Analysis of Factors that Affect the Follow-up Pattern after Dental Treatment under Conscious Sedation in Children (어린이의 진정법 하 치과치료 후 정기검진에 영향을 미치는 요인 분석)

  • Children need regularly dental check-ups after dental treatment because there are a lot of changes of oral conditions such as tooth exfoliation and eruption, occlusion, and recurrence of dental caries. The purpose of this study is to determine the factors that affect the follow-up pattern after dental treatment under conscious sedation. Patients who were treated under sedation at the department of pediatric dentistry of Ewha Womans University Mokdong Hospital from 2009 to 2013 were included in this study. The final samples comprised of 562 patients that were classified into 4 groups according to the number of visits during 3 years. The chi-square test was used to determine the factors affecting regular follow-up. There were statistically significant differences between each treatment groups and factors such as the change of doctor, overall treatment time, the number of treated teeth under sedation, restorative treatment, minor operation, and preventive treatment (p < 0.05). This study identified factors that affect regular follow-up after dental treatment under conscious sedation. It is recommended to educate the importance of regular check-ups to parents who are less likely to return.

An analysis of the associated factors which influence DMFT index of the primary school children's in Pocheon city (포천시 초등학생의 주관적 구강건강인식 및 구강보건 행태와 우식경험영구치지수와의 관련성)

  • Objectives : We supposed that identifying the influencing factors on DMFT index in the primary school children in Pocheon be indispensible for the primary school children's oral health program establishment in Pocheon. So, to prepare the basic materials for the primary school children's oral health program establishment in Pocheon, we analyzed the associated factors which could have an influence on the primary school children's DMFT index in Pocheon. Methods : We selected 3,676 primary school children in Pocheon, whose age were from 6 to 11, and surveyed those children by a questionnaire and the oral examination based on WHO's oral health survey method. Data were analyzed with a $x^2$-test and multiple regression analysis using SPSS $15.0^{(R)}$. Results : The results of oral survey in the primary school children in Pocheon revealed as follows: DMFT index, National Health Oral health survey of 2006 people nationwide in the results when compared to younger grade children and older grade in the low and the higher. Subjective oral health awareness, high of viscosity junk food snacks, fruit, gender, dental visits and humidity, oral health education classes, including the variable of attitude was 6(p<0.05). Their higher awareness of dental health, oral health education classes ever the more positive attitude to DMFT index was statistically significant(p<0.05). Solid in the habit of eating junk food snacks to the students of a liquid relative to student intake was higher DMFT index indices(p<0.05). Conclusions : We could reason that one's oral health recognition and behavior should have a relationship with his(her) DMFT index.

Significant (SiC) index of the 12-year-old child's permanent teeth caries experience (12세 아동의 우식경험영구치 Significant Caries(SiC) Index에 대한 조사 연구)

  • Objectives : The purpose of the study is to investigate the significant(SiC) index of the 12 year old children's permanent teeth caries experience. A total of 428 children aged 12 years old were recruited from metropolitan city(215 children) and rural area(213 children) in this study. SiC index was obtained from DMFT index. Data were analyzed by gender, region, and DMFT. Methods : DMFT index and Sic index were analyzed and compared by pit and sealant fissure treatment, dental caries, gender, and areas. A total of 428 children aged 12 years old were recruited from metropolitan city(215 children) and rural area(213 children) in this study. Results : Boy students(6.73) tended to have a higher SiC index than girl students(7.84). There were significant differences(p<0.05). DMFT index in urban area was 2.59, and that in rural area was 3.35. In SiC index, rural children showed a higher index(7.77) than urban children(6.72).There existed a significance between DMFT index and SiC index(p<0.05). Conclusions : It is important to educate and to develop the oral disease prevention program for the high risk group children. This study showed the relationship between DMFT index and SiC index in 12 year old children in urban and rural areas.

EFFECT OF ROASTED BARLEY TEA ON THE ADHESIVE PROPERTIES ON SALIVA-COATED HYDROXYAPATITE BEADS OF CARIOGENIC MUTANS STREPTOCOCCI (보리차(Hordeum vulgare var, hexastichon)가 수산화인회석에 대한 우식유발성 세균의 부착에 미치는 영향)

  • Effect of the roasted barley tea in commercial markets on the adherence to the saliva-coated hydroxyapatite(HA) beads and the cell surface hydrophobicity of Streptococcus mutans and Streptococcus sobrinus as cariogenic microorganism was examined in vitro. Adherence activity and hydrophobicity in bacteria tested in all the barley tea samples decreased and the values were different according to the type of tea and the type of treatment. The inhibition of bacterial adsorption to HA beads suggest that barley tea active molecules as catechins and melanoidins may adsorb to a host surface, preventing the tooth receptor from interacting with any bacterial adhesions. The obtained results showed that the barley tea may inhibit bacterial adherence, the first step of the pathogenesis of dental caries in which these microorganism are involved.

Isolation and identification of the abundant bacteria in dental caries in children (소아의 치아우식 부위별 우점 세균 분리 및 동정)

  • Objectives: The study aimed to isolate the abundant bacteria in dental caries in children and to investigate the bacterial species involved in addition to those that have been previously reported. Methods: The specimens were collected from the supragingival plaques of each dental caries area, pit and fissure caries, deep dentinal caries, smooth surface caries, and dental caries, and from healthy subjects in the control group. Bacteria were cultured from these specimens, DNA was extracted from the isolated bacteria, and the 16S rRNA gene sequences were analyzed and identified. Results: Based on the results of the 16S rRNA gene sequence analysis for the 90 strains of dominant bacteria from the 45 specimens, 5, 7, 8, 7, and 13 species were identified from the supragingival plaques from healthy teeth, pit and fissure caries, deep dentinal caries, smooth surface caries, and dental caries, respectively. In healthy teeth, Actinomyces naeslundii dominated. Corynebacterium durum, Ralstonia pickettii, and Streptococcus intermedius showed equal distribution. The dominant bacterial species in dental caries, S. sanguinis, showed the greatest difference in prevalence in pit and fissure caries. In deep dentinal caries, S. mutans and Lactobacillus rhamnosus were dominant; in smooth surface caries, S. mutans and S. sanguinis were dominant; and in the supragingival plaques of dental caries, S. sanguinis and S. mutans were dominant. Conclusions: The bacterial species isolated from dental caries encompassed four phyla, eight genera, and 22 species. In addition, the SS1-2 strain, belonging to the genus Neisseria, was identified as a new species from among the isolated strains.

