Radiologic Diagnosis of Nontuberculous Mycobacterial Pulmonary Disease (비결핵마이코박테륨 폐질환의 영상의학진단)

  • The incidence and prevalence of nontuberculous mycobacterial pulmonary disease (NTM-PD) is increasing worldwide, including in Korea, and the clinical importance of NTM-PD is also rapidly increasing. The diagnosis and management of NTM-PD is difficult. Radiologic evidence is mandatory to diagnose NTM-PD, and the radiologic findings may be the first evidence of the disease in many patients. Traditionally, NTM-PD demonstrates two different radiologic forms: fibrocavitary and nodular bronchiectatic. However, the disease also shows non-specific and a wide spectrum of radiologic features. Radiologists must be aware of the radiologic features of NTM-PD and should include them in the differential diagnosis. This review focuses on the epidemiology in Korea, diagnostic criteria, and radiological features of NTM-PD for radiologists.

  • Cavernous hemangioma of the breast is a rare benign vascular tumor. It is usually diagnosed as non-palpable mass during imaging examination, incidentally. Common sonographic finding of the breast hemangioma is a superficially located, oval shape, circumscribed margin mass. But the appearance of breast hemangioma is variable and it can be difficult to diagnose in preoperatively. Here, we report a 68-year-old female case with palpable breast mass and increased size on follow up ultrasonography.

  • Thoracic foreign bodies (FBs) are serious and relatively frequent in emergency departments. Thoracic FBs may occur in association with aspiration, ingestion, trauma, or iatrogenic causes. Imaging plays an important role in the identification of FBs and their dimensions, structures, and locations, before the initiation of interventional treatment. To guide proper clinical management, radiologists should be aware of the radiologic presentations and the consequences of thoracic FBs. In this pictorial essay, we reviewed the optimal imaging settings to identify FBs in the thorax, classified thoracic FBs into four types according to their etiology, and reviewed the characteristic imaging features and the possible complications.

  • A variety of medical devices have evolved throughout the years. Commonly used devices have typical radiological appearances which are familiar to radiologists. However, some new devices, as well as devices that are not commonly used, may be missed or misinterpreted by radiologists. It is even more difficult to identify a certain medical device with limited clinical history. Therefore, accurate identification of medical devices is crucial to diagnose malposition and potential complications. In this article, we provide a pictorial review of medical devices of the abdomen and pelvis according to classifications that include gastrointestinal devices, hepatobiliary devices, genitourinary devices, and miscellaneous. We also comprehensively review the clinical and radiologic features of complications related to these medical devices.

  • To provide high-quality training to residents in a rapidly changing medical environment, it is very important to improve the annual training curriculum centered on competency and ensure that training hospitals maintain an environment suitable for training. The Korean Society of Radiology (KSR) has been steadily improving the training system and has suggested the improvement of the training system by strengthening the competency-based evaluation and faculty development. Currently, KSR was selected for the second annual training curriculum systematization construction project in July 2021, and developed entrustable professional activities, core competencies, and assessment guidelines required by the construction project. Therefore, the development process and assessment guidelines will be introduced to residents and the faculty.

  • 현재 많은 병원의 핵의학과에서 핵의학 장비를 이용하여 많은 수의 핵의학영상을 생성하고 있다. 생성된 핵의학영상은 환자의 질병을 진단 또는 치료하기 위해 기능적 정보를 많이 포함하고 있다. 하지만 이렇게 중요한 기능적 정보가 현재의 PACS 에서는 그 중요한 기능적 정보를 모두 표현하지 못 하는 문제점이 있다. DICOM 에서는 핵의학 영상 및 데이터에 대하여 표준을 정해놓고 그 표준을 따르도록 규정하고 있다. 이러한 DICOM 3.0 표준에서 핵의학 영상 및 데이터에 대하여 표준을 정해놓은 일은 비교적 최근의 일이어서 많은 수의 핵의학 영상 장비나 PACS에서는 핵의학영상에 대한 특징이 반영되지 않고 있는 실정이다. 이에 핵의학 영상의 호환성을 향상과 PACS와 핵의학 장비간의 호환성을 향상시키기 위하여 DICOM 3.0 Part 3에 정의된 IOD 중 꼭 필요하다고 생각되는 최소한의 Tag들을 선별하여 Guideline을 작성하여 DICOM 영상을 Guideline의 내용을 토대로 분석하였고 핵의학 영상이 PACS에서 제대로 활용되지 못 하는 원인을 분석 하였다.

Radiologic Evaluation of Uterine Lesions Using a Pattern Recognition Approach (패턴 인식 접근법을 기반으로 한 자궁병변의 영상의학적 평가)

  • It is important to distinguish uterine lesions from other lesions occurring in the pelvic cavity for the proper management. The primary radiological evaluation of uterine lesions is performed using transvaginal ultrasonography, and if the lesion is too large or shows atypical benign imaging findings, magnetic resonance imaging should be performed. Analyzing radiological findings of uterine lesions through a pattern recognition approach can help establish the accurate diagnosis and treatment plan. In this pictorial assay, we describe imaging characteristics of various lesions arising from the uterus and evaluate them based on the pattern recognition approach.

  • Polyacrylamide gel (PAAG) has been used for augmentation mammoplasty in certain countries, such as China or Eastern European countries. However, owing to various complications, its use is currently banned in most countries. Nevertheless, because of the migratory stream of female from these countries, we also encounter patients with PAAG mammoplasty in Korea. Owing to the various imaging features of PAAG mammoplasty, it may interfere with the identification of malignancy or other pathologic conditions. Therefore, it is important to identify the imaging findings related to PAAG mammoplasty. However, there is limited literature on the radiologic findings of complications related to PAAG mammoplasty in Korea. Herein we report four cases of PAAG mammoplasty, the complications faced, and depict various radiologic features through multimodality imaging.

  • Physical abuse is a significant cause of morbidity and mortality in the pediatric population. Young children, particularly in the first year of life, are most vulnerable to physical abuse. To evaluate suspected physical abuse, radiologists play a vital role by detecting radiological findings suggestive of physical abuse and differentiating them from other pathologies. This review focuses on radiologic findings, including those for fractures, abusive head trauma, spinal injury, and thoracoabdominal injury, commonly discovered in physically abused children, with special emphasis on biomechanical forces that produce injuries.

  • The Korean Society of Radiology and the Korean Society of Thoracic Radiology have prepared recommendations for the use of diagnostic imaging for COVID-19 in various clinical scenarios. We have tried to grasp the situation in the real world, aggregated opinions from the chest radiologists, and reviewed available references, in order to suggest the most reasonable recommendations possible at this moment. As circumstances change and new evidences emerge, the recommendations should immediately be modified accordingly.
