The Effect of Samul-tanggahyangbuja on Anti-Depressive Behavior and Immunity (사물탕가향부자가 항우울행동 및 면역기능에 미치는 영향)

  • Objectives: Samul-tang and Rhizome of Cyperus rotundus L. have been frequently used in gynecologic disease. The purpose of the present study is to explore the behavioral and neurobiological effects of Samul-tanggahyangbuja (SGH) on ovariectomized rats and to form a basis for clinical treatment. Methods: Ovariectomized rats were repeatedly stressed for over 2 weeks. After orally medicated with SGH (200 or 400 mg/kg/day), the anxiety response was tested using the Elevated Plus Maze (EPM) in rats. The serum levels of estradiol and IL-4, and immunohistochemical changes of IL-4 in the Locus coeruleus (LC) and Paraventricular Nucleus (PVN) were measured. Results: 1. In the EPM, SGH 400 mg significantly increased time spent on the open arms and decreased time spent on the closed arms, compared with the control group (p<0.05). 2. SGH tended to increase numbers of crossings in the open and closed arms in the EPM. However, it did not reach statistical significance. 3. SGH significantly increased the serum levels of estradiol compared with the control group (p<0.05). 4. SGH 400 mg significantly increased the serum levels of IL-4 compared with the control group (p<0.05). 5. IL-4 immunoreactivity was reduced in the control group compared with the normal group (p<0.05). However, SGH groups (200 and 400 mg) did not produce any significant effects on levels of IL-4 in the LC and PVN. Conclusions: These results suggest that SGH possesses the anti-depressant and immuno-modulatory effects on ovariectomized rats.

Relationship between Eating Attitude in Female Adolescent and Psychological, Biological Factors ; Depression, Aggression, Impulsiveness and Blood Cholesterol Level (여고생 식이태도와 심리적 및 생리적 요인과의 상관관계 ; 우울감, 공격성, 충동성 및 혈중 콜레스테롤에 대하여)

  • The purpose of this study is to evaluate correlation of adolescent eating attitude and psychological factors which might influence the eating attitude such as depression, aggression and impulsiveness. The author also tried to identify relationship between adolescent eating attitude and biological factors such as serum cholesterol level and triglyceride level. Two hundred and forty one female high school students without physical disorder were selected as study subjects. For the study, Eating Attitude Test-26KA (EAT -26KA), Beck Depression Inventory (BDI) -Korean version, Barrat Impulsiveness scale (BIS) -Korean version, and Buss-DurKee Hostility Inventory (BDHI) -Korean version were used. Serum cholesterol level and triglyceride level were also investigated. For the purpose of identifying factors which could influence eating attitude of adolescence, stepwise multiple linear regression analysis was done. Total score of BDHI could be most significant factor to explain eating attitude, and BDI, serum cholesterol/triglyceride level be second, third position respectively. But, BIS was not significant predicting parameter. The contributing portion of these factors to explain eating behavior was 25.9%.

Convergence Factors Affecting Suicide Risk of University Freshmen (대학신입생의 자살위험성에 영향을 미치는 융합적 요인)

  • The purpose of this study was to investigate the correlations between ADHD, depression, anxiety, suicide risk and suicide risk of university freshmen, and to investigate factors affecting suicide risk of university freshmen. As a result, the correlation between suicide risk and ADHD, depression, and anxiety were statistically significant. The correlation between suicide risk and ADHD (r=-.411) was moderately negative, while depression (r=-.594) and anxiety (r=-.563) were high negative correlations. According to the logistic regression analysis, the significant factors affecting suicide risk of college freshmen were ADHD, depression, and anxiety. In the abnormal group of variables, ADHD (OR=3.17; 95% CI 1.36-7.38), depression (OR=10.80; 95% CI 4.32-27.02) and anxiety (OR=3.39; 95% CI 1.55-7.41) were identified as influencing factors. As such, suicide risk groups of college freshmen are highly associated with ADHD, depression, and anxiety. Therefore, personal counseling and regular psychological tests of college freshmen will be provided to the high-risk suicide program to help students' psychological stability and adjustment to school life.

