  • The purpose of this study is to propose the policy improvements of management evaluation system of municipal medical institutes. Management evaluation system of municipal medical institutes has been carried out every each year. This makes a great improvement in management practices. But in some respects, management evaluation system of municipal medical institutes has some problems in mechanism and measures. To overcome these, appraisal measures should be composed to improve medical efficiency. If we want to reconcile two objects which require both profit and public service, more appropriate measures should be designed. In addition, we should make measures to promote people work hard. For this purpose, we should make sure a more theoretical and practical ground back to evaluate the effectiveness of management consulting.

Performance Evaluation of Local Governmental Public Hospitals using Profit Analysis (이익분석을 통한 공공의료원 경영성과분석에 대한 일 접근)

  • The aim is to analyze a performance evaluation of local governmental public hospitals. Data were collected from public disclosure system DB of National Tax Service on 15 July 2013. Twenty-two income statements of local governmental public hospitals were analyzed using descriptive statistic analysis. The analyzing factors were medical operating profit, contribution margin, and trend of operating profit. The important result was that the medical operating profit was negative, average medical operating profit was -4,678.9 million won. However, the contribution margin was positive in all local governmental public hospitals, and average contribution margin was 12,572.5 million won. The trend of operating profit was positive in 15 local governmental public hospitals between 2011 and 2012. The average increased operating profit was 1,299.1 million won. Through this result, we suggest that not only individual public health hospital perspective, but also the enterprise wide perspective like nation was significantly considered to make decision of closure or maintain of local governmental public hospitals.

The Effects of Core Competencies on the Management Performance of Hospitals : With a Focus on the Mediating Roles of Medical Service Orientation and Internal Customer Orientation (의료기관 핵심역량과 경영성과의 구조적 인과관계 분석 : 의료서비스 지향성과 내부고객 지향성)

  • The Purpose of this study was to empirically investigate the effects of core competencies on the management performance of hospitals and the mediating roles of medical service orientation and internal customer orientation. The sample of this study was composed of 307 employees from 18 general hospitals in Incheon Metropolitan City and Bucheon City. Data were collected with self-administered questionnaires and analyzed using structural equation modelling technique. This study found that core competencies had positive effects on the management performance of hospitals and that medical service orientation and internal customer orientation mediated the relationship between core competencies and the management performance of hospitals. The theoretical and practical implications of the results were discussed and the directions for future research were suggested.

Operation Financing Method for Management Activities and Effect on Management Performance in Regional Public Hospitals (지방의료원의 경영활동 운영자금 조달방법과 경영성과에 미치는 영향)

  • This study used the financial information of 29 Regional public hospitals registered in the HASPA. Regional public hospitals analyzed the ratio of operating financing by management activities and how they affect net profit. The results of the study, Regional Public Hospitals ratio of fund financing for management activities was 83.50 percent for medical revenues, 9.53 percent for Incidental Revenue of Medical, 4.54 percent for Contribution Revenue, 4.42 percent for Other non-medical profits, 1.21 percent for depreciation Expense and 0.73 percent for Amount of reserve for unique purpose project. As a result of examining the effect of financing by management operation on net profit, the ratio of operating funds by medical revenue, by incidental revenue of medical, by contribution revenue and by other non-medical profits had a positive impact on net profit of Regional public hospitals. And It has been confirmed that the ratio of amount of reserve for unique purpose project has a negative impact on net profit. Therefore, it is necessary for Regional public hospitals to manage costs in consideration of revenues and internal reserves.

The association between the social presence and trust of chatbots and the sociodemographic characteristics of artificial intelligence chatbots users in general hospitals : focusing on sex and age (의료기관 인공지능 챗봇 이용자의 인구사회학적 특성과 챗봇의 사회적 실재감 및 신뢰감의 관련성 연구 - 성별과 연령 중심으로)

