Case Study on the Use of Songwriting to Promote Self-Acceptance in Adolescents in Palliative Care (노래 만들기를 통한 완화의료 청소년의 자기수용 증진 사례 연구)

  • This study aimed to examine the effects of songwriting on the self-acceptance of adolescent in palliative care. Three participants were recruited from a university hospital in an urban area in South Korea. The songwriting intervention was conducted in six sessions, and each session was composed of an introduction, song writing and wrap-up. Participants were asked to complete the Unconditional Self-Acceptance Questionnaire (USAQ) before and after the intervention. Their verbal expressions and written song lyrics observed during the intervention were also measured. The results showed that the USAQ scores increased after the intervention, indicating the improvement in their acceptance of the self without judgement or high reactivity to feedback. In addition, the participants exhibited greater recognition of their emotions and acceptance of themselves as they are. They were also better able to discover internal resources and psychological competence. The participants reported that they could more freely express their emotions through song writing and that recording the songs they created was a means of communicating their feelings to others. Based on this study, it is expected that adolescents in palliative care will be able to experience self-acceptance through such brief song writing interventions.

Development of Comprehensive Oro-Facial Function Scale (포괄적 구강안면기능척도(Comprehensive Oro-Facial Function Scale; COFFS)의 개발)

  • Objective : This study aimed to develop a Comprehensive Oro-Facial Function Scale (COFFS) that can evaluate oro-facial function in patients with dysphagia. Methods : To verify the item composition and reliability of the COFFS, preliminary items were collected by selecting and analyzing four previous studies, and the Content Validity Ratio (CVR) was derived through a second survey of experts. Cronbach's 𝛼 was calculated for the internal validity of the evaluation items, and the test-retest reliability and inter-rater reliability were calculated using the internal classification coefficients (ICC). Results : The content validity ratio of all items was 0.67; in the case of Cronbach's 𝛼 value for each domain, 0.849 for communication domain, -0.224 for the oro-facial structure and shape, 0.831 for the ability to perform orofacial movements, and 0.946 for mastication and swallowing function. The test-retest reliability was 0.974 and the inter-rater reliability was 0.937, showing high reliability. Conclusion : In this study, the evaluation tool of COFFS was finally selected from 34 items in four areas and developed on a 3-5 point scale according to the evaluation items. In future studies, additional research is needed to prove its validity through correlation with other evaluation tools that measure oro-facial function.

Effect of Oral Motor Facilitation Technique on Oral Motor Function in Stroke Patients (구강운동촉진기술(Oral Motor Facilitation Technique)이 뇌졸중 환자의 구강운동기능에 미치는 효과)

  • Objective : This study was conducted to confirm the effect of the oral motor facilitation technique (OMFT) on oral motor function in stroke patients. Methods : This study was conducted on 72 stroke patients with dysphagia were included. Thirty-six patients were randomly assigned to the experimental and control groups were randomly classified into 36 patients each using a random table, and a two-group pre-post test was designed. The experimental group underwent OMFT, and the control group underwent traditional dysphagia therapy for 30 min, once a day, 5 times a week for 4 weeks, for a total of 20 sessions. The Comprehensive Orofacial Function Scale (COFFS) was used to evaluate oral motor function. Repeated-measures analysis of variance (ANOVA) was performed to confirm the effect of the period, and an independent t-test was performed to analyze the difference in change between the two groups. Results : Total COFFS scores improved in both groups. The experimental group showed significant changes in mandibular and lip movements, cheek blows, and tongue movements. In addition, there were significant differences depending on the intervention period in terms of masticatory distribution, food spillage, swallowing of solid and liquid foods, and voice changes. There were significant differences in the mandibular opening and closing categories between the two groups. Conclusion : OMFT is effective in improving oral motor function in stroke patients with dysphagia and can be used as basic evidence in clinical practice.

