Study on the Usefulness of Ultrasonography for Postpartum Depression and Thyroid Disease (출산 후 우울증과 갑상선질환에 대한 초음파검사의 유용성에 관한 연구)

  • Postpartum depression(PPD) of women with depression increased frequency of thyroid disease, and so the correlations for depression and thyroid disease has been the subject of discussed whether. The purpose of this study was to predict the prevalence of PPD and the correlation between PPD and thyroid disease through ultrasonography. January 2010 to November 2011, Obstetrics & Gynecology in M-clinical center admitted 230 patients within 1 year postpartum were enrolled. EPDS by PPD scale depression screening and general characteristics of subjects were investigated and thyroid was examined that ultrasonography and thyroid blood tests. A total of 230 patients non PPD group were 53.0% and PPD group were 47.0%. In ultrasonography, among 27 patients who changed in size of thyroid, non PPD group were 14.8% and PPD group were 85.2%. Among 124 patients who thyroid nodules were presence, non PPD were 35.8% and PPD group were 64.2%. In ultrasonography, PPD group were higher incidence than non PPD group were changes in size of thyroid and the presence of nodules. There was significant difference between the changed in size of thyroid and thyroid nodules were presence the two group. Definitive histopathological diagnosis was benign in 33 patients (non PPD group were 45.5%, PPD group were 54.5%), malignancy in 5 patients (only PPD group were 100%), thyroiditis in 3 patients (non PPD group were 33.3%, PPD group were 66.7%). The results of thyroid blood tests, abnormal TSH level were 7 patients (non PPD group were 28.6%, PPD group were 71.4%) and abnormal Free T4 level were 9 patients (non PPD group were 44.4%, PPD group were 55.6%). There was no significant difference between the abnormal TSH level and Free T4 level of the two group. 5 patients were diagnosed as thyroid dysfunction. Of these, 2 patients were subclinical hyperthyroidism in non PPD group, 2 patients were subclinical hyperthyroidism and 1 patient was subclinical hyperthyroidism in PPD group. This study was significant the correlation between PPD and thyroid gland disease through ultrasonography. And the objective results of this study might be able to provide guideline that understanding, prevention and treatment for PPD and thyroid disease.

Alexithymia in Major Depressive Disorder and Subclinical Depression (주요 우울장애와 준임상적 우울증에서의 감정표현불능증)

  • Objectives : The objective of the present study was to investigate alexithymia in major depressive disorder(MDD) and subclinical depression(SCD). Methods : Three hundred eighty-six community-dwelling adults(238 females and 148 males, age 19-79; mean age $43.0{\pm}13.9$) were recruited. Structured clinical Interview for DSM-IV(SCID) was conducted for the diagnosis of MDD or other Axis I psychiatric disorders. The Center for Epidemiological Studies for Depression Scale(CES-D) and the Totonto Alexithymia Scale(TAS) were administered to assess depressive symptom and alexithymia, respectively. Among subjects without MDD, those who had minor depressive disorder on the SCID or high scores(i.e. over 16) on the CES-D were defined as subjects with SCD. TAS total score and factor I, II, III scores of TAS in MDD, SCD, and non-depressive controls were compared. Results : Among 386 subjects, 38 subjects(9.8%) were diagnosed as MDD by DSM-IV criteria, while 57 subjects(14.8%) were classified into SCD group. There were significant differences between 3 groups(MDD, SCD and non-depressive controls) in total score($F_{2,383}=14.0$, p<0.01), factor I(difficulty in identifying feeling)($F_{2,383}=23.4$, p<0.01) and factor II(difficulty in describing feeling)($F_{2,383}=7.8$, p<0.01), but not factor III(external oriented thinking)($F_{2,383}=1.8$, p=0.16) of TAS. In post-hoc analysis, both MDD subjects and SCD subjects had higher scores in TAS total, factor I and factor II, compared to non-depressive controls(all p<0.01). In contrast, there were no significant differences between MDD subjects and SCD subjects in any TAS factor. Conclusion : In this study, both MDD subjects and SCD subjects were more alexithymic than non-depressive control subjects. These findings suggest that SCD, as well as MDD, is also related to alexithymia.

  • 대학생의 취업불안, 우울과 자살생각간의 관계를 확인하기 위해 서술적 조사연구를 진행하였으며 연구결과 대학생들의 취업불안은 중간이상이었으며 취업불안을 유발하는 원인에 대한 불안이 높았다. 또한 우울은 경정도의 우울을 나타내고 있었으며 자살생각도 중간정도였다. 대학생들의 취업불안은 우울 및 자살과 중정도 이상의 상관관계를 보였으며 대학생의 자살생각에 대해 취업불안과 우울은 21.8%의 설명력을 보였다.

