대구ㆍ경북지역 대장직장암환자의 식행동 특성

  • 본 연구는 대구ㆍ경북지역 대장직장암 환자의 일반 생활 상황과 식행동 특성을 분석 평가하여 지역민의 대장직장암 예방을 위한 영양교육에 활용할 수 있는 식생활 관련 기초 자료를 얻고자 수행되었다. 최근 경북대학교 병원에서 대장직장암 진단을 받고 입원한 수술 전후 환자 123명 (남 79명, 여 44명)을 환자군 대상으로 하였고 대장질환이 없는 정형외과 환자 등 182명(남 117명, 여 65명)을 비교군 대상으로 하였다. 대상자의 일반 특성, 식습관, 식행동 특성, 식품섭취 빈도 등의 내용을 포함한 설문지를 이용하여 개인 면담을 통하여 조사하였다. 현재 BMI와 과거 가장 높았던 체중의 BMI는 남녀 모두 환자군에서 유의하게 높게 나타났다. 환자군의 교육 수준은 비교군에 비해 낮았고 주 거주지역 분포는 농촌이 가장 많았으며 가계 소득은 환자군에서 비교군에 비하여 높은 것으로 나타났다. 주요 일상활동 강도는 비교군에 비해 환자군에서 유의적으로 가벼운 것으로 나타났다. 발병 전 일상생활에서 느끼는 스트레스의 정도가 환자군에서 비교군에 비해 유의적으로 높았던 것으로 나타났다. 자신의 성격 유형을 비관적인 것으로 생각하는 비율과 가족의 암 병력이 환자군에서 유의적으로 높게 나타났다 식사의 규칙성에서도 환자군에서 끼니를 거르는 비율이 유의적으로 높았다. 환자군은 비교군에 비해 육식을 유의적으로 높게 선호하였으며 외식시 고기류를 선호하는 비율이 높았다. 평소 물 섭취량은 환자군에서 비교군에 비해 적었다. 환자군이 비교군에 비해 짠맛에 대한 선호도가 유의적으로 높았고 단 맛과 매운 맛은 비교군에 비해 싫어하는 경향을 보였으며 기름진 맛을 선호하는 경향이 환자군에서 유의하게 높았다. 환자군에서 굽기, 튀기기를 좋아하는 경향이 유의적으로 높았다. 환자군에서 음주 빈도, 음주량 그리고 커피 섭취량이 높게 나타났으며, 1일 흡연량이 많았고, 흡연 시작 연령은 빨랐으며, 금연 연령은 늦어 흡연 기간이 유의적으로 긴 것으로 나타났다. 환자군의 영양지식 정도와 영양태도는 비교군에 비해 유의적으로 낮았고 환자군의 영양지식 수준이 높을수록 좋은 영양태도를 가지는 것으로 나타났다. 과거 10년간 식품 섭취 빈도 조사 결과 환자군은 등푸른 생선류와 콩류 및 된장, 두부를 비교군에 비해 유의적으로 더 많이 섭취하였고 해조류, 생 녹황색채소류, 익힌 녹황색 및 담색채소류와 녹차를 더 적게 섭취한 것으로 나타났다. 본 연구의 결과는 일상생활의 스트레스와 가족의 암 병력 및 비관적 성격 유형이 대장직장암 발병의 위험인자가 될 수 있음을 제시하며 육식 선호, 적은 양의 물 섭취, 짜고 기름진 맛에 대한 높은 기호도, 음주, 흡연, 커피의 섭취 그리고 나쁜 식습관이 대장직장암 발생과 관련된 식생활 위험인자가 될 수 있음을 제시한다. 해조류, 녹황색채소, 담색채소, 녹차가 대장직장암 발생의 보호인자의 가능성도 제시되었다. 따라서 향후 지역적 식문화 특성을 고려한 보다 광범위하고 체계적인 조사 연구를 통해 이 지역의 대장직장암 발생의 위험인자와 보호인자를 재확인할 필요가 있다고 보며 본 연구의 결과는 지역민의 대장직장암 예방을 위한 영양교육 자료로서 활용될 수 있다고 본다.

