper planning models and strategies to reorient them. For these purposes, the hospitals' financial indicators were analysed, the process of profit planning were reviewed, and strategic assessment were made, using relevant data. The analysis shows that failures both in proper capital investment to fixed assests and in effective containment of operating costs have been causing the worsening of profitability. For the improvement of the profitability management, seven procedural and behavioral strategies were suggested from the innovative and rationalizing perspectives, together with necessary prerequisite conditions to be equipped with for their implementation. This study concludes that the top management should attempt the changs on their own initiative.

  • This proposal of eco-friendly Resort wear targeting medical tourists of Jeju intend to contribute to the local economy by creating more added-values and profits. With high-quality leisure outfits, the medical tourism could provide tourists more pleasantness as well as a souvenior, which may help them to cherish the memory in Jeju. Well-developed Galot leisure outfits matched with other Galot products could result in additionary buying of other Galot items. Furthermore, by developing the size system as well as pattern grading, we can help local Galot manufacturers who currently require these standardized creation system. The summary of this research is as follows: 1. we examined the current issues and disadvantages of the local Galot through a survey. 2. We defined the Galot leisure outfit for Jeju medical tourism, conducted market research, and reflected these into representative designs for them. 3. We developed a size system as well as a pattern grading to standardize the manufacturing process.

The Job Stress and Mental Health of the Insurance Reviewer (보험심사 근무직의 직무스트레스와 정신건강)

  • The net function of the domestic medical insurance system is highly regarded, but due to the problem of incomplete coverage, the public wants to secure coverage through private medical insurance subscription. As a result, the subscription rate of private medical insurance has recently increased, and the billing rate has also increased. As the number of people seeking private medical insurance increased, workers at private medical insurance companies are experiencing increased job stress and side effects, especially for insurance reviewers who are in charge of paying insurance, such as communicating with customers who claimed insurance and contributing to the company's profit. In response, this study analyzed the effects of job stress on mental health of insurance reviewers and conducted a descriptive survey study to reduce job stress of insurance reviewers and promote mental health. The analysis shows that job stress for insurance reviewers has a significant impact on mental health (+). In detail, job stress has a significant impact on all four factors: social performance and self-confidence, depression, sleeping disturbance and anxiety, and general well-being and vitality. This study showed that job stress in insurance reviewers has a significant (+) impact on mental health. Job stress can cause side effects in organizational aspects, such as reducing enthusiasm for job performance and increasing turnover and resignation rates, but it can also worsen individual physical health and cause diseases such as depression and anxiety, causing mental health to be impoverished. Therefore, in order to prevent this, appropriate work stress prevention methods and countermeasures should be provided to help reduce work stress and improve mental health.

Comparison of Operational Efficiency and Quality Efficiency of Medical Services by Country : Focused on OECD Member Countries (국가별 의료서비스의 운영효율성과 품질효율성 비교: OECD 회원국들을 중심으로)

  • This study analyzed the efficiency of medical services in OECD member countries by dividing it into operational efficiency and quality efficiency. For this purpose, data from 2017-2019 OECD Health Statistics were used. As the analysis method, super efficiency was measured by applying an output-oriented Variable Returns to Scale (VRS) model. As a result of the analysis, Switzerland, Korea, and Italy were included in the high group of operational efficiency, Canada, Greece, Denmark, etc. in the medium group, and Belgium, Germany, and Spain in the low group. Based on quality efficiency, Norway, Switzerland, and Spain are in the high group, and Greece, Denmark, Mexico, etc. are in the medium group, and the Netherlands, Germany, Belgium, etc. were included in the low group. As a result of comparative analysis of efficiency by OECD member countries as of 2018, it was found that Korea's operational efficiency was the most efficient and quality efficiency was inefficient. Korea (0.998) should improve life expectancy by 0.2 (0.2%) and subjective health perception by 44.2 (138.1%) by benchmarking Greece (0.422), Switzerland (0.207), and Spain (0.371) to improve quality efficiency. Unlike most previous studies that focused on operational efficiency, this study measured quality efficiency together and analyzed the efficiency of the medical service industry in each OECD member country. Through this, this study has implications in that it confirmed the international competitiveness of the domestic medical service industry and suggested ways to improve efficiency.
