Malignant Pancreatic Intraductal Papillary Mucinous Neoplasm with Splenic Invasion: A Case Report (악성 췌관내 유두상 점액성 종양의 비장 침범: 증례 보고)

  • Intraductal papillary mucinous neoplasm (IPMN) is a tumor originating from pancreatic ductal epithelial cells, leading to excessive mucus secretion and dilation of the pancreatic duct. Pathologically, IPMNs display a wide range of dysplasia, ranging from low-grade dysplasia to invasive carcinoma. Invasion into surrounding organs, especially into spleen, is rare and has not been reported in Korean journals. Worldwide, only two cases have been reported. Here, the authors report their experience with a rare case of IPMN in the pancreas that invaded the spleen.

Immunohistochemical Diagnosis of Renal Pelvis Transitional Cell Carcinoma in a Dog (개의 신우 이행상피암종의 면역조직화학적 진단)

  • A 9-year-old, male, Doberman pinscher dog with 5-month history of intermittent hematuria, vomiting and glucosuria was referred to local animal hospital. Abdominal ultrasonography showed an irregular and hyperechoic mass in the renal medulla of the enlarged left kidney. Grossly atrophied renal cortex and medulla and marked hydronephrosis were observed on the cut surface of kidney. A single, numerous papillary projected, pedunculated mass 4~5.5 cm in diameter was occupied in renal pelvis, and extended from pelvis to the inlet of ureter. Histopathologically, the mass had numerous papillary structures with arboriform pattern. These papillae were consisted of fibro-vascular stalks that were lined by multiple layers of neoplastic urothelium (transitional epithelium) with marked cellular atypia. Immnohistochemical (IHC) staining demonstrated that the neoplastic cells showed strong positive reactions for cytokeratin (CK) 7, CK 19, CK clone MNF116 and CK high molecular weight, but negative signals for CK 8 low molecular weight. Based on the gross findings, histopathology and CKs profile using IHC staining, this mass was diagnosed as renal pelvis transitional cell carcinoma in a dog.

  • Solid papillary carcinoma of the breast is a distinctive form of intraductal papillary carcinoma frequently associated with both mucinous carcinoma and infiltrating ductal carcinoma, not otherwise specific. To our knowledge, this case is the first description of the cytologic aspects of solid papillary carcinoma of the breast in the Korean literature. We experienced a case of solid papillary carcinoma of the right breast diagnosed by fine needle aspiration cytology(FNAC) in a 70-year-old female. FNAC from the right breast showed high cellularity consisting of mostly tight clusters of tumor cells and a few scattered tumor cells. The nuclei were monotonously round to oval in shape with inconspicuous nucleoli. The cytoplasm was abundant and finely granular. Scant amount of mucinous material was present on the background. The diagnosis was confirmed histologically and immunohistochemically.

Metastatic Papillary Thyroid Carcinoma Masquerading as Type II Branchial Cleft Cyst : A Case Report (제2형 새열 낭종으로 오인된 전이성 갑상선 유두상 암종 1예)

  • In case of lateral cervical cystic lesions, the differential diagnoses include branchial cleft cyst(BCC), teratoma, dermoid, hemangioma and lymphangioma etc. But sometimes metastatic cystic lymph nodes may exist in lateral neck. In such circumstance, the primary lesions are known to stem from oropharynx, nasopharynx, salivary and thyroid gland etc. A-66-year-old-male came to our clinic, due to the lateral cervical mass for 5 years. We performed the neck CT, sonography and sono-guided FNAC. He was initially diagnosed with the benign cyst such as BCC. We performed the excisional biopsy on left level II, but the pathologic report was revealed as metastatic papillary thyroid carcinoma(PTC). And then he received the total thyroidectomy with neck dissection. The final diagnosis was cystic metastasis from PTC. We learn a valuable lesson form this case in the following. Even if the simple cervical cyst is presumed with radiology and clinical pattern, more careful considerations on the basis of history and radiologic findings are mandatory.