A Study on the Relationship between Health Food and Health-Related Factors by Residence and Sex in Tong-Yeong Area (거주지역 및 성에 따른 통영지역주민의 건강식품 이용실태 및 건강관련 제요인과의 관련성)

  • In order to investigate the relationship between intake conditions of health food and health-related factors by residence and sex in Tong-Young area, a survey was carried out from 1,303 adults. Health foods were classified 3 groups including vitamin and mineral supplements, toner foods and manufactured health food supplements. Health-related factors were stress, fatigue, smoking and drinking. The $29.5\%$ of the subjects had taken some health food for health. Especially the male took more toner foods habitually than the female did. In take of vitamin and mineral supplements by residence, there was a significant difference $(p\leq0.01)$ as follows. The subjects in island $(20.0\%)$ who took vitamin/mineral supplements were about two times as compared with the subjects in Dong $(10.8\%)$, or Eub-Myeon $(10.0\%)$. The subjects taking supplementary food replied over fair $(82.8\%)$, the subjects taking toner food replied over fair (90.3$\%$) scored higher than who replied bad or very bad in self-perceived health status. Therefore, the better the subjects felt self-perceived health status, the more they took health foods for health themselves. In self-perceived stress status, the subjects who replied a little $(50.0\%,\;45.3\%)$ or little $(19.9\%,\;26.4\%)$, took vitamin and mineral supplements or manufactured health foods a lot. In toner food there was a significant correlation $(p\leq0.05)$ as follows. The less the subjects felt stress, the more they took dietry supplement. No smoker $(12.9\%)$intake rate of vitamin and mineral supplements was higher than smoker $(8.8\%)$. Smokers $(6.5\%)$ intake rate of toner food was higher than no smoker $(4.0\%)$. It was not significant the relationship between intake condition of health food and drinking. The main motivation for taking health food were by self-decision and invitation of friends or neighbors.

Analysis of Correlation between Smoking and Gingivitis an Actual Condition of Oral Health Care in Some Vocational High School Boy Students (일부지역 실업계 고등학교 남학생의 구강관리실태 및 흡연과 치은염과의 상관성 분석)

  • The purpose of this study was to examine correlation between smoking and gingivitis in boy students targeting 281 students in the 2nd grade for vocational boys' high schools where are located in Cheongju city. As a result of carrying out questionnaire research and oral examination, the following conclusions were obtained. 1. The smoking-experience rate in subjects of this study accounted for 59.4%. The current smoking rate was indicated to be 41.3%. Students' average gingivitis prevalence accounted for 61.5%. Out of those students, the students with experience of smoking accounted for 44.8%. The average gingivitis index was $0.20{\pm}0.27$. Out of those students, the students with experience of smoking were indicated to be $0.29{\pm}0.31$. 2. In case of having experience of smoking, the probability of having gingivitis was high(p = 0.000). The more drinking experience led to the higher gingivitis prevalence(p = 0.010). 3. In the multi-variate analysis, a case of gingivitis prevalence was higher in the group with experience of smoking by 5.5 times(p = 0.000) compared to the non-smoking group. A case that the smoking volume for a day is over 6 pieces was higher by 8.4 times(p = 0.000) compared to the non-smoking group. A case of being over 25 months in the smoking period was higher 13.2 times(p = 0.000) compared to the non-smoking group.

Influence of Panax Ginseng on Hepatic DNA Synthesis in Mice (고려인삼이 마우스의 간조직 DNA 합성능에 미치는 영향( I ))

  • It was planned to evaluate the influence of Panax Ginseng upon hepatic DNA synthesis in mice by observing incorporation of $[^3H]$ thymidine into the tissue cells. Thirty male mice$(body\;weight:\;18{\sim}20\;g)$ were divided equally into the ginseng and the saline groups. Each animal of the ginseng and the saline groups received every day (subcutaneously) 0.05 m1/10 g body weight of ginseng extract (4mg of ginseng alcohol extract in 1 ml of saline) and the same amount of saline, respectively, for 5 days. On the 5th experimental day, all animals received $1\;{\mu}Ci/g$ body weight of $[^3H]$ thymidine intraperitoneally 2 hours after the last medication. Five animals, at a lime, of each group were sacrificed 1, 10, and 24 hours after thymidine administration, and their hepatic radioactivity was measured autoradiographically in terms of the % number of radioactive cells in 1,000 cell counts (Radioactive Index, R.I.). Following results were obtained: 1. The hepatic radioactive indices obtained from the saline group 1, 10, and 24 hour after $[^3H]$ thymidine administration were $3.23{\pm}0.23,\;5.20{\pm}0.21,\;and\;6.00{\pm}0.30\;(mean{\pm}S.D.)$, respectively. 2. The corresponding values obtained from the ginseng group $(4.22{\pm}0.33,\;6.32{\pm}0.32,\;and\;7.42{\pm}0.35)$ were significant higher than the values of the saline group. The inference from the above results was that the ginseng facilitated hepatic DNA synthesis.

