Comparison of Exposure Dose by Using AEC Mode of Abdomen AP Study in Radiography (복부 전후 방향 검사의 자동노출제어 사용 시 선량 비교 연구)

  • We evaluated the effectiveness of TL (Time Limit) method by comparing with NTL (Non-time limit) method when it is used for examinations for abdomen Anterior Posterior (AP) in this paper. The evaluation was conducted based on the comparison of dose, and of signal to noise ratio (SNR) and contrast to ratio (CNR) on both methods. The experiments were conducted with XGEO GC 80 (Samsung, Korea), Unfors ThinX RAD (Unfors, Sweden) and Rando Phantom (Alderson research laboratories, USA) and shielding material with the size of $5.5{\times}9{\times}0.1cm^3$. It was set to activate only two upper ionization chambers in automatic exposure control(AEC) mode and the tube-voltage was set to 80kVp. When the exposure time was limited, it is limited to 51 msec. The images both by NTL AEC method and TL AEC method were acquired when with and without attachment of shielding material on the upper ionization chambers. The images were evaluated by SNR and CNR which are the image evaluation methods using 'Image J'. The NTL AEC method showed increases in dose as much as 130.7% at maximum and 80% at minimum than other methods. The TL AEC method showed decreases in mAs and exposure dose than the NTL AEC method as much as 43.8% and 44.4% respectively. There were no significant differences in SNR or CNR for the experiments (($p{\geq}0.05$). Therefore, it is suggested that the TLAEC mode is more effective when examining patients who have high BMI index or a patient with a metallic substance in the body after surgery.

The Role of Radiotherapy in the Treatment of Portal Vein Thrombosis from Advanced Hepatocellular Carcinoma (진행된 간세포암에서의 간문맥 혈전증에 대한 방사선치료 효과)

  • Purpose: To determine the role of radiotherapy for the treatment of portal vein thrombosis (PVT) from hepatocellular carcinoma (HCC). Material and Methods: A retrospective analysis was performed on 70 patients that had been diagnosed with HCC and were treated with three-dimensional conformal radiotherapy (3D-CRT) for the PVT. The radiation dose ranged from 40 Gy to 60 Gy (median dose: 48 Gy) and the biological effective dose (BED) ranged from 31.3 Gy to 78.0 $Gy_{10}$ (median dose: 61.6 $Gy_{10}$). Response was determined by measuring the extent of the PVT on a CT image at 0, 1 and 3 months after completion of the radiotherapy. The median follow-up period was 9 months. Results: The response rate was 47.1% (33 patients), with two patients (2.9%) showing a complete response, 31 patients (44.3%) showing a partial response, and 35 patients (50%) showing stable disease or no response. The 1-year progression-free survival rate was 60%, and the median progression-free survival time was 17 months. The median overall survival time was 11 months, the median survival time in the responders was 15 months and in the nonresponders was 8 months (p=0.032). Four patients (5.7%) had transient liver function impairment during treatment. Radiation induced liver disease (RILD) was observed in only one patient (1.4%). Conclusion: Three-dimensional conformal radiotherapy for the treatment of PVT from advanced HCC was a relatively effective and safe method.

Development of Predictive Model for Annual Mean Radon Concentration for Assessment of Annual Effective dose of Radon Exposure (라돈 노출 유효선량 평가를 위한 연간 평균 라돈 농도 예측모델 개발)

  • This research, sponsored by the Korean Ministry of Environment in 2014, was the first epidemiological study in Korea that investigated the health impact assessment of radon exposure. Its purpose was to construct a model that calculated the annual mean cumulative radon exposure concentrations, so that reliable conclusions could be drawn from environment-control group research. Radon causes chronic lung cancer. Therefore, the long-term measurement of radon exposure concentration, over one year, is needed in order to develop a health impact assessment for radon. Hence, based on the seasonal correction model suggested by Pinel et al.(1995), a predictive model of annual mean radon concentration was developed using the year-long seasonal measurement data from the National Institute of Environmental Research, the Korea Institute of Nuclear Safety, the Hanyang University Outdoor Radon Concentration Observatory, and the results from a 3-month (one season) survey, which is the official test method for radon measurement designated by the Korean Ministry of Environment. In addition, a model for evaluating the effective annual dose for radon was developed, using dosimetric methods. The model took into account the predictive model for annual mean radon concentrations and the activity characteristics of the residents.