Oral Health and Oral Health Behavior as Risk Factors for Depression (우울증에 대한 구강건강 및 관리행태 위험요인)

  • This study investigated the association between depression and oral health and the factors influencing depression in adults. Data on 13,199 people (male 5,793, female 7,406; age ≥19 years old) were extracted from the Korean National Health and Nutrition Examination Survey VIII (KNHANES VIII) (2016~2018) and analyzed using frequency, percentage, an x2 test, a T-test, and logistic regression analysis. Depression was defined as either a medical diagnosis or a score ≥10 on Patient Health Questionnaire-9. The probability of depression was found to be statistically different for general characteristics, such as gender, age, school grade, income, and drinking. The logistic regression analysis showed that the independent variables, general health awareness (OR=9.094, 95% CI 7.139~11.585) and oral health awareness (OR=1.936, 95% CI 1.465~2.560), were associated with depression, and speaking discomfort, chewing discomfort, oral pain (within 1 year), and prosthesis were found to increase depression probability. The depression probability was also was found to significantly increase by 1.81 times if brushing frequency was less that once a day, if people used dental floss (OR=1.42), and had to have an oral examination (OR=1.31). These results indicated that oral health and oral health behaviors are associated with the depression, with the influencing factors having oral health and behavior-related characteristics. Therefore, proper oral health and oral health risk factor behavior management programs should be developed to assist in reducing depression.

Affecting Factors on Depression among Female Labor Workers (생산직 여성근로자의 우울에 영향을 미치는 요인)

  • This study was to examine factors related to depression of female labor workers. In order to identify health promotion strategies and intervention program to reduce depression of female labor workers. The participants were 176 female workers of manufacturing industries. Data were collected using a structured questionnaire that consisted of personal characteristics, duty related, work related characteristics and depression. Using a SPSS/WIN 17.0 Program, descriptive statistics, $x^2$-test, ANOVA and Stepwise Multiple Regression procedures were employed. As follows on the results; In Correlation analysis, work intensity, work concentration, vibration, work difficulty and job stress were positively correlated with depression. In multiple Regression, work intensity and work difficulty were significantly explained by depression. Given the findings, Do nevertheless support the need to develop effective depression intervention programs to reduce factors which work related condition and depression.

The Effects of Life Stress and Circadian Rhythm on Depression in College Students (전문대학생들의 생활스트레스, 일주기성이 우울감에 미치는 영향)

  • The purpose of this study was to measure the depression level of college students and to analyze the effects of college life stress and circadian rhythm on depression. A questionnaire survey was conducted to 720 students of college in Gwangju area from October 10, 2016 to October 31, and 687 of the collected questionnaires were analyzed. As this result, 34.5% of the students needed treatment for depression, and 71.6% of the students had mild depression. Second, depression was higher in female students, in higher grade students and students living apart from their parents. Third, evening activity type showed higher depression than morning activity type. Fourth, regression analysis revealed that depression was influenced by stress of value problem, circadian rhythm, academic stress, and getting a job stress. The results of this study suggest that students should be able to detect depression early and run a mental health program to relieve college life stress in college. However, this study is limited to some local college students and is not suitable for generalization. It is necessary to conduct research to compare university students and college students in the future.

Effect of Preschool Teacher's Job Stress and Depression on Burnout : Focusing on the Moderating Effect of Depression (유아교사의 직무스트레스 및 우울이 직무소진에 미치는 영향 : 우울의 조절효과를 중심으로)

  • This research is about the effects of job stress and depression on burnout. One hundred seventy nine Kindergarten school teachers in Kyounggi Incheon province participated in this research. They answered the questionaires about job stress, depression and burnout. Average, standard deviation, correlation analysis, simple period analysis and statistical considerations were used. Four meaningful conclusions were found. First, factors which indicates nursery school teachers' work stress have meaningful relations with depression and burnout. As nursery school teachers' work stress goes up, they were more depressed, and their burnout decreases. Second, factors which indicates nursery school teachers' work stress have considerable effect on the work performance. Third, Kindergarten school teachers' burnout is directly and mutually influenced by the job stress and depression. Therefore, Effort to lesson burnout is needed to decrease not only job stress but also depression.