Factors Influencing Compliance with Anti-Tuberculosis Therapy (폐결핵 환자의 치료 순응과 관련된 요인)

  • The purpose of the study was to determine factors influencing compliance with anti-tuberculosis therapy. The study subjects were 104 tuberculosis patients who have received the initial treatment in 3 health centers of Kyongju-City, Dalseong-Gun in Teagu and Kumi-City. Data were collected between September and October 1995. The patients were classified into the improved group and the non-improved group according to outcomes of 3 month treatment with short-term therapeutic regimen. To find factors influencing compliance with anti-tuberculosis therapy, multiple logistic regression was made. There was no significant differences between the improved group and the non-improved group in sex, age, education level, occupation, family pattern, and habitual change regarding smoking and drinking. The level of knowledge about anti-tuberculosis therapy in the improved group was significantly higher than the non-improved group(p<0.01). Multiple logistic regression analysis revealed that family support for not forgetting medication (p<0.05) wis a predictor of improvement and knowledge about anti-tuberculosis therapy(p=0.054), regularity of medication(p=0.062), and consultation to family, doctor and nurse(p=0.075) were marginal predictors of improvement. Treatment must be given to every patient confirmed as having tuberculosis and must be given free of charge to the patients. The requirements for adequate chemotherapy are prescribed in the correct dosage and taken regularly by the patient for a sufficient period to prevent relapse of the disease after cure. It is suggested that education to the patients should be reinforced and connectedness between patients and tuberculosis control workers and family should be solidated.

A Study on the Change of Contrast Sensitivity with Breath Alcohol Concentration in Various Luminance (다양한 휘도에서 호흡 알코올 농도에 따른 대비감도 변화 연구)

  • Purpose: This study was to investigate the effect of breath alcohol concentration (BrAC) increase with drinking alcohol on contrast sensitivity in the conditions of restricted BrAC. Methods: 23 males in 20s (average age $21.17{\pm}2.19$ years, body mass index (BMI) $22.09{\pm}2.16$) were selected and administered the amount of alcohol to reach 0.05% and 0.08% BrAC calculated by BAC (blood alcohol concentration) Dosing Software program, which was developed as basis of Watson's formula. Then, the contrast sensitivity in various luminance conditions (photopic, mesopic, and mesopic with glare) was measured and compared between these conditions. Results: The contrast sensitivity in all spatial frequency was decreased with BrAC increase. Although BrAC was increased, the peak of contrast sensitivity didn't change as 6 cycle per degree (cpd) in the photopic condition and 3 cpd in the mesopic condition, respectively. But, in the mesopic condition with glare, the peak of contrast sensitivity was shifted from 6 cpd at 0% and 0.05% BrAC to 3 cpd at 0.08% BrAC with increase of alcohol concentration. Conclusions: The increase of BrAC by drinking alcohol induces the decrease of contrast sensitivity in all spatial frequency and the shift of peak of contrast sensitivity, which can cause safety accidents, and may have an effect on various visual tasks.

A Study on Smoking among Korean Rural Residents (일부(一部) 농촌주민(農村住民)들의 흡연(吸煙)에 관(關)한 조사(調査))

  • The present study was conducted to investigate smoking status and the relationship of smoking to hypertension and diseases on 1,504 subjects who were over 30-year-old age in Kyungsan Gun, Kyungpook province, during the period of 12 months from March 1978 to February 1979. The results obtained are summarized as follows: Of 1,504 subjects, 52.2 percent smoked. The percentage of males smoking showed 84.8 and the percentage of females was 30.7. There were no statistically significant differences between the rate of smoking and educational levels. Protestants demonstrated lower rates of smoking compared with other groups. Regarding the age started to smoke, 66.4 precent of male smokers and 16.9 percent of female smokers commenced smoking before the age of 19. In the regard to the average number of cigarettes smoked daily, 45.8 percent of male smokers consumed some 20 cigarettes and 24.1 percent of female smokers consumed some 10 cigarettes. 3.2 percent of male smellers smoked 40 or more cigarettes a day. Alcohol consumption was closely associated with smoking. 90.7 percent of male alcohol drinkers and 60.1 percent of female drinkers smoked. Cigarette smokers have higher rates of ailments than nonsmokers as a whole. The complaints of cough with sputum and shortness of breath revealed higher rates among smokers than among nonsmokers. Frequency of illnesses showed higher for smokers than for nonsmokers. Smokers were more likely than nonsmokers to suffer from chronic obstructive lung diseases such as chronic bronchitis, asthma, and emphysema. Abnormal systolic blood pressure (>160mmHg) and diastolic (>100) revealed significantly higher among smokers than among nonsmokers.

