Assessment of Detection Rate Applying the Digital Convergence Mammographic Imaging Methods(FFDM, DBT, BMRI): Based on Breast Density (디지털 융복합 유방 영상 검사기법(FFDM, DBT, BMRI)을 사용한 검출률 평가 : 유방밀도에 근거하여)

  • This study aimed to assess detection rate applying the mammographic imaging methods. The evaluation of the shape and margin of tumor was conducted by score oriented to 180 patients who had undergone FFDM, DBT and BMRI scans among breast cancer patients, and the number of calcification was classified by size. There was no significant difference between DBT and BMRI for tumors larger than 1 cm in low-density breasts and for tumors larger than 2 cm in high-density breasts. Moreover, as for the detection rate of number of fine calcification, the highest rate was observed in FFDM followed by DBT and BMRI in the order named regardless of size. In conclusion, DBT was able to be detected even without BMRI if the mass was larger than 1 cm in the low-density breast and larger than 2 cm in the high-density breast. The detection rate of calcification turned out to be the highest in FFDM followed by DBT regardless of size, and calcification was not observed by BMRI. The appropriate use of FFDM, DBT and BMRI with respect to mass tumor will contribute to serving as a guide to examination methods of reducing burden of patients.

Application of Silver Ion for Clinical Mastitis in Holstein Cows (임상형 유방염을 가진 홀스테인(Holstein) 소에 대한 은 이온의 적용 연구)

  • Mastitis is an inflammation of the mammary gland that develops as a response to pathogenic microorganisms. Somatic cell count (SCC) and bacteriological analysis are the most accepted tools to determine udder health. This study evaluated the effects of intra-mammary gland infusion of silver ion against clinical mastitis in Holstein cows. Silver ion (20 ${\mu}g$/ml) was infused through the intra-mammary gland in quarters having clinical mastitis and milk was collected to determine SCC, and levels of lactoferrin and bovine serum albumin (BSA). Silver ion infusion decreased udder inflammation, firmness and swelling, and reduced clots, BSA, lactoferrin and SCC in milk. However, milk yield and circulating blood cells remained unchanged. The silver ion-mediated reductions of BSA and SCC indicate reduced inflammation and bacterial activity in silver ion-treated mammary glands in Holsteins with mastitis, which may be exploited in a curative strategy.

  • Objective.: Sparganosis locations in humans are usually presented with a subcutaneous tissue of abdominal wall, chest, abdominal vicera and brain, but are rarely found in the breast. Methods. A case of sparganosis was confirmed by surgical excision of two parasites in a 76-year-old female patient present to a palpable mass in the right breast (presumed to have been sparganosis approximately 3 years ago). She had no history to direct ingestion of snakes or frogs, but had the history of drinking contaminated water. Mammography, ultrasonography, MRI, and FDG PET/CT imaging findings for patient were characteristic of sparganosis due to suspicion of breast cancer. Conclusions: The first route of infection in humans is drinking contaminated water. The second route is the ingestion of raw or partially cooked snakes or frogs. The third route is infected wound snake, frog muscle that attach to the case. However, only a few cases of drinking contaminated water have been reported in the country. Ultrasonography, MRI is known to be helpful for diagnosis of breast sparganosis. However, Mammography, ultrasonography, MRI, and FDG PET/CT for breast sparganosis is not reported present in the country. Reported the case and reviewed the related literature briefly.

A Case Report of Intraductal Carcinoma Detected in a Patient Undergoing Surveillance for Benign Breast Mass (유방 양성 종괴 추적 관찰 환자에게서 발견된 관상피내암 증례 보고)

  • Breast ductal carcinoma in situ(DCIS) refers to cases in which breast epithelial cells have become malignant but are still limited to normal ducts, and has been increasing rapidly in recent years. In this case, a two-year follow-up revealed findings on mammography and ultrasonography that indicated a small mass classified as BI-RADS Category 3, However far from typical malignant tumor these findings were. In the second year of follow-up, a hypoechoic mass with an indistinct boundary of the right breast in the 6 o'clock direction, on mammography appeared to be about 2.1×1.3 cm in size, and biopsy diagnosed it as ductal carcinoma. Since ductal endothelial cancer has no characteristic clinical findings and can show positive clinical and imaging findings in the early stages, regular follow-up is considered important for early diagnosis, and detection of ductal endothelial cancer through mammography and ultrasound is important for improving the prognosis of all breast cancer patients. During the initial examination conducted four years ago, we reported cases of intra ductal cancer in which asymmetric shading, microcalcification, and branched mass, indicative of intra ductal cancer, were observed during follow-up. It is advisable to concurrently explore methods for reducing X-ray dosage to mitigate potential side effects of contrast medium.

  • 2002년 암 발생 통계 자료에 의하면 유방암은 현재 우리나라에서 여성암 발생률 1위를 차지하고 있다. 유방암 발생은 특히 식습관과 밀접한 관련을 가지며 따라서 여성의 유방암 예방을 위한 식생활지침의 마련이 시급한 실정이나, 이를 위한 기초자료들이 서구의 연구에 의존하고 있다. 식생활이 각 나라와 민족, 사회와 문화적인 환경에 따라 서로 상이함을 고려할 때 서구에서 보고된 자료를 우리나라에서 그대로 적용하는 데는 무리가 있을 것으로 보인다.(중략)

  • 유방 질만 분석을 위한 병리진단에서는 암종세포의 세관형성 정도와 핵의 다형성과 유사분말정도를 기준으로 하여 나누는 방법이 용이하고 재현성이 높다. 그 중에서도 세포핵의 크기와 다형성, 핵과 세포질의 비율은 양성종양과 악성종양을 분류하는데 있어서 중요한 요소중의 하나이다. 그러므로 본 논문에서는 유방 질단 영상에서 세포핵을 추출하여, 핵의 형태학적 특징값인 핵의 연적 둘레, 가로·세로(장·단축)의 길이를 구하고 핵과 세포질의 비율을 계산한 후, 추출한 형태학적 특성 칸들이 양성종양과 악성종양으로 분류하는데 있어서 유의한 특성 값인지 비교·분석하였다.

