Variation Analysis of Medical Service Utilization in Oriental Medicine Frequent Disease of Rural Area (농어촌지역 한방 외래 다빈도 상병의 의료이용 변이분석)

  • The objectives of this study are to identify whether the small area variation also exists in the oriental medicine and, if it exists, what causes, to expand our boundary of research interests on the small area variation observed at the western medicine toward the oriental medicine as one of the fundamental research foundations and to provide any fundamental findings from this study results to the healthcare politicians to promote consumer's rational behaviors for the use of healthcare. This study analyzed the health insurance claim data (2010, 2011) which were the patients of western medicine and the outpatients of the oriental medicine with the top 10 most frequent diseases and looked into the variation of healthcare utilization among the areas after grouping resident area into an 86-area category. The study result shows that the small area variation was also observed at the part of the oriental medicine in which the characteristics of patients critically affect the healthcare expenditure per visit day rather than those of providers and the characteristics of both patients and providers equally affect the healthcare expenditure per patient. Therefore, this study suggests that government set up healthcare policies on the standardization of oriental medicine to prevent its over-utilization and unmet need, enforcing the roles of oriental medicine in the markets, enhancing the appropriate health care utilization, and expanding provision and sharing the health care information to reduce unnecessary health care utilization.

Factors Associated with Health Service Utilization of the Disabled Elderly in Korea (장애노인의 의료이용에 영향을 미치는 요인)

  • The purpose of this study is to examine the factors associated with the probability and levels of the health service utilization among the disabled elderly in Korea. The sample includes 2,111 people older than 65 who are extracted from the 2008 National Survey on People with Disabilities. More than half (54.3%) of the sample experienced at least 1 outpatient physician visit within two weeks and 26.7% were hospitalized within a year. The key factors associated with the outpatient visits were health insurance status, the existence of chronic disease, self-rated health, the Activities of Daily Living (ADLs), as well as renal impairment. Similarly, the utilization of inpatient care was related to health insurance status along with the existence of the internal organ disabilities such as cardiac or respiratory disorders. The study implies the need for the health care policies regarding the prevention of chronic diseases, dependency for daily activities of the elderly, and a management system that specifically targets those with internal organ disabilities. Moreover, the study suggests that financial supports for the low-income group would be helpful to increase their access to health service utilization.

  • u-Healthcare integration between ICT and healthcare service is able to utilize effectively for people's healthcare anywhere and any time. The purpose of this study is to explore the perception and influence factors on intention to use u-Healthcare service of physicians and staffs working for medical institutions. Ninety eight(98) valid cases have been collected for this study. It is also analysed by SPSS 18.0 and SmartPLS 2.0M3. Empirical findings provide important insights as follows: firstly, performance expectation, effort expectation and social influence positively influence intention to use u-Healthcare service; secondly, perceived risk negatively influences usage intention, and finally, performance expectation, effort expectation and perceived risk are moderated by voluntariness.

  • 본 논문에서는 응급 환자가 발생하여 가까운 병원이나 대형 병원에 도착하기 전에 환자의 정보를 이용하여 응급처치가 가능하도록 한다. 그리고 응급 의료 기관에서는 환자 진료 준비를 사전에 갖출 수 있도록 하여 응급 환자 진료의 효율성을 높였다. 우리가 흔히 연락을 쉽게 할 수 있는 119번호를 이용하여 다양한 복합 응급 신고를 접수하고 있고, 유관기관과 통합적 대응 체계를 구축하는 효율적 응급의료서비스 새로운 전략을 제시하고자 한다.

  • 병원의 의료 서비스에 대한 고객의 불만과 고충 내용을 분석하여 불만족을 파악함으로써 고객 불만족의 재발을 막고 이에 대한 해결방안을 제시하고 적용하는 고객만족 경영 전략을 수립하고자 분석한 결과 다음과 같다. 1. 조사대상자의 성별은 남자가 높은 분포를 보였고 계절별로는 환자측면과 보호자측면 모두 여름에 높았고 요일별로는 모든 측면에서 월요일에 불만족이 많았다. 이용형태별로는 환자측면은 외래가, 보호자측면은 입원이 높았고 발생부서별로는 환자측면과 보호자측면 모두 진료부에 대한 불만사항이 많았고 소리함을 통한 접수가 많았다. 2. 불만요일별 항목 특성 중 진료서비스 항목은 진료정 확성이 높은 분포를 보였고 보호자는 진료서비스에서, 환자는 절차서비스에서 높은 불만을 보였다. 3. 발생부서별로는 절차서비스는 진료지원에서 높은 점수가, 진료서비스는 진료부에서 높은 점수를 보였으며 친절서비스에서는 간호부가 높은 점수를 보였고 편의환경서비스에서는 행정부가 높은 점수를 보였으며 통계적으로 유의한 차이를 보였다.4. 불만요인별 상관관계의 결과는 진료서비스는 성별의 역 상관관계를 보였다. 친절서비스는 성별과 연령이 정상관 관계를 보였고, 진료서비스는 역상관 관계를 보였다. 절차서비스는 성별과 연령, 진료서비스, 친절서비스가 역상관 관계를 보였다. 편의환경서비스는 성별과 연령이 정상관 관계를 보였고, 진료서비스, 친절서비스, 절차서비스는 역상관 관계를 보였다. 현 의료서비스에 대한 고객의 불만 및 고충 내용이 병원 의료서비스의 질을 개선하는데 이바지할 수 있는 기초자료로써 그 의의가 있다.

