Effect of Eversion Characteristics on Knee and Ankle Joint of Trans-tibial Amputees (인공의족의 외반 특성이 하퇴절단자의 무릎과 발목에 미치는 영향)

  • One of the important functions of prosthetic foot is the foot inversion-eversion which is so important when walking on uneven surfaces. The aim of our study was to evaluate the effect of foot eversion angle especially on knee and ankle joint for transtibial amputees by motion analysis. The experimental data were collected from three transtibial amputees and then ten healthy individuals. To simulate walking on side sloping ground, we used custom-made slope (5, 10, 15 degrees). Motion analysis was performed by 3-dimensional motion analyzer for 6 dynamic prosthetic feet. The results showed that knee abduction moments of amputated leg were decreased but those of sound leg were mainly increased as foot eversion angle increased. And ankle abduction moments of sound leg were inconsistent in magnitude and tendency between control and experimental group. Therefore foot eversioncharacteristics should be considered to develop advanced prosthetic foot.

Patellar Inferior Pole: New Landmark for the Anteromedial Instrument Portal for Arthroscopic Surgery of the Medial Meniscus Posterior Horn (슬개골 하극: 내측 반월상 연골판 후각부에 대한 관절경 수술을 위한 전내측 기구 삽입구의 새로운 표식)

  • Purpose: We prospectively evaluated the clinical usefulness of the patellar inferior pole (PIP) as a landmark of the anteromedial (AM) portal for the arthroscopic surgery of the medial mensiscus posterior horn (MMPH). Materials and Methods: Group 1 (50 normal left knees of adults), Group 2 (10 normal knees under anesthesia), and Group 3 (50 consecutive knees undergoing elective arthroscopic surgery for relatively simple intraarticular pathologies, or diagnostic arthroscopy) were included. In Group 1 and 2, the true lateral (A) and valgus stress lateral radiographs (B) on $30^{\circ}$ flexion were obtained, and the lines (AM portal line) passing through the PIP and distal-most medial femoral condyle (MFC) were drawn under the condition without considering the thickness of articular cartilage of MFC (1, 2-A, B group), and considering it as 2.5mm on B (1, 2-C group). Then, we investigated the meeting point of the AM portal line with medial tibial plateau (C-D percentage), and measured the distance between the PIP and the anterior joint line (E-length), and medial tibial-femoral joint space (F-length). In Group 3, the AM portal was made at the PIP level and clinical usefulness of the approach to the MMPH and body of the lateral meniscus (LM) was analyzed. Results: The average C-D percentage came out as 85.8, 101.3, 69.1% for each Group 1-A, B, C, and 102.4, 144.6, 116.8% for each Group 2-A, B, C. Measured E-length was an average of 15.1 (Group 1-A), 15.5 (Group 1-B, C), 13.1 (Group 2-A), and 12.9 mm (Group 2-B, C) and the change by valgus stress had no statistical significance. The F-length increased about 1.2 (Group 1) and 3.6 mm (Group 2) when valgus stress was applied, which had statistical significance (p<0.001, p<0.001). In Group 3, 49, 48 knees were classified as good for the MMPH, and the body of LM in aspect of the clinical usefulness of AM portal made on the PIP level. Conclusion: We identified the clinical usefulness of the PIP as a skin landmark of AM portal for the arthroscopic surgery of the MMPH.

However, the studies of ankle taping is focused only ankle joint. The purpose of this study was to examine the effect of taping on lower extremity during jump landing in subjects with functional ankle instability. we collected the data from VICON. Joint range of motion for sagittal plane, frontal plane and transverse plane were measured during a jump-landing task. Taping used in this study appeared to restrict ankle motion with altering the knee and hip joint ROM. We found ankle taping had effects of reducing PF, initial contact PF in sagittal plane, while increasing knee flexion, hip flexion, intial contact hip flexion statistically. It decreased ankle inversion, knee valgus, initial contact knee valgus, hip abduction in frontal plane and ankle abduction, hip internal rotation in transverse plane statistically. Ankle taping showed significant change Knee and hip joint ROM with ankle joint ROM.

