Quality Improvement Nurses' Experiences of Working for Healthcare Quality in Public Hospitals (공공병원 의료 질 관리 전담자의 의료 질 향상 활동 경험연구)

  • Purpose : To describe and understand the experience of QI (quality improvement) nurses related to the hospital QI activities in public general hospitals. Methods : Purposive sampling was conducted 10 QI nurses and who have QI work experience for more than 1 year. Data were collected through focus group interviews. Interviews were recorded, transcribed and analyzed with qualitative content analysis using Strauss and Corbin's methodology. Results : The core categories of experience with QI activities were 'the success and failure of the medical care and overall health service sector', 'the degree of activation of current QI activities', 'characteristics of public hospital QI activities', 'what is needed to activate future QI activities'. The key themes were derived as follows. 'Success of quality enhancement activities according to the characteristic of public hospitals', 'activation of public hospital QI activities through leadership and QI education', 'reorganizing the role of regional hospitals in public hospitals'. Conclusion : Physician participation is important in the success of QI activities in public hospital practice. To lead these physician participation, Sharing doctor's QI experience and providing the necessary knowledge in QI activities and helping their leadership in QI activities are needed. QI nurses at public hospitals should lead QI activities to improve national hospitals' care quality through cooperating with local hospitals.

우리나라 보건의료 정보체계와 정보화사업의 현황 및 문제

  • 국가 보건의료체계(health care system)가 올바르게 기능하기 위해서는 의료체계를 구성하는 각 요소들간의 유기적인 상호 작용이 대단히 중요하다. 정보체계는 이러한 유기적인 상호 작용을 위해 보건의료체계의 각 영역에서 꼭 필요한 시스템이다. 보건의료 서비스를 효율적으로 생산하기 위해서 투입하는 자원의 개발과 배치 체계는 물론 생산된 보건의료 서비스를 소비자가 효과적으로 이용할 수 있도록 하는 서비스 전달체계(delivery system)의 전과정에서 보건의료정보체계는 중요한 인프라가 되고 있다. 정보체계는 궁극적으로 보건의료체계가 지향하는 목적을 달성할 수 있도록 짜여져야 한다. 다시 말해, 의료체계의 각 하부영역에서 의료이용의 접근성(accessibility), 형평성(equity), 서비스의 의과학적 질(scientific technical quality), 포괄성(comprehensiveness) 등 보건의료체계가 추구하는 가치를 정보체계에서도 그대로 담아 낼 수 있어야 한다. 또한 정보체계를 구축하기 위한 요건으로 각 사업 영역별 표준화, 사업 영역간의 상호관계, 공공과 민간의 역할 분담, 정보 관련 시선과 인력, 정보기술 등의 인프라, 각 사업 영역별 정보화에서 담아야 할 정보 내용(contents, DB) 등에 대한 사전 기획이 수립되어야 한다. 그 동안 보건의료분야의 정보화사업은 1996년부터 보건소기관의 행정, 진료, 보건 업무지원을 위한 보건소정보화사업과, 1995년부터 초고속공공응용서비스 사업의 일환으로 추진해 온 전염병, 혈액유통, 장기이식, 외래진료예약, 원격진료, 응급의료, 원격치매진료 시스템 구축사업 등이 있으며, 응급의료, 혈액관리, 장기이식, 전염병정보, 외래진료예약시스템을 통합한 보건의료정보통합서비스시스템(1998년) 구축사업 등이 있었다. 이러한 사업들에 대해 현재 평가되고 있는 문제점들로는 정보화 사업에 대한 전략과 비젼이 부재한 상태에서 지나치게 정보기술 중심으로 추진되다 보니 보건의료 정보의 공유나 활용이 전혀 이루어지지 않거나 매우 제한적이었고, 시범사업의 경우는 초기 사업을 추진하기 위한 주체가 명확하지 않았고, 관련 인력의 훈련과 양성이 안 되었으며, 사업 확대를 위한 법적 기반의 취약, 정보화 사업이 부실한 데 대한 평가 기준과 대안 제시 등이 매우 취약한 것이 문제점으로 나타났다. 특히, 정보 내용과 DB 구축을 위한 표준화 등에 대한 사전 설계 부재는 개발된 정보시스템의 운영을 결정적으로 어렵게 만든 요인이 되고 있다. 전반적으로 기존의 정보화사업은 사업 추진을 위해서 꼭 필요한 비젼 수립, 비젼에 따른 목표 설정, 전략 개발, 관리체계 구축, 운영방안 수립 등 단계적인 정보화 계획이 부재한 것으로 평가되고 있어 보건의료정보체계 구축을 위한 종합 비젼 기획(ISP)이 시급한 실정이다.

