Objective: The purpose of this study was to evaluate how airway changes influence facial growth during puberty in Korean children. Methods: Thirty-six patients aged 9 to 11 (mean age: 10.7 years) were selected. Cephalograms and hand-wrist x-rays were taken at pre-treatment (T1) and post-treatment (T2). The sample was divided into narrow (5.2 - 8.6 mm, AW-Narrow), medium (8.9 - 11.5 mm, AW-Medium), and wide (11.7 - 16.0 mm, AW-Wide) groups according to the airway width at T1. Cephalometric measurements at T1, T2 and growth from T1 to T2 were compared between groups. Results: The degree of increase in airway size in each group was 4.55 mm, 3.84 mm and 1.94 mm in the AW-Narrow, AW-Medium, and AW-Wide groups, respectively. Moreover, the differences were statistically significant. The significant smallest posterior facial height was found in the AW-Narrow group at T1. For the growth values from T1 to T2, the AW-Narrow group showed significantly larger values of PFH/AFH, facial axis, ANS (T1 - T2), and Gn (T1 - T2) than the AW-Medium group. Conclusions: The compensational changes in the airway width and facial growth were found in the narrow group. Surgical approach of the airway to prevent unfavorable facial growth in these years of age should be carried out, but with careful deliberation because these problems may be improved naturally.

The aim of this study was to investigate the relationship between the C-factor and shrinkage strain values of composite resin and examine the strain values in different incremental filling techniques. Experiment consisted two aims. First, we compared with strain value in two different C-factors(3.7 and 1.0). Second, we examined the strain values in three different filling techniques. The results of the present study can be summarized as follows : 1. High C-factor groups showed higher contraction stress values than low C-factor groups at 900 sec after polymerization. 2. Hybrid resin showed higher contraction stress values than flowable resin in high C-factor cavities. But contraction stress was not revealed significant difference between hybrid resin and flowable resin in low C-factor cavities (P>0.05). 3. Bulk felling with hybrid resin(Group 1) showed high contraction stress and lining with flowable resin followed hybrid resin (Group 5) showed lower contraction stress. 4. Contraction stress were increased during 900 sec after polymerization in high C-factor groups but decreased gradually after 900 sec. 5. Low C-factor groups showed tight marginal seal between resin and cavity wall but high C-factor groups showed gaps formed between resin and cavity wall in part. On the basis above results, layering techniques in high C-factor cavity showed advantages in reducing contraction stress and gap formation between cavity wall and resin restoration.

Objectives: This study is designed to suggest the health service goals necessary for providing the more efficient services relevant to the requests of the community, through the evaluation on the operating status of the unified health subcenters. Methods: We visited total 5 unified health subcenters comprising 3 ones located in Gyeongsangbuk-do and 2 ones located in Gyeongsangnam-do from December 2000 to January 2001, and interviewed about the pre- and post-unified status related to manpower, facilities, equipment, medical service and health service quality, and the problems and improvement plans of the unified management. Results: According to the evaluation on the manpower before and after the unification of the health subcenters, the total employees increased by 2.8 persons on average from 6.8 to 9.6 persons in the investigated subjects. The numbers of doctors, dentists and nurses were almost the same as before. There were no clinical pathologic technician and radiological technician before but they were appointed to duty in 3 unified health subcenters later. The unification of the health subcenters has produced slight increases in the frequency of the medical service and dental treatment and considerable increases in that of the physical therapy and laboratory tests. In relating to the changes of the health service, the cases of visiting health care and ambulatory medical service, and the total number of health education participants were greatly increased after the unification. The number of cases undergoing the vaccination and cervical cancer screening was similar to that of the pre-unification while the patient number of the registration to hypertension or diabetes showed a tendency to increase a little. Since the unification of the health subcenters, the frequency of laboratory tests has been increased, but the quality of health service has not been improved yet. Nevertheless, the unification seems to be positive according to the result of the great improvement in visiting health care, ambulatory medical service and health education service. The problems of the unification of the health subcenters were indicated in indefiniteness of the service details between the workers; excessively large building hard to be effectively managed; insufficient medical instruments, inappropriation of working expenses, lack of professional training for the health education, etc. Conclusions: For further active functions of the unified health subcenters, the minimal allocation basis to appoint doctors, nurses and administrative workers to do the duty should be differentiated from the basis for a health subcenter, and the fundamental instruments needs to be expanded to improve the quality of the medical service and visiting health care service. Moreover, the unified health subcenter needs to have definite service details between the workers, and should improve the working efficiency through the development of service-related guidelines.

