A Study on the Knowledge of Nurses, Performance and Preventive Practice of Pressure Ulcer among Nurses in Long-Term Care Hospital (노인요양병원 간호사의 욕창간호지식, 욕창예방 간호수행 및 간호수행 장애요인에 관한 연구)

  • This study is the descriptive research intended for identifying the reciprocal relationship between the knowledge of nursing and performance of pressure ulcer prevention that registered nurses at the aged care hospital possess. A total of 317 nurses who had been working at the aged care hospital for 4 months from March 1 2014 to June 1 2014 were chosen as research group. The data were collected by means of self-reported questionnaire and data were analyzed with descriptive statistics. The knowledge of nursing on pressure ulcer depending on general characteristics of research group had the significant difference in each item of clinical experience, education program, education time and education frequency(p<.05), In the performance of pressure ulcer prevention only the education frequency among various general characteristics showed significant differences(p=.40). The knowledge of nursing related to pressure ulcer had the low negative correlation with the performance of nursing(rho=-.137, p<.05). Among the factors disturbing nursing performance of pressure ulcer prevention the shortage of nursing staff appeared to be 57.7%. From the result of this study it was revealed that both increasing the education frequency of preventive nursing and managerial consideration concerning the shortage of nursing staff were needed more rather than the knowledgement of preventive nursing to improve the nursing performance of pressure ulcer prevention.

Analysis of Pressure Ulcer Nursing Records with Artificial Intelligence-based Natural Language Processing (인공지능 기반 자연어처리를 적용한 욕창간호기록 분석)

  • The purpose of this study was to examine the statements characteristics of the pressure ulcer nursing record by natural langage processing and assess the prediction accuracy for each pressure ulcer stage. Nursing records related to pressure ulcer were analyzed using descriptive statistics, and word cloud generators (http://wordcloud.kr) were used to examine the characteristics of words in the pressure ulcer prevention nursing records. The accuracy ratio for the pressure ulcer stage was calculated using deep learning. As a result of the study, the second stage and the deep tissue injury suspected were 23.1% and 23.0%, respectively, and the most frequent key words were erythema, blisters, bark, area, and size. The stages with high prediction accuracy were in the order of stage 0, deep tissue injury suspected, and stage 2. These results suggest that it can be developed as a clinical decision support system available to practice for nurses at the pressure ulcer prevention care.

  • The number of bedsores patients is increasing every year due to the aging society, and the importance of bedsores management is emerging as a burden on nurses due to the pandemic situation of COVID-19. Curses are a disease that causes ulcers due to floating posture, and it is a disease that is burdensome for nurses to periodically change their body position. Therefore, this study proposes a system that can reduce the burden on nurses by detecting the risk of inducing bedsores and monitoring the status of calls in real time. Through this system, it will be possible to increase nursing work by solving the difficulties of nursing in the bedsores of nurses.

Capstone Design Trail in Nursing Education and Its Outcome (간호학에 적용한 캡스톤 디자인의 적용사례 및 결과)

  • This study is a methodological study analyzing the results of applying capstone design to nursing education. Among the nursing college students who completed the fundamental nursing subject, the students who agreed to participate in the research were composed of the project team and proceeded as team activities. It was conducted through knowledge - team composition - topic selection / analysis - design / development - simulation - evaluation / feedback process starting from knowledge learned through fundamental nursing class. The research period was from January 1, 2015 to October 30, 2015 for 6 months. BBS (Bed Sore Socks) was developed for the prevention of pressure ulcers, and BBS was applied for seven days to randomly selected patients in the long term care facility. The incidence and pressure ulcer risk scores were assessed. The results of the pilot study showed that the intervention group had an effect on the incidence of pressure ulcer ($x^2=.40$, p = .500) and the pressure ulcer risk score (z = -.45, p = .690), but it was not statistically significant. Capstone design was trailed in the field of nursing education and produced the bed sore socks. In the nursing education, not many capstone design trails appeared yet, this study might be the first trail in the field of nursing, and it could be challenging for expanding of nursing education.

Knowledge, Performance, and Incidence Rate of Pressure Injury Using Skin Observation Records in Long-Term Care Hospitals (피부관찰기록지 이용여부에 따른 요양병원 간호 인력의 욕창간호 지식과 수행도 및 욕창발생률)

  • Purpose: To demonstrate the importance of comprehensive skin observation as an effective intervention for pressure injury prevention in elderly long-term care hospital patients. Methods: The survey was conducted with 70 nursing staff members working at two long-term care hospitals with 200 beds or less in D city. Data were collected from October 16 to October 23, 2019 and analyzed using descriptive statistics, the chi-square test, and the independent t-test with the SPSS 25.0 program. Results: Nursing care knowledge for pressure injury was similar between the two groups. Conversely, nursing care performance for pressure injury degree of the nursing staff in the hospital using skin observation records performed better than those who did not (t=6.11, p<.001). Furthermore, comprehensive skin assessments in long-term care hospitals using skin observation records showed a lower incidence rate of pressure injury than that showed using general skin assessments (t=-5.28, p=.006). Conclusions: Comprehensive skin assessment is important for pressure injury prevention in elderly long-term care hospital patients. To implement this effectively, it is necessary to devise institutional regulations, guidelines, and systematic education programs.

