The purpose of this study was to analyze the characteristics of violent experiences, the types of responses to violent experiences, the coping methods among nurses working in long-term care hospitals. Data were collected using structured questionnaires from 226 nurses in 12 long-term care hospitals in J province. Data collection was performed between April 15, 2016 and April 29, 2016. The data were analyzed by using descriptive statistics and ${\chi}2-test$. Of all the respondents, 55.8% had experienced violence in the past 1 year. Of the perpetrators of the violence, 54.6% and 60.0%, were aged 65 and over and had cognitive functional decline, respectively. The most frequent type of violence was verbal abuse(90.5%). Following the violent experiences, emotional responses had the highest score($2.8{\pm}0.8$). The major coping behavior was "told their peers(84.1%)", followed by "disregarded it and worked although it felt bad(75.4%)". These results indicate the need to establish systems for violence prevention and coping, as well as applicable educational programs, tailored to the circumstances of long-term care hospitals and the characteristics of their patients.
The main purpose of this study is to provide the sustainability and continuous development of Long-term care Insurance in projecting changes of the Long-term care Insurance beneficiary population and Cost. We conducted a transformed cohort-component projection method that are employed for the beneficiary population projection and applied the previous experiences in Japan and German. A transformed cohort-component method means that we also projected the increasing beneficiary of long-term care insurance for using the data of geriatric disease in NHIC and estimated the cost of insurance's financial resources. First of all, beneficiary increase and strategy of extending to level 4 are categorized 2 and the expense account projection are categorized 2. If it is thought experience of Japan and German, The Level 4 extend of insuree is projected 2012 or 2013. With the results of this study, we proposed that extended level 4 insuree include the 40%~90% of geriatric disease in elderly people. The number of beneficiaries in 2011 is expected to reach to about 342,896 and in 2015 is 415,905 on scenario 1. Scenario 2(40%of geriatric disease in elderly people), the number of beneficiaries in 2011 is 342,896 and in 2015 is 483,453. Scenario 3(90%of geriatric disease in elderly people), the number of beneficiaries in 2012 is 545,068 and in 2015 is 565,565. The cost of beneficiaries insurance benefit of scenario 1 are projected from 3,000billion in 2012 to 3,500billion won in 2015. Scenario 2 are projected from 3,100billion in 2012 to 4,000billion won 2015. Finally, The cost of Level 4 extending are need minimum 300billion to maximum 1,400billion won.
In medical litigation, there are various cases where a doctor's 'explanation' of a patient becomes problematic. Medical explanations and guidance are required from the doctor, starting from the beginning of diagnosis, through treatment processes such as surgery, when hospitalization is necessary for treatment, during hospitalization, upon discharge, and after discharge. Furthermore, notification from the doctor or medical institution may be requested regarding the economic costs that will be incurred due to medical treatment. South Korea's judiciary has been developing legal principles regarding such doctor's explanations by distinguishing between explanations for obtaining consent for medical treatment and medical explanations related to guidance on patient treatment methods, taking into account related laws such as the stage of treatment and the Medical Service Act. Additionally, the Constitutional Court recently ruled on the non-benefit cost notification system linked to the explanation of economic costs. However, holding a doctor accountable solely because the doctor's explanation was insufficient has aspects that do not correspond to the actual situation in clinical reality, and may have a reflexive disadvantage that results in a decline in legal rights. Therefore, the doctor's explanation needs to be examined from both perspectives: guaranteeing the patient's right to self-determination and protecting his or her right to decision.
Purpose: We conducted a descriptive correlational study to determine a relationship between nurses' awareness of good death and attitudes toward terminal care, which in turn could be used as basic data for improvement of the quality of terminal care at geriatric hospitals. Methods: From April 3, 2013 through April 22, 2013, data were collected from 230 nurses working at geriatric hospitals. Results: Nurses' attitudes toward terminal care showed no significant correlation with awareness of good death, but it was positively correlated with a sense of closeness, a subfactor of awareness of good death. There was negative correlation between emotions regarding a deathbed, a subfactor of attitudes of nurses in charge of terminal patients, and awareness of good death. We found positive correlation between terminal care performance and awareness of good death. Conclusion: This study warrants the need for nursing education catered to characteristics of geriatric hospitals and development of diverse intervention strategies to help them to attain a positive attitude toward death by familiarizing themselves with the concept of good death and enhancing job satisfaction.
This research is an empirical case study that suggests the practical practice and output of action plan, the actual performing activity of risk management system established to prevent and respond to risk of the elderly care facility. For this, this research analyzed the action plan practice process and outcome upon 2 years' practice (2014~2015) by A Elderly care facility which has conducted performance activity by establishing risk management system to prevent and respond to danger for the first time in Korea. As the research result, first, risk management system action plan was established in the choice and performance stage of risk prevention and response handling method, the 3rd among 4 staged process of risk management system establishment. Second, as the outcome along with risk management system action plan performance, as the result of comprehending the investigation on risk change for 2 years(2014~2015), risk decreased by 28% in 2015 compared to 2014, displaying effect in risk management activity along with performing action plan. Based on this result, it was determined that action plan for the effective action plan establishment and practice in the elderly care facility should be established with systematic promotion schedule to be well connected with its strategy, achievement goal, and achievement project, etc. based on vision and strategy, instead of being established individually, based on clear matter of responsibility, utilizing such technique as Gantt chart, etc., composing concretely by schematizing in order to view all contents to be practiced clearly.