Effects of obesity on dental caries in adolescents of 13-18 years (13-18세 청소년의 비만이 치아우식에 미치는 영향)

  • Objectives: This study investigated the relationship between obesity and dental caries of Korean adolescents. Methods: Data from the Korea National Health and Nutrition Examination Survey (KNHNES VI) (2013 to 2015) were used and targeted adolescents aged 13 to 18 years. For statistical analysis, the SPSS 18.0 program was used, and the significance level was set at 0.05. Results: For the general and health-related characteristics, female students (p=0.001) showed higher decayed, missing, and filled teeth (DMFT) index than male students. Furthermore, DMFT showed higher tendency, as age was higher (p<0.001). Smoking (p=0.011) and drinking experience (p=0.027) revealed higher DMFT. The dental caries experience was significantly different among students who were female (p=0.033), older (p=0.001), and overweight or underweight (p=0.022). In the BMI for the general and health-related characteristics, male students had higher obesity rate than female students (p=0.003), and obese subjects had higher dental checkup in the last year (p=0.013). In multiple linear regression analysis of obesity and DMFT, the unadjusted model showed that the obesity group's DMFT was about 0.8 higher than that of the normal group (p=0.024). However, statistical significance was not shown in the adjusted models. In the logistic regression analysis of obesity and dental caries experience, the unadjusted model 1 (1.93 times), the adjusted model 2 (1.79 times), and model 3 (1.76 times) showed significantly higher dental caries experience. Conclusions: Both obesity and dental caries are related to diet habits and lifestyles and have a negative impact on adult health. Therefore, it is necessary to create a healthy social environment around the adolescent, and proper intervention through education is required to form healthy habits.

Synergistic Effect of Methanol Extract of Salvia Miltiorrhiza and Antibiotics against Dental Caries Pathogens (치아우식증유발세균에 대한 단삼 메탄올추출물과 항생제와의 병용효과)

  • Salvia miltiorrhiza Bunge (S. miltiorrhiza) is a traditional Korean medicine that is commonly used for the treatment of inflammatory diseases such as edema, arthritis, and hepatitis. The present study investigated the antimicrobial activity of methanol (MeOH) extract of S. miltiorrhiza roots against oral bacteria using broth the microdilution method and the checkerboard and time-kill methods evaluated the synergistic effects of treatment with antibiotics. The MeOH extract was demonstrated as a higher antibacterial activity (MICs, 8 to $64\;{\mu}g/mL$; MBCs, 16 to $64\;{\mu}g/mL$) against all tested oral bacteria. Additionally, the extract was observed to have a synergistic effect with ampicillin or gentamicin. A time-kill study evaluating the effects of the extract indicated that the extract treatment in combination with ampicillin or gentamicin showed rapid bactericidal activity. The results suggest that MeOH extract of S. miltiorrhiza could be employed as a natural antibacterial agent against dental caries.

  • Loss of permanent molar by multiple dental caries traumatic influence or hereditable disease can cause loss of the vertical dimension in children. However traditionally reconstructive treatment to restore vertical dimension in children has been provided by using simple methods such as celluloid crown form and stainless steel crown. The presented case report describes an alternative treatment modality of vertical dimension by using removalbe appliance This appliance is made with average of facial height and maximal clenching force by using T-scan.

The Significant Caries(SiC) Index of High School Students in Ulsan City (일부 고등학생의 구강보건인식도와 Significant Caries(SiC) Index 조사연구)

  • To prepare basic data for oral health promotion of high school students through the survey of the recognition of oral health, a questionnaire survey was conducted for 369 students. Statistical analysis was conducted using the SPSS 11.5 with $x^2$-test, fisher's exact test, t-test. The obtained results were as follows 1. The average of the high school student's DMF rate, DMFT index and SiC Index was 87.53%, 3.36, 6.50. 2. The average of the high school student's oral health knowledge was more than 3, 70% of highschool did right toothbrushing. But toothbrushing frequency of student of more than 60% was less than 2 a day. 3. In SiC Index, Only 30% of high school students received oral health education and 80% of them recognized oral health manpower's education needs.

  • Preserving the pulp is important in the treatment of carious pulp exposure in young permanent teeth. Pulpotomy is a vital pulp therapy in which a portion of the coronal pulp tissue is surgically removed, and the remaining radicular tissue is covered with suitable material that protects the pulp from further injury and permits and promotes healing. It is important to develop biocompatible treatment directed at maintaining pulp vitality and increasing tooth longevity. Platelet-rich fibrin (PRF) has been referred to as a second-generation platelet concentrate. Two clinical cases in which PRF was applied as a medicament after pulpotomy of an immature permanent tooth are presented. After isolation, caries removal and pulpotomy with PRF was performed. A layer of mineral trioxide aggregate (MTA) was placed over the PRF, and the final restoration was performed. Postoperatively, the patient had no pain or discomfort, and follow-up radiographs revealed normal periodontal ligament space and trabecular bone pattern.
