A Method for Gene Group Analysis and Its Application (유전자군 분석의 방법론과 응용)

  • In microarray data analysis, recent efforts have focused on the discovery of gene sets from a pathway or functional categories such as Gene Ontology terms(GO terms) rather than on individual gene function for its direct interpretation of genome-wide expression data. We introduce a meta-analysis method that combines $p$-values for changes of each gene in the group. The method measures the significance of overall treatment-induced change in a gene group. An application of the method to a real data demonstrates that it has benefits over other statistical methods such as Fisher's exact test and permutation methods. The method is implemented in a SAS program and it is available on the author's homepage(http://cafe.daum.net/go.analysis).

Systematic Review of Research on Adherence among Hemodialysis Patients in Korea (혈액투석환자의 이행에 관한 체계적인 문헌고찰)

  • The purpose of this study was to analysis instruments, identify relevant factors and analyze the effect size of intervention studies in reference to adherence among hemodialysis patients. Electronic literature searches were conducted for PubMed, CINAH, RISS, Naver academic, KISS, NDSL, Nanet, Kci, DBpia and KoreaMed. Of 437 studies identified, 37 met inclusion criteria, and 5 were used to estimate effect size. As a results, total of 11 original instruments were found, 61 factors were considered as relevant factors for adherence. Types of intervention were individual education program, self-care program using SMS, E-mail, or telephone counseling, supportive care program and drug education program. The effect size using random model effect was high (d=1.04, 95% CI: 0.60, 1.49, p<.00001).

Lexical and Phrasal Analysis of Online Discourse of Type 2 Diabetes Patients based on Text-Mining (텍스트마이닝 기법을 이용한 제 2형 당뇨환자 온라인 담론의 어휘 및 구문구조 분석)

  • This paper has identified five major categories of the T2D patients' concerns based on an online forum where the patients voluntarily verbalized their naturally occurring emotional reactions and concerns related to T2D. We have emphasized the fact that the lexical and phrasal analysis brought to the forefront the prevailing negative reactions and desires for clear information, professional advice, and emotional support. This study used lexical and phrasal analysis based on text-mining tools to estimate the potential of using a large sample of patient conversation of a specific disease posted on the internet for clinical features and patients' emotions. As a result, the study showed that quantitative analysis based on text-mining is a viable method of generalizing the psychological concerns and features of T2D patients.

Partial Left Ventriculectomy as a Bridge to Cardiac Transplantation in a Patient of End-Stage Heart Failure -Case Report- (말기 심부전 환자에서 심장이식의 교량으로서 좌심실 축소술 - 중례보고 -)

  • A 40-years-old male with dilated cardiomyopathy(DCMP) and end-stage heart failure had undergone partial left ventriculectomy(PLV) in July 1997 and then underwent cardiac transplantation in January 1999. Three months later he showed increased ejection fraction (EF) from 26% to 42.6%, decreased left ventricular end diastolic diameter(LVEDD) from 71mm to 45mm, cardiac output(CO) 3.95 L/min and cardiac index(CI) 2.28 L/min/m$^2$echocardiographically. Eight months later, left ventriclular end diastolic and systolic diameters increased to 56 and 51 mm respectively and EF decreased to 17% in echocardiographic follow-up. He had been on maximum medication until he underwent cardiac transplantation 18 months after the PLV. Consecutive myocardial biopsies (1, 3 and 6 month later) showed ISHLT (international society of heart and lung transplantation) class la and the treatment for rejection was not needed until now on. We report a partial left ventriculectomy as a successful bridge to cardiac transplantation in a patient with DCMP and end-stage heart failure.

  • The College entrance examination stress syndrome is a kind of anxiety disorder. The underlying cause of this disorder is not a test anxiety itself. One's hidden inner and familial conflicts are more likely attributed to this disorder. Patient's the most common complaints are various psychosomatic symptoms but in severe cases underlying psychopathology may be activated and progress to major psychosis. In a broad sense adolescent's delinguent behavior, drug abuse, school drop-out and sucide are closely related to this syndrome. In clinical management of these patients, considering the special situation of impending examination, the therapist must access to central conflict theme in a short time without severe resistance. The authors classified this syndrome into 5 clinical subtypes ; 'the anxious group', 'the exhaustion group', 'the despair group', 'the emptyness group' and 'the boredome group'. Typical case of each subtype and it's management methods were presented briefly.

