A Survey on Emergency Medical Service of Cerebral Infarction (뇌경색 환자의 구급의료 실태)

  • 우리나라의 병원 전 응급의료체계는 비교적 짧은 기간 동안에 소방서를 축으로 운영되고 있다. 병원 전 단계에서 응급처치의 적절성을 높이기 위해서는 119구급대에 의한 응급처치에 대하여 체계적으로 조사를 시행함으로써 기초 응급의료에 대한 지속적인 질 관리를 해 나가는 것이 매우 중요하다. 특히 뇌경색은 급성기 치료를 위하여 혈전용해제 치료가 중요하며 증상 발현 후 치료가 시작되기까지의 소요시간이 매우 중요하므로 뇌경색 환자의 발생과 동시에 응급의료체계의 반응시간과 뇌경색을 인지하기 위한 환자평가 및 인지 후 응급처치 정도를 소방 119구급서비스를 통하여 살펴보고자 한다.

  • In the medical field, augmented reality is being used for surgical and medical education. However, augmented reality technology is not applied to emergency patients. In this paper, we propose a medical service support model that can support rapid medical service to emergency patients through an augmented reality - based IT device. The proposed model has the function of collecting the information necessary for the first aid simply through the IT equipment based on the reality of reality, and also receiving the first aid method appropriate for the emergency situation to the medical staff and supporting the service. In addition, the proposed model hierarchically collects information related to emergency patient information inquiry, emergency patient status and emergency treatment based on Analytic Hierarchy Process (AHP). The collected information uses a pair of comparison matrices to compensate for the ambiguity between the information. In particular, the collected information is stored in the server of the medical staff, and in addition to the unique information of the collected information, the collected information can be reflected in the medical service of the medical staff.

Smart App for Remote Medical Direction Support (원격 의료 지도 지원을 위한 스마트 앱)

  • In emergency situations, first aid workers are the main task of trauma evaluation and care. However, the scope of first-aid treatment is small and they are mainly carried out to the emergency room of the hospital. If a specialist who is in charge of an emergency situation is connected and emergency medical treatment through remote medical direction is performed, accurate diagnosis and appropriate care can be helped. This paper has developed an smart application(app) to support remote medical direction for emergency medical services. The developed smart app allows emergency rescuers to call a doctor at a remote location and transmit real-time status of emergency patients to vital sign and video. This will help to diagnose the patient's condition accurately. In addition, emergency care can be instructed and response in the emergency room can be made quickly.

  • Emergency care provides primary care and stability for the patient in the early stages. It saves a patient's life and minimizes the risk until that treated in a hospital. In the recent years, An attempts have been made to convergence medical technology and IT technology to overcome the limitations of temporal, spatial, and medical technology applications. In this study, we established an environment for emergency information services. The developed smart application for emergency rescue activities support can easily identify emergency situation and First Aid. Then, it can be transmitted to a remote medical guidance doctor so that emergency treatment and hospital response can be performed quickly.

  • 기존 연구에서 환자의 응급 상황이 발생했을 경우 병원에 도착하기 전 신속하고 정확한 체계적인 관리가 환자의 생명에 많은 영향을 미친다는 연구 결과가 보고되고 있다. 대부분의 어플리케이션에서는 많은 응급 상황에 대해 실시간 생체 신호들을 모니터링이 하기 위해 전송되는 생체 신호들은 많은 제한을 두고 있으므로 이런 생체 신호들을 무선 통신 기반에서 효율적인 설계가 요구되고 있다. 따라서 본 논문은 응급 상황의 이동 중에 환자의 생체 데이터와 환자의 영상을 실시간으로 전문가에게 전송하는 시스템을 설계함으로써 병원에 도착하기까지 숙련된 전문 의료 서비스 지원을 통해서 의사가 환자의 데이터를 분석 및 평가하고 치료 과정에서 환자의 처방을 즉각적으로 지시할 수 있다.

