Effects of Core Exercise Program on the Low Back Function in Private Guard and Security (경호원들의 코어프로그램 수행과 요추부 기능개선에 관한 연구)

  • Background: This study was to investigate the effects of spinal strengthening exercise and lumbar stabilization exercise(core exercise program) on trunk muscle strength, flexibility and balance, lumbar function. The subjects of this study were the eighteen subjects who was registered in private guard company. The each exercise group of 9 persons were chosen by random controlled trial. Methods: We used instrument BTE, Libra, Ruler Measuring tape to measured trunk muscle strength, flexibility, balance and lumbar function. Results: The result of this study were summarized below; The flexor muscle strength was improve in lumbar stabilization exercise(core exercise) group(p<.05). The extensor muscle strength was improve in lumbar stabilization exercise(core program) group(p<.05). The trunk flexibility was improve in spinal strengthening exercise group and lumbar stabilization exercise group(p<.05). The balance ability was improve in spinal strengthening group and lumbar stabilization exercise group(core program) group(p<.05). Conclusion: With the above results, demonstrated effects of spinal strengthening exercise and lumbar stabilization exercise in each private guard and security in this study.

  • Objective : In contrary to ganglion cyst that commonly develops in connective tissue of peripheral joint or tendon, spinal ganglion cysts have been rarely reported. The authors report five cases of spinal ganglion cysts which originated from posterior longitudinal ligament of the lumbar spine. Methods : The authors retrospectively analyzed clinical, radiological, and surgical findings of five cases of spinal ganglion cysts and speculated the pathogenesis of spinal ganglion cyst. Result : Cysts were excised totally and symptoms improved without complications in all cases. Intraoperative findings revealed cysts that were tightly adherent to posterior longitudinal ligament. Conclusion : Five cases of ganglion cyst which caused similar symptoms and signs those of lumbar disc herniations were excised successfully. MRI and operative findings suggested spinal ganglion cyst of posterior longitudinal ligament were closely associated with disc degeneration which imply disc degeneration or herniation may play an important role in the pathogenesis of ganglion cyst.

Loss of Disc Height after Spontaneous Regression of a Herniated Lumbar Disc - A Case Report - (탈출된 요추 추간판 자연 소실 후 발생한 추간판 간격 감소 - 증례 보고 -)

  • Study Design: Case report. Objectives: We report 3 cases of loss of disc height after spontaneous regression of a herniated lumbar disc. Summary of Literature Review: Reports of spontaneous regression of a herniated lumbar disc were identified. Materials and Methods: We conservatively treated 3 patients who were diagnosed with a herniated lumbar disc. During outpatient follow-up, radiating pain improved in all patients, but they complained of chronic lower back pain. Magnetic resonance imaging (MRI) was performed for diagnostic purposes. Results: On MRI, spontaneous regression of the herniated lumbar discs was observed, but loss of disc height was also found. Conclusions: A herniated lumbar disc may be a risk factor for loss of disc height. It is important to recognize that a patient with a herniated lumbar disc can struggle with chronic lower back pain even if spontaneous regression of the herniated lumbar disc occurs.

Influence of the Lumbar Spine Adjustment using the Lumbar Roll Support on Head and Neck Posture in Older Adults (요추 지지대에 의한 노인의 요추만곡 조절이 머리와 목 자세에 미치는 영향)

  • The objective of this study was to identify the effect of lumbar lordotic curve adjustment on head and neck posture in older adults. Methods Twenty healthy older participants (mean age 71.32) were photographed while watching monitor in sitting with or without lumbar roll support(length 28cm, diameter 10cm). The upper cervical angle and lower cervical angle were measured using the NIH ImageJ 1.32. Comparisons between upper and lower cervical angle with or without lumbar roll support were made using paired-t test analysis. Results Subjects demonstrated a significant difference in the mean upper and lower cervical angle. Mean difference of the upper cervical angle was about $2.83^{\circ}$ with and without lumbar roll support(p<0.005). Mean difference of the lower cervical angle was about $4.44^{\circ}$ with and without lumbar roll support(p<0.0001). Conclusions This study showed that healthy older adults demonstrated more ability to maintain an upright posture of cervical spine during lumbar lordotic curve maintenance with lumbar roll support than without lumbar roll support. When the clinicians consider improvement of the head and neck posture in older adults, they must incorporate adjustment in the lumbar region.