The Development of Wholeness Program for Effects Dementia-Buffering Testing of the Demented Elderly (노인의 치매행동 관련 홀니스 프로그램 개발과 완충효과 검증)

  • This study of the Wholeness Program relevant to dementia behavior and verification of buffering effects. For the purpose of this, among those who were attending Day Care Centers belonging to Seniors Welfare Center of M1 and M2 located in Seoul. Among those who met all these requirements, 40 senior citizens were finally selected. After carrying out a preliminary test to the selected subjects, 20 elderly of Seniors Welfare Center of M1 were randomly placed as an experimental group, while other 20 people of Seniors Welfare Center of M2, as a control group. The Wholeness Program relevant to dementia behavior was carried out from March 14 to May 30 of 2016, for 12 weeks, twice a week. As for the measurement tool, this study depended on the Korean version of Mini Mental State Examination (MMSE-K), the Korean version of Modified Barthel Index (K-MBI) and the Korean version of Geriatric Depression Scale (GDS-K). While the elderly were participating in the Wholeness Program relevant to dementia behavior, they were measured two times in total, before and after the participation in dementia behavior. Based on SPSS-PC (version 21.0), firstly, independent t-test was carried out to understand the homogeneity between the experimental group and the control group. Secondly, matched-pair t-test was carried out between preliminary test and post-test to inquire into the effects of MMSE-K, K-MBI and GDS-K. Thirdly, ANOVA was conducted to confirm the variation (change amount) between preliminary test and post-test. According to the survey results, the Wholeness Program relevant to dementia behavior of the elderly shows overall effects in all spheres of changes of cognitive ability (MMSE-K), changes of physical capacity (K-MBI) and changes of depression level (GDS-K).

A Case of Sheehan's Syndrome Mimicking Psychotic Depression (정신병적 우울증 양상을 나타낸 Sheehan씨 증후군 1례)

  • We experienced a case of 51-year-old female patient who showed symptoms of persecutory delusion, auditory hallucination and hallucinatory behavior, severe insomnia, psychomotor retardation and social withdrawal, along with some clinical signs of the deficiency of various hormones those gradually progressed after massive postpartum vaginal bleeding 13 years ago. She was admitted to a psychiatric ward under the impression of psychotic depression. However careful history taking and evaluation of clinical feature gave rise to the possibility of underlying medical condition. Laboratory work-up revealed panhypopituitarism, hypoglycemia and hyponatremia. After replacement of thyroid hormone and cortisol for 1 week, her clinical symptoms including psychiatric symptoms were improved. Taken together, these findings were compatible with the diagnosis of Sheehan's syndrome. On reporting this case, we would like to emphasize again the importance of differential diagnosis of medical problems causing psychiatric symptoms those are easily neglected in the clinical approach toward psychiatric patients.

NEW ANTIDEPRESSANTS IN CHILD AND ADOLESCENT PSYCHIATRY (소아청소년정신과영역의 새로운 항우울제)

  • Objectives:As increasing number of new antidepressants have been being introduced in clinical practice, pharmacological understanding has been broadened. These changes mandate new information and theories to be incorporated into the treatment process of children with depressive disorders. In light of newly coming knowledge, this review intended to recapitulate the characteristics of new antidepressants and to consider the pivotal issues to develope guidelines for the treatment of depression in childhood and adolescence. Methods:Searching the Pub-Med online database for the articles with the key words of 'new', 'antidepressants' and 'children' ninety-seven headings of review articles were obtained. The author selected the articles of pertinent subjects in terms of either treatment guideline or psychopharmacology of new antidepressants. When required, articles about the clinical effectiveness of individual antidepressants were separatedly searched. In addition, the safety information of new antidepressants was acquired by browsing the official sites of the United States Food and Drugs Administration and Department of Health and Human Services. Results:1) For the clinical course, treatment phase, and treatment outcome, the reviews or treatment guidelines adopted the information from adult treatment guidelines. 2) Systematic and critical reviews unambiguously concluded that selective serotonin reuptake inhibitors(SSRIs) excelled tricyclic antidepressants( TCAs) for both efficacy and side effect profiles, and were recommend for the first-line choice for the treatment of children with depressive disorders. 3) New antidepressants generally lacked treatment experiences and randomized controlled clinical trials. 4) SSRIs and other new antidepressants, when used together, might result in pharmacokinetic and/or pharmacodynamic drug-to-drug interaction. 5) The difference of the clinical effectiveness of antidepressants between children and adults should be addressed from developmental aspects, which required further evidence. Conclusion:Treatment guidelines for the pharmacological treatment of childhood and adolescence depression could be constructed on the basis of clinical trial findings and practical experiences. Treatment guidelines are to best serve as the frame of reference for a clinician to make reasonable decisions for a particular therapeutic situation. In order to fulfill this role, guidelines should be updated as soon as new research data become available.