Analysis of political conducts of the political players on privitization of healthcare service and public healthcare service after the democratization (민주화 이후 정치행위자들의 의료민영화 및 의료공공성 관련 정치적 행위 분석)

  • This paper analyzed experientially political conducts of the political players on privitization of healthcare service and public healthcare sercive aftrer the democratization in order to determine the influence of democracy on the development of a welfare state. For this purpose, the contents of the major daily newspaper reports from 1993 to 2012 were analyzed. As a result of such analysis, it was found that the political parties did not drive the policy even after the democratization reflecting the demands of the people, political parties had a strong conservative stance and did not show a consistent ideological inclination in their policies. Second, even though the role of the civic movement in the public healthcare service issue became larger after the democratization, policy decisions were driven by the government in an authoritative manner. In addition, the degree of contribution by the civic movements to the development of public healthcare service did not offset the influence of the president and the finance ministry on the development of privatization. Third, both the development of public healthcare service and privatization progressed simultaneously under the Kim Dae Jung, Rho Mu Hyun and Lee Myung Bak administrations after the democratization and it is thus difficult to argue that the qualitative nature as a welfare state was changed after the democratization. However, the degree of development as a welfare state differed depending on the ideological inclination of the presidents, the capacity of the welfare ministry and the ministry that drove the privatization.

Regional Hub Public Hospital Medical Service Quality and Customer Loyalty Relationship (지역거점 공공병원의 의료서비스 질과 고객충성도와의 관계)

  • This study is significant for its proposed health care quality improvement methods to study the relationship between the medical service quality and customer loyalty in the regional hub public hospital. The subjects were selected 604 people who visited the regional hub public hospital to customers from June 9 to June 30, 2014 as the final survey. The analysis was conducted hierarchical regression analysis to identify factors affecting the reliability factor analysis and customer loyalty. In conclusion showed that accessibility factors affecting customer loyalty, awareness hospital, the perceived value of cost-effectiveness, human services on a significant impact. The regional hub public hospitals by focusing efforts I have a continuity of customer care through constant management in order to provide quality health care, increase the frequency of use and loyalty are needed.

The Determinants and Medical Care Utilization Behavior of Private Health Insurance (민간의료보험의 가입요인과 의료이용행태)

  • This study analyzes actual conditions of private health insurance empirically by using the decision to join Korea Health Panel data. First, the study investigates how sociodemographic factors influence joining the private health insurance. Next, the relationship between the private health insurance membership and the frequency of medical service utilization is analyzed. The main results of these analyses can be summarized as follows: the socially vulnerable are less likely to join private health insurance relative to the middle and upper classes. The frequency of medical service utilization of private insurance members is relatively higher. This fact contains the possibility that the middle and upper classes are able to receive more medical services through private health insurance membership, while the socially vulnerable have difficulty with obtaining medical services in spite of much need. Therefore, policies are needed which manage the private health insurance more effectively and secure the publicity of health service socially.

