The purpose of this study is to analyze the use of hospital, hospitalization, medical service, discharge and power of medical care patients who are concerned about moral hazard. We conducted focus group interview with 3 medical care patients and their families and 5 workers who had worked for more than 4 years in a nursing hospital. The main results and implications are as follows. First, admission to nursing hospitals was mostly based on the linkage between the medical institutions and the competition to attract the patients rather than the choice of the patients. Second, the main cause of the long-term hospitalization of medical assistance patients was the lack of social protection measures such as absences of residence and care giver, although there are factors that cause moral hazard such as low self-pay. Third, most of the patients were in need of treatment, but they were admitted to the hospital even though their needs were not higher than those of the health insurance patients. Fourth, the rehabilitation service is the mainstay of the medical service of the nursing hospital, and the roles of nursing staff and care givers are important. Fifth, medical care patients are paying medical expenses for nursing hospitals due to cost of living and family support, but they are exempted from the hospital expenses or the burden of their own expenses in the hospital. Sixth, public institutions and social welfare institutions have not managed continuously since commissioning patients to nursing hospitals and have neglected the connection with community services after discharge.

This study was conducted to provide basic data to evaluate the appropriateness of convalescent hospitalization treatment by investigating the number of hospitalization days and the total treatment expenses of a total of 44,037 monthly billing statements requested from the Health Insurance Review and Assessment Service. Evaluated data consisted of medical care expenses of patients of the diem payment system hospitalized at convalescent hospitals in Daejeon, Chungnam, Chungbuk and Sejong from January through December of 2014. According to the analysis result of the general characteristics of the study objects and the canonical correlation analysis of the top 15 main diagnosis names, 7 canonical functions have been deducted. Among them, six canonical functions were shown to be statistically significant (p<0.001), and canonical function 1 had a chi-squared value of 5955.49 and 98 degrees of freedom at p<0.001 level. Overall, the results indicated that if health and welfare service in the regional society is magnified, social hospitalization can be reduced.

The purpose of this study was to examine the incidence rates of pressure ulcers depending on the types of healthcare organizations and to determine whether the characteristics of patients and facilities influence on the incidence of pressure ulcers in long term facilities. We analyzed data on 796,857 patients of the 2009 National Patient Sample which was extracted from to claims for medical fees to Health Insurance Review & Assesment Service(HIRA). A total of 3.2% of patients(n=25,339) had at least 1 pressure ulcer during their hospitalization. The pressure ulcer rates were highest in long term care facilities(8.2%, n=11,895) following general hospital(2.7%, n=8,052), hospital(1.7%, n= 5,059). According to logistic regression analysis, urinary incontinence (Odds ratio(OR)=2.462, 95% confidence interval(CI)=2.038-2.974), hypertension(OR=1.456, CI=1.400-1.515), peripheral vessel diseases (OR=1.357, CI=1.200-1.534) were significant predictors of pressure ulcers. As the number of diagnoses, age, and the number of doctors per 100 beds increased, the incidence of pressure ulcers increased. In addition, more number of beds was associated with fewer pressure ulcers.

This study investigates the characteristics of patients hospitalized at a convalescent hospital, by considering patient groups based on their medical record information. It further analyzes and correlates the factors, thus providing basic data required to improve the patient classification system at convalescent hospitals. The data includes total information of 213 medical records of patients discharged from a convalescent hospital in the Jeonbuk region during a period of one year, from January to December 2016. The study examines and correlates the days of hospitalization and the number of diseases, revealing a positive correlation having a correlation coefficient, thereby indicating that infliction with a greater number of diseases results in longer hospitalization. Based on these findings, the study raises the need to revise and supplement items on the patient assessment report to help determine the patient groups and identify medical efforts to be actually provided to patients. In addition, a proper care service system for each patient group based on their respective problems that are intensively managed according to the patient groups will be an essential element in the efficient management of convalescent hospitals. Furthermore, an important task addressed will be in managing the health of the elderly population at the national level.
This work has performed to find what activities of daily living are required for the intensive interests when inpatient elderly more than 3 months has been supported and convalescent care, where the inpatient elderly were judged by the inpatient assessment report in the time of December, 2013. According to the estimation with logistic function of the relationship between the state of decision making recognition technology and the Activities of Daily Living(ADL), the intensive cares for the elderly are required in the parameters of 'Having meal' and 'transferring sitting' when they are severed and convalescently cared as the degree of functional independence for ADL are severly proceeded. In addition, the senescence and disease the activities except 'Having meal' and 'transferring sitting' seem to be influenced by the decline of body function more than the state of decision making recognition technology for daily living.