5 YEARS EVALUATION OF COMPOSITE RESIN RESTORATION ON PERMANENT FIRST MOLAR IN CHILDREN (어린이 제 1 대구치 복합 레진 수복물의 5년 후 임상평가)

  • Clinical performance of resin-based composite material depends on case selection and on the proficiency of the operator. Especially, composite resin restoration on permanent first molar in children have more limitations clinically than adult patients according to stage of tooth eruption and patient's compliance. This study was designed to evaluate the clinical performance of posterior composite resin restoration in children after 5 years. 35 teeth of 16 patients who were received composite resin restoration on permanent first molar in Department of Pediatric dentistry, Pusan National University Hospital between January 2001 and December 2001 were evaluated based on Modified USPHS criteria. From the finding in this study, following conclusions can be made. 1. 6 teeth(17%) of 35 teeth was replaced, so 5-years survival rate of posterior composite resin restoration is 82.9%. 2. As results of each evaluation criteria, on color match, anatomic form, surface roughness, sensitivity/ discomfort, ideal A grade score was 86.2%, 93.1%, 86.2%, 86.2%, clinically accepted B grade score was 13.8%, 0%, 13.8%, 10.3%. On marginal adaptation and marginal discoloration, A grade score was 13.8%, 44.8% and B grade score was 79.3%, 34.5% and secondary caries rate was 20.7%. 3. 69.1% of teeth (20 teeth) was clinically accepted on all evaluation criteria.

PRESENT STATE AND PROSPECT OF PUBLIC DENTAL HEALTH SERVICE FOR CHILDREN AND ADOLESCENTS IN KOREA (우리나라 어린이와 청소년의 구강건강을 위한 공공서비스의 현황과 전망)

  • To achieve the dental health of children and adolescents which is the objective of the pediatric dentistry, the role of the public service should be increased. The basis of the public service is established by many laws of which the Dental Health Law is most important. The percentages of primary schools that had the school dental health clinic, that implemented the fluoride rinsing program, and that implemented the group toothbrushing after lunch were 7.2%, 57.5%, and 46.9%, respectively, and the percentages of primary school children that received the dental health education and that received the fissure sealing were 48.0% and 12.4%, respectively(2006). About 42% of infants and preschool children from 0 to 6 years received preventive dental care in the last one year, at the nursery or kindergarten(18%) or at the health center(1%)(2005). The percentage of the health centers that implemented water fluoridation was 11.3%, and the percentage of the population who drank the fluoridated water was 5.7%(2006). It was suggested that the school dental health administration should be unified, that dental health teachers should be employed, and that the comprehensive dental health care should be supplied to all the children and adolescents through the circuit school dentists and the school dental hospitals in the long term. Also, the dentist in charge system for the children and adolescent was suggested.

Clinical Characteristics of Human Parvovirus B19 Infection in Children

  • Purpose: Human parvovirus B19 infection is widespread and has a heterogeneous clinical spectrum, ranging from asymptomatic infection to potentially life-threatening complications. We investigated the various clinical features of human parvovirus B19 infection during an outbreak of the virus in our community. Methods: A retrospective chart review study was conducted at the Pusan National University Children's Hospital from December 2017 to April 2019. We investigated the clinical features of children with parvovirus B19 immunoglobulin M or parvovirus B19 DNA detected using polymerase chain reaction. Results: A total of 24 children were diagnosed with parvovirus B19 infection. Twelve (50%) had lace form rashes, and four (16.7%) had petechial rashes. Two (8.3%) were diagnosed with fever without a focus. Six (25%) developed aplastic crisis as a complication of infection, of whom three were previously diagnosed with hereditary spherocytosis and three with acute lymphoblastic leukemia. Conclusions: In addition to erythema infectiosum, the parvovirus B19 infection can present clinically with various types of rashes and fever without a focus. Furthermore, hematologic manifestations such as neutropenia and aplastic crisis can occur during infection.

A STUDY ON THE TRAUMATIC INJURIES TO PRIMARY TEETH (유치 외상에 관한 연구)

  • The purpose of this study was to research the traumatized primary teeth and investigate following factors: sex, age, cause, place and time(of the year) of injury, elapsed time, area and type of injury, dental treatment and prognosis. The analysis includes total of 1533 traumatized primary teeth from 758 children aging from 6 months to 6 years(mean age: 2.8) from 2003 to 2007. The result follows : 1. The children of age between 1-2 and 2-3 were involved in the largest number of injuries in both sexes with boy/girl ratio of 1.77:1(p<0.001). 2. Fall and collision were the main causes of traumatic dental injury, especially in younger children(p<0.05). Places of injury occurrence varied: home, outdoors, and kindergarten. Warm climate accounts for frequent outdoor injuries in May, September and October(p<0.001). 3. Most of the children visited dental clinic within 24 hours of the injury(77.6%). From March to September, dental trauma occurrences were distributed evenly, except for Winter period(p<0.001). 4. Upper central incisors were the most commonly affected teeth, and the injuries usually involved 1 tooth or 2 teeth. 5. Periodontal tissue injuries dominated and subluxation was the most common type. Lateral luxation, enamel fracture, intrusion and root fracture followed.