  • Insular carcinoma(poorly differentiated thyroid cancer) is defined as a aggressive, follicular-derived thyroid carcinoma with behavior intermediate between follicular/papillary and anaplastic carcinomas. It was described by Carcangiu in 1984, but its prognosis, classification and the origin is not yet clear. And preoperative fine needle aspiration cytology of insular carcinoma has not been satisfactory. We experienced a case of advanced thyroid insular carcinoma with invasion of the sternum. So we intend to present the case with a review of the related literatures.

  • Oxyphilic (Hurthle) cells have abundant eosinophilic granular cytoplasms and occur in both benign and malignant neoplasms of the thyroid gland. Most published studies described mainly oxyphilic tumors of follicular type, and reports on oxyphilic papillary thyroid carcinomas are rare. The oxyphilic variant of papillary carcinoma differs from the classic papillary carcinoma in its more aggressive biological behavior. We report a case of oxyphilic papillary carcinoma exhibiting exclusively oxyphilic cells in a follicular pattern without papillary structures. The pattern of papillary carcinoma was confirmed by fine needle aspiration material and lymph nodal metastasis, both of which revealed typicaled papillary structures with characteristic nuclear features, psammoma bodies, and oxyphilic cytoplasm.

Lymphocyte Rich Papillary Oxyphilic Carcinoma of Thyroid - A Case Report - (림프구 침윤이 풍부한 갑상선 유두상 호산성세포 암종 - 1예 보고 -)

  • Lymphoid infiltration can be seen in some lesions such as Hashimoto's thyroiditis, subacute thyroiditis and several neoplasm of the thyroid. In case of malignancy, there are a few reports of lymphoid infiltration in the diffuse sclerosing variant of papillary carcinoma. But heavy lymphoid infiltraton without evidence of sclerosis is uncommon. We experienced a case of papillary oxyphilic carcinoma with massive lymphoid infiltration, which looks like Warihin tumor of salivary gland. However cytological feature of epithelial cells exhibit that of papillary carcinoma.

A Case of Thyroid Hyalinizing Trabecular Tumor Mistaken for Papillary Carcinoma in Aspiration Cytology (흡인 세포검사에서 유두상 암종으로 오인된 갑상선 유리질 소주형 종양 1예)

  • Hyalinizing trabecular tumor (HTT) of the thyroid gland is a rare neoplasm and only less than 100 cases have been reported so far. It is characterized by hyalinizing stroma with trabecular growth pattern and has an indolent clinical course. Because of its histologic features, it is frequently misdiagnosed as papillary or medullary carcinoma in fine needle aspiration cytologic findings. The tumor is benign or low malignant potential and thyroid lobectomy is recommended for adequate treatment. We recently experienced a case of thyroidal HTT in a 57-year-old man, who presented with a right thyroid nodule that was suspicious of papillary carcinoma in aspiration cytology. We report the unique and rare disease entity with brief literature review.

A Case of Papillary Carcinoma arising in a Thyroglossal duct cyst (갑상설관낭종에서 기원한 유두상 암종 1례)

  • Thyroglossal duct cysts are the most common anomaly in thyroid development. Generally, duct cysts are benign, but 1 per cent of cases may be malignant. Tumors of the thyroglossal duct are very rare and are usually papillary thyroid carcinomas. In most cases the diagnosis is only established after excison of a clinically benign thyroglossal duct cysts. The etiology of such tumors is unclear, but do novo origin and spread from a primary thyroid gland tumor has been suggested. We reported a case of papillary carcinoma arising in a thyroglossal duct cyst treated successfully with surgical excision in 37-year-old-female.

Fine Needle Aspiration Cytology of Papillary Carcinoma of the Thyroid with Numerous Multinucleated Giant Cells (다수의 다핵 거대세포를 동반한 갑상선 유두상 암종의 세침흡인 세포학적 소견)

  • An unusual case of papillary carcinoma of the thyroid revealing numerous multinucleated giant cells in the aspiration biopsy cytology is reported. Papillary carcinoma is the most common malignant neoplasm of the thyroid and is frequently diagnosed by aspiration biopsy cytology. Recently, we experienced a case of papillary carcinoma with many multinucleated giant cells in a 55-year-old woman. The cytologic features are described.
