Investigation into Occupational Disease of Merchant Crew (상선승무원들의 질병실태 조사)

  • The purpose of this study was to find out morbidity rate and pattern of disease and affect of variables related disease and medical management of seafares'. The subjects this study were 1049 seafares' were took education in Korea Institute of Maritime and Fisheries Technology. This questionnaire was focused on finding the basic data for prevention of disease and promotion health for the seafares'. The collected data were analyzed by using descriptive statistics, Chi-square, cross tab, linear regression by SPSS 10.1 package. The result of this study are as follow. 1)The morbidity rate within recent 12 months was 69.0%. 2)there were significant differences qf occurred disease in age(p<0.05), income(p<0.01), career of ship on board(p<0.01), rank(p<.01), perceived health status(p<0.01), worry of health(p<0.01), fatigue symptoms(0.01), satisfy of job(p<0.05), rest time(p<0.05) 3) Considering disease unable to work more than 4 hour, the number of those who had oral disease 7.3%> tinea 6.6%> gastric ulcer 6.4, and musculoskeletal disease group were 20.9%, which revealed the highest rate oral disease 13.6%> skin disease> 12.4%, digestive disease> 12.1%. 4) As refer to medical management, The pain above 31days 35.7%, hospitalization and treatment below 7days were each 50.2%, 42.8%, medical service were doctor's office 27.9, which revealed the highest rate.

Influence of Hippocampectomy and Adrenalectomy upon Gastric Ulceration in Rats (흰쥐의 위궤양 발생에 미치는 뇌해마 제거 및 부신 적출의 영향)

  • This study was conducted to see whether the hippocampectomy exerted facilitatory influence upon gastric ulceration in animals, and if so, whether the effect of hippocampectomy could be suppressed by adrenalectomy. 107 male rats were divided into 5 groups: rats that had over 90% of their hippocampal tissue removed through an opening on each side of the cerebral cortex(hippocampal group, N=21), rats that received bilateral adrenalectomy(adrenal group, N=29), rats that received adrenalectomy as well as hippocampectomy(hippocampo-adrenal group, N=10), rats that received damage to each side of the cortex over the hippocampus(cortical control group, N=20), and rats that had solely their head skin incised(normal control group, N=27). All rats were kept without restraint or food deprivation until on the 25th day after surgery, the stomach of each rat was inflated with 7ml of physiological saline and then removed under deep anesthesia. The mucosal surface was sketched under dissecting microscope, and enlarged photographs$(4{\times})$ were taken. The percentage of animals developing gastric ulcer in each animal group was calculated, the number of ulcer in each stomach was counted, and the total area of ulceration per stomach was measured on the Photograph with the aid of superimposed graph paper and expressed as permillage of total area of the glandular mucosa. Results obtained were as follows: 1. The percentage of animals developing gastric ulcer was significantly larger in the hippocampal group than they were in the hippocampo-adrenal, the adrenal, the cortical, and the normal control groups. 2. The mean number of ulcer per stomach was significantly larger in the hippocampal group than they were in the adrenal, the cortical control, and the normal control groups, while no significant difference existed between the hippocampal and the hippocampo-adrenal groups. 3. Total area of ulcer per stomach was significantly larger in the hippocampal group than they were in the cortical control and the normal control groups, but no significant differ-ence existed among the hippocampal, the adrenal, and the hippocampo·adrenal groups. 4. All measured values of the adrenal group were not significantly different from those of the hippocampo-adrenal, the cortical control, and the normal control groups. It is inferred from the above results that the hippocampus exerts an inhibitory influence upon gastric ulceration and that the hippocampal influence is mediated only partly through suppression of pituitary·adrenal activity.

  • In order to study the immunohistochemical effects of Opae-san on gastric ulcer induced by HCl-aspirin in rats, experiments were done by oral administration and measure histological features of ulcer lesion, scaning electron microscopic appearance, the changes of numbers of parietal cells, chief cells, gastrin and somatostatin-immunoreactive cells. The obtained results are as follows: 1. Ulcerative lesions were numerously detected in control groups especially in junction of cardiac-fundic gastric mucosa and histologically very severe injury to gastric epithelium were observed too but in the Opae-san administrated groups, no gross lesion of ulcer were detected and histologically minor injury of gastric mucosa were observed. Most slight injuries to gastric mucosa were observed in 5 days after treatment. 2. The numbers of parietal cells were remarkably increased in control group but in Opae-san administrated groups appeared significant decrease compared to control groups. Most remarkably decrease of the numbers of parietal cells compared to control groups were observed in 5 days after treatment. 3. The numbers of chief cells were remarkably decreased in control group but in Opae-san administrated groups appeared significant increase compared to control groups. Most remarkably increase of the numbers of chief cells compared to control groups were observed in 5 days after treatment. 4. The numbers of gastrin-immunoreactive cells were remarkably decreased in control group but in Opae-san administrated groups appeared significant increase compared to control groups. Most remarkably decrease of the numbers of gastrin-immunoreactive cells compared to control groups were observed in 5 days after treatment. 5. The numbers of somatostatin-immunoreactive cells were remarkably decreased in control group but in Opae-san administrated groups appeared significant increase compared to control groups. Most remarkably decrease of the numbers of somatostatin-immunoreactive cells compared to control groups were observed in 5 days after treatment. 6. Scaning electron microscopically, severe denude and degeneration of gastric mucosa were observed in control groups but in Opae-san administrated groups the lesions were remarkably decreased compared to control groups.