Prevalence of Allergic Rhinitis between Urban and Rural Residents in a Local Community (일개 시 지역의 도심과 농촌 주민의 알레르기비염 유병률)

  • Objectives: The purpose of this study was to examine the prevalence of allergic rhinitis between urban areas in the adjacent areas to a steel industrial complex and rural areas and the impact of pollutants in the industrialized city on allergic rhinitis. Methods: From July 28 to August 9 of 2008, 1,043 residents of urban and rural areas in a local community had enrolled in health screening and questionnaire survey. One thousand thirty-three patients also underwent a skin prick test. Prevalence rates of allergic rhinitis were calculated according to residential areas, and the used statistical analysis were Fisher's exact test and chi-square test. Results: In a survey, the fraction of adults, who complained of sneezing, nasal obstruction and rhinorrhea, during a recent 1 year period, showed the significant difference between urban (30.5%) and rural areas (22.4%). The fractions of positive skin prick tests were not different between two areas in each age group. The prevalence of allergic rhinitis was 8.4% in urban areas and 6.9% in rural areas. Considering the age groups, the adults group only showed the significantly higher prevalence of allergic rhinitis in urban areas (8.2% vs. 3.7%). Conclusions: Unlike the children and adolescents groups, the prevalence of allergic rhinitis in adults group was higher in the industrialized urban areas.

Papillary Renal Cell Carcinoma in Transplanted Kidney and Xp11.2 Translocation/Transcription Factor E3-Rearranged Renal Cell Carcinoma in the Native Kidney: A Case Report (이식신장에 생긴 유두모양 신세포암종과 고유신장에 생긴 Xp11.2전위/전사인자E3-재배열 신세포암종: 증례 보고)

  • Concomitant renal cell carcinomas (RCC) of both native and allograft kidneys are extremely rare, and only a few cases have been reported in the available English literature. A particularly rare variant within the adult population is the Xp11.2 translocation/transcription factor E3 (TFE3)-rearranged RCC. Although few case reports of TFE3-rearranged RCC have been reported in children who underwent kidney transplantation (KT), no case of adults with TFE3-rearranged RCC following KT has been reported. Herein, we presented the radiological and pathological findings of a rare metachronous papillary RCC in the allograft kidney and TFE3-rearranged RCC in the native kidney. The TFE3-rearranged RCC in the native kidney exhibited slow expansion in size over five years. Radiologically, it appeared as a slightly enhanced, lobulated mass on contrast-enhanced CT. MRI revealed high signal intensity on T1-weighted images and low signal intensity on T2-weighted images.

THE PREVALENCE OF WHITE SPOT LESIONS ON THE MESIAL SURFACES OF THE 1ST MOLARS IN CHILDREN AND MINIMAL INVASIVE APPROACH - A PILOT STUDY (어린이 제1대구치 근심면 초기 우식의 유병률과 최소 침습적 접근)

  • In clinical pediatric dentistry, we have many chances to encounter the white spot like incipient enamel lesions on the mesial surfaces of the 1st molars with direct vision, especially just after the 2nd primary molars were exfoliated. But it was thought highly desirable to assess if these lesions are properly and effectively managed yet. This study aims at surveying the prevalence of incipient lesions on the mesial surfaces of the 1st molars in children through direct observation and examining the suitability of adhesive sealing on them as a pilot trial in searching for their proper management. 1. Among the 124 mesial surfaces of the 1st molars examined, 34% were sound, 53% had incipient carious lesions and 13% had cavitated lesions. 2. In the sectional views of the specimens, 20% showed microleakage after thermo-cycling and it was thought not recommendable as a permanent method. Therefore in order to effectively fight against the incipient caries lesions in children‘s permanent teeth, it was thought proper not to rely on any one method, but to perform reinforcing oral hygiene and promotion of remineralization in combination with therapeutic sealing which is stronger in short-term sealing effect. Although therapeutic sealing has been considered as the core in minimally invasive concept to treat the white spot lesions, its long-term clinical trials have not been suggested. Continuous research is strongly required for making this approach to acquire permanent nature, especially in regards of proper pretreatment and high molecular materials deeply penetrable into enamel.