  • Objectives: This study explores the impact of age groups on social presence and trust among users of medical artificial intelligence chatbots. Furthermore, we investigate the existence of gender differences within these relationships. Method: We collected data through a survey from people who had interacted with general hospital chatbot services, either by making reservations or seeking consultations. Multiple linear regression analysis was conducted to examine the relationship between general characteristics of study population and social presence and trust of artificial intelligence chatbots. Additionally, we conducted stratified analysis to confirm the presence of gender differences within these relationship. Results: Among 300 participants, those aged 50 and older had higher social presence of artificial intelligence chatbots and greater trust of artificial intelligence chatbots (social presence, 𝛽=0.543, p=0.003; trust, 𝛽=0.787, p=0.000). In stratified by sex, women aged 50 and older had higher social presence and trust of artificial intelligence chatbots compared to those in their 30s age group (social presence, 𝛽 = 0.925, p=0.002; trust, 𝛽=0.645, p=:0.007). However, there was no statistically significant relationship between age and chatbot social presence and trust in men. Conclusion: This study demonstrates that advanced age plays a significant roles in users' social presence and trust in medical artificial intelligence chatbots. Futhermore, our findings reveal gender differences with women aged 50 and older showing the most substantial levels of social presence and trust. Therefore, it is expected that this finding can serve as valuable evidence to enhance the satisfaction of medical institution service users, offering crucial insights into the effective utilization of chatbot services.

  • 본 연구는 말콤볼드리지 의료서비스 분야 평가기준을 기반으로한 한국형 의료서비스 분야 국가품질상 평가도구의 타당성을 검증 하고자 한다. 이를 위해 미국의 말콤볼드리지 국가품질상(The Malcolm Bladrige National Quality Award)의 평가기준인 리더십, 전략계획, 환자 ${\cdot}$ 고객 ${\cdot}$ 시장 중시, 측정 ${\cdot}$ 분석 ${\cdot}$ 지식관리, 인적자원 중시, 프로세스 관리, 사업성과의 7 개 항목과 21 개의 세부항목을 토대로 국내 산업에 맞는 측정항목을 개발하였고, 이러한 평가항목들을 이용해 한국 의료서비스 기업들이 국가품질상에 대한 자체평가를 위해 활용할 수 있는 한국형 경영품질지수를 개발하고, 실제로 한국의 병원들의 사례를 통해 한국 의료서비스 산업 품질경영상태를 파악하고자 한다.

  • 일본의료법의 기본적인 내용은 1948년 제정 당시 그대로 이어서 인구의 고령화,질병구조의 변화,의학기술의 진보에 충분히 대응할수 없다는 문제점이 지적되어 왔다. 이에따라 1985년 의료계획의 창설을 골자로 하는 제1차 개정이 행하여 졌다. 이 개정은 의료공급체제 개혁에의 제1보로써 제1차 개정이라고 불려지고 1992년 행해진 제2차개정은 이에 계속된 제도개혁으로 지금까지 전혀 손대지 않았던 의료시설 기능의 세분화와 체계화에 깊이 손을 댄것으로써 앞으로의 병의원 경영에 있어서 커다란 전화점이 될 것이다. 제2차 개정의 목적은 양질의 의료를 효율적으로 제공하는 의료공급 체제의 확보에 있다. 개정의 주요 포인트는 특정기능 병원과 요양형병상군의 제도화이다. 지금까지 병원이라고 하면 모두 똑같은 체제이었으나 제2차 의료법의 개정으로 병원의 기능에 적합한 인원과 구조설비를 갖추는 것이 가능하게 되었다. 이번호에서는 1992년 4월 개정된 의료법의 내용과 배경을 설명하고 의료법 개정에 즈음하여 행해진 진료수가의 개정내용을 설명한 후에 차기 의료법 개정의 방향을 전망해 본다

Relationship between Relevance Index and Hospital Management Performance (지역 의료이용 친화도(RI)와 병원 경영성과의 관계)

  • The purpose of this study is to present the effective management strategy of RI based on the results of research on the causal relationship between the regional medical care capacity and the hospital management performance by calculating the RI of the general hospitals in Korea, This research has significance. The results of this study are as follows: First, statistically significant differences were found in Number of beds and recurring profits in urban areas by the general characteristics. Second, the correlations between the RI and the variables of the regional medical use are as follows: Recurring profit ratio, New Outpatient Visits, Operating Margin, Daily Outpatient Visits per 100 Beds, Daily Inpatient Days per 100 Beds, and Average Charge per Inpatient Day. Based on the results of this study, the significance of this study is as follows. First, we calculated the affinity for local medical use, which is the index of local medical utilization. Secondly, it is analyzed according to internal and external environmental factors such as city size, hospital size, etc. It can be said that the hospital provided basic data for establishment of hospital management strategy to increase the utilization rate of local medical care.

