Spinal Ganglion Cyst of Lumbar Posterior Longitudinal Ligament (요추부 후종인대에서 발생한 결절종)

  • Objective : In contrary to ganglion cyst that commonly develops in connective tissue of peripheral joint or tendon, spinal ganglion cysts have been rarely reported. The authors report five cases of spinal ganglion cysts which originated from posterior longitudinal ligament of the lumbar spine. Methods : The authors retrospectively analyzed clinical, radiological, and surgical findings of five cases of spinal ganglion cysts and speculated the pathogenesis of spinal ganglion cyst. Result : Cysts were excised totally and symptoms improved without complications in all cases. Intraoperative findings revealed cysts that were tightly adherent to posterior longitudinal ligament. Conclusion : Five cases of ganglion cyst which caused similar symptoms and signs those of lumbar disc herniations were excised successfully. MRI and operative findings suggested spinal ganglion cyst of posterior longitudinal ligament were closely associated with disc degeneration which imply disc degeneration or herniation may play an important role in the pathogenesis of ganglion cyst.

Correction of Fluoroscopic Image for Nucleoplasty in Lumbar Disc (요추디스크 수핵감압술을 위한 투시영상의 교정)

  • Fluoroscopy is performed when tissue or organ in the human body is examined, and it is used for diagnosis and procedure in back ailments. With regard to fluoroscopy equipment, distortion occurs on the peripheral part of fluoroscopic image rather than on its central part. This study measured distortion factors of vertical spacing ratio and distortion factor of diagonal spacing ratio before and after correction by applying a correction algorithm. According to measuring the vertical spacing ratio, post-correction standard deviation decreased by 0.04 in comparison with pre-correction one. Also measuring the diagonal spacing ratio, post-correction standard deviation decreased by 0.06 in comparison with pre-correction one. Consequently, the distortion of fluoroscopic image decreased after correction. A decrease in the distortion of image through the application of correction algorithm and the improvement of performance will be helpful in finding a correct position of lumbar puncture in nucleoplasty to treat lumbar disc herniation in the future.

  • 허리는 요추(허리척추뼈), 척추골사이의 디스크(추간판), 척추와 디스크 주위의인대, 척수와 신경, 요추의 근육, 골반과 복부내의 기관들로 구성되어 있다. 우리 몸에서 고장이 잘 나는 곳은 움직임이 많은 곳이다. 사지도 움직임이 많은 관절에 고장이 잘 생긴다. 척추의 움직임이 많은 곳은 허리이다. 허리는 매우 불안정하여 전 인구의 약80%가 한번은 통증을 경험한다고 한다. 이러한 허리통증은 일반적으로 일상생활에서의 잘못된 자세와 생활습관에 기인하는 경우가 많다. 수술이나 치료 후에도 자세와 생활습관을 교정하지 못하면 재발의 경우가 있는 등 환자 자신이 허리통증에 대한 이해와 노력이 없으면 궁극적인 치료를 기대하기 어려우므로 환자 자신의 관심과 노력이 무엇보다 중요하다.이번 호에서는 인체의 신비 허리에 대해 척추전문에스병원 이승철원장과 경희대학교 동서신의학병원 척추센터 김기택 교수의 도움말로 알아본다.

Effects of Decompression Therapy for 6 Cases with Lumbar Herniated Disc (감압치료가 요추간판탈출 6(증)례에 미치는 영향)

  • The Purpose of this study was to report magnetic resonance imaging(MRI) changes of six cases of Lumbar herniated disc which was treated with spinal decompression therapy, oriental medical therapy. 'Disc heights' were measured on sagittal view of $T_2$-weighted MRI. Size of the herniated disc was measured by MRI and 'disc herniations index'. The grading system and algorithm for 'disc degeneration' were based on MRI signal intensity, disc structure, distinction between nucleus and anulus, and disc height. Data for disc height, disc herniations index and disc degeneration grade were collected before and after the treatment together with calculation from the MRI. Case studies were showed that decompression therapy for the herniated disc has an positive effect on disc herniations index, disc regeneration, not disc heights. The clinical evidence for the use of decompression in herniated disc remains inconclusive because of limited researches. Further trials, which give attention to these areas, are needed before any firm conclusions may be made.

  • Normal lumbar vertebrae function only when soft tissues are in position, constituting vertebral body, discs and facet feints. Considering the mechanism of supporting bodily weight, the widest movement of vertebral column reaches a lumbar sacral joint to cause structural changes. The feet is proved that lumbago is the damage of lumbar vertebrae accompanied with the change of soft tissues surrounding lumbar vertebrae, rather than simple pain in a certain lesion. It is based on the mechanism of vertebral body and intervertebral discs in the anatomical structure of the lumbar region. In my opinion, it is necessary to prove more accurately the cause of lumbago, escaping from the conventional cause of the abnormality of disc.

