The psychological consequences of indirect trauma exposure through the news on the Sewol ferry disaster (세월호 뉴스 노출을 통한 간접 외상의 심리적 영향)

  • The purpose of the present study was to examine psychological consequences of indirect trauma exposure through the disaster news. Participants(N=439) completed some self-report questionnaires such as Posttraumatic Risk Checklist(PRC), Impact of Event Scale-Revised(IES-R), and Multidimensional Fear of Death Scale(MFODS) at 68.11(±18.47) days after the Sewol ferry disaster. The data were analyzed with structural equation modeling by AMOS 23.0 program. The results showed that fear of death and periand post-traumatic crisis factors mediated the association of news exposure immediately after disaster and posttraumatic stress symptoms. Findings in this study indicated that news exposure immediately after disaster influenced fear of death which led to the peri- and post-traumatic crisis, and these crisis factors increased posttraumatic stress symptoms. Finally, limitations of this study and suggestions for future study were discussed.

Relationship between Severity of Physical Trauma and Subsequent the Severity of PTSD Symptoms in Traffic Accident Related PTSD Patients (교통사고 관련 외상후 스트레스 장애 환자에서 신체적 외상의 정도와 외상후 스트레스 장애 증상의 심각도 사이의 관계)

  • Objective: This study was designed to evaluate the etiological factors of the PTSD(post traumatic stress disorder) by examining the relationship between severity of physical trauma and subsequent the severity of PTSD symptom in traffic accident related PTSD patients. Method: Subjects were 21 psychiatric inpatients with history of traffic accident related PTSD(DSM-IV criteria), the purpose of evaluation of mental disability and no evidence of organic brain leisons. The severity of physical trauma was assessed by McBride number of nonpsychiatric department and the presence / absence of loss of conciousness. The severity of PTSD symptom was assessed by Hovens' self rating inventory for PTSD. And then we evaluated the correlation between these two factors. And we also evaluated relationship between severity of PTSD symtom and clinical variables. Results : There were no significant relationship between McBride number of nonpsychiatric department severity and symptomatic severity(r= 0.17, p<0.05), the presence / absence of loss of conciousness and symptomatic severity(p>0.05). And significant relationship between symptomatic severity and clinical varibles such as sex, education level, marital status(p<0.05). Conclusions : These data did not support data of previous studies that traumatic severity was correlated with symptom severity but, suggested that other variables affecting the severity of PTSD symptom is more important indirectly. And that the 'trauma' in PTSD is psychological meaning rather than physical meaning is also suggested.

Factors Influencing Post-Traumatic Growth in Traffic Accident Patient (교통사고 환자의 외상후 성장 영향요인)

  • This study is a descriptive correlation study investigating the effects of stress disorder symptoms, resilience, and social network on post-traumatic growth in traffic accident patients. The participants were 158 traffic accident cases enrolled from five 100-bed hospitals situated in city C. Data were collected from July 1 to August 31, 2018, and analyzed by t-test, ANOVA, Pearson's correlation coefficients, and multiple regression using SPSS / Win23. The explanatory power of post-traumatic growth was determined to be 36.9%, and the factors affecting post-traumatic growth were social network and post-traumatic stress disorder. In addition, social networks completely established the relationship between resilience and post-traumatic growth. Our results confirmed that a wider social network and increased symptoms of post-traumatic stress disorder of the traffic accident patient are associated with higher post-traumatic growth. Therefore, it is necessary to explore approaches that improve the social networks and resilience to help post-traumatic growth of traffic accident patients. Additional research is required through repetitive and long-term observation of the accident victims.

  • 목적: 일반인과 외상성 스트레스 장애환자들(PTSD)에서 동일한 공포 자극에 대한 기능적 뇌 자기ㆍ공명영상에서 뇌 활성화 부위의 차이를 알아보고자 하였다. 대상 및 방법: 대조군 (여자9명)과 외상성 스트레스 장애로(PTSD) 진단된 환자군 (여자9명)을 대상으로 하였고 모두 오른손잡이였다. 외상성 스트레스 장애군은 모두 교통사고와 관련되어 있었다. 1.5T MR 기기에서 EPI BOLD 기법을 이용하여 24개의 axial slice를 얻었으며, 시각자극으로 교통사고 현장 사진과 비교군으로 체크 무의 사진을 무작위로 배열하여 각각 1초씩 보여 주었다 (event related design). SOA (Stimulus Onset Asynchrony)는 3.5초로 하였고 전체 영상은 10분 동안 얻었다. 모든 환자에서 기능적 자기공명 영상은 1차례 시행하였다. 영상후 처리는 SPM 분석 프로그램을 사용하였으며 황성화 신호의 유의수준은 p=0.01을 기준으로 활성화 영상을 얻었다. 활성화 신호를 육안으로 비교 분석하였고 해마, 편도핵, 전전두엽의 활성화 정도를 중심으로 평가하였다.