Complex relationship between Hospital management performance and the degree of the regional competition (Focusing on the Regional Public Hospital) (의료기관의 지역 내 경쟁정도와 경영성과간의 융복합적인 관계(지방의료원을 중심으로))

  • The purpose of this study is to measure the level of competition in the degree of Regional Public Hospital that can provide a basis for establishing an effective management strategy and analysis of the financial performance. Investigation period of the Regional Public Hospital HHI(Herfindahl-Hirschman Index) over three years from 2010, which was unknown until 2012, index and management performance evaluation criteria, a total of 31 were selected as Regional Public Hospital surveyed. In the conclusions, it showed that the most concentrated market with high financial performance, a significant correlation between the degree of local competition and financial indicators showed. The Regional Public Hospital for trends according to the degree of competitive local medical market and therefore is required to establish policies and practices to ensure competitiveness and public interest in the region. The Regional Public Hospital in order to preempt the lead for the competitiveness of other institutions will be considered in the strategy, such as improving medical services and capital investment.

A Study on Application for CRM Marketing and QI Activities in Healthcare Organizations (보건의료조직에 있어서 CRM 마케팅과 QI 활동의 적용에 관한 연구)

  • CRM의 장점은 원하는 고객에 대해서 마케팅 노력을 기울일 수 있고 그 결과로 마케팅 비용을 줄이고 효율을 높일 수 있는 것이다. 그리고 마케팅 전략 수립에 대한 방법의 변화와 그 수행방법의 변화가 CRM에 있기 때문에 여기에 따른 절차와 문화, 조직구조까지 변화 시킬 수 있다. 이를 방지하기 위하여 CRM교육이 필요하며, 그 중요성을 인식하고 실행하여야 한다. 그리고 경영환경에 적합한 마케팅 전략 수립이 필수적이다. CRM의 정확한 이해와 적합한 수행이 있어야만 병원경영의 목표달성에 도움이 된다. 그러나 CRM자체의 문제가 없는 것도 아니다. 무엇보다 전문 인력이 부족하면 시스템 구축과 마케팅이나 컨설팅 인력을 확보하여 프로젝트의 부실화를 방지하여야 한다. 다음은 실업자들의 업무패턴을 파악하고 사용자의 편의를 고려한 적용이 있어야 한다. 고가의 비용을 투입하여 CRM을 실행하였다 해도 사용자 교육의 미비가 생기면 효율이 감소될 것이다. 역시 CRM의 적용에 있어서 호환이 되지 않으면 통합비용으로 별도의 비용이 지출해야 하므로 비용의 증가를 감수하여야 한다. 환자의 의료의 질에 대한 만족은 환자의 주관적 개념으로 동일한 의료서비스에 대하여 환자들은 저마다 다르게 의료의 질을 평가하게 된다. 대부분의 환자들은 자신의 욕구와 의료의 질에 대한 기대를 가지고 병원을 방문하여 의료의 질을 평가하게 된다. 대부분의 환자들은 자신의 욕구와 의료의 질에 대한 기대를 가지고 병원을 방문하여, 그것을 의료제공자가 충족시키는 정도가 환자의 만족도를 결정하게 된다. 의료의 질은 의료서비스에 대한 만족과 밀접한 관련이 있다. 의료의 질과 만족은 강하게 연관이 있으면서도 양자는 분명히 다르다. 의료의 질에 대한 평가와 만족도는 매우 일치도가 높다고 볼 수 있다. 이것은 의료의 질적 수준이 높으면 환자만족도도 높다는 것을 의미한다. 따라서 본 연구는 병원의 환자관리를 위한 CRM 마케팅의 전기를 실증적으로 역설하고 이의 도입을 통한 효과와 문제점, 이에 대한 대응방안을 제시하고, 모범적인 CRM운영 사례를 제시함과 동시에 효과적인 의료의 질 경영을 구현하고 이것이 환자의 의료서비스 만족도를 높이는 결과인 것을 모색하는 것이 본 연구의 목적이다.