Antitumor and Immuno-potentiating Activity against Mouse Sarcoma 180 by Crude Polysaccharides Extracted from Fruiting Body of Tricholoma matsutake (송이(Tricholoma matsutake)의 자실체에서 추출한 조다당류가 생쥐의 Sarcoma 180에 미치는 항암 및 면역증강 작용)

  • Tricholoma matsutake, one of edible and medicinal mushroom belonging to Tricholomaceae of Agaricales, has been known to contain some curing effect on gastric cancer and ulcer, and inhibitory effect on sarcoma 180 and Ehrlich sarcoma. Neutral salt soluble (0.9% NaCl), hot water soluble and methanol soluble substances (hereinafter referred to Fr. NaCl, Fr. HW and Fr. MeOH, respectively) were extracted from fruiting body of the mushroom. In vitro cytotoxicity tests, crude polysaccharides were not cytotoxic against cancer cell lines such as Sarcoma 180, HepG2, HT29 and NIH3T3 at the concentration of 2.0 mg/ml. Intraperitoneal injection with crude polysaccharides showed life prolongation effect of 23.4$\sim$37.2% in mice previously inoculated with Sarcoma 180. Fr. MeOH and Fr. HW exhibited the immuno-potentiating activity of B lymphocyte by increasing the alkaline phosphatase activity by 2.2$\sim$11.9 folds compared with control at the concentration of 0.2$\sim$0.5 mg/ml. In case of Fr. NaCl, the numbers of peritoneal exudate cells and circulating leukocytes were increased by 6.0 and 1.5 folds at the concentration of 50 mg/kg, respectively. Therefore, it is concluded that crude polysaccharides extracted from fruiting body of Tricholoma matsutake showed antitumor and immuno-potentiating activity against Sarcoma 180 of mouse.

Effects of Sasa quelpaertensis Extract on mRNA and microRNA Profiles of SNU-16 Human Gastric Cancer Cells (SNU-16 위암 세포의 mRNA 및 miRNA 프로파일에 미치는 제주조릿대 추출물의 영향)

  • Sasa quelpaertensis Nakai leaf has been used as a folk medicine for the treatment of gastric ulcer, dipsosis, and hematemesis based on its anti-inflammatory, antipyretic, and diuretic characteristics. We have previously reported the procedure for deriving a phytochemical-rich extract (PRE) from S. quelpaertensis and how PRE and its ethyl acetate fraction (EPRE) exhibits an anticancer effect by inducing apoptosis in various gastric cancer cells. To explore the molecular targets involved in this apoptosis, we investigated the mRNA and microRNA profiles of EPRE-treated SNU-16 human gastric cancer cells. In total, 2,875 differentially expressed genes were identified by RNA sequencing, and gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) analyses indicated that the EPRE-modulated genes are associated with apoptosis, mitogen-activated protein kinase, inflammatory response, tumor necrosis factor signaling, and cancer pathways. Subsequently, protein-protein interaction network analysis confirmed interactions among genes associated with cell death and apoptosis, and 27 differentially expressed microRNAs were identified by further sequencing. Here, GO and KEGG pathway analysis revealed that EPRE modified the expression of microRNAs associated with the cell cycle and cell death, as well as signaling of tropomyosin-receptor-kinase receptor, transforming growth factor-b, nuclear factor kB, and cancer pathways. Taken together, these results provide insight into the mechanisms underlying the anticancer effect of EPRE.

Gastroesophageal Reflux in Peptic Ulcer Patients (소화성 궤양 환자에서 위식도 역류)

  • Background: It is well known fact to the patients of duodenal ulcer that their condition is frequently accompanied with reflux esophagitis. Therefore this condition is called an "acid-related disorder" because it is commonly associated with increased acidity. But there has been disputes on the effect of Helicobacter pylori eradication in these two conditions and whether H. pylori infection may have a protective role in reflux esophagitis. Only few reports have dealt with the prevalence of reflux esophagitis and gastroesophageal reflux in patients with peptic ulcer The aim of this study is to estimate the prevalence of gastroesophageal reflux and to analyze the pattern of the pathologic reflux in peptic ulcer patients. Materials and Methods: The study population consisted of 57 patients with endoscopically confirmed duodenal and/or gastric ulcer who all underwent 24hr ambulatory esophageal pH monitoring. Results: The prevalance of gastroesophageal reflux in peptic ulcer patients was 54.2% and 54,5% in gastric ulcer, and 62.5% in duodenal ulcer, 50% in combined ulcer, respectively. The prevalence of gastroesophageal reflux in the control group was 22.7% Conclusion: We discovered significantly higher prevalence of gastroesophageal reflux in patients with peptic ulcer disease than in those without it. In conclusion, the presence or absence of gastroesophageal reflux must be considered in the setting of peptic ulcer disease management.

