The Experience and Competence of Physicians Who Provide Emergency Health Care at Public Health Sub-Centers on Remote Islands in Korea (도서지역 보건지소 공중보건의사의 응급의료 경험 및 대처능력 고찰)

  • Objectives: To investigate the experience and competence of physicians providing emergency medical services at public health sub-centers on remote Korean islands. Methods: This study enrolled 79 doctors who work at public health sub-centers on remote Korean islands. Data were collected in December 2009 via self-administered e-mail questionnaires. The response rate was 44.3%. Results: Emergent situations occurred at most (58.68%) of the public health sub-centers that were surveyed in December 2009. An average of 1.92 cases required treatment by public health physicians. Only 20.25% of the physicians were specialists in emergency medicine, while the remainder were general practitioners (GPs) without clinical experience as emergency doctors. We also found that the physicians we surveyed had insufficient knowledge of emergency medical care. At some health centers only one doctor was available, and there was no medical team in holiday, although most of the physicians indicated that the ideal number of doctors per center was two or three. In cases of emergency, patients were often sent to the mainland by ship without receiving first-aid treatment. The public health sub-centers lacked the necessary medical equipment to save lives in emergencies and lacked escort systems for emergency patients. Conclusions: The Korean government should address the importance of providing emergency care in remote areas. Health administrators should provide suitable manpower, medical equipment, guidelines for emergency medicine, and education for public health physicians on remote islands.

  • 안전교육을 위한 방법으로 체험교육은 일반 국민들이 사고예방법이나 사고발생시 올바른 대처방법을 체험하게 함으로서 스스로를 안전하게 지킬 수 있는 능력을 습득하는데 매우효과적인 중재전략이다(강신광, 2003: 142-143 전세중, 2008). 또한 학생들을 대상으로 소방안전교육을 실시하고 있지만 교과서에 수록되어 있는 그림이나 이론 위주의 주입식 교육이 주를 이루고 있어 혼란스러운 응급 상황에서 올바르게 대응하기 위해서는 반드시 실제와 같은 가상 상황에서 반복적인 훈련을 통하여 체험할 수 있어야 한다(김현기, 2006 전세중, 2008). 그리하여 김태환(2001: 83-117)은 국민의 안전의식의 향상을 위한 안전 개선에는 주민 스스로가 안전사고에 대한 예방과 유사시 긴급대응을 할 수 있도록 안전에 대한 적극적이고 능동적인 체험교육을 위한 교육시설이 필요하다고 하였다. 안전체험관 설립을 위한 시민의식 조사 연구(김태환, 2001: 83-117)에서도 대부분의 응답자들은 안전사고 발생에 대한 불안감을 가지고 있으나 대처요령을 잘 모르고 있다고 함으로서, 주입식 안전교육의 한계를 극복하고 일반인의 안전의식 결여를 해결하기 위한 체험식 교육을 시행할 수 있는 재난안전체험관의 설립이 시급하다고 주장하였다. 재난발생시 위기상황에서 적용할 수 있는 현장 체험식 안전교육이 필요하다는 사회적 요구에 따라 2003년 최초로 안전종합체험관 형태의 서울시민안전체험관이 개관되었다. 이를 계기로 여러 지방자치 단체에서도 현장과 유사한 교육상황을 제공할 수 있는 안전체험교육의 필요성을 절감하고 잇달아 안전체험관을 설립하여 운영하고 있다.

The Development of X-ray Unit of Remote Emergency medical System (원격응급시스템에 적합한 X-ray 장치 개발)

  • The X-ray unit developed by this study is to diagnose emergency cases which is too far from a hospital and to classify the patients. We have to use the X-ray in a ambulance or the scene of an accident where we cannot use the AC220 outlet because of the distance from a hospital as well. The X-ray unit developed has a characteristics as follows. First of all, as the unit has a condenser in itself where there is no electric supply, we can use the X-ray inspector in a mountain area or a island. Second, we can detect by digital detector the information taken by X-ray from DC 12[V] electricity and store as a form of file. A control circuit can secure the reliability of the X-ray unit by using the Pic16F84A X-ray and provide various functions. The X-ray unit which suits remote emergency system can be efficiently used for the emergent cases who is too far from a doctor and a hospital or in the situation where it is difficult to diagnose, transcribe and treat simultaneously.

