  • 본 논문에서는 실시간 데이터 분석을 통해 소아 응급 진료가 가능한 응급실을 안내함과 동시에 실시간 응급실 데이터베이스를 제공하여 응급실 진료 효율을 증진시키는 APP을 제안한다. 이 제안은 서울시의 응급실 이용 통계 중 야간 시간대와 소아 응급 진료율이 높다는 것에서 착안, 이를 효율적으로 안내 및 순환시키는 것을 목적으로 한다. 사용자의 연령 데이터를 수집하여 일반 응급실 환자와 소아 응급실 환자를 분류하며, 사용자의 위치 데이터를 수집하여 가까운 응급실의 정보를 제공한다. 또한 병원별 응급실의 실시간 혼잡도 현황을 제공한다.

The Effect of Information on the level of need fulfillment and anxiety of the emergency patient's family members (정보제공이 응급실 환자 가족의 요구 충족 및 불안 감소에 미치는 영향)

  • The purpose of this study was to examine the effect of nursing information on the level of need fulfillment and anxiety of the emergency patient's family members. The quasi-experimental study was designed using a noneqivalent control group non-synchronized design. During the first period, 30 subjects were assigned to the control group and 25 to the experimental group at a late period. The experimental group was provided with nursing information via guide booklet designed by the researcher. The control group received only routine care. Data was collected from January 31 to April 16 in 1996 at the K hospital in Taegu and analysed by chi-square test, t-test, ANOVA and Pearson correlation with SAS program. The instruments used for this study were the Family Needs Scale developed by Jung and the State-Trait Anxiety Inventory developed by Spielberger. The results of this study were summarized as follows : 1. The first hypothesis that the family members who received nursing information will have greater need fulfillment than family members who did not receive nursing information was supported. 2. The second hypothesis that the family members who received nursing information will have lower anxiety level than family members who did not receive nursing information was not supported. 3. The third hypothesis that the more the need of family member of emergency patient was met, the lower the anxiety level, was not supported. In conclusion, it was proved that nursing information about the emergency room provides family members with more need fulfillment, but did not decrease the anxiety level.

Clinical Characteristics of Asthmatic Patients Who Visited Emergency Room (응급실로 내원한 천식환자들의 임상적 특성)

  • Background : Despite remarkable progress of understanding the pathophysiology and therapy of bronchial asthma, asthma morbidity and mortality are on the rise. Also hospitalization and attending rates of emergency department for asthma have been increasing gradually. We analyzed clinical characteristics and prognosis of patients who visited emergency room due to asthma attack in order to define clinical characteristics of these group of patients. Method : We reviewed 105 adult asthmatic patients who attended emergency department of Korea University Hospital between August 1995 and July 1996, retrospectively. Results : 103 patients(56 female, 47 males, mean age : 48.6 years) attended-68 self referral, 18 practitioner referral and 17 OPD transfer- and 86 patients(83.5%) were admitted. Attending emergncy department was clearly more frequent in December(13.6%) and May(12.6%). Time lag between onset of asthmatic attack and arrival at the hospital was $14.2{\pm}15.5$ hour and initial peak expiratory flow rate was $166.7{\pm}68.3L/min$.(43.3% predicted) The commonest cause for visiting emergency room was aggravation of asthma due to upper respiratory tract infection in mild asthmatics. About half of them had history of previous ER visits. Their prognosis was not bad, but after discharge, about half of patients escaped from OPD follow-up. Conclusion : As a group they merit detailed attention and follow up arrangement. Clinician need to monitor and review the treatment plans, the medications, the patient's management technique, and the level of asthma control. For this group, plans for longer term treatment, including asthma education program and adjustment of overall treatment plan should be made.

  • Korea Centers for Disease Control and Prevention(CDC) has been provided not only manual of managing infectious patient but also functional requirement of space in emergency department(ED) by distributing "Guideline for infection control in emergency department(GICED)" in 2009. To understand how much the guideline enforces its functional requirement on ED planning practice, it is compared to Emergency Medical Service Act(EMSA) a basic standard for ED planning. As a result, it is clear that those have different focal point in functional program and don't share infection control issue. By reviewing target hospitals' EDs opened around 2009, all ED have satisfied with the EMSA requirement but guideline. Those are selectively adapted infection control related spaces CDC guideline suggested regardless of open year so that target EDs are not to be influenced by the guideline. This research can support as a reference research when the EMSA are going to be reinforced by infection contol issue.