Clinical Aspects in Patients with Thyrotoxic Periodic Hypokalemic Paralysis (갑상선 중독성 주기성마비 환자의 임상적 고찰)

  • Background: Thyrotoxic periodic paralysis is an uncommon illness characterized by intermittent flaccid paralysis of skeletal muscle, usually accompanied by hypokalemia, in patient with hyperthyroidism. However, the pathophysiology of thyrotoxic periodic paralysis remains largely unexplained and controversial. This report describes the clinical and biochemical findings in 19 patients with thyrotoxic periodic paralysis who were examined at the Yeungnam University Medical Center(YUMC) during the past decade. Methods: The medical records of 997 YUMC patients, seen between 1986 and 1996, with diagnosis of hyperthyroidism were reviewed. Nineteen patients out of 997 hyperthyroidism patients were diagnosed, and examined by history, physical examination, serum electrolyte value, and thyroid function test during paralysis. On the basis of these results, comparisons were made on age, sex, precipitating factors, timing, affected limbs, prognosis, serum potassium and serum phosphate and thyroid hormone levels. Results: The prevalence of periodic paralysis in hyperthyroidism was 1.9 percent and the male to female prevalence ratio was 30:1 and in all patients, the development of perodic paralysis was correlated with hyperfunctional state of the thyroid gland. Eleven cases of periodic paralysis were associated with hypokalemia and their thyroid hormone levels were significantly more increased than those of the patients without hypokalemia. Interestingly, our study shows the recurrence of paralysis after treatment. Conclusion: Although the precise pathophysiology of the disease is as yet undefined and controversial, it occurs primarily in Asians with an overwhelming male preponderance and prevalence of 2 percent in hyperthyroidism. The interactive roles of thyroid hormone, Na-K pump, and genetically inherited defect in the cellular membrane potential of the skeletal muscle can be speculated. Further investigation will be needed to firmly establish the mechanism of thyrotoxic periodic paralysis.

A Study on Health Condition and Nutritional Status of Female University Students in Masan Area (마산시 일부 여대생의 건강상태 및 영양섭취 실태 조사)

  • The purpose of this study was to evaluate the health condition and nutritional status in female university students in Masan. The 285 subjects aged 20.1 years (18~26 years) were assessed with a set of questionnaire composed of general information, food, smoking, drinking and exercise habits, anthropometric measurements, blood pressure measurements, 24-hour recall and food frequency questionnaire. The rates of smoking and alcohol drinking habits were 1.8% and 82.5%, respectively. All the subjects had the experience of passive smoking. The 11% of the students exercised regularly and 11.6% ate meal regularly. The average height and weight of subjects were 161.0 ㎝ and 53.9 ㎏, respectively and the BMI was calculated as 20.8${\pm}$2.6 ㎏/$m^2$:The 13.3% of subjects were underweight, while 6.7% of them were overweight. The mean value for body fat was 26.4% and subscapular and tricep skinfolds thickness were 14.2 ㎜ and 16.9 ㎜, and waist-hip ratio was 0.72. The systolic/diastolic blood pressure of the subjects was 115.9 ㎜Hg/70.9 ㎜Hg. Except protein, phosphorous, vitamin E, B_6 and C, the average intakes of energy and nutrients were below the Korea recom-mended dietary allowance (RDA). Especially, intakes of calcium, iron and zinc were the lowest, as <70% of RDA. The proportion of energy derived from carbohydrates : protein : fat was 58.0 : 15.8 : 26.2. The 10 frequently consumed food were ssalbap (47.2/mon), kimchi (35.6/mon), onion (16.8/mon), japgokbap (15.1/mon), sesame oil (14.1/mon), carbonated drink (13.3/mon), grapes (12.7/mon), ice cream (12.4/mon), milk (11.9/mon) and water-melon (10.6/mon). Results of this study could be useful for planning nutrition education programs for female university students in this area to improve their dietary habit and health status.
