조직학적으로 유용성이 입증된 산탈회법을 이용한 인접면 비교적 초기 치아 우식의 병소를 형성하여 진단율을 조사하였다. 산 용액을 이용하여 20개 인접면 치아우식을 20개 소구치에 형성하였고, 37개 인접면 치아우식을 30개 대구치에 형성하였다. 건전한 소구치 20개, 대구치 30개를 포함하여 총 96개 치아를 4개씩 나누어 24개의 블록을 형성하였고, 각각 2개 블록의 교합면을 교합시켜서, 교익촬영을 하였다. 촬영 결과를 36명의 치과의사들이인접면 치아우식의 유무를 기록하고, 동시에 및 ROC 분석을 위한 5 개 범주의 판독 기준으로 판독하여 기록하였다. 인접면 치아우식증 유, 무만으로 판독한 결과 진단의 sensitivity는 0.71, specificity는 0.78 이였다. ROC 분석 한 결과의 곡선도표 아래부분의 평균 면적은 약 0.806 이였다. 치아우식증 유무만으로 진단한 결과는 특정한 sensitivity와 specificity 만을 나타내지만, ROC 분석 결과는 주관적 진단 기준과 구별되는 고유의 진단 능력을 표시하는 1-specificity(False Positive)의 변화에 따른 sensitivity(True Positive)의 변화를 연속적으로 나타내어 주었다.
치아 우식증은 영유아기를 거쳐 학령기에 이르기까지 광범위하게 감염된 질환으로써 구강 위생 평가를 통한 적절한 대응과 조기 진단을 통한 조기 치료와 적절한 예방 프로그램을 통해 건강한 구강 건강관리를 도모할 수 있겠다. 우식 위험 평가에 대한 미국소아치과학회의 지침 내역을 정리해 보면 다음과 같다.

The purpose of study was to compare the prevalences of severe early childhood caries(S-ECC) using two different diagnostic criteria. The subjects of this study included 401 preschool children from 36 to 71 months of the randomly selected seven nurseries in Iksan city. Severe early childhood caries was defined as (1) 1 or more cavitated, missing (due to caries), or filled smooth surfaces in primary maxillary anterior teeth or (2) a decayed, missing, or filled score of ${\geq}$4 (age 3), ${\geq}$5 (age 4), or ${\geq}$6 (age 5) surfaces. The prevalence determined by maxillary anterior caries and the prevalence determined by dmfs were 26.7%, 28.9% in 3 years, 32.4%, 30.4% in 4 years, 39.4%, 41.3% in 5 years, and 35.4%, 35.9% in all, respectively. The results suggest that the two diagnostic criteria of the severe early childhood caries yield almost the same results.

The aim of this study was to evaluate the proximal caries detecting ability and identify the optimal cut-off values of two types of laser fluorescence (LF) devices; classic type (DD) and pen type (DDpen). The number of proximal surfaces participated in this study were 164 surfaces in primary dentition and 438 surfaces in permanent dentition. Each tooth surface was sequentially assessed by two types of LF devices, and bitewing radiograph. The radiographs were classified into 3 groups in primary dentition ($pR_0$, $pR_1$, $pR_2$), and 4 groups in permanent dentition ($PR_0$, $PR_1$, $PR_2$, $PR_3$) according to the depth of caries, and used as gold standard. In primary dentition, the area under the curve (AUC) values of DD were 0.851 and 0.890, and those of DDpen were 0.883 and 0.917, respectively in enamel caries and dentin caries. In permanent dentition, the AUC values of DD were 0.762 and 0.886, and those of DDpen were 0.828 and 0.958, respectively in enamel caries and dentin caries. When detecting proximal caries in posterior teeth with LF devices, DDpen is more useful than DD in both primary and permanent dentition. However, in primary dentition, DD can also be useful to detect proximal caries.

The purpose of this study was to evaluate the specificity, sensitivity, and diagnostic power of caries activity test using LED fluorescence. The subjects of this study were 55 children of $6{\sim}7$ years old. LED light were irradiated to labial or buccal surface of all teeth. Fluorescence from initial carious lesion of teeth illuminated by an LED light was observed through barrier filter and the number of teeth showing lesion, size and position of lesion were counted. Streptococcus mutans colony counting and dDfFtT rate test were also done and their correlation was compared. And then specificity, sensitivity, diagnostic power of optical caries activity test using LED light were evaluated. 1. There was positive $correlation({\gamma}=0.43)$ between LED fluorescence test and Streptococcus mutans count(P<0.05). 2. When visual examination was defined to standard testing method, the specificity, sensitivity, diagnostic power of LED fluorescence test were 100%, 76.1%, and 100%. 3. When dDfFtT rate was defined to standard testing method, the specificity, sensitivity, diagnostic power of LED fluorescence test were 88.9%, 47.8%, and 95.7%. 4. When S. mutans colony counting was defined to standard testing method, the specificity, sensitivity, diagnostic power of LED fluorescence test were 100%, 58.7%, and 100%. Considering the above results, optical caries activity test using LED light could be regarded as a practical method because of its close relationship with microbiological caries activity test.