The purpose of this study is concerned with an investigation of the actual condition of traumatic injuries of teeth and supporting structures in children to set up possible criteria for prevention and treatment of injured teeth. The materials consisted of the clinical records of 431 traumatically injured teeth of 212 children, accumulated during two years from the first of April in 1998 to the end of March in 2000, supplied from the Dept. of Pediatric Dentistry, Chonbuk National University Hospital. The incidence of injuries in boys was extremely high for permanent teeth, twice as high as in girls. Accidents to primary teeth were most common from 1 to 2 years of age and to permanent teeth were most from 8 to 9 years of age. The most common cause of trauma was a fall for both primary and permanent teeth, followed by collision. The anterior teeth in maxilla was most frequently affected by trauma in both the primary and permanent teeth. The most common type of trauma were loosening for the primary teeth, followed by luxation types which included the intrusion, displacement and extrusion and complete avulsion types. For the permanent teeth, the most common type of trauma were tooth fracture. The most common trauma of soft tissue was laceration of upper lip, lower lip and gingiva of maxilla Concerning treatment at the first visit, primary teeth with only loosening and concussion were not usually treated. Permanent teeth were often treated by crown restorations for crown fractures and by endodontic procedures for pulpal exposure. Though we could elucidate actual condition of traumatic injuries of teeth in children, we should make a follow-up survey to ensure the prognosis of injured teeth and establish the most desirable criteria for traumatized teeth in children.
Background: In January 2018, revised elderly fixed outpatient copayment for the elderly were implemented. When people ages 65 years and older receive outpatient treatment at clinic-level medical institutions (clinic, dental clinic, Korean medicine clinic), with medical expenses exceeding 15,000 won but not exceeding 25,000 won, their copayment rates have decreased differentially from 30%. This study aimed to examine the changes of health utilization of elderly after revised elderly fixed outpatient copayment. Methods: We used Korea health panel data from 2016 to 2018. The time period is divided into before and after the revised elderly fixed outpatient copayment. We conducted Poisson segmented regression to estimate the changes in outpatient utilization and inpatient utilization and conducted segmented regression to estimate the changes in medical expenses. Results: Immediately after the revised policy, the number of clinic and Korean medicine outpatient visits of medical expenses under 15,000 won decreased. But the number of clinic outpatient visits in the range of 15,000 to 20,000 won and Korean medicine clinic in the range of 20,000 to 25,000 won increased. Copayment in outpatient temporarily decreased. The inpatient admission rates and total medical expenses temporarily decreased but increased again. Conclusion: We confirmed the temporary increase in outpatient utilization in the medical expense segment with reduced copayment rates. And a temporary decrease in medical expenses followed by an increase again. To reduce the burden of medical expense among elderly in the long run, efforts to establish chronic disease management policies aimed at preventing disease occurrence and deterioration in advance need to continue.
Background: The purpose of this study was to investigate the effect of self-perception of halitosis on oral health behaviors of college students according to whether they worked in a non-service or service-related job. Methods: A self-reporting questionnaire of 150 university students working in service jobs and 150 university students working in non-service jobs in the metropolitan area was conducted from 26th April to 5th May 2018. The survey tool consisted of 3 general topic areas. There were 5 questions on oral health awareness behavior, 3 questions related to their self-perception of halitosis, and 8 questions on their oral health care practice. All were measured on the Likert 5-point scale. The frequency of their oral health behaviors and their self-perception of halitosis were analyzed according to the subjects' occupation. Pearson's correlation analysis and a linear regression analysis were conducted to confirm the influence of the two. The significance level for the statistical significance test was set to α = 0.05 (two-tailed). Results: In both the service and non-service groups, a coated tongue and food debris were found to influence halitosis(x2=10.95, p=0.027). According to the t-test, taking into account the self-perception of halitosis and oral health behaviors, both were higher in those that had a service job. Self-perception of halitosis and oral health behaviors were found to have a negative correlation(γ=-0.11, p<0.05). As the self-perception of halitosis increased, oral health care practices also increased(Beta=-0.185, p=0.020). Conclusions: Systematic interventions such as oral health programs and health promotion are needed to improve the oral health of service workers.
The purpose of this study was to analyze the priorities of oral health education contents for preschool children by targeting teachers and parents using the Borich priority formula and The Locus for Focus Model. The survey was conducted in 212 teachers and 215 parents from December 26, 2017 to January 21, 2018. The priorities of oral health education contents were based on a 3-step analysis method, including the paired sample t-test, Borich priority formula, and The Locus for Focus Model. As a result of this study, the number of items about oral health education for preschool children that were prioritized by teachers was 7, while that by parents was 9. The top priorities that teachers and parents had in common were the following 5 items; "The progression of dental caries," "Symptoms of dental caries," "How to prevent dental caries," "Eruption sequence of permanent teeth," and "Method for emergency management of avulsed teeth." The teachers' priorities of the oral health education contents were the same between teachers and parents; "Eruption sequence of deciduous teeth" and "The function of the permanent teeth" were added. The parents' priorities of the oral health education contents were the same between teachers and parents; "The effect of fluoride application," "The number of permanent teeth," "How to prevent malocclusion," and "The appropriate timing of malocclusion treatment" were added. Based on the results of this study, when developing oral health education programs for teachers and parents, oral health education for teachers should include 7 items and oral health education for parents should include 9 items.

