  • Lumbar disc herniation in the adolescent is rare. The clinical features of adolescent lumbar disc herniation are typically different from those of adults. Many adolescent with that disease may be misdiagnosed as minor trauma, muscle sprain, tight hamstrings or scoliosis. Published series reiterate that adolescents, as opposed to adults, do not respond well to conservative treatment. In adolescent lumbar disc herniation with hamstring tightness, the neurologic defects are improved shortly after, discectomy, but the hamstring tightness continue and remain even after 1 year. Earlier discectomy and adjunct postsurgical conservative measures can provide a greater opportunity for correction and stabilization of scoliotic posture. We here report on a case of adolescent lumbar disc herniation that was misdiagnosed by knee injury during 2 years. After posterior partial discectomy, the hamstring tightness and scoliosis was disappeared without complication.

Effects of Lumbar Stabilization Exercise on Static and Dynamic Balancing and Gait of Stroke Patients (뇌졸중 환자에 대한 요추부 안정화 운동이 정적, 동적균형 및 보행에 미치는 영향)

  • The purpose of this study was to identify the effects of an exercise program for lumbar stabilization on static and dynamic balancing and gait of stroke patients. The subjects were 27 stroke patients, who were divided into an experimental group(n=14) and a control group(n=13). The exercise was conducted for 30 minutes a day, four times a week, and for six weeks. The variables included static and dynamic standing balance and gait, which were verified before and after the lumbar stabilization exercise. Using the IBM SPSS(19.0 version), data were analyzed with t-test and two-way ANOVA to validate the differences before and after the experiment and between the groups. The significance level was set at α = .05 and the results of this study are as follows. First, statistically significant differences were found in the static standing balance after the exercise (P< .05). Second, there were statistically significant differences in the dynamic standing balance after the exercise (P< .05). Third, statistically significant differences were found in the gait of the patients after the exercise (P< .05). In addition, the interactions between the two groups showed significant differences. These results suggest that stroke patients need a variety of lumbar exercises and that the lumbar stabilization exercise can improve physical treatment and health of the patients, having positive effects on their quality of life. Further studies are necessary to explore the applications of various lumber exercises.

Analysis of Bone Mineral Density and Related Factors after Pelvic Radiotherapy in Patients with Cervical Cancer (골반부 방사선 치료를 받은 자궁경부암 환자의 골밀도 변화와 관련 인자 분석)

  • Purpose: This study was designed to evaluate the effects on bone mineral density (BMD) and related factors according to the distance from the radiation field at different sites. This study was conducted on patients with uterine cervical cancer who received pelvic radiotherapy. Materials and Methods: We selected 96 patients with cervical cancer who underwent determination of BMD from November 2002 to December 2006 after pelvic radiotherapy at Kosin University Gospel Hospital. The T-score and Z-score for the first lumbar spine (L1), fourth lumbar spine (L4) and femur neck (F) were analyzed to determine the difference in BMD among the sites by the use of ANOVA and the post-hoc test. The study subjects were evaluated for age, body weight, body mass index (BMI), post-radiotherapy follow-up duration, intracavitary radiotherapy (ICR) and hormonal replacement therapy (HRT). Association between the characteristics of the study subjects and T-score for each site was evaluated by the use of Pearson's correlation and multiple regression analysis. Results: The average T-score for all ages was -1.94 for the L1, -0.42 for the L4 and -0.53 for the F. The average Z-score for all ages was -1.11 for the L1, -0.40 for the L4 and -0.48 for the F. The T-score and Z-score for the L4 and F were significantly different from the scores for the L1 (p<0.05). There was no significant difference between the L4 and F. Results for patients younger than 60 years were the same as for all ages. Age and ICR were negatively correlated and body weight and HRT were positively correlated with the T-score for all sites (p<0.05). BMI was positively correlated with the T-score for the L4 and F (p<0.05). Based on the use of multiple regression analysis, age was negatively associated with the T-score for the L1 and F and was positively correlated for the L4 (p<0.05). Body weight was positively associated with the T-score for all sites (p<0.05). ICR was negatively associated with the T-score for the L1 (p<0.05). HRT was positively associated with the T-score for the L4 and F (p<0.05). Conclusion: The T-score and Z-score for the L4 and F were significantly higher than the scores for the L1, a finding in contrast to some previous studies on normal women. It was thought that radiation could partly influence BMD because of a higher T-score and Z-score for sites around the radiotherapy field. We suggest that a further long-term study is necessary to determine the clinical significance of these findings, which will influence the diagnosis of osteoporosis based on BMD in patients with cervical cancer who have received radiotherapy.