  • The purpose of this study was to measure the absorbed dose and calculate the effective dose for cone beam computed tomography (CBCT) and panorama units and to estimate usefulness of x-ray protective. Rando phantom and glass dosimeters were used for dosimetry. The absorbed doses were measured at 15 organs and 14 remainder from correspond to ICRP 2007 recommendations. The absorbed dose was highest in salivary glands as measured CBCT 2.420mGy, panorama 0.307mGy. Absorbed dose in another organs were high in order of thyroid, brain, skin, esophagus. The effective dose was CBCT 0.100mSv, panorama 0.011mSv and effective dose of panorama was higher than that of CBCT by 10 times. In case of wearing x-ray protective, reducing effective dose of CBCT by 0.066mSv (66%) and panorama by 0.008mSv (72%). Effective dose were reduced by radiological shielding but it needs further optimization studies, where dosimetric data are analyzed in combination with image quality with keep the patients' exposure as low as possible.

Analysis of Radioactivity Concentrations in Cigarette Smoke and Tobacco Risk Assessment (담배연기와 담뱃잎 내 함유된 방사능 농도분석 및 위해도 평가)

  • In this study, radioactivity quantitative analysis was performed on radon contained in cigarette, and the effective dose was calculated using the result value to determine the amount of exposure caused by smoking. A total of 5 types of cigarettes were sampled. Cigarette smoke was collected by using activated carbon, and tobacco were measured by homogenizing for quantitative analysis. For each sample, Bi-214 and Pb-214 were subjected to gamma nuclide analysis to observe the uranium-based radioactive material contained in cigarette, and a measurement time of 30,000 seconds was set for the sample based on the results of previous studies. As a result of measuring the radioactivity of tobacco, a maximum of 0.715 Bq/kg was derived, and in the case of cigarette smoke measured using activated carbon, a maximum of 3.652 Bq/kg was derived. Using this measurement, the average effective dose to the lungs is 0.938 mSv/y, and it was found that there is a possibility of receiving exposure up to 1.099 mSv/y depending on the type of tobacco. It was found that the exposure dose due to cigarette occupies a large proportion of the annual effective dose limit for the general public. Therefore, more diverse studies on radioactive substances in cigarette are needed, and measures to monitor and reduce the incidental exposure to radon should be established.

Effect of Field Size on the Clinical Exposure Index for Lumbar Spine X-ray Examination (노출지수를 이용한 요추 X선 촬영의 조사야 유효성 평가)

  • The field size of the lumbar spine X-ray examination, which belongs to the most frequent examination in general radiography, is 5 times wider than the width of the lumbar spine. Exposure index (EI) as per International Electrotechnical Commission has a proportional relationship with the dose incident on the image receptor for clinical protocols in addition to RQA5, which is a calibration beam quality. In this study, the effectiveness of the set field size was evaluated through the change of EI according to the size of field during lumbar spine X-ray examinations. Lumbar anterior-posterior and lateral examinations was performed using a whole-body phantom, and the national average exposure conditions of Korea investigated in 2017 were introduced for the X-ray exposure. As a result of comparing the EI displayed on the console of digital radiography system for the three field size in ① 18 × 36 cm2 ② 25 × 36 cm2 ③ 36 × 36 cm2, the EI values showed a tendency to increase as the field size increased. Since the patient dose, such as organ dose around the lumbar spine, increases as the field size becomes larger, thus, if the EI obtained from the field size at a level that does not interfere with diagnosis is set as a reference, the effectiveness of the field size can be evaluated through the EI displayed on the console when the lumbar spine X-ray examination is conducted.

Evaluation for the Usefulness of Copper Filters according to Mode Change in Digital Radiography System (DR 시스템에서 모드 변화에 따른 구리필터의 유용성 평가)

  • This study confirmed the usefulness of the copper filter according to the mode change by comparing and analyzing the energy change according to the application of the copper filter and the change in effective dose and image quality according to the distance to the subject in the DR(Digital Radiography) system. The average energy increased when the copper filter was applied and the reduction rate by 50% of mAs was increased as the thickness of the copper filter increased according to the application of the 10 kVp rule in AEC mode. The effective dose decreased as the thickness increased when the copper filter was applied in AEC(Automatic Exposure Control) mode and manual mode according to the application of the 10 kVp rule, and the decrease rate decreased with increasing 10 kVp increments. As a result of analyzing the dicom images for AEC mode and manual mode with Image J. the PSNR(Peak Signal to Noise Ratio) values were approximate values of less than 30 dB for each mode and for each copper filter thickness. When the copper filter was applied, the average energy increased, so when the 10 kVp rule was applied, the mAs for each mode could be reduced, and the effective dose could also be reduced. However, as the distance and tube voltage increased, the reduction rate of mAs decreased, and the quality of the image was found to decrease when the copper filter was applied, but there was no difference in quality of the image when the copper filter thickness increased.