The Correlation Between Defense Mechanism and Posttraumatic Stress Disorder in Burn Patients (화상 환자의 방어기제와 외상후 스트레스 장애와의 관련성)

  • Objectives : This study was designed to investigate the correlation between defense mechanism and posttraumatic stress disorder in burn patients. Methods : We recruit 40 burn patients and we evaluate sociodemographic characteristics, characteristics related to burn, korean version of Impact of Event Scale-Revised (IES-R), Korean Version of Center for epidemiologic studies depression scale (CES-D) and Ewha Defense Mechanisms Test (EDMT). Multiple regression analysis was performed to evaluate the correlation between EDMT score and IES-R score. Results : Factor 2 of EDMT which consists of Ego-expansion subjects was significantly correlate with IER-S score. In factor 2, Controlling (β=-0.299, p<0.05), Sublimation (β=-0.276, p<0.05) is significantly correlate with IER-S score. Conclusions : Some subjects of defense mechanism was significantly correlate with IES-R score. it might be helpful to understand defense mechanism for the posttraumatic stress disorder in burn patients.

  • 외상후 스트레스 장애(PTSD)는 장기적으로 심각한 기능 장애를 초래하므로 적절한 치료가 필수적이다. 현재 PTSD 치료법 중 효과가 검증된 주류 심리치료는 환자에게 정서적 고통을 유발하여 치료 중도 포기를 야기하고 치료 효과를 감소시키는 주요한 원인이다. 본 연구에서는 생성형 AI를 적용하여 사용자의 맞춤형 트라우마 이미지를 무의식적으로 노출시키는 방식으로 게임에 적용하였다. 개발된 게임은 디지털 치료기기로 사용함으로써 비침습적인 방법으로 치료의 효과를 증대한다.

Change of Temperament and Personality in Post-traumatic Stress Disorders (외상후 스트레스장애에서 기질 및 성격의 변화)

  • Objectives : This study investigate to identify the stability of temperament and personality characteristics of patients including Posttraumatic Stress Disorder (PTSD) patients. Methods : The number of subjects was 102, of which 33 were PTSD patients and 69 were non-PTSD patients. To demonstrate the change in individual temperament and personality characteristics, Temperament and Character Inventory (TCI) were administered repeatedly on the subjects. Repeated measures ANOVA and simple main effects analysis were conducted. Results : When analysed by dividing the subjects into PTSD groups and non-PTSD groups, the differences between the primary and secondary tests did not appear in the non-PTSD groups, but the differences between the primary and secondary tests were significant in Harm Avoidance (HA), Reward Dependence (RD), Self-Directedness (SD), Cooperativeness (C). In addition, it was noted that the time and group interaction effects of HA, RD, SD and C were significant, and that the main effects of time of HA, RD, SD and C were significant. Conclusions : This study is meaningful in that in the course of experience and recovery of traumatic events, we have clinically confirmed that changes in the temperament, known as stable variables, are possible.

Childhood Trauma and Pharmacotherapy Retention among Outpatients with Panic Disorder (공황장애 외래 환자의 아동기 외상과 약물치료 유지)

  • Objectives : This study investigated lifetime experiences of trauma, treatment retention, and psychiatric symptoms among outpatients with panic disorder after initiation of pharmacotherapy. Our research hypothesis was that panic patients with childhood trauma would display more severe symptoms and less treatment retention compared to those without such history. Methods : A total of 135 first-visit outpatients with DSM-IV panic disorder were approached during the period from March 2012 to August 2016. Fifty-three patients (39%) either refused or returned incomplete questionnaires, leading to a final sample size of 82. Participants completed the Trauma History Screen, the Beck Depression Inventory, the Beck Anxiety Inventory, and the Abbreviated PTSD checklist. Results : The number of lifetime trauma was significantly correlated with treatment retention (rho=-0.269, p=0.015). Among subtypes of trauma, only childhood physical abuse was significantly correlated with treatment retention (rho=-0.298, p=0.007). Conclusions : Our results indicated that psychological trauma, particularly of an interpersonal nature from childhood, can affect pharmacotherapy treatment retention in panic disorder. This may be mediated by poor patient-doctor relationships originating from trust issues among childhood trauma survivors or lack of perceived improvement due to the more severe symptoms and unfavorable course experienced by those with childhood trauma. Further studies are needed to explore the reasons for poor treatment adherence in this population.