Combined Exercise in Premenopausal Women Effects on Body Composition and Bone Mineral Density (복합운동이 폐경 전 성인여성의 신체조성 및 골밀도에 미치는 영향)

  • The purpose of this study was to compare and analyze changes in body composition and bone mineral density (BMD) during combined exercise in premenopausal women who did not take regular diet or regular physical activity within 6 months, In addition to being able to recognize the importance of pre-menopausal women's health and exercise, it also provides basic data for the development of educational programs for early prevention of osteoporosis prevention education I want to. The average age of the subjects in their 30s was 35.44 years and their height was 158.89 cm in their 30s. The average age of the subjects in their 40s was 41.89 and their average height was 160.78 cm in their 40s. Body composition, BMI, and body fat percentage were higher in the 30s, and bone mineral content, lean body mass, skeletal muscle mass, body weight, body fat mass and waist circumference were higher in their forties. Lumbar spine BMD and femur density were higher in their 30s than their 40s. Body composition, skeletal muscle mass, and fat mass increased in the 30s body composition by age - related complex exercise in premenopausal women, body fat mass, body fat percentage, waist circumference decreased. Body mass, body fat, body fat percentage, and BMI decreased in the 40s. The lumbar spine, the lumbar spine, the lumbar spine, and the femur were increased in the lumbar spine, the lumbar spine, the femur, and the femur. In the post-analysis results, changes in body composition were statistically significant due to decrease in muscle mass, body weight, body fat mass and BMI after combined exercise. In the change of bone density, lumbar spine BMD was measured as lumbar spine 1, lumbar spine 2, lumbar spine 3, And the femur density was increase.

Factors Associated with Conversion from Conservative to Surgical Treatment in Single-Level Lumbar Spinal Stenosis Patients (보존적 치료 중인 단분절 요추관 협착증 환자에서 수술적 치료로 전환과 관련된 연관 인자)

  • Study Design: Retrospective study of prospectively-collected data. Objectives: To determine the factors associated with conversion from conservative to surgical treatment in single-level lumbar spinal stenosis patients. Summary of Literature Review: Various reports have presented clinical outcomes after the surgical and nonsurgical treatment of spinal stenosis. However, few reports have investigated factors predicting conversion to surgery during the course of conservative treatment. Materials and Methods: We analyzed 40 patients who visited our hospital from May 2010 to May 2015 and were traceable for at least 3 years after being advised to undergo surgery following 3 months of conservative treatment. Of these patients, 20 underwent surgery and 20 did not. We then investigated the factors associated with conversion to surgical treatment. Clinical assessments were conducted using a questionnaire, and the overall area of the spinal canal and the muscle area within the spinal canal were measured using magnetic resonance imaging. Results: The average area of the spinal canal was $81.40{\pm}53.61mm^2$ in the surgical group, compared to $127.75{\pm}82.55mm^2$ in the nonsurgical group (p=0.042). The muscle area in the spinal canal was $5.17{\pm}1.30cm^2$ in the surgical group, whereas it was $6.40{\pm}1.56cm^2$ in the nonsurgical group (p=0.010). The patients in the surgical group were more likely to have experienced repetitive strain and to have frequently visited health clubs (p=0.047, p=0.037, respectively). However, regular stretching was more common in the nonsurgical group (p=0.028). Conclusions: The factors associated with conversion to surgical treatment were a narrow spinal canal, a small muscle area within the spinal canal, visiting health clubs, repetitive sprain, and not stretching. A small muscle area within the spinal canal can be considered as a key factor related to surgical conversion.