A Study on the Influence of Socio-economic Deprivation on Depression : Focusing on Latent Growth Modeling Analysis (사회경제적 박탈이 우울에 미치는 영향에 관한 연구: 잠재성장모형 분석을 중심으로)

  • This study attempts to investigate the trajectories of socio-economic deprivation on the depression trajectories among middle-aged individuals using a latent growth model over the last five years. Using the Korea welfare panel study data ranged from the year 2013 (8th wave) to the year 2017 (12th wave), a sample of 7,437 aged over 40 was selected. Three hypothesis was proposed as follows. Is the trajectory of depression changing over the years? Are the initial values of socio-economic deprivation causally related to the initial values of depression? Are the rates of change in socio-economic deprivation causally related to the rates of change in depression? Results showed that the growth curve of depression tended to decline negatively over the five-year period. As the intial values of socio-economic deprivation increased, so did the initial values of depression move upwards. Finally, as the changes of socio-economic deprivation tended to get larger, those changes of depression also accordingly increased as well. The public policy implications based on the current study were discussed to pay more attention to those middle-aged individuals suffering from socio-economic deprivations.

CLINICAL CHARACTERISTICS OF CHILD AND ADOLESCENT PSYCHIATRIC INPATIENTS WITH MOOD DISORDER (입원한 기분장애 소아청소년의 임상특성 - 주요 우울증과 양극성장애의 우울삽화 비교를 중심으로 -)

  • The purpose of this study is to find out the characteristics of depressive episode about major depression and bipolar disorder in child and adolescent. The subjects of this study were 34 major depression patients and 17 bipolar disorder patients hospitalized at child and adolescent psychiatry in OO university children's hospital from 1st March 1993 to 31st October 1999. The method of this study is to review socio-demographic characteristics, diagnostic classification, chief problems and symptoms at admission, frequency of symptoms, maternal pregnancy problem history, childhood developmental history, coexisting psychiatric disorders, family psychopathology and family history and therapeutic response through their chart. 1) The ratio of male was higher than that of female in major depressive disorder while they are similar in manic episode, bipolar disorder. 2) Average onset age of bipolar disorder was 14 years 1 month and it was 12 years 8 months in the case of major depression As a result, average onset age of major depression is lower than that of bipolar disorder. 3) The patients complained of vegetative symptoms than somatic symptoms in both bipolar disorder and depressive disorder. Also, the cases of major depression developed more suicide idea symptom while the case of bipolar disorder developed more aggressive symptoms. In the respect of psychotic symptoms, delusion was more frequently shown in major depression, but halucination was more often shown in bipolar disorder. 4) Anxiety disorder coexisted most frequently in two groups. And there coexisted symptoms such as somartoform disorder, mental retardation and personality disorder in both cases. 5) The influence of family loading was remarkable in both cases. Above all, the development of major depression had to do with child abuse history and inappropriate care of family. It is apparent that there are distinctive differences between major depression and bipolar disorder in child and adolescent through the study, just as in adult cases. Therefore the differences of clinical characteristics between two disorders is founded in coexisting disorders and clinical symptoms including onset age, somatic symptoms and vegetative symptoms.

A Study of the Depressive Symptoms and the Quality of Life in Patients with Breast Cancer in a University Hospital (일 대학병원 유방암 환자의 우울증상과 삶의 질)

  • Objectives : This study was designed to investigate the prevalence of depressive symptoms in patients with breast cancer and to identify demographic variables and clinical characteristics impact on depressive symptoms and health related quality of life in patients with breast cancer in a university hospital. Methods : Fourty-one patients with breast cancer were selected, who had visited the department of General surgery of the Wonkwang University hospital with a diagnosis of breast cancer regularly during the period November, 2010-May, 2011. All of subjects were evaluated for the depression, anxiety and the health related quality of life with Beck Depression Inventory(BDI), anxiety subscale of Personality Assessment Inventory(PAI) and Short-Form 36 Health Survey-Korean version(SF-36-K). Patients were divided into depressive symptoms and non-depressive symptoms group according to the BDI score. We compared SF-36-K between two groups, and analized multiple regression with depression and health related quality of life as criterion variables and demographic and clinical characteristics. Results : The prevalence of depression in patients with breast cancer in a University hospital was 36.4%. Compared to the non-depression, depressed patients with breast cancer appeared significantly lower mean scores on six subscales in SF-36-K : Physical function(p<.01), Role-physical(p<.001), General health(p<.05), Social function(p<.001), Role-emotional(p<.001) and Mental health(p<.001). But there was no significant difference between two groups in Vitality and Bodily pain. Anxiety, level of education and presence of enforcement of chemotherapy(63.6%) were significant explanation variables for depressive symptoms. And type of surgery and age(55.8%) were significant explanation variables for health related quality of life. Conclusions : The prevalence of depressive symptoms in patients with breast cancer in a University hospital was 36%. The depressive symptoms had not only negative impact on the health related quality of life but also important explanation variable for health related quality of life. These results suggest that depressive symptoms in patients with breast cancer should be evaluated and treated for improving patient's health related quality of life.