The purpose of this study was to evaluate the surface change after 15% carbamide peroxide home bleaching to various restorative materials (composite resin [CR], resin modified glass ionomer [RMGI] and glass ionomer [GI]) and to observe the effect of surface condition of the materials on re-staining. Three esthetic restorative materials (Filtek Z250, 3M, USA; Fuji II LC, GC, Japan; Fuji II, GC, Japan) were used in this study. Twenty specimens per material group were made and divided into two groups (bleached and control). The specimens were immersed in coffee after applying bleaching agent. The color change and surface roughness were measured before and after bleaching and after immersion in coffee. The data were analyzed with SPSS 18.0. The results were as follows: 1. The color of all experiment groups was significantly changed after bleaching (p<0.05). RMGI was the greatest value of ${\Delta}E^*$ and ${\Delta}L^*$. GI and CR groups were in ordering (p<0.05). The ${\Delta}a^*$ value was decreased GI, RMGI and CR. RMGI was only significantly decreased in ${\Delta}b^*$ value (p<0.05). 2. The surface roughness before and after bleaching was significantly different on CR, RMGI and GI (p<0.05). 3. After staining with coffee, the value of ${\Delta}E^*$ was increased in GI, RMGI and CR, furthermore GI and RMGI showed significant difference in the bleaching groups (p<0.05). The ${\Delta}L^*$ value of GI and RMGI was significantly decreased. 4. The change of surface roughness after staining was not significantly different in all groups (p>0.05). The maintenance of color stability in esthetic restorations is one of the most important properties. Tooth whitening is for the aesthetic. Therefore, dental professionals should notice to patients about re-staining after tooth whitening. They should give an instruction that how to prevent and which kinds of agents could be stained.
It is important to practice preventive method to control oral disease. Dental caries tendency has been decreased in developed countries, but early childhood caries(ECC, BBTD) became serious dental problems in many countries. To slove these problems, more positive and definite prenatal programs on infants and child are needed. Mostly the control of oral health was affected by a mother's knowledge and behavior in this stage. According to many studies, prenatal education is the most effective method to promote oral health in children. But in Korea, there is little oral health education programs for pregnant women, nursing mothers, infants and children. The purpose of this study was to access the knowledge and opinions of caries prevention among gravid women. 330 pregnant women(primiparae=181, multiparae=149), aged 23 to 39 years were randomly selected at prenatal health education courses in Seoul, Korea, 2000. Face-to-Face interviews were conducted by a trained interviewer using questionnaires included demographics, attitude toward children's dental cares, knowledge of early childhood caries, knowledge concerning pregnancy and oral health. The obtained results were as follows; (1) Most respondents were very concerned about their children's oral health. (2) 58% of the respondents were unaware of early childhood caries and the proportion was significantly higher among primipara (p<0.05). (3) Only 43% of the respondents believed that improper breast feeding could develop ECC, the proportion was significantly higher among primipara(p<0.05). (4) 82% of the respondents reportedly believed that the fetus takes away calcium from the mother's teeth during pregnancy. (5) 71% of the respondents believed that dental treatment during pregnancy was unsafe. There is a relatively low level of accurate knowledge regarding infant nutritions, maternal need for oral health and dental caries prevention among pregnant women. Oral health education and promotion programs that are science-based are needed for gravid women.
This study examined the relationship between the quantity of Streptococcus mutans and Lactobacillus spp. related to dental caries and the degree of acidity in saliva. A total of 240 saliva samples were taken from 80 subjects at the faculty of dentistry in Skopje, Macedonia. The saliva samples were taken by stimulating saliva production stimulation with paraffin chewing. However, no stimulation was applied when obtaining the samples used for measuring the pH. The data showed that in the caries group, S. mutans in 1 ml of saliva formed colonies with confluent growth (CFU > $10^6$ and $10^4-10^5$) in 100% of samples, whereas the Lactobacillus spp. formed colonies with confluent growth in 78.3%. In contrast, no colonies with confluent growth (CFU > $10^6$ and $10^5$) were found in the control group (with healthy intact teeth). In the caries group, the pH of the saliva was slightly acidic (pH = 5.90 - 6.50) and the buffering capacity was very low (below 0.7 ml of saliva per min). On the other hand, the pH of the saliva in the control group was neutral (pH 7.01 - 7.7) and the buffering capacity was high (over 1 ml of saliva per min). The increased number of S. mutans and Lactobacillus spp. in 1 ml of saliva (above $10^5$ CFU or more) from the CRT (Caries Risk Test, Vivadent, Liechtenstein) bacteria test can indicate an increased caries risk as well as slightly higher acidity of the saliva. Overall, these results reveal that the caries risk can be predicted by simply measuring the pH and buffering capacity of saliva, and can be used to monitor the effect of dental hygiene practices with the aim of preventing dental caries.