  • Subacute Ankle Sprain patients, 3 tests have been acknowledge for diagnostic precision and thus most commonly used to test their soft tissues, muscles, and ligaments: ankle stress-valgus test, anterior-draw test, and MRI scan. Although not much different from the rest two in its diagnostic value, MRI costs patients some money and time. Also, we have reached the conclusion through an experiment with a group of patients in a certain hospital that the rest two methods is as efficient and useful as MRI in diagnosing ankle joint patients.

  • Purpose : The purpose of this study is to investigate an occurrence of valgus deformity of ankle joint after vascularized fibular graft in children. Materials and Methods : Four children under 15 years who were surgically treated with vascularized fibular graft were studied. The age of the patients was from 4 years to 13 years, the follow-up period was from 24 months to 108 months. The causes of vascularized fibular graft were open fracture (1 case), congenital psuedarthrosis (2 cases), hypoplastic ulna (1 case). The tibiofibular synostosis was done in 3 cases and not in 1 case. We measured the tibiotalar angle and bimalleolar angle at immediately postoperative and final radiography, and checked ankle motion, pain, and instability of ankle joint. Results : The A-P mortise angle was not different between initial and final radiography in all cases. The intermalleolar angle increased in all cases at the final radiography. There were no pain, instability and limitation of ankle motion. Conclusion: We consider the tibiofibular synostosis can prevent from ankle valgus deformity after vascularized fibular graft in children.

  • Past research has reported that the common causes of ankle arthritis include trauma, congenital deformity, and degeneration. Among them, fracture-induced post-traumatic arthritis is most common. For patients with ankle fractures, an anatomical reduction is performed through surgical treatment. However, insufficient reduction or malunion of the fracture site may change the alignment of the ankle joint, resulting in valgus or varus deformities. Currently, most operative options for valgus arthritis aim to either restore joint alignment and/or reduce the uneven load on the cartilage. In this report, we would like to share our clinical experience of a patient with posttraumatic valgus ankle arthritis caused by severely comminuted fracture and dislocation. A satisfactory outcome could be obtained with combined fibular lengthening osteotomy and medial displacement calcaneal osteotomy.

  • Background: Investigation in gender differences of kinetics and kinematics for individuals with patellar femoral pain syndrome (PFPS) was not sufficiently performed. Objects: The purpose of this study is that whether there is a difference depending on gender from muscle activity and strength and knee valgus angle during controlled single-leg squat which is widely used as clinical movement test for the patient with PFPS. Methods: 20 young adults (10 men, $20.0\pm}2.1$years; 10 women, $20.4{\pm}2.1$years) with PFPS were voluntarily recruited in this study. Muscle activity and strength and knee valgus angle were collected during single-leg squat. Independent t-test and Mann-Whitney test were used to compare the differences between groups of male and female. Results: Rectus femoris (t=-2.204, p=.041) and vastus medialis oblique (t=-2.151, p=.045) muscle activity of women were significantly higher than male group. Normalized muscle strength of hip and knee muscles showed a significant difference between men and women (p<.05). Valgus angle of the knee in women (t=-2.450, p=.025) were increased significantly than men. Conclusion: The therapist would consider the characteristics of these gender differences during performing movement test, exercise, and education for the individuals with PFPS.

The effects of hallux valgus on ankle plantarflexor and dorsiflexor torque (무지외반이 족관절 저굴근과 배굴근의 우력에 미치는 영향)

  • The purpose of this study was to find out the effects of hallux valgus on the ankle plantarflexor and dorsiflexor peak torque, on the total work, on the average power, and on the Peak torque of plantarflexor to dorsiflexor ratio values. Isokinetic ankle plantarflexor and dorsiflexor strength was evaluated in 30 women(control: 15, hallux valgus: 15) by Cybex NORM System. Test data was gathered in the right ankle at a speed of 60/sec, 90/sec, 120/sec in the control group and the hallux valgus group. Statistical analysis was performed using SPSS 10.0 for windows software. Mean and standard deviations were measured and calculated for the General characterristic of subjects. A paired t-test was used to examine the differnces in the means for the ankle plantarflexor and dorsiflexor between two groups. Pearson coefficient correlation was used to examine the correlation of the hallux valgus and the peak torque. Analysis of variance indicated that isokinetic values of the control group were greater than the hallux valgus group(p<.05, p<.01).