Upper Gastrointestinal Bleeding in Full-Term Infants (만삭아에서 발생한 상부위장관 출혈에 대한 임상적 고찰)

  • Purpose: The aim of this study was to evaluate the clinical characteristics, endoscopic findings and risk factors related to the upper gastrointestinal bleeding (UGIB) seen in full-term infants. Methods: A clinical analysis for 9 cases with UGIB confirmed by endoscopy was carried out retrospectively. Patients were admitted to the Department of Pediatrics, Eulji Hospital, from January to December 2003. Results: UGIB from gastric or duodenal mucosal lesions has been seen in 0.13% in newborns infants. All patients were full-term AGA neonates without asphyxic findings at birth. Hematemesis, melena or recurrent vomiting developed within $4.4{\pm}3.8days$ after birth. Vital sign and laboratory test was normal on admission. Endoscopic findings showed hemorrhagic gastritis in 6 cases and peptic ulcers in 3 cases. All patients were successfully managed by medical treatment for $18.6{\pm}5.0days$. On treatment, clinical symptoms improved within $0.9{\pm}0.3days$. Follow-up endoscopy was not performed because there was no recurrence of symptoms in all patients. Case mothers had no history of gastritis, ulcer or anti-ulcer medications before and during pregnancy. Conclusion: If the healthy full-term infants express UGIB within a few days after birth, it is necessary to take careful history of family, mother and delivery process and to practice endoscopy for mucosal lesions of the patients. A follow-up endoscopy dose not seem to be necessary if the infant is clinically well.

Cultural characteristics of Inonotus obliquus isolated from Betula costata at Mt. Jumbong in Korea (점봉산 거제수나무에서 분리한 차가버섯의 배양특성)

  • The sterile form of Inonotus obliquus is used for preparation of the medicine befungin that is effective in the treatment of gastritis, gastric ulcers, and several tumors. The fungus is known to be produced mainly on the stems of Betula platyphylla var. japonica that grows at high altitudinal (above 1,100 m) region in Korea. But, we found the mushroom on the stem of Betula costata at Mt. Jumbong in Korea. We isolated a pure culture of the fungus from the stem of B. costata by use of potato dextrose agar (PDA) medium with streptomycin. We could isolate the fungus from plant's tissue filled with hyphae, but not from other parts. The spore collected from the sclerotia showed $6.0{\sim}10.0{\times}4.5{\sim}6.0{\mu}m$ in diameter, and the hypha was $2.5{\sim}5.0{\mu}m$ in thickness. The colony showed irregular features and scattered yellow color at the center as the culture ages. We could find brownish setae at the yellow region of colony at 20 days of culture, and the size ranged $4{\sim}6{\times}100{\sim}420{\mu}m$. The oatmeal agar (OA) provided best growth for I. obliquus among five media (CDA, CMA, MA, OA and PDA). Optimum temperature ranged $25{\sim}30^{\circ}C$, and optimum pH was relatively alkaline with the range of pH 8.0~9.5.

  • This study has been to produce of anti-H. pylori antibody in milk produced from cows immunized with antigen of Helicobacter pylori and to search the relationship between vaccine dosage and antibody formation and impact of vaccine dosage on cows. The content of anti-H. pylori antibody in serum and whey increased in accordance with vaccine dosage volume. It has been confirmed that the formation of high-quantified antibody was produced in all groups with vaccine dosages of 10 mL, 20 mL and 30 mL. It has been turned out that the antibody was formed most in 20 mL dosage. It was inclined to $12\%$ reduce caused by vaccine injection, but recovered after about maximum 1 week. In measurement of body temperature of cows after vaccine injected, it was inclined to rise with the normal scope in comparison with the controlled conditions.

The Role of Central Adrenergic Activity in Stress-induced Ulcerogenesis (스트레스성 궤양발생에 대한 중추 아드레날린성 활성도의 역할)

  • The role of central adrenergic activity in the genesis of stress ulcers was investigated by intracerebroventricular (i.c.v.) administration of catecholamines and clonidine in pylorus-ligated rats restrained for 4 hours at a temperature of $4^{\circ}C$. 1. The stress-induced ulceration was markedly decreased by the i.c.v. administration of norepinephrine, epinephrine, dopamine or low dose of clonidine. 2. After an i.c.v. administration of norepinephrine or epinephrine, the volume of gastric juice, and both acid and pepsin secretion were markedly decreased. 3. Dopamine or a low dose of clonidne decreased the volume of gastric juice and acid secretion but did not affect pepsin secretion. 4. Isoproterenol caused a decrease in the volume of gastric juice and acid secretion, however, the ulcerogenesis was similar to that of the control. 5. Gastric function as well as ulcerogenesis was little affected by a high dose of clonidine. From the above results, it is suggested that central adrenergic activation inhibits cold-restraint induced ulcerogenesis via adrenergic alpha and dopaminergic receptors, and that this effect may be mediated by a decrease in gastric acid secretion. It is also suggested that other factors may be involved in this antiulcerogenic effect.