This study examined and compared the droplet infection prevention activity and cough etiquette knowledge among nursing hospital workers. Data were collected from March 4 to 29, 2019, through personal interviews using a questionnaire in 177 nursing hospital workers (health care provider: n=61, non-health care provider: n=116). The data were analyzed using a x2-test, Fisher's exact test, independent t-test, and ANOVA. The general characteristics were statistically significant between the two groups in terms of sex and age. The prevention activities of droplet infection showed statistically significant differences between the two groups in usually carrying a tissue, usually carrying a handkerchief, usually carrying a mask, carrying a handkerchief at respiratory symptoms, hand washing frequency, and cough etiquette compliance. Knowledge of cough etiquette showed significant differences between the two groups. The knowledge of cough etiquette, according to the non-health provider's prevention activities of droplet infection, was statistically significant in usually carrying a mask and cough etiquette education. Therefore, it is necessary to raise the knowledge about cough etiquette and develop strategies for habitual cough etiquette for each nursing hospital worker.

Objective : Purpose of this study was to investigate the effect of depression and self - efficacy on the quality of life of the elderly hospitalized in a nursing hospital and to suggest a new direction for improving the quality of life of the elderly. Methods : The target was 212 senior citizens who were hospitalized at a nursing hospital located in K city in North Gyeongsang Province. The questionnaire was structured by adding the characteristics of the subjects, the Korean Senior Depression Index (GDS-K), the Self-effective Index (WHQOL-BRFA), and the World Health Organization's Life-Enhanced scale. coded using SPSS ver.18, the depression, self-efficiency, and quality of life differences according to the general characteristics (t-test, ANOVA), and Self-efficacy, depression, and the correlation coefficient between quality of life and A multI-sentence analysis to see the impact of depression and self-efficacy on the quality of life. Results : Self - efficacy and depression according to subject characteristics were different according to length of stay, and quality of life was different in religion. The correlation between depression, self - efficacy, and quality of life of admitted elderly showed statistically significant negative correlation (p <.01) with depression in both the quality of life and self - efficacy sub - variables. The depression and the self - efficacy of the elderly had the greatest effect on the quality of life. The variables were depression (${\beta}=-.328$), social efficacy (${\beta}=.248$), and physical efficacy (${\beta}=.193$). Conclusion : In order to improve the quality of life of the elderly, it will be necessary to provide and develop medical care services that reduce depression and improve self - efficacy (physical and social).
The purpose of this study is to investigate the burden of caring for the care-givers of the elderly in Korea by using an integrated literature review method. A total of 23 studies were analyzed using a search database. When care-givers had higher sense of filial and guilt or more than two diseases, they showed high level of feeling of burden. Also, they had high level of feeling of physical burden by their oldness, service period (especially at the period of 1-3 years). In the feeling of economic burden, they had high level of feeling of burden by their oldness, or elderly's disease periods. The feeling of burden by psychological condition was found in elderly in aged and the beginning of admission of nursing home. The feeling of burden by environment situation was found when the functional status of the elderly was bad. Therefore, we need to concern care-givers's feeling of burden with elderly people in the nursing home. In the future, I believe that the findings of this study will be helpful for development of the intervention program for alleviate burden for the care-giver.
Purpose: This descriptive study is aimed at identifying how nursing hospital workers' performance of end-of-life care is influenced by their knowledge and awareness of hospice palliative care, attitude towards end-of-life care, performance, importance, awareness of death and the factors. Methods: A self-reported questionnaire was used to collect data from 113 workers at an accredited nursing hospital in K province. Variables were their knowledge and awareness of hospice palliative care, attitude towards end-of-life care, end-of-life care performance and importance and awareness of death. An analysis was performed with the frequency, percentage, mean, standard deviation, t-test, ANOVA, Scheffe's test, Pearson's correlation coefficient and multiple regression using IBM SPSS 21.0. Results: The factors affecting the nursing hospital workers' end-of-life care performance were the importance of end-of-life care and their marital status, which showed an explanatory power of 38.2%. Conclusion: In order to improve the nursing hospital workers' end-of-life care performance, a training on the importance of end-of-life care should be provided. Therefore, we would like to propose establishing administrative measures such as 1) efficient staffing to help the caregivers better perform what they think is important, 2) development of a training program that can improve their performance of end-of-life nursing care and 3) a study to verify the effectiveness of the program.
The main purposes of the study were to identify factors affecting health-related quality of life of elderly stroke survivors, and to suggest social work interventions for improving their quality of life. The participants were 328 elderly stroke survivors over 55 years old recruited from 18 long-term care hospitals and 13 nursing care facilities in Chung-Nam Province, Korea. The data was collected through a direct survey method from Dec. 16, 2008 to Jan. 17, 2009. Statistical analysis revealed that the overall health-related quality of life among participants was relatively low, especially in the social relationships domain. As a result of hierarchical multiple regression analysis, depression emerged as the most significant predictor of elderly stroke survivors' health-related quality of life, followed by financial burden in future, activities of daily living, acceptance of disability, comorbidity, and major financial supporter(spouse). Health-related quality of life of elderly stroke survivors in long-term care facilities was affected by a variety of variables from disease-related factors to environmental ones. Particularly psychosocial variables were the most powerful factors to predict HRQOL of elderly stroke survivors. Based on the results, social work implications were discussed in the context of developing clinical strategies to alleviate elderly stroke survivors' health-related quality of life in long-term care facilities. Living with chronic illness and disability is a great challenge to the elderly. Most of all long-term residence in care facilities can cause deterioration of quality of life in old age. From now on, studies on health-related quality of life of elderly with chronic illness and disability should be activated in social work field.