  • I argue that the National University Hospital has to take its responsibility as the third health care service center for leading other public health care corporations, as well as its responsibility for educating, researching, and serving as a health center, as it written on its act of incorporation. As its management system had changed from a state-owned enterprise system to a self-supporting accounting system of a special corporation, the National University Hospital has pursued more heavily upon the profitability without showing any differences with the private university hospitals, even though the National University Hospital is a state-owned and public health care corporation. Therefore, after I have specifically analyzed the correlation between public service and profitability in a National University Hospital, I present my ideas on how to assign the public health care responsibilities to the National University Hospital in the current situation of pursuing heavily on profitability. Also how to partake in the service roles of the National University Hospitals in harmony with other health care corporations, and how to balance public service and profitability in the National University Hospitals.

  • 생체의료 및 보안검색 응용을 위한 테라헤르츠파 기술에 대하여 조사하였다. 테라헤르츠파는 기존의 X-선이나 초음파, 적외선, 밀리미터파 등에 비하여 독특한 특성을 가지고 있어 새로운 분광 및 영상기술을 확립할 가능성을 가지고 있다. 테라헤르츠 기술의 발전과 더불어 생체의료분야에서는 암진단, 치과질환 및 바이오칩 등에서 큰 시장을 형성할 것으로 전망되며, 보안검색분야는 공항, 항만, 지하철, 우편물 취급 등의 공공부문에서부터 군사목적에 이르기까지 응용이 가능하다. 최근 수 년 동안 소형, 경량 및 저가격의 테라헤르츠 시스템이 개발되면서 시장전망에 크게 높아졌다. 초기에는 공공시설 및 대형 병원에서 시작하여, 기술적 안정성 및 시장 활성화에 의한 경제성이 확보되면 민간 부문의 시장으로 크게 파급될 것으로 가대된다.

  • 국내 병원들은 의료서비스 시장 개방을 눈앞에 두고 있다. 의료서비스의 시장 개방은 의료 공공성을 유지하면서 의료서비스의 질 향상을 기대할 수 있는 반면, 의료비의 상승과 의료의 양극화 우려를 동시에 내포하고 있다. 의료산업에서 가장 중요한 것은 물론 의료서비스의 품질이다. 그러나 이제 글로벌 경쟁에 대비해야 하는 국내 상황에서 다양한 경영전략이 필요한 때이다. 여기서는 급변하는 병원산업에서 향후 허용될 것으로 예상되는 의료기관의 다양한 광고전략 중에서 정보기술을 기반으로 하는 브랜드 커뮤니티에 관련하여 병원이 대비해야 할 활용전략을 제시한다. 여타 산업에서의 브랜드 커뮤니티 성공요인과 사례를 바탕으로 병원종사자 뿐만 아니라 의료서비스 이용자들을 위한 의료산업에서의 온라인 커뮤니티 구축에 초점을 두었으며, 본 연구에서 제시하는 병원의 브랜드 커뮤니티를 활성화하는 6가지 요소는 운영자의 역량, 프리미엄 가치의 제공, 독특한 컨셉, 보안과 확장성이 보장된 장, 오프라인 모임, 그리고 다양한 홍보루트이다.

A Study on the Relationship between the Name Awareness of Public Medical Institutions and Patients' Intent to Revisit (공공의료기관 인지도와 재방문여부와의 관계 연구)

  • This study has significance in terms of its intent to contribute to the increase in hospital revisits by proposing measures to enhance name awareness through an examination of the relationship between the name awareness of public medical institutions and patients' revisit, and its corresponding influences. The study findings were drawn by employing analysis methods such as frequency analysis, reliability and exploratory factor analysis, t-tests, ANOVA, correlation analysis, and hierarchical multiple regression analysis. The results of the correlation analysis showed statistically significant correlations between all independent variables and the patients' revisit or not. The perception of service quality and the environmental factor of medical treatment increased the explanatory power of independent variables, thereby exhibiting a high correlation with the revisit to medical institutions. The usual image, medical service, and medical treatment environment were revealed to have a positive influence on the patient's intent to revisit. This study could confirm a correlation between the name awareness of medical public institutions and the patients' revisit or not. Consequently, to improve the quality and infrastructure of their medical services, public medical institutions are required to perform studies on measures to increase the revisits of patients by identifying their image, medical services, and treatment environment on a consistent basis.
