Fatal plastic bronchitis with eosinophilic casts in a previously healthy child (건강하였던 소아에서 발생한 치명적인 호산구성 증식성 기관지염)

  • Plastic bronchitis is a rare disease characterized by the recurrent formation of branching mucoid bronchial casts that are large and more cohesive than those that occur in ordinary mucus plugging. Casts may vary in size and can be spontaneously expectorated, but some require bronchoscopy for removal. Plastic bronchitis can therefore present as an acute life-threatening emergency if obstruction of the major airways occurs. Three of 22 reported patients with eosinophilic casts were fatal, with death due to central airway obstruction. Here, we report a child with no history of atopy, allergy, or congenital heart disease who was diagnosed with plastic bronchitis with eosinophilic casts. Although he was administered intravenous (iv) antibiotics; iv corticosteroids; and a vigorous pulmonary toilet regimen, including chest physiotherapy and routine bronchoscopic removal of casts, he had brain death secondary to hypoxic brain damage. Plastic bronchitis can be fatal when casts obstruct the major airways, as in the present case. Clinicians should intervene early if a patient exhibits signs and symptoms consistent with plastic bronchitis.

Free Muscle Transplantation of the Chronic Lower Extremity Osteomyelitis (만성 하지 골수염에 시행한 유리 근 이식술)

  • Chronic osteomyelitis have been treated with wound dressing and antibiotics therapy often results in healing but foul odor pus discharges from the fibrotic soft tissues reactivates and requires appropriate control of the infection. Debridement of the wound, curettage and sequestrectomy, bone graft and immediate free flap transplantation is the curative protocol for the chronic osteomyelitis in the lower extremity. Authors have treated 7 cases of chronic osteomyelitis in the lower extremity with microsurgical free tissue transplantation at Department of Orthopedic Surgery, Chonbuk National University Hospital from December 1993 through February 1998. The results are as follows. 1. The chronic osteomyelitis occurred in tibial shaft in 4 cases, in calcaneus 2 cases and in femur 1 case. 2. Duration of the chronic osteomyelitis was at average 31.6 years. 3. Squamous cell carcinoma in the surrounding fibrotic tissue was biopsied in 1 case. 4. 4 cases had no trauma and occurred through hematogenous infection and 3 cases had fracture trauma. 5. Wound debridement and immediate free muscle transplantation had done in 5 cases and wound debridement, sequestrectomy and immediate free muscle transplantation in 2 cases. 6. Rectus abdominis muscle transplantation had peformed in 4 cases(57.1%), latissimus dorsi mucle 1 case(14.3%), latissimus dorsi myocutaneous 1 case(14.3%) and gracilis 1 case (14.3%). 6 cases of 7 were success(85.7%). 7. 1 case of failed latissimus dorsi musculocutaneous flap in thigh had done above knee amputation and 1 case of chronic posttraumatic osteoarthritis of the ankle joint had done below knee amputation at other hospital.

Analyses of Larg Cell Area MCFC System Dynamics (대면적 용융탄산염 연료전지 시스템 동특성 분석)

  • The steady state and dynamic characteristics of large cell area MCFC stacks were analyzed to solve the problems such as temperature difference generated in stacks and pressure difference between anode and cathode. Manipulated variables (current density, duel utilization rate, oxidant utilization rate) and controlled variables (temperature difference, anode and cathode pressure difference) which had an important effect on the MCFC stack performance were determined using operation results of two types of MCFC stacks (5kW (3,000 $\textrm{cm}^2$, 20 ea). 3kW (6,000 $\textrm{cm}^2$, 5ea)). The stability and transfer function representing system dynamics were obtained by steady state gain rate which showed the relative change between MVs and CVs. The transfer function was a 3$\times$3 matrix and a typical first order system without time delay. The optimal operating condition of large cell area MCFC stacks could be determined by analyzing dynamic characteristics. In case of a 5 kW MCFC stack, pressurized operation with recycle flow should be used to control the outlet temperature less than 68$0^{\circ}C$ and to control the MCFC system effectively. MIMO control or decoupler should be used to remove the interaction between MVs and CVs. This result will be used as important data in determining the control structure design and operation mode of large cell area MCFC systems in the future.

Surgical removal of a left ventricular thrombus caused by acute myocarditis (급성 심근염에 의한 좌심실 혈전의 수술적 제거 1례)

  • Left ventricular thrombus is mainly caused by anterior myocardial infarction or severe cardiac wall dysfunction of the apex, and is rarely caused by a complication of acute myocarditis. A 12-year-old female who developed symptoms of motor dysphasia and incomplete hemiparesis of the right side was admitted to the hospital. The brain MRI taken on the day of her admission showed acute cerebral infarction in the left basal ganglia and the frontoparietal lobe. The echocardiogram showed a movable thrombus, which was $19{\times}28mm$ sized and located in the apex of the left ventricle. So in order to prevent further thromboembolic event we performed open cardiac surgery via the atrium and removed the thrombus of the left ventricle. After the removal of the thrombus her symptoms improved and she was discharged from the hospital. Thrombus formation in acute viral myocarditis are considered to be related with endocardial injury and blood flow stasis. Treatment with anticoagulants in left ventricular thrombosis may not be effective and may even cause a major thromboembolism. When the thrombus is laminar and fixed, one should consider anticoagulant therapy. But if the thrombus is pedunculated and movable, which means that there are higher possibilities of major embolism or there may be already one, one should consider surgical removal. We report a 12-year-old girl who required surgical removal of a left ventricular thrombus caused by acute viral myocarditis.