  • The spine is one of the most important skeletal joints, and it strongly affects the health of the musculoskeletal system. A normal spine has an S-shape, and it is very important to maintain this shape. Recently, spinal diseases such as low back pain have increased rapidly, especially among the elderly. Some of these diseases are caused by congenital spinal disorders and sporting and accident injuries as well as by bad postures. Improper spinal postures could generate excessive disc pressure, which is related to degeneration and pain. Therefore, in this study, we investigated the three-dimensional kinematic parameters of the thoraco-lumbar joint in several bad postures using a motion capture analysis technique. Different bad postures created a significant amount of flexion/extension, side bending, and axial rotation angle compared with neutral postures. Further study is necessary to investigate the disc pressure and ligament force due to the increase in joint rotation from the bad postures.

  • In this study, comparisons were made among three representative methods for predicting compressive forces on the lumbosacral disc: LP-based model, double LP-based model and EMG-assisted model. Two subjects simulated lifting tasks that are normally performed in the refractories industry. In the refractories lifting tasks, vertical and horizontal distance, and weight of load were varied. To calculate the L5/Sl compressive forces, EMG signals from six trunk muscles were measured and postural data were recorded using the Motion Analysis System. The EMG-assisted model was shown to reflect well all three factors considered here. On the other hand, the compressive forces of the two LP-based models were only significantly affected by weight of load.

  • The intervertebral disc in the anterior portion of the function unit gives the spine its flexibility. The disc is attached closely to the vertebral endplates. Between these endplates and the annulus fibrosus, the nucleus pulposus of the lumbar disc is enclosed in a circle of unyielding tissues. Compressive pressure placed on the disc is dissipated circumferentially in a passive manner In response to the greater axial forced exerted on the lumbar spine in comparison to the cervical and thoracic spines, the nucleus pulposus has its greatest surface area in the lumbar spine. The intervertebral disc is not only structure that helps diss pate stresses placed on the spine. With flexion, extension, rotation, or shear stress, the load distribution on the function unit is shared by the intervertebral disc, anterior and posterior longitudinal ligaments, the facet joints and capsules, and other ligamentous structures like the ligamentum flavum, interspinous and supraspinous ligaments, which attach to the posterior elements of the functional unit.

Effect of Lumbar Stabilizing Exercise SEBT Training on Isometric Lumbar Strength, Dynamic Balance Ability and Range of Motion in Open Laser Lumbar Microdisectomy Patients (미세 현미경 레이저 요추 디스크 절제술 환자의 요부안정화운동과 SEBT 트레이닝이 등척성 요부근력과 동적균형능력, 관절가동범위에 미치는 영향)

  • This study examines the effects on isometric lumbar extension strength, dynamic balance ability, and range of motion, after administering 8 weeks SEBT exercise (dynamic balance exercise) and Lumbar Stabilizing exercise, to open laser lumbar microdiscectomy patients. Totally, 14 patients who underwent preservation treatment for 6 weeks after undergoing open laser lumbar microdiscectomy, were enrolled for the study. Patients were randomly assigned to the Lumbar Stabilizing Exercise Group (LSG, n=7) and SEBT Exercise + Lumbar Stabilizing Exercise Group (SGLS, n=7). Results indicate that isometric lumbar extension strength ratio significantly decreases after 8 weeks rehabilitation exercise in both the LSG (p=0.007) as well as SLSG (p=0.024) groups. Normalized reach distance of the three directions in the Y-balance test to examine the dynamic balance capability showed a significant increase in both groups. The dynamic balance capability showed significant increase to the left (LSG, p=0.010; SLSG, p=0.002) and right (LSG, p=0.002; SLSG, p=0.002). Moreover, significant increase was also obtained in the range of joint operation, in both LSG (p=0.006) and SLSG (p=0.017) groups. These results indicate that both groups of rehabilitation exercise achieve positive outcomes on the isometric lumbar extension strength, range of motion, and dynamic balance ability. However, some results suggest that the SEBT program is likely to have a better efficacy.

Graphic-based guidance point display system design for obtaining and improving lumbar exercise effect (요추 운동효과 향상을 위한 그래픽 기반 유도점 표시 시스템 설계)

  • The use of computers has increased greatly, and services have been developed very much for the convenience of users. As the use of computers increases, human activity has declined, and symptoms of deterioration in health are increasing. As the time to sit and live increased, humans are more exposed to diseases of low back pain, and low back pain includes pain related to the lower back centering on the herniated disc. Most of the back pain is caused by abnormalities in the muscles and ligaments that support the spine and trunk. In this paper, we present an overall design for developing an app that can obtain lumbar exercise effects to reduce low back pain or help treatment. In order to use this effectively, we propose a plan to obtain lumbar exercise effect, and aim to present an app design that can help effectively treat low back pain by using graphic-based guidance points.