  • 본 연구는 응급실 간호사의 직무 스트레스와 대처방안을 파악하여, 응급실 간호사의 스트레스를 보다 합리적으로 대처할 수 있는 방법을 모색하여 직무만족 및 효율적인 간호활동에 도움을 주기위한 자료를 제공하기 위한 것이다. 이를 위하여 스트레스와 동기부여 이론을 제 고찰해보고, 응급실 간호사들의 직무스트레스가 간호사들의 동기부여에 영향을 미치는지를 살펴보기 위한 개념적 모형을 제시하였다.

Factors Related to Emergency Department Healthcare Providers' Attitudes towards End-of-Life Care (응급실 의료진의 임종 돌봄태도에 미치는 영향 요인)

  • Purpose: The purpose of this study was to determine the factors that influence healthcare providers' attitude towards end-of-life care (EOLC) in the emergency department (ED) in hospital settings. Methods: From June 1 through June 30, 2014, a descriptive correlational study was performed with 41 doctors and 105 nurses stationed in the ED. Results: According to a regression model on the factors affecting healthcare providers' professional attitude towards EOLC, 28.1% of variance (F=15.185, P=0.000) was explained by awareness of death, gender and personal attitude towards EOLC. And 34.1% of the healthcare providers' personal attitude was related with awareness of death, experience of hospice education, occupations and professional attitude towards EOLC. Conclusion: This study demonstrated that attitude towards EOLC was influenced by awareness of death and personal characteristics. Healthcare providers in the ED should be provided with tailored training to improve their understanding of death. Also an educational program should be developed and provided to ED healthcare providers to improve their awareness of death.

  • The research about nursing students' experience on the clinical environment observation is essential to developed effective strategy for clinical education and to improve quality for clinical practice of nursing education. This study is aimed to above goal by the report about nursing students' experience on emergency room environmental observation. The number of participants is 30 nursing students who are senior in the practicing at emergence room. Data analysis was centered on scrutinizing the contents of the participants' replies in order to deduce any relevant meaning. Also, data were analyzed by the meaning of learning based on self-examination through the practical learning. The positive and negative topics stated. Nursing students' experience on emergency room observation will supply the thinking about their own self-portrait in near future, and will give self reflection about correct nursing interventions to nursing students.

  • Emergency room nurses whom exposed to more confused environment, violence, and medical disputes feel anxiety, due to increasing of demands and needs of emergency room services. Emergency room nurse anxiety needs to intervention in order to relieve emergency room nurses from anxiety, because it is an important factor that decreases job skill and satisfaction. In this paper, we examine whether to improve the effect of music intervention on emergency room nurse anxiety through appreciation of music. We used vocal music and a SAI(State Anxiety Inventory) tool for measuring the effect of music on emergency room nurse anxiety and physiological responses. Experimental results showed that vocal music intervention reduces emergency room nurse anxiety and physiological responses.

A Study on Operation Problems for the Emergency Medical Process Using Real-Time Data (실시간데이터를 활용한 응급의료 프로세스 운영에 관한 연구)

  • Recently, interest in improving the quality of EMS(emergency medical services) has been increasing. Much effort is being made to innovate the EMS process. The rapid progress of ICT technology has accelerated the automation or intelligence of EMS processes. This study suggests an emergency room management method based on real-time data considering resource utilization optimization, minimization of human error and enhancement of predictability of medical care. Emergency room operation indices - Emergency care index, Short stay index, Human error inducing index, Waiting patience index - are developed. And emergency room operation rules based on these indices are presented. Simulation was performed on a virtual emergency room to verify the effectiveness of the proposed operating rule. Simulation results showed excellent performance in terms of length of stay.

Relationships among professional identity, role conflict and job satisfaction of nurses in emergency department(ED) (응급실 간호사의 전문직 정체성, 역할갈등 및 직무만족도의 관계)

  • The purpose of this study was to determine the relationships among professional identity, role conflict and job satisfaction of nurses in and emergency department and to verify the factors that influence the job satisfaction. The subjects were 120 nurses in the emergency department of 2 tertiary hospitals and 5 secondary hospitals located in B city from July 1, 2013 to July 31, 2013. The professional identity and role conflict, and the professional identity and job satisfaction showed positive correlations. The factors influencing the job satisfaction were found to be the professional identity and 'hoping to work at an emergency department in the future'. The full explanatory power of the model was 13.0%. Therefore, participating in a symposium and specialty enhancing training will be needed to enhance the specialty of emergency department nurses.