A Study on Situations of Safety Education in Universities and University Students' Awareness of Coping Methods for Safety Accidents (대학의 안전교육 현황 및 대학생의 안전사고 대처방법 인식에 관한 연구)

  • This study researched methods for coping with safety accidents by university students, after considering the situation of safety education by universities in Korea. For this study, a survey of 411 students of universities in the cities of Gwangju and Daejeon was conducted. According to the results, the ratio was high for answers such as willingness to participate in safety education beginning with the departments of engineering, art and physical training, education, art and humanity, social sciences and natural sciences, and medical sciences. It was found that university students most preferred first aid among the contents of safety education. University students of all departments answered that they had experienced safety education, but were afraid to be embarrassed in the case of actual safety accidents, as was found through the results of investigations of their awareness of coping methods with safety accidents. It was verified that many times their suffered skin and other wound in an earlier safety accident that they had experienced. This study suggests that safety education needs to be accomplished in the university curriculum, predicting and practicing for causes and factors of danger in advance, in order to improve the level of safety awareness and safety competence among university students.

Factors Affecting Posttraumatic Stress of Emergency Medical Personnels (구급대원의 외상성 스트레스 영향요인)

  • This study not only investigates emergency medical personnel's posttraumatic stress(PTS), social support, work burden, and coping style but also identifies related factors which were exerted influence on PTS and active coping method of emergency medical personnels among the firefighters. The data were gathered from 143 emergency medical personnels who were receiving training. The data were collected by IES-R, consisting of twenty-two questions that determine the level of PTS, method used in Cho's research(2000) that deal with work burden, method used in Oh's article (2006) relating to social support, and sixty-two questions that deals with coping methods. Then the data gathered were analyzed using SPSSWIN 14.0 program. PTS of general characteristics showed significant difference in age, marriage status, education, and position. High risk group of PTS was 74(51.7%). Work burden, active and passive coping method, and three symptoms in PTS are significantly high(p=0.000) in high risk group. PTS was correlated with work burden(r=0.508, p=0.000), active coping method(r=0.375, p=0.000), and passive coping method(r=0.505, p=0.000) but not with social support. Related factors of PTS were work burden(0.371) and passive coping method(0.366). Also related factors of high risk group of PTS were work burden(odds ratio=1.064, 95% confidence interval:1.031-1.103) and passive coping(odds ratio=1.050, 95% confidence interval:1.022-1.080). Related factors of active coping method were PTS(0.392) and social support(0.158). To minimize the PTS of emergency medical personnels, the new policy should decrease their work burden and passive coping method and strengthen the social support to encourage active coping method.

Use of Ketamine Hydrochloride for Pediatric Dental Patient at General Hospital (응급실에서 소아외상환자의 치과적 처치를 위한 케타민 진정법의 사용 현황)

  • Children commonly experience orofacial injuries and often need emergency treatment. Due to fear and anxiety, children tend to be uncooperative in emergency rooms. Ketamine hydrochloride is a well-known sedative agent at medical-based emergency rooms which has been used for procedural sedation. In this paper, we will discuss the sedation of uncooperative young patients, who needed dental treatments in the emergency room at Wonju Severance Christian Hospital, using ketamine. We collected the records of patients under 18-years-old who visited the emergency room for dental treatment from January 2010 to May 2014. The data was categorized by age, sex, required dental treatments and application of ketamine sedation. Among 659 pediatric patients who visited for emergency dental treatments, 118 patients were treated under sedation using ketamine. Majority of patients were under the age of 6 (110 patients), and the most frequent cause of sedation was suture of oral laceration (105 patients). Though ketamine should not be used by dentists alone, dentists in emergency rooms can easily meet the patients under deep sedation using ketamine. Hence, dentists in emergency rooms need to be aware of the clinical effects, considerations, and potential adverse effects of ketamine.