  • 본 연구에서는 유한요소 해석 방법을 이용하여 척추 퇴행성 질환을 가진 환자의 관절 불안정 모델을 개발 및 검증하였다. 관절 불안정 모델은 정상요추 유한요소 모델의 인대 및 협부를 결손시켜 개발하였으며, 요추 유한요소 모델의 제 5 요추체를 완전히 고정시키고 추적 경로 방향의 400 N의 압축력을 가한 상태에서 10 Nm의 굴곡과 신전 모멘트를 가하였다. 굽힘 및 신전 시 관절 불안정 모델들의 회전량이 정상의 요추 모델의 회전량에 비하여 크게 나타났다. 또한 굽힘 시 관절 불안정 모델들의 전위거리가 정상에 비하여 크게 나타났다. 본 연구의 결과는 척추 불안정성의 원인 및 기전의 이해뿐만 아니라 다양한 인체정보 콘텐츠 분야에 널리 활용될 수 있을 것으로 생각된다.

  • Spinal hematomas are a rare but serious complication that is typically observed in the epidural space. Spinal subdural hematomas are a dangerous clinical situation because of their potential to cause significant compression of the neural elements and can be mistaken easily for spinal epidural hematomas. This paper reports a case of a severe whole lumbar subdural hematoma after a simple epidural injection that was treated with surgical decompression with excellent clinical results.

The effects of motorized flexion-distraction treatment on the lumbosacral region angle in patients with chronic low back pain (자동 굴곡-신연기법이 만성요통 환자의 요천추부 각도에 미치는 효과)

  • This study is to examine effects of motorized flexion-distraction treatment on the pain, lumbosacral angle, lumbar lordosis angle, and lumbar 5 (L5) intervertebral disc angle in patients with chronic low back pain. We selected 30 cases of chronic low back pain, which were evenly divided into two groups: experimental group and control group. We applied the same hot pack, interferential current therapy, and ultrasound therapy to both groups. The experimental group had additional treatment of motrized flexion-distraction therapy and control group had additional of stretching exercise. For each subject, the pain, lumbosacral angle, lumbar lordosis angle, and lumbar 5 (L5) intervertebral disc angle were measured before and after treatment, While experimental groups showed significant improvements after treatment, more significant effects were found in the experimental group.

Intradural Schwannoma Associated with Lumbar Spinal Stenosis: A Cese Report (요추부 척추관 협착증과 동반된 경막내 신경초종: 증례 보고)

  • In the patient who has intradural mass associated with spinal stenosis, if the operation for spinal stenosis is performed alone, the symptom may remain. We report with literature review that we achieved the successful outcome after simultaneous decompression of spinal stenosis and space occupying mass removal in the case of intradural and extradural compression. A 71-year-old female patient suffering from low back pain and radiating pain of both lower extremities admitted. In magnetic resonance imaging, spinal stenosis on L4-5 and spondylolisthesis on L5-S1 compressed dural sac and intradural space occupying mass on L4 level compressed. By posterior approach, decompression and interbody fusion were carried out. Then mass was removed with median durotomy. Pathologic diagnosis was schwannoma and the symptom was improved remarkably.

  • 목적 : 경사계와 각도기로 경추, 요추 및 견관절의 운동 범위를 반복 측정하여 두 도구의 측정자내 신뢰도 우위를 추정하여 보고자 하였다. 방법 : 각 관절에 별다른 증상이 없는 104명의 학생들을 대상으로 경추, 요추 및 견관절의 능동 운동 범위를 경사계와 각도기로 각 도구 당 2회씩 측정하였다. 측정하기 바로 전에, 한 명의 교육자가 경사계와 각도기를 이용한 경추, 요추 및 견관절의 운동 범위 측정 방법을 교육하였다. 104명은 두 명 씩 쌍을 이룬 후 각도기, 경사계, 각도기, 경사계를 이용하는 순서로 서로의 경추, 요추 및 견관절의 운동 범위를 측정하였다. 결과 : 견관절의 수평 외전과 외회전에서 경사계의 측정값이 각도기의 측정값보다 통계적으로 의미 있게 안정적으로 반복되었다(P<0.05, 독립 이 표본 t-검정). 결론 : 견관절의 운동 범위 측정에 있어서 경사계가 각도기보다 높은 측정자내 신뢰도를 가지는 것으로 추정되어, 경사계와 각도기의 신뢰도를 급간내 상관 계수(intraclass correlation coefficient)를 이용하여 비교해보는 연구의 예비 조사로서 실시된 이번 연구의 결과는 향후 시행될 연구에 있어서 견관절의 운동 범위 측정에 보다 집중적인 연구를 진행할 수 있는 근거를 마련하였다.