Development of Graphene Nanocomposite Membrane Using Layer-by-layer Technique for Desalination (다층박막적층법을 이용한 담수화용 그래핀 나노복합체 분리막 개발)

  • Forward osmosis (FO) desalination system has been highlighted to improve the energy efficiency and drive down the carbon footprint of current reverse osmosis (RO) desalination technology. To improve the trade-off between water flux and salt rejection of thin film composite (TFC) desalination membrane, thin film nanocomposite membranes (TFN), in which nanomaterials as a filler are embeded within a polymeric matrix, are being explored to tailor the separation performance and add new functionality to membranes for water purification applications. The objective of this article is to develop a graphene nanocomposite membrane with high performance of water selective permeability (high water flux, high salt rejection, and low reverse solute diffusion) as a next-generation FO desalination membrane. For advances in fabrication of graphene oxide (GO) membranes, layer-by-layer (LBL) technique was used to control the desirable structure, alignment, and chemical functionality that can lead to ultrahigh-permeability membranes due to highly selective transport of water molecules. In this study, the GO nanocomposite membrane fabricated by LBL dip coating method showed high water flux ($J_w/{\Delta}{\pi}=2.51LMH/bar$), water selectivity ($J_w/J_s=8.3L/g$), and salt rejection (99.5%) as well as high stability in aqueous solution and under FO operation condition.

A Study on the Separation of Electrolyte from Amino Acid Solution through Electrodialysis (전기투석법을 이용한 아미노산으로부터 전해질 분리정제에 관한 연구)

  • The separation of inorganic salt from amino acid solution using was performed electrodialysis. In order to review the availability of electrodialysis using isoelectric point of amino acid as a bio-separation technique, electrodialysis stacks were designed using ion exchange membrane. Separation of NaCl from amino acid solution was performed in the condition similar to amino acid fermentation process. To obtain otimum conditions of separation, leakage of amino acid depending of pH and limiting current density were measured. On the basis of optimum condition, removal of NaCl and leakage of amino acid were investigated quantitatively in batch and continuous process, and current efficiencies were also obtained. As a result of batch experiment for 11 hours each amino acid solution, removal efficiencies of NaCl were in the ranges of 96.1~96.2%. Amino acid leakage rate of glycine, methionine, alanine were 2.5, 1.7, 2.0% respectively. Current efficiencies were in the ranges of 44.5~44.6%. As a result of continuous experiment in various flow rate of each amino acid solution, it took 120 ~ 150 min to reach to steady state. Removal efficiency of NaCl was increased as the flow rate was decreased, but current efficiency was decreased. At the steady states, there were no leakage of amino acid.

  • Effects of alkaline additives on the $CO_2$ removal reaction have been investigated by a thermogravimetric analyzer. $Li_2ZrO_3$ was synthesized by soild reaction of $ZrO_2$ with $Li_2CO_3$ and then alkali chemicals were added to the synthesized $Li_2ZrO_3$ and then heat treatment was carried out. Addition of alkali chemicals enhanced the reactivity of $Li_2ZrO_3$ with the following order; $K_2CO_3>NaCl>LiCl>Na_2CO_3$, which were resulted from the formation of partially melted $Li_2CO_3$. SEM photographs showed the presence of melted state and the XRD results showed that the chemical states of added salts were not changed. Addition of NaCl caused the induction time of about 60 min at the initial reaction stage and the addition of $Na_2CO_3$ inhibited the decomposition of $Li_2CO_3$ at about $700{\sim}750^{\circ}C$.

  • Successful root canal treatment can be obtained by the removal of microorganisms from the pulpal space of the root canal system through biomechanical technique with instruments and irrigation. Due to the difference in the dimension of the pulpal structure such as thin wall of the root, large portion of chamber, the primary teeth should be considered in a different way of approach. Traditionally, fluids have been dispensed passively into the root canals for improve the cleansing. The use of sodium hypochlorite as an irrigant in root canal treatment is widespread and common, because it meets requirements for the ideal irrigants.

  • In this study, cellulose acetate (CA) membrane was prepared by phase inversion precipitation and then evaluated the forward osmosis (FO) membrane performance. Differences in water flux and salt rejection between RO and FO with prepared membranes were observed. The different structure membranes were prepared with various solvent which evaluate the influence of membrane structure on permeability. The structure of the prepared membrane was confirmed through scanning electron microscopy (SEM) and the permeability changes were estimated using the bench-scale FO test equipment.