Age classification of emergency callers based on behavioral speech utterance characteristics (발화행태 특징을 활용한 응급상황 신고자 연령분류)

  • In this paper, we investigated the age classification from the speaker by analyzing the voice calls of the emergency center. We classified the adult and elderly from the call center calls using behavioral speech utterances and SVM(Support Vector Machine) which is a machine learning classifier. We selected two behavioral speech utterances through analysis of the call data from the emergency center: Silent Pause and Turn-taking latency. First, the criteria for age classification selected through analysis based on the behavioral speech utterances of the emergency call center and then it was significant(p <0.05) through statistical analysis. We analyzed 200 datasets (adult: 100, elderly: 100) by the 5 fold cross-validation using the SVM(Support Vector Machine) classifier. As a result, we achieved 70% accuracy using two behavioral speech utterances. It is higher accuracy than one behavioral speech utterance. These results can be suggested age classification as a new method which is used behavioral speech utterances and will be classified by combining acoustic information(MFCC) with new behavioral speech utterances of the real voice data in the further work. Furthermore, it will contribute to the development of the emergency situation judgment system related to the age classification.

Wearable oxygen saturation measurement platform for worker safety management (작업자의 안전관리를 위한 웨어러블 산소포화도 측정 플랫폼)

  • It is important to grasp biometric data in real time for prompt action in the event of a safety accident at a work site where the risk of safety accidents exists. Among them, blood oxygen saturation is the most important factor in maintaining human life, so real-time oxygen saturation measurement and monitoring is necessary according to the situation as a preemptive response for worker safety management. By receiving real-time bio-signals from workers wearing health and life-risk protective clothing, and sharing and analyzing the worker's risk status in an external system, it is possible to diagnose the worker's current condition and efficiently respond to emergencies that may occur to the worker. In this paper, we propose a wearable oxygen saturation measurement platform technology that can monitor the risk of harmful gases and oxygen saturation of the wearer in real time and ensure the wearer's activity and safety in order to cope with emergency situations at the scene of an accident. If we overcome the limitations identified through the results of the proposed system later and apply improved biodata such as motion correction to the platform, we expect that it will be usable not only in hazardous gas environments, but also in hospitals and homes for emergency patients.

  • 현재와 미래 정보통신 기술의 눈부신 발달에 따라 흥 네트워킹 산업의 발전과 에너지 시장 환경의 변화가 소비자 중심으로 이루어지고 있는 향후 미래 디지털 사회에서는 부하관리와, 정전이나 과부하같은 응급 상황을 대처할 수 있는 지능형 에너지 관리 시스템 개발이 필요하다. 따라서 많은 인공지능 기법을 이용해 에너지 관리 시스템 개발이 진행중이다. 그러나 대부분의 시스템은 에너지 관리 시스템에서 발생되는 대용량의 정보들을 고려하여 개발한 시스템은 거의 이루어지고 있지 않다. 이에 본 논문에서는 지능형 실시간 에이전트를 이용해 정기적으로 에너지 정보를 요구하는 이벤트와 비정기적으로 데이터를 요구하는 이벤트 및 계절별, 시간대별 및 요일별등으로 나누어, 요구되는 에너지 정보를 보다 효율적으로 처리할 수 있는 가법을 적용한 지능형 원격 에너지 관리 시스템을 설계하고 개발하였다.

인간환경 세미나-천연자원의 관리와 해양오염

  • 대기오염, 해양, 하천오염, 인구격증, 자연자원고갈 등 이른바 인간환경 문제는 금세기 인류가 극복해야 할 가장 심각하고도 철박한 도전임은 두 말 할 나위도 없다. 인류의 미래에 암영을 주고있는 이러한 환경의 도전에 적극 대처하기 위한 응급책은 선,후진국을 막론하고 시급한 요청이 아닐 수 없다. 이에「하나 밖에 없는 한국」이라는「슬로건」을 내걸고 인간환경세미나가 6월 스톡홀름 유엔인간환경회의에 앞서 지난 5월 23일 부터 동 25일까지 3일간 서울 아카데미하우스에서 개최된 것은 주지의 사실이지만 기간 여기서 발표된 논문 원고의 수집과 정리가 늦어져 이제야 본지에 실리게 된 것을 만시지탄이 없는 바는 아니나 매우 송구스럽게 생각한다. 그러나 독자제현도 과학기술인으로서 건망증을 탓할 것이 아니라 일상생활 주변에서 늘 듣는 공해문제를 상기하고 이에 대한 우려를 넘어서 적극적인 연구 검토를 빌어 마지 않는 바이다.