Characteristic Analysis of Flexibility and Muscle Strength according to Exercise using Lumbar Strengthen Exercise Instrument (요추강화 운동기기의 훈련을 통한 유연성 및 근력 특성 분석)

  • In this paper, We investigated the characteristic analysis of flexibility and muscle strength for exercise to verify capacity in rehabilitation exercise of lumbar using lumbar strengthen exercise instrument. We have experiment in 20th years man and woman who are 20 subject with no medical history, we divided subjects into control group with no exercise and training group with lumbar strengthen exercise. We used Hi-Spine(Medicalscience.korea) also, provided exercise 40 minute a day, three days a week and progressed total four weeks. Moreover in our experiment, subjects exercised four postural position as lay down, sit, stand and stretch each ten minute. We measured trunk extension backward, trunk flexion forward, evaluation of based physical fitness and lumbar joint torque. The reults have shown that there more improved all for flexibility, based physical fitness and lumbar joint torque in training group than control group. We indicated that by rotating 3-D axis movement flatform of exercise instrument, muscle spindle in subject have been stimulated and these rotation direction and angle caused muscle tonus and contraction that makes muscle, flexibility and based physical fitness improve more. Our study can be used rehabilitation exercise program to aged people and patient with lumbar injury.

  • 요통은 산업화된 현대사회에서 흔히 발생하는 질환이다. 요통의 다양한 원인 중에서도 척추의 정상 가동범위를 넘은 불안정성이 요통의 한 원인으로 보고되고 있다. 이 문헌고찰의 목적은 요추 부위의 안정성을 부여하는 운동 프로그램 후 근육 기능의 강화, 통증의 감소를 보고한 논문들을 비교, 분석하는 것이다. 그리하여 요통의 한 원인인 척추의 안정성을 증가시켜 요추 부위의 통증을 감소시키고 기능을 향상시키는 운동 프로그램의 효과를 입증하는 것이다.

Follow-Up MR Imaging Assessment of Natural History of Lumbar Disc Herniation in Patients with Recurred Low Back Pain (재발성 요통을 호소하는 환자들의 추적 요추부 자기공명영상에서 보이는 요추간판 병변의 변화)

  • Purpose : To compare lumbar disc changes between initial lumbar spine (L-spine) MRI and follow-up (f/u) MRI that were performed due to recurred backaches. Materials and Methods : A total 50 patients who had undergone f/u L-spine MRI were retrospectively reviewed. Five discs (L1-S1) were surveyed in each f/u MRI. Lumbar disc changes were defined as no change, aggravations, or improvements compared to initial disc states. These states were defined on the basis of morphologic status and disc levels. Results : In a total of 250 discs in 50 patients, 31 discs (12.4%) showed morphologic changes of disc lesions, whereas 219 discs (87.6 %) showed no changes. Among the 31 disc lesions, 24 were aggravated and 7 were partially improved. And on the basis of disc status, initially abnormal discs revealed any morphologic changes of the degree of disc herniation. A total of 33.3% of the morphologic changes are noted in initially extruded discs. Fifteen morphologic changes of disc lesions were located at the L4-5 level. Conclusion : Our results suggest that correlations between lumbar disc herniations and back pain symptoms are limited, and that evaluations of extra disc lesions are required.

Analysis of the factors influencing headache and backache following lumbar puncture (요추 천자 후 발생하는 두통 및 요통에 영향을 주는 요인의 분석)

  • Purpose : This study aimed to examine the factors influencing the appearance of headache and backache following diagnostic lumbar puncture in children, focusing on the need for strict bed rest after lumbar puncture. Methods : We studied 70 two-fifteen-year-old pediatric patients who underwent diagnostic lumbar puncture from July 2005 to July 2007 at Konkuk University Hospital. We divided them into two groups. Patients in the first group (n=24) were allowed free mobility and patients in the second group (n=46) were to have strict bed rest for four hours after puncture. Data were analyzed by age, sex, number of puncture attempts, cell counts and pressure in the cerebrospinal fluid (CSF), duration of bed rest, and occurrence of headache and backache. Results : The rate of complications was not significantly related to sex, age, presence of enterovirus, CSF pressure, or postural headache. The occurrence of headache was significantly correlated with white blood cell (WBC) count in CSF (P=0.043). Symptom frequency did not differ significantly between the groups. Backache was significantly related to the frequency of puncture attempts (P=0.046). Conclusion : Strict bed rest following diagnostic lumbar puncture in children does not influence headaches and backaches. These are respectively related to the WBC count on the CSF profile and the frequency of attempts. Therefore, after lumbar puncture, absolute bed rest is not necessary and patients are more comfortable with free mobility.
