The sonophoresis, a representative low-intensity ultrasonic therapy, is a technique for delivering the drugs into the epidermis, dermis and skin appendages by using physical vibration and heat effects of the ultrasonic waves. Sonophoresis could increases the delivering and absorption efficiency of the drugs usually consisting of hydrophilic molecules and macromolecules. In addition, it has the advantage of being effective in delivering drugs with relatively large molecular sizes such as insulin or lipid. In this study, we proposed a multi-structure ultrasonic transducer assembly with a large-size single piezoelectric element and a drug delivery function at the treatment site for efficient sonophoresis treatment. Futhermore, a transducer assembly structure capable of raising and maintaining the temperature of the treatment site was proposed and evaluated for effectiveness. The transducer assembly proposed in this study is expected to improve the efficiency of sonophoresis by providing a constant amount of drug, and assisting drug delivery through heating the treatment site.
To investigate the enhancing effects in transdermal permeation of drug using newly designed ultrasound apparatus of 500 kHz, the transdermal permeation studies through the hairless mouse skin were conducted with lidocaine. The ultrasound apparatus of 500 kHz frequency and transducer were newly developed. The drug permeation studies were performed according to the ultrasound frequencies such as 1 MHz and 500 kHz at $1W/cm^2$ in intensity in continuous mode or pulsed mode, respectively. The results on transdermal permeation of lidocaine according to ultrasound intensity showed that the drug permeation increased as the intensity was higher.
This study investigated effects of different sound frequencies on development and insecticide susceptibility of the green peach aphid, Myzus persicae. Under a constant sound intensity (95 dB), different frequency (0-5,000 Hz) sounds were treated on entire developmental period of the aphids. With increase of sound frequencies, nymphal to adult development of the aphids showed significant retardation and the females exhibited a significant decrease in fecundity. The greatest disrupting effects were induced by treatment of 5,000 Hz, which also caused marked change in protein expression of the aphids analysed by two dimensional electrophoresis. Sound frequencies above 2,000 Hz significantly increased susceptibility of the aphids against imidacloprid. This study clearly suggest that there is a stress sound, which can be sensed by M. persicae and induces its physiological alteration.

This study investigated the adverse effects of sound treatment on physiological processes of the American leafminer, Liriomyza trifolii, during several developmental stages. Larval feeding activity was analyzed by measuring feeding tunnel length. It was significantly suppressed by sound treatment (5,000 Hz, 95 dB). Sound treatment delayed the pupal period at 315 - 5,000 Hz and prevented adult emergence at 1,000 - 5,000 Hz. Female oviposition was also inhibited by the stress sound treatments. However, phototactic adult movement was not affected by sound treatment. Pupae treated with 5,000 Hz showed marked changes in protein patterns analyzed by two dimensional electrophoresis. MALDI-TOF analysis of specific protein spots indicated that trafficking protein particle complex I, triosephosphate isomerase, hypothetical protein TcasGA2_TC013388, polycystin-2, paraneoplastic neuronal antigen MA1, and tropomyosin I (isoform M) were predicted in the control insects and disappeared in the insects treated with sound. By contrast, DOCK9, cytoskeletal keratin II, and F0F1-ATP synthase beta subunit were predicted only in the sound-treated insects. Furthermore, stress sound significantly increased the susceptibility of L. trifolii to insecticides. These results suggest that physiological processes of L. trifolii are altered by sound stress, which may be exploited to develop a novel physical control tactic against L. trifolii.

This study analyzed effects of different sound treatments in frequencies and intensities on digestion and immune physiological processes of the beet armyworm, Spodoptera exigua larvae. Without effect on egg hatch, sound treatments with 100-5,000 Hz at 95 dB suppressed feeding behavior and inhibited a digestive enzyme activity. In addition, two dimensional electrophoresis of midgut luminal proteins indicated a marked difference of the sound-treated larvae. In response to 5,000 Hz at 95 dB, larvae showed a significant decrease in hemocyte nodule formation against fungal challenge along with significant suppression in phospholipase $A_2$ activity in hemocyte and plasma. With increase of sound frequencies, the treated larvae showed an enhanced susceptibility to insecticides. Such sound frequency effect was significantly modulated with different sound intensities. These results suggest that sound treatment may give adverse stress to physiological processes of S. exigua larvae and may be applied to a nonchemical insect pest control.

This study was to compared the effectiveness of piroxicam gel phonophoresis with conventional ultrasound therapy in knee osteoarthritis. Eighteen patients with a means age of $55.7{\pm}5$ years were randomly assigned to phonophoresis or ultrasound groups. Continuous ultrasonic waves of 1 MHz frequency and $1\;W/cm^2\;SATA_i$ were applied for 5 minutes to the target knee joint. This study indicated their pain level by marking on a visual analog scale(VAS) at the start of treatment and at the end of 3day. Osteoarthritis VAS scores, pain on rest state, non weight bearing range of motion, 20 meters walking, and walking step by the patients were evaluated before and after therapy. Both therapeutic modalities were found to be effective and generally well tolerated after 3 therapy sessions. But piroxicam phonophoresis was not superior to conventional ultrasound in patients with knee osteoarthritis.

The purpose of this study investigated the anesthetic effects of lidocaine gel by phonophoretic transdermal delivery. The anesthetic effects were evaluated by two aspects as quantitative sensory testing and sensory nerve conduction study. Twelve healthy males(aged $23.25{\pm}2.09$ years) were studied. Exclusion criteria were ; pain, history of sensory disturbances and skin conditions in the areas to be examined. The subjects were divided into two groups; group I(lidocaine gel without ultrasound) and group II(lidocaine gel with ultrasound). The following results were obtained; 1. In changes of tactile threshold and electrical pain threshold, all groups were significantly increased(p<0.05). 2. In changes of electrical pain threshold, it was significantly differenced between the groups(p<0.05). We conclude that the transdermal delivery of lidocaine gel by phonophoresis has a possibility to use for surface anesthesia and the pain control of the superficial tissue.
This study was performed that how phonophoresis using ultrasound for piroxicam affects transdermal permeation and anti-inflammative effects. Transdermal permeation study conducted by using hairless mouse had two categories: control group and ultrasound group. Transdermal permeation was observed according to duty cycle and intensity. Anti-inflammatory effects were determined using in Sprague-Dawley rat. The subjects were divided into three groups of six SD rat each 24 hour, 48 hour, 72 hour. The results of this study were as follows: 1. Transdermal permeation of piroxicam was measured according to ultrasound duty cycle. This research demonstrates that ultrasound group retains more transdermal permeation than control group, and that pulsed ultrasound group holds a little more transdermal permeation than continuous ultrasound group. 2. The transdermal permeation of piroxicam is closely related with ultrasound intensity. Effect of each group of transdermal permeation was significant rises in proportion to ultrasound intensity. 3. By observing inflammation of the tissue caused by trauma, phonophoresis group showed more significant of anti-inflammatory effect. The conclusion of phonophoresis was found to improve significantly the transdermal permeation and the anti-inflammatory effect.