  • 체세포 수(SCC = somatic cell count)는 유방염과 밀접한 관계가 있는 생리적 반응의 표시이다. 그래서 유질과 유방염의 관리에 대단히 중요한 영향인자로 중요하게 사용된다. 현재 그 수에 의한 세계적인 평가는 나날이 연구의 성과가 보고되고 있다. 우리는 체세포 수로 많은 애로를 가지고 있으면서도 감소대책과 수에 대한 국제적 흐름에 민감하지 못한 것이 현실로 보인다. 이상과 같은 것을 감안하여 근래의 체세포 수와 유방염에 관한 정보를 제공하려고 한다.

Association of Prototheca with Bovine Mastitis (Prototheca에 의한 소 유방염 발생예)

  • 7세의 홀스타인 암소에서 엽록소가 없는 Prototheca에 의한 만성적인 진행성 유방 염의 발생에 대해 보고한다. 유방염 우유로 만든 습윤표본에서 명확한 세포벽과 내생포자를 갖고 있는 구형 내지 난원형의 병원체가 직접 관찰되었고, Sabouraud배지에서의 순수배양물 에 대해 Wright염색 및 PHOL염색을 했을 때 algae가 증명되었으며, nutrient배지에서 세균 이 중식하지 않아서 Prototheca가 유방염의 원인으로 제시되었다. 이 병원체는 자연상태에서 는 사물기생성이지만 감염원은 확실하지 않았다. algae와 진균의 형태학적인 연구를 하는데 있어서 PHOL염색액을 광범위하게 응용할 수 있음을 제시하였다. Prototheca의 ecology와 병원성을 규명하기 위해 더 많은 연구가 이루어져야 될 것으로 생각된다.

Eating patterns and use of nutritional information in breast cancer survivors treated with radiation therapy in South Korea (일반인과 유방암 환자간의 식행동 및 영양정보에 관한 인식조사)

  • The purposes of this study were 1) to investigate eating behaviors and patterns in breast cancer patients using a newly developed food frequency questionnaire and 2) to examine perception and use of nutritional information about breast cancer treatment among cancer patients treated with radiation therapy. Sixty breast cancer patients (case group) undergoing radiation therapy in Ajou University Hospital, Suwon, South Korea and 79 healthy women (control group) participated in this study. Mean age of subjects in the control group was $46.00{\pm}7.88$ years and BMI was $23.12{\pm}2.85kg/m^2$, and that of the case group was $50.06{\pm}11.64$ years and $22.32{\pm}3.24kg/m^2$. The results of eating behaviors showed several significant differences between control and case groups. Breast cancer patients ate meals on a more regular basis, on time, and more frequently compared to control subjects. In addition, they preferred more salty or spicy and bland food compared to healthy women. According to answers from the food frequency questionnaire, breast cancer patients consumed significantly lower amounts of boiled white rice, meats and processed food, fish and shellfish, coffee, milk, and cheese, whereas they consumed a significantly large amount of boiled multigrain rice, vegetable, seaweeds, soybean and processed food, and yoghurt compared to healthy women. This study also observed the way in which cancer patients and healthy control subjects obtain information about breast cancer treatment and its reliabilities. Results showed that healthy women did not hesitate to obtain information from mass media, while breast cancer patients would obtain nutritional information from specialists rather than mass media. Results of this survey confirmed that breast cancer patients avoided intake of red meat protein, even though they already recognized the importance of dietary protein intake for recuperation and treatment of the disease. These results could be used for future diet and nutrition guidelines for breast cancer patients.

  • Breast cancer is the most common form of cancer among korean woman. Therefore, the early detection activities of breast cancer such as breast self-examinations, clinical breast examinations, mammography are important. A yearly mammography examination has been recommended for women aged 40 and older for the early detection of breast cancer in asymptomatic periods. However, the glandular tissue of breast is the most radiation-sensitive tissue, and the determination of average glandular dose (AGD) forms an important part of the quality control of the mammographic systems. Because of the difficulty of estimating AGD directly, it is often estimated from the measurements of the incident air kerma and by applying the appropriate conversion factors. The primary objective of this study was to standardize the method of measuring AGD. The secondary objective was to evaluate the relationships between AGD per various composition and thickness of the breast using Monte Carlo simulations. As a result, we standardized the method of measuring AGD according to International Atomic Energy Agency (IAEA) guidelines (CoP: an international code of practice). Overall, AGD for mammographic practice in Korea was less than 3.0 mGy recommended by the Korea Food and Drug Adminstration (KFDA) protocol, and Korean Institute for Accreditation of Medical Image (KIAMI). The measured and simulated AGD for a given condition were calculated as 1.7 and 1.6 mGy, respectively. For the AGDs obtained, there was no significant difference between them. The simulated AGD was dependent on the fraction of glandular tissue of the breast. The AGD increases with increasing of the breast glandularity due to increasing absorption of low energy photons. The AGD also increases as a function of breast thickness. In conclusion, the results of this study could be used as a baseline to establish a reference level of radiation dose in mammography.