  • Traditional drugs(herbal and animal drugs) of Korea, China and Japan have developed essentially from the same origin, since the traditional medicine of three countries has been originated from ancient China. Due to different geographical locations and discrepancy of plant resources of the traditional drugs, some divergency in terms of systematic botany in traditional drug materials has appeared in the three countries. Present report aims to survey traditional herbal drugs that have been called same traditional names in three countries, but they are actually different with respect to systematic botanical view-point. The official drug compendia(pharmacopoeia and natural drug standards) of three countries were subject to examination. Survey was conducted by the following categories. Traditional drugs were listed under same name, however, 1) they belong to different genus; 7 traditional herbal drugs were listed. 2) they belong to same genus, but different species; 24 traditional herbal drugs were found. 3) a variety of related species are used; 15 traditional drugs were listed. 4) actually same plant, but taxonomical name is differently called and/or different parts of plant are used; 10 traditional drugs were counted. 5) animal drugs belong to one of the above categories; 7 traditional animal drugs were found. Total 63 traditional drugs(herbal and animal) were found to comprise different taxonomical names when the official drug compendia of Korea, China and Japan were examined.

A Case Report of Degenerative Spondylolisthesis Treated By Oriental Medical Treatment (한방치료를 적용한 퇴행성 척추전방전위증 환자 치험 1례)

  • Objectives : This study was performed to report the effect of oriental medical treatment including chuna manual therapy for the spondylolisthesis patient. Methods : We treated him by oriental medical treatment including acupuncture, herb therapy and chuna manual therapy. And the result was assessed by numerical rating scale(NRS), walking distance at once and radiological examination. Results : After treatment, NRS changed from 8 to 4, walking distance at once changed from 175m to 500m, percent of slip changed from 28.62% to 19.80% and slip angle changed from $20.0^{\circ}$ to $22.8^{\circ}$. Conclusions : In this study, oriental medical treatment including chuna therapy was effective in spondylolisthesis patient. But additional studies will be needed.

A Clinical Study on floor of Mouth Cancer (구강저암의 임상적 고찰)

  • We analyzed retrospectively 100 cases of floor of mouth cancer, treated at Korea Cancer Center Hospital from Jan. 1985 to Dec, 1992. The most prevalent age group were 6th, 1th decades, the sex ratio was M:F =4.6:1. 95 patients were smoker, 73 patients smoked more than 1 pack per day. 51 patients were heavy drinker (So-ju more than 1 B/day). Histopathologically, the squamous cell carcinoma (95 cases) was most common, followed by the adenoid cystic carcinoma (4 cases), mucoepidermoid carcinoma (1 case). The most common clinical staging was stage IV, and lymph node metastasis were 71 cases, histopathologically. Distant metastasis were 2 cases, multiple primary cancer was 1 case. 5 years survival rates according to treatment modality were 71.4% in operation only group that were almostly in early stage, 0% in radiation therapy group that were almostly in advanced stage, 32.5 % in combination therapy with operation and radiation therapy.

Continuous Intravenous Infusion of Morphine and Ketorolac for Postoperative Pain (Morphine과 Ketorolac의 지속적 정주에 의한 술후 통증 완화 효과)

  • Despite their sometimes fatal complications such as respiratory depression when used for postoperative pain control, intravenous and epidural narcotics remain the mainstay of treatment regimens. Because of the problems, anesthesiologists are seeking alternatives. We compared the analgesic effect and complications of continuous intravenous morphine with ketorolac. Ketorolac is a non-steroidal agent with potent analgesics and moderate anti-inflammatory activity. Forty ASA physical status I or II patients were given morphine(20 patients) or ketorolac(20 patients):In the morphine group, an initial bolus dose of 2 mg i.v. was given followed by continuous infusion at a rate of 1 mg/hr for 48 hours. The ketorolac group was given initial bolus of 30 mg i.v. This was followed by continuous infusion at a rate of 3.75 mg/hr for 48 hours using a Baxter Daymate Infuser. We checked systolic, diastolic and mean arterial pressure, heart rate, visual analogue scale(VAS) and the Prince Henry Score(PHS). This was done before the initial bolus, at 5, 15, 30 and 60 min, at 2, 6, 12, 24 and 48 hours after administration. We observed the side effects nausea and vomiting, pruritus, hypotension, somnolence, urinary retention and respiratory depression. From our study we believe ketorolac in selected patients, is as effective as morphine in alleviating postoperative pain without side effects of morphine.

Perceived Usefulness and Attitude toward Smart-glass for First-aid Remote Support among Coast Guards in Korea (응급처치 원격지도용 스마트글래스 사용에 대한 한국 해양경찰의 인지된 유용성 및 태도)

  • This study was to investigate the types of emergencies transported by the Southwestern Coast Guard, the need for telemedicine guidance, and the perception and attitude of smart glasses as a communication method targeting 31 coast guards. A relatively high frequency and training requirement were confirmed for bleeding, abrasion, and abdominal pain. The demand for telemedicine guidance on medication and triage was higher, and the perceived usefulness and attitude scores for the use of smart glasses were 3.76±0.61 and 3.64±0.45, respectively. A moderate correlation between perceived usefulness and attitude toward smart glasses was confirmed (r=.630, p<.01). With the development of technology, it is time to actively introduce new devices such as smart glasses.