Effects of Song Discussion on Depression and Rehabilitation Motivation in Stroke Patients (노래 가사 토의가 뇌졸중 환자의 우울 및 재활동기에 미치는 효과)

  • This study investigated the effects of song discussion on depression and rehabilitation motivation in stroke patients. Older adults with chronic stroke participated in this study: nine for the experimental group and eight for the control group. The experimental group was divided into three subgroups and participated in 12 sessions over 6 weeks. Target lyrics were selected by the investigator among popular songs from the participants' young adulthood. The song-based discussion was facilitated to address issues targeted at supportive, insight-focused, or reconstructive stage. The control group was provided with delayed intervention. At pre and posttest, the short form of Geriatric Depression Scale and the Rehabilitation Motivation Scale were measured. The experimental group showed significantly decreased depression and significantly increased rehabilitation motivation (p < .01), while the control group showed no significant changes. Positive changes were also observed in all subcategories of rehabilitation motivation in the experimental group, particularly in significantly increased task-oriented motivation and decreased amotivation. This study suggests that song discussion will be effectively applied in rehabilitative settings to address psychological issues of older adults with stroke.

Differences in Social Maturity and Behavioral Problems According to the Level of Sleep Problems in Infants With Autism Spectrum Disorder (자폐스펙트럼장애 영유아의 수면문제 경계선 및 임상 수준 집단과 정상 수준 집단 간 사회성숙도와 문제행동의 차이)

  • Objective : The purpose of this study was to examine differences in social maturity and behavioral problems according to the level of sleep problems in children with ASD. Methods : The participants were 102 mothers of infants with ASD aged 1-5 years. The Social Maturity Scales (SMS) and Child Behavior Checklist 1.5-5 were used as the measuring tools. Results : The level of sleep problems in infants with ASD was 56.58T on the sleep problems scale. The participants were grouped based on scores on the sleep problems scale: those with scores <65 (good sleepers) or scores ≥65 (poor sleepers). Sleep problems significantly correlated with all the scales on the CBCL. However, no association was found between sleep problems and social quotients. Poor sleepers achieved significantly lower scores on the social quotient scale of the SMS than good sleepers. Poor sleepers achieved significantly higher scores in internalizing problems, externalizing problems, and DSM-oriented scales on the CBCL compared to those in the good sleepers. Conclusion : The significance of this study is that it has verified the severity of sleep problems in infants with ASD and has examined the differences in social maturity and behavioral problems between poor sleepers and good sleepers.

A VALIDITY STUDY OF PARENT BEHAVIORAL RATING SCALES AS DIAGNOSTIC TOOLS OF ATTENTION DEFICIT/HYPERACTIVITY DISORDER (주의력결핍/과잉운동장애(ADHD) 아동의 진단도구로서 부모용 행동 평가지의 타당도 연구 - 한국아동인성검사와 아동 ${\cdot}$ 청소년 행동평가척도를 중심으로 -)

  • This study was designed to examine the validity of HPR subscale in Korean Personality Inventory for Children(KPI-C) and Attention Problems subscale in Korean Child Behavior Checklist(K-CBCL) as diagnostic tool for Attention-Deficit/Hyperactivity Disorder(ADHD). Nineteen ADHD-1 type, twenty-three ADHD-H type, sixteen Neurosis, and fifteen normal children with the age from 6 to12 were selected based on DSM-IV, and their responses of the KPI-C and CBCL were analyzed. Omnibus F-test results showed that there were significant differences in the F scores of HPR and Attention Problems T scores(p<.05). But in Posthoc analysis, the HPR and AP scores in three clinical groups were significantly higher than in normal group, but there was no group difference among three clinical groups(p<.05). These results shows that HPR subscale and Attention Problems subscale may be useful tools for screening clinical groups(vs normal group) but there was a limit to the clinical validity of two subscales as diagnostic tools for the subtypes of ADHD.

DIAGNOSTIC CLASSIFICATION AND ASSESSMENT OF PSYCHIATRICALLY REFERRED CHILDREN WITH INATTENTION OR HYPERACTIVITY (주의산만 ${\cdot}$ 과잉운동을 주소로 소아정신과를 방문한 아동의 진단적 분류와 평가)

  • This study assessed psychiatrically referred 5-to 13-year-old children who presented inattention or hyperactivity as chief complaints. Demographic characteristics, primary diagnosis, and comorbid psychiatric conditions of them were identified, and they were assessed using questionnaires and neuropsychological tests. Primary diagnoses included ADHD, anxiety disorder, mental retardation, depression, oppositional defiant disorder, developmental language disorder and others. functional enuresis, conduct disorder, and developmental language disorder were among the secondarily diagnosed disorders. In patients diagnosed as ADHD, overall comorbidity rate was 55.3%. The disorders that frequently co-occured with ADHD were specific developmental disorder, conduct disorder, oppositional defiant disorder, anxiety disorder and other. ADHD groups with or without comorbidity differed in performance IQ and CPT scores. ADHD group differed from externalizing disorders group in the information subscore of IQ, MFFT, and CPT scores, and differed in teachers rating scales, the uncommunication factor of CBCL, and CPT card error compared with internalizing disorders group. The authors concluded that inattentive or hyperactive children should be assessed using various instruments to differentiate other disorders and to identify possible presence of comorbid conditions.