In this study, professional oral care was provided and 3 times a day twice a week by a dental hygienist for 3 months from July 24 to October 25, 2011, to 43 long-term patients in a single nursing facility located at the city of D, and the number of mutans strepococci colonies within the saliva and the level of salivation were then analyzed after 3 samplings with an interval of 1 month to compare the intraoral conditions among the subjects and examine the effects of professional oral care on the Streptococcus mutans population in the intraoral region of long-term patients in a nursing facility. The level of salivation was elevated from 5.8 ml to 6.4 ml after one month and to 7.5mL after two months. The population of bacteria decreased from $6{\times}10^8CFU/ml$ to $3{\times}105CFU/ml$. In terms of the change in the bacterial population in accordance with systemic diseases, the patients with hypertension and diabetes showed a meaningful decrease in the population. As more dental charge and prosthesis are present, the number of bacteria decreased significantly. On account of the close relationship between a professional oral care and the population of Streptococcus mutans at the intraoral region of long-term patients in nursing facilities, various dental health programs should be researched and developed to consider the characteristics of long-term patients in nursing facilities and manage them continuously but effectively.
The purpose of this study was to investigate the relationship among the consumption value and hospital satisfaction and patient caregiver's psychological well-being(PWB) in geriatric hospitals. Data from 355 subjects were collected and analyzed. The findings are as follows. Functional value and emotional value influenced hospital satisfaction positively, while rarity value influenced negatively. Hospital satisfaction influenced patient caregiver's "PWB" positively. And family's support, which had a positive effect on patient 'caregiver's "PWB", played a role in moderating variable between functional value and hospital satisfaction. From this study, the implications of the findings were discussed and hospital management research project that geriatric hospitals should improve hospital satisfaction and "PWB" of patient caregiver was proposed.
This study analyzed the characteristics of the presence of multiple chronic diseases (MCDs) in older adults who transferred from long-term care hospitals (LTCHs) to emergency departments (EDs). According to the data from the national emergency department information system from January 1, to December 31, 2019, the number of older adults transferred from LTCHs to the ED due to chronic diseases was 13,608. Among those who MCDs, 79.9% were over 75 years old, and 74.0% were hospitalized for MCDs. The length of stay in the ED differed according to the presence of MCDs (P<0.001). As for the prevalence of MCDs, the odds ratio (OR) of the ED and in the hospitalized patients was high in Gwangju (OR 8.899 vs. 8.142) and Jeonbuk (OR 13.865 vs. 10.676). As described above, the characteristics of patients regarding the presence of MCDs varied according to age and region.
This paper is aimed at providing fundamental data for marketing strategies needed to determine the direction of policy to operate hospitals in an effective way. From the analysis, it was found that: first, expectation toward hospitality of the staff, facilities, environment and administrative service of hospitals for the elderly exercises significantly positive(+) effects on satisfaction of patients; second, trust in and satisfaction with administrative service of hospitals for the elderly exerts significantly positive(+) effects on the intent of patients for revisit and information by word of mouth; and third, the intent for revisit influences positively the intent for information by word of mouth significantly. It is therefore needed to try to understand the motives and tendencies of the elderly in selecting hospitals, to promote their satisfaction and to cultivate the spirit for patient-oriented service. Furthermore, efforts should be made to gain trust of medical staff, effects of medical treatment as well as medical equipment in addition to laborious endeavors to work out marketing strategies backed up by price competitiveness of paradigm.
Objective: After introduction of the Elderly Long Term Care Insurance, number of geriatric hospitals increased greatly. However, few studies were conducted on the satisfaction of the patients of the geriatric hospitals especially on the intention to reuse geriatric hospitals. Methods: The current study modified Sheth's consumption value model, useful for the development of an effective marketing strategy for geriatric hospitals, and tested it in Korean circumstances. The survey was conducted at two municipal hospitals and three private hospitals in Seoul and Gyeonggi Province between August 1 and September 30 of 2012. Data were collected from 472 inpatients who are 60 years or older. The structured research tool was developed and distributed. Factor analysis and hierarchical logistic regression analysis were conducted using SPSS(version 14.0) Results: The result of the research suggested that the satisfaction on rehabilitation services, which is a functional value, among the consumption values was the strongest factor on the intention to reuse geriatric hospitals. Impression of high-end luxury, reliability of care, and outside locus of control on health followed. Conclusion: It is recommended that geriatric hospitals ought to improve rehabilitation programs, facilities, and equipments and enhance kindness of staff to encourage intention to reuse geriatric hospitals.