The Effects of Pressure Injury Nursing Knowledge and Pressure Injury Nursing Attitude on Pressure Injury Nursing Practices of Nurses in Geriatric Hospitals (노인병원 간호사의 욕창 간호 지식과 욕창 간호 태도가 욕창 간호 수행에 미치는 영향)

  • Purpose: This study investigated the effect of pressure injury nursing knowledge, and pressure injury nursing attitudes, on pressure injury nursing practice. Methods: Participants in this descriptive study were 141 nurses at 20 geriatric hospitals. Data was collected August 24, 2021- April 4, 2022, and analyzed in terms of Independent t-test and one-way ANOVA, Scheffé test, Pearson's correlation coefficient, and multiple regression, using the SPSS/WIN 27.0 program. Results: Pressure injury nursing practice positively correlated with pressure injury nursing attitudes (r= .44, p< .001). Factors influencing pressure injury nursing practice were pressure injury nursing attitudes (β= .43, p< .001), and gender (β= .21, p= .006). The model used in this study explains 22.8% of pressure injury nursing practice (Adjusted R2= 22.8, F= 11.30, p< .001). Conclusion: Results show factors that influence pressure injury nursing practice of nurses in geriatric hospitals. Based on the results, pressure injury nursing practice programs must include factors that improve pressure injury nursing attitudes. A follow-up study to confirm the impact of developing a program for increasing pressure injury nursing practice is also recommended.

Deep Learning-Based Pressure Ulcer Image Object Detection Study (딥러닝 기반 욕창 이미지 객체 탐지 연구)

  • 본 논문에서는 딥러닝 기반 욕창 감지를 위한 욕창 객체 탐지를 연구한다. 객체 탐지 딥러닝 기법으로 RCNN, Fast R-CNN, Faster R-CNN, YOLO 등 다양한 기법이 존재하며, 각 모델의 특징 또한 다르다. 욕창은 단계별로 피부, 조직에 손상의 정도가 다르다. 낮은 단계의 경우 일반적인 피부색과 유사하게 나타나며, 높은 단계의 경우 근육, 뼈, 지지 조직 등의 괴사로 인해 삼출물 또는 괴사조직이 나타난다. 논문에서는 One-Stage Detection 기법인 YOLO를 기반으로 욕창 이미지 내부에서 욕창 탐지를 진행한다. 현재 보유하고 있는 이미지 데이터 수가 많지 않아 데이터 증강기법을 통해 데이터를 증강하여 학습에 활용하였다.

Identifying Minimum Datasets for Pressure Ulcer Assessment and Analysis of Nursing Records in Home Nursing (가정간호의 욕창 의사결정지원 서비스를 위한 욕창 사정 MDS 규명 및 간호 기록 분석)

  • Purpose: The purpose of this study was to identify minimum datasets for ulcer assessment and to map the minimum datasets to paper-based nursing records for pressure ulcer care in homecare setting. Methods: To identify minimum datasets for pressure ulcer assessment, the authors reviewed four guidelines for pressure ulcer care. The content validity of the minimum datasets was assessed by three homecare nurse specialists. To map the minimum datasets to nursing records, the authors examined 107 pressure ulcer events derived from 45 pressure ulcer patients who received home nursing from two hospitals in Gyeonggi Province. Results: The minimum datasets for initial assessment were anatomical location, stage, size, tissue, exudate, condition of periwound skin, undermining, odor, and pain. 'Location' was recorded best, accounting for a complete recording rate of 98.1%. 'Exudate' and 'pain' showed the poorest record, accounting for 2.8% and 0%, respectively. The minimum datasets for progress assessment were wound size, tissue, and exudate, each accounted for 31.8%, 2.8%, and 4.7%, respectively. Conclusion: This study concluded that data on pressure ulcer assessment was not sufficient homecare and it can be improved by adopting minimum datasets as identified in this study.

  • This study is an algorithm for clinical decision-making using machine learning, and it is an algorithm to classify pressure sores to be used in the development of a system to help prevent pressure sores when nursing staff care for patients who lie down for a long time. As a result of machine learning, the learning accuracy of the algorithm was 82.14% and the test accuracy was 82.58%.