The aims of this study were to compare the accuracy, sensitivity and specificity of cnventional visual examination, radiography and a new laser fluorescence method, KaVo Diagnodent, for the detection of occlusal caries lesions. One hundred sound human premolars and molars which had no restorations or interproximal cavities were tested by three methods. Tooth lesions depth was assessed at histologic examination using Caries detector dye The following results were obtained. 1. Diagnodent show 7.8 in sound tooth, 25.4 in initial caries, 30.5 in enamel caries, and 53.8 in dentin caries with average score 2. Spearman and Pearson relation coefficient was high between tooth-specimen test with dye and Diagnodent(0.736, 0.619), visual examination(0.664, 0.666), and was low between tooth-specimen test with dye and radiographic examination(P<0.01, total) 3. Accuracy of occlusal caries was highest on Diagnodent(65%) and lowest on radiographic examination(35%) 4. In initial caries, the sensitivity and specificity of Diagnodent method was the highest. In enamel caries, the sensitivity of visual examination was the highest and specificity of Diagnodent method was the highest. In dentinal caries, the sensitivity and specificity of Diagnodent method was the highest and sensitivity of visual examination was the lowest.

This study aimed to evaluate the effectiveness of deep convolutional neural networks (CNNs) for diagnosis of interproximal caries in pediatric intraoral radiographs. A total of 500 intraoral radiographic images of first and second primary molars were used for the study. A CNN model (Resnet 50) was applied for the detection of proximal caries. The diagnostic accuracy, sensitivity, specificity, receiver operating characteristic (ROC) curve, and area under ROC curve (AUC) were calculated on the test dataset. The diagnostic accuracy was 0.84, sensitivity was 0.74, and specificity was 0.94. The trained CNN algorithm achieved AUC of 0.86. The diagnostic CNN model for pediatric intraoral radiographs showed good performance with high accuracy. Deep learning can assist dentists in diagnosis of proximal caries lesions in pediatric intraoral radiographs.

The purpose of this study was to evaluate the applicability of virtual reality simulation after experience of dental caries diagnosis for dental students before exposing to clinical pediatric practice. A pediatric patient model of a five-year-old child with primary dentition was developed and a caries model that is amenable to VR(virtual reality) diagnosis was organized and set-up. The dental student's were allowed to use the simulated model for fifteen minutes and their experiences were evaluated using a self-reported questionnaire to evaluate presence and usability of this application. Overall, virtual presence and appearance area of the simulation were highly scored. The result indicates that the VR model has no significant difference from the actual clinical caries regardless of grade of students, gender and VR experience. If the prototype is continuously advanced, its applicability in dental education will increase.

Dental caries is an important dental disease among children and adolescents that can continue for a lifetime. Early detection of dental caries in deciduous dentition is significant because it can influence the permanent teeth. It is also critical to prevent dental caries by performing fluoride treatment and pit-and-fissure sealant for high-risk children. Various methods have been developed for the early detection of dental caries; however, many studies are still seeking to discover more effective methods. In general, visual examination and radiographic images are used, but these techniques have several limitations such as errors and radiation exposure. In this study, clinical application of the newly developed DIAGNOcam caries identification device and its possible applications were examined. DIAGNOcam was applied to diagnose dental caries in the posterior teeth of patients in the Department of Pediatric Dentistry, and it was confirmed that it could be used to detect proximal caries, the margin of restoration, and the extent of dental caries lesions.

To evaluate the quantitative nature of laser fluorescence(LF) and dye-enhanced laser fluorescence(DELF) on early enamel caries, artificial caries were induced on the bovine enamels and argon laser was irradiated on the surfaces of lesions. Fluorescence radiance from enamel specimen was recorded with CCD camera which was connected to PC image analyzing system. The difference of fluorescence of radiance(DFR) between the carious and the sound enamel in each sample of the LF and the DELF groups were measured by image analyzing program. The DFR of lesion surfaces measured with laser fluorescence and lesion depths were evaluated and the results are as follows: The caries lesions were discriminated from sound enamels by the darkness with LF and by the brightness with DELF. The DELF was more sensitive than the LF at the earlier stage of demineralization.