  • In the recent years, an attempts have been made to convergence medical technology and IT technology to overcome the limitations of temporal, spatial in the medical technology applications. Emergency care is primarily intended to provide primary care and stabilization to the patient, thereby saving the patient's life and minimizing the risk until hospital care. In this study, we developed an application S/W to support emergency services for emergency medical services. Paramedics can easily keep a log of emergency activities and monitor the patient's condition in real time with this application S/W on smart devices. In particular, medical signals and information emergency status of emergency patients can be measured or photographs of affected areas can be taken. In addition, this report can be delivered to remote medical doctors for rapid emergency measures and immediate hospital response.

  • 본 논문에서는 효율적인 환자 이송 및 치료를 위한 응급구조사와 의료기관 간의 의사소통 및 질 관리를 위한 EC&M (Efficient Communication and Management) 시스템을 제안한다. 이는 관련 앱을 효율적으로 사용하여 응급차 배치부터 병원 도착 시 환자 수속 작업 등을 최소화하고, 각 작업의 수행시간을 최소화 한다. 또한 이 시스템의 사용자는 소방청, 의료기관, 119 응급 구조사이고, 이 중 응급구조사에 초점을 두었다. 이는 PRE-KTAS를 소방청과 의료기관에 동시전달하여 전달된 PRE-KTAS를 통한 적절한 의료기관을 배치하고 선정된 응급의학 전문의에게 직접의료지도를 요청할 수 있다. 그리고 응급구조사는 의무기록활동지를 작성하여 의료기관에 전달하여 내용전달의 효율성을 높인다. 본 논문에서는 시뮬레이션을 통하여 제안하는 응급구조사와 의료기관 간의 정보 공유 시스템은 기존에 제안하는 앱을 사용하지 않고 발생했던 많은 문제를 해결 할 수 있을 것이므로 기존의 앱보다 우수할 것으로 기대된다.

  • Tele-medicine and emergency medical system are necessary for moving from an accidental point or far distance to a hospital and emergency treatment or home treatment before a hospital. Emergency treatment is extremely important in the case of death before arriving a hospital and deformed of disabled by medical treatment delay. A necessary element for this medical system is the emergency communication system. This system is on preparing for an ability of furnishing patient status to a corresponding health service by monitoring the patient at an ambulance of the accident place. This is the transportation of basic biological information of a patient to a medical center by wireless communication system and the corresponding hospital of medical center examine the patient by monitoring, then they can send emergency medical order to the patient for emergency treatment. The TRS is most efficient way of emergency medical communication system, which is currently used with popularity. In this paper studied simultaneously a way of detecting and transporting bio-logical signals, and monitoring of transporting data with communication of voice in the accident place of ambulance.

  • Recently, medical technology and IT technology have been trying to converge to overcome limitations of the time and the space in medical technology application. In this study, an app was developed to support remote medical direction for emergency medical services. The developed app allows doctors in remote locations to receive real-time emergency patient situations from emergency paramedics. It can also guide the patient's condition diagnosis and emergency treatment and enable rapid response from the emergency room.

  • 현재 응급 의료 시스템에서는 환자의 소재를 파악하기 힘들 뿐만 아니라, 환자의 이송과정에서도 환자를 치료할 수 없는 병원에 이송하는 등의 문제점을 지닌다. 또한 이송을 위한 대기중인 구급차가 없거나, 환자의 병명에 따른 시한을 지키지 못함으로써 환자를 위급한 상태까지 이르게 하는 경우도 존재한다. 본 논문은 이러한 문제를 해결하기 위해 자동화된 중앙 관리 체계를 기반으로 한 u-EMS의 시공간 환경과 행위를 명세 및 분석하기 위한 CARDMI기반의 HAP 스케줄러를 제안한다. 이를 통해 응급 의료 시스템에서 발생할 수 있는 문제점을 u-EMS를 통해 환자를 적시적소에 EMS서비스를 제공할 수 있도록 스케줄링 하였다.