Evaluation of the Usefulness of MapPHAN for the Verification of Volumetric Modulated Arc Therapy Planning (용적세기조절회전치료 치료계획 확인에 사용되는 MapPHAN의 유용성 평가)

  • Purpose: Latest linear accelerator and the introduction of new measurement equipment to the agency that the introduction of this equipment in the future, by analyzing the process of confirming the usefulness of the preparation process for applying it in the clinical causes some problems, should be helpful. Materials and Methods: All measurements TrueBEAM STX (Varian, USA) was used, and a file specific to each energy, irradiation conditions, the dose distribution was calculated using a computerized treatment planning equipment (Eclipse ver 10.0.39, Varian, USA). Measuring performance and cause errors in MapCHECK 2 were analyzed and measured against. In order to verify the performance of the MapCHECK 2, 6X, 6X-FFF, 10X, 10X-FFF, 15X field size $10{\times}10$ cm, gantry $0^{\circ}$, $180^{\circ}$ direction was measured by the energy. Evaluation of Radiation effective dose by Naturally Radionuclides in the Soil of Busan (부산지역 토양 내 천연방사성핵종 분석 및 유효선량율 평가)

  • The presence of $^{238}U$, $^{232}Th$ and $^{40}K$, which are naturally residing radionuclides, in the ordinary soil of Busan, the 2nd largest city in Korea, was anlayzed and the residents' radiation exposure to ordinary soil was evaluated. Regarding the measurement methods, to conduct a detailed analysis of the naturally residing radionuclides in the soil of Busan, this study divided the 16 administrative districts into a lattice structure with 3 spots, and collected a total of 48 soil samples (July 2012 and April 2013). ICP-MS was used to analyze the concentration of the radioactivity of $^{238}U$ and $^{232}Th$ in the soil, and a HpGe detector, a gamma ray detector, was used to analyze the radioactivity of $^{40}K$. The measurement values of this study were compared with the concentration of radioactivity of East Asian regions. The concentration of $^{238}U$ nuclides in Korea was lower than the mean, whereas the concentration of $^{232}Th$ and $^{40}K$ nuclides was higher than the mean. The higher mean concentrations of $^{232}Th$ and $^{40}K$ than the mean were attributed to the many granite areas that contain a great deal of naturally occurring radionuclides.

Evaluation of Effective Dose and Exposure Levels of Radon in Office and Plant Buildings (일부 제조업 사업장의 사무 및 공장동에서의 라돈농도 수준 및 유효선량 평가)

  • Objectives: Radon may be second only to smoking as a cause of lung cancer. Radon is a colorless, tasteless radioactive gas that is formed via the radioactive decay of radium. Therefore, radon levels can build up based on the amount of radium contained in construction materials such as phospho-gypsum board or when ventilation rates are low. This study provides our findings from evaluation of radon gas at facilities and offices in an industrial complex. Methods: We evaluated the office rooms and processes of 12 manufacturing factories from May 14, 2014 to September 23, 2014. Short-term data were measured by using real-time monitoring detectors(Model 1030, Sun Nuclear Co., USA) indoors in the office buildings. The radon measurements were recorded at 30-minute intervals over approximately 48 hours. The limit of detection of this instrument is $3.7Bq/m^3$. Also, long-term data were measured by using ${\alpha}-track$ radon detectors(${\alpha}-track$, Rn-tech Co., Korea) in the office and factory buildings. Our detectors were exposed for over 90 days, resulting in a minimum detectable concentration of $7.4Bq/m^3$. Detectors were placed 150-220 cm above the floor. Results: Radon concentrations averaged $20.6{\pm}17.0Bq/m^3$($3.7-115.8Bq/m^3$) in the overall area. The monthly mean concentration of radon by building materials were in the order of gypsum>concrete>cement. Radon concentrations were measured using ${\alpha}-track$ in parallel with direct-reading radon detectors and the two metric methods for radon monitoring were compared. A t-test for the two sampling methods showed that there is no difference between the average radon concentrations(p<0.05). Most of the office buildings did not have central air-conditioning, but several rooms had window- or ceiling-mounted units. Employees could also open windows. The first, second and third floors were used mainly for office work. Conclusions: Radon levels measured during this assessment in the office rooms of buildings and processes in factories were well below the ICRP reference level of $1,000Bq/m^3$ for workplaces and also below the lower USEPA residential guideline of $148Bq/m^3$. The range of indoor annual effective dose due to radon exposure for workers working in the office and factory buildings was 0.01 to 1.45 mSv/yr. Construction materials such as phospho-gypsum board, concrete and cement were the main emission sources for workers' exposure.