The Effects of Medical Service Quality of the Nursing Hospital in Japan on the Inpatients' Satisfaction and Reuse Intention (일본 요양병원 의료서비스 품질이 입원환자 만족도와 재이용의도에 미치는 영향)

  • The purpose of this study is to examine the effect of the medical service quality of the nursing hospital on the inpatients' satisfaction, reuse intention and to verify the mediating effect of the inpatients' satisfaction. The subjects of this study were 255 male and female inpatients in Hiroshima in Japan. The collected data were used as a model of path coefficients obtained through analysis of covariance structure and hypothesis test. As a result of verification, level of medical team and medical service, reception, facility and fee of the medical service quality of the nursing hospital perceived by the inpatients showed a statistically significant positive correlation with the inpatients' satisfaction. The inpatients' satisfaction showed a statistically significant positive correlation with their reuse intention. The level of medical team and medical service, reception, facility and fee of the medical service quality of the nursing hospital perceived by the inpatients shows a statistically no significant positive correlation with the reuse intention. But, they show a statistically significant positive correlation with the reuse intention with the mediating effect of the inpatients' satisfaction. Finally, the result of this study is to confirm the effect of the medical service quality on the inpatients' satisfaction, reuse intention of the nursing hospital. In this process, it is meaningful to verify the role and function of medical service quality of the nursing hospital.

  • 본 논문에서는 병원과 외부서비스 간의 의료정보 교환 프로토콜을 설계하고, 설계한 프로토콜에 ECC 알고리즘을 적용하였다. 이를 위하여 관련연구로 CCR, ECC 알고리즘에 대해 분석하고, 의료정보 교환 프로토콜을 정의하였으며, 병원과 외부 서비스 간의 정보를 구분하고, ECC 알고리즘을 적용한 의료정보 교환 프로토콜을 설계하였다.

  • According to a rapid development of medical technology and increasing requirements of the users on prevention and control of diseases, whole healthcare services are changing into user-oriented services. There are many attempts and studies on integrating users' medical information but it is so difficult to implement a true user-oriented medical services because carrying the information from each medical facility to the integrated medical information system involves many conflicts of interests and authentication problems. This paper presents integrated medical information system which provides real-time medical services, allowing the users to be a critical player who can receive the medical information they want from any medical facility on their mobile devices without any change in the form of documents in relation to those issued by that facility and give that information themselves to the system through the mobile devices.

The Effected Factors on Customer Satisfaction of Medical Service and Willingness to Revisit among Selected Hospital Users in a Local City (일 지방 도시의 종합병원 이용자들의 의료서비스 만족도와 재이용 의사에 미치는 요인)

  • Objectives: This study was to find the effected factors on customer satisfaction for medical service and the willingness to revisit among hospital users Methods: The data was collected by a questionnaire survey from February 1 to April 30, 2004, and 600 samples have been analysed among users of university hospital, private hospital and public hospital in a local city. Results and Conclusions: The satisfaction total score to use hospital was 113.54 points(out of 175 point), these scores were constituted 39.10 points(out of 55 point) on satisfaction score for kindness of hospital employee, 36.28 points(out of 60 point) for equipment utilization and service formality, 18.59 points(out of 30 point) for environmental status and 19.57 points(out of 30 points) for reliability in medical examination and treatment service. The factors effected on satisfaction total score to use hospital were type of visiting hospital, age of customer, convenience to visit the hospital, experience of using other hospitals(R2=0.171). The effected factors of willingness to revisit scores were such as satisfaction score in medical examination and treatment service, satisfaction score of kindness hospital employee, experience of health examination and age of customer($R^2=0.370$). In conclusion, to raise the response's willingness to revisit. This must be reinforced by employee's kindness education and medical service quality.

  • Recently, various medical services are provided with the advance of IT. However, there is a problem that a third party would exploit medical information when the information is sent and received through wired or wireless connection. In this paper, a patient information management scheme using group index information for the third party not to illegally exploit a patient's medical information without his consent is proposed. This proposed scheme creates index information in each hierarchical level to be used with access information so that not only medical staff can have access to patient's medical information hierarchically but also it can manage access level in groups. The scheme aims to enable the medical staff to minimize the time spent to analyze the type of disease and to prescribe for it so that they can improve patients' satisfaction. Plus, the scheme aims to improve work efficiency by minimizing the medical staff's workload according to the authority to access patients' medical information.
