Prevalence and Predictors of Nocturia in Patients with Obstructive Sleep Apnea Syndrome (폐쇄성수면무호흡증 환자의 야간뇨 유병률 및 관련인자)

  • Objectives: Several studies suggest that nocturia may be related to obstructive sleep apnea syndrome (OSAS). The mechanism by which OSAS develops nocturia has not been determined. The present study aimed to determine the prevalence of nocturia among adults with OSAS and to identify factors that may be predictive in this regard. Methods: Retrospective review of clinical and polysomnographic data obtained from patients evaluated at the sleep clinics of the St. Paul's Hospital between 2009 and 2012. The urinary symptoms were assessed on the basis of the International Prostate Symptom Score (IPSS). Pathologic nocturia was defined as two or more urination events per night. OSAS was defined as apnea-hypopnea index (AHI) ${\geq}5$. A multivariate analysis using logistic regression was performed to examine the relationship between polysomnographic variables and the presence of pathologic nocturia, while controlling for confounding factor. Results: A total of 161 men >18 years of age (mean age $46.7{\pm}14.1$), who had been referred to a sleep laboratory, were included in the present study. Among these, 27 patients with primary snoring and 134 patients with obstructive sleep apnea were confirmed by polysomnography. Nocturia was found in 53 patients with OSAS (39.6%) and 8 patients with primary snoring (29.6%). The AHI was higher in patients with nocturia than in those without nocturia (p=0.001). OSAS patients with nocturia had higher arousal index (p=0.044), and lower nadir oxyhemoglobin saturation (p=0.001). Multiple regression analysis showed that age (${\beta}$=0.227, p=0.003), and AHI (${\beta}$=0.258, p=0.001) were associated with nocturia, and that the presence of pathologic nocturia was predicted by age (OR 1.04 ; p=0.004) and AHI (OR 1.02 ; p=0.001). Conclusion: Nocturia is common among patients with OSAS. The strongest predictors of nocturia are age and AHI in patients with OSAS.

Urinary Calcium Excretion in Children with Primary Monosymptomatic Nocturnal Enuresis (일차성 단일증상성 야뇨증 환아에서 소변내로의 요칼슘배설에 대한 연구)

  • Purpose : Hypercalciuria has been suggested to be involved in the pathogenesis of nocturnal enuresis associated with polyuria. We have measured the amount of calcium excretion in the urine from children with monosymptomatic nocturnal enuresis and studied the association with enuresis. Methods : Thirty-six children (20 males and 16 females, age 5 to 15.6 years) presenting with monosymptomatic nocturnal enuresis were recruited for this study. Results : Among 36 patients, 6 patients had hypercalciuria, providing a 16.7% frequency. The mean Ca/Cr ratio of hypercalciuria group showed $0.23{\pm}0.10$ on daytime, and $0.33{\pm}0.10$ on nighttime showing higher values in nighttime, but it was not statistically significant. Average 24 hour calcium excretion was 8.95 mg/kg. Night/day time urine volume ratio in hypercalciuria group was $0.87{\pm}0.20$, and that in normocalciuria group was $0.81{\pm}0.33$, also showing no difference. Daytime Ca/Cr ratio and nighttime Ca/Cr ratio from all enuresis patients showed a significant correlation (P=0.0001). However, Ca/Cr ratio between daytime and nighttime urine volume had no significant correlation, respectively (daytime P=0.08; nighttime P=0.07). Also, daytime and nighttime Na concentration, urea concentration, and osmolality also had no significant correlation with urine Ca/Cr ratio, respectively. Conclusion: Hypercalciuria shown in some of enuresis patients is not directly caused by primary monosymptomatic nocturnal enuresis.

The Effects of Electrical Stimulation Therapy with Microchip for the Treatment of Bladder Irritability Symptoms in Stress Urinary Incontinent Women (방광 자극증상을 호소하는 복압성 요실금 환자에서 마이크로칩을 이용한 전기자극치료의 효과)

  • Purpose: This study was carried out to evaluate the efficacy of intravaginal pelvic floor electrical stimulation (FES) therapy on bladder irritability symptoms in stress urinary incontinent women. Materials and Methods: The evaluation before and after treatment included the Blaivas's female Bladder Questionnaire, the quality of life questionnaires and the overall satisfaction with present voiding state using visual analogue test (VAT). All patient were treated for 20 minutes, twice a week for 6 weeks, using a new intravaginal electrical stimulator with microchip (PIC16C74). Results: After the FES therapy, the overall success rate of the SUI was 50.4.% at 9 months. The bladder irritability symptoms such as frequency, nocturia, urgency, residual urine sensation and lower abdominal discomfort were improved. In particular, the symptoms of frequency, urge incontinence, dysuria were significantly improved after the FES therapy at 9 months. The VAT score of the overall satisfaction with the present voiding state was significantly lower after the FES therapy. 73.7% of patients were satisfied with the FES therapy and complications such as hemorrhage, vaginitis, urinary tract infection and pain were not encountered. Conclusion: These results suggest that FES therapy with microchip improves some bladder irritability in SUI women. Therefore, electrical stimulation therapy should be considered as a first line therapy in SUI women with bladder irritability symptoms.

  • Patients with moderate to severe degrees of Henoch-Sch$\ddot{o}$nlein purpura (HSP) nephritis receive high-dose intravenous methylprednisolone pulse therapy (IMPT). Although the regimen is generally safe and effective, various complications occasionally develop. administration of excessive corticosteroid can induce urinary potassium wasting leading to hypokalemia. Polyuria, one of the complications of hypokalemia, is related to both increased thirst and mild nephrogenic diabetes insipidus. And hypokalemia itself also impairs the maximal renal urinary concentration ability. Although polyuria or nocturia after IMPT is not common, it is correctable immediately by oral potassium supplementation. Therefore, during IMPT, careful history taking of nocturia as well as monitoring urine volume, serum and urine potassium level at regular follow-up are necessary because even mild hypokalemia can provoke urine concentrating ability defect. We experienced a case of 11 year-old boy with HSP nephritis who suffered from hypokalemia-induced polyuria with nocturia right after IMPT.

A Prospective Study of the Effect of Direct Moxibustion for Treatment of Nocturia (야간뇨에 적용한 직접구 시술의 효과에 대한 전향적 연구)

  • Objective: The purpose of this study was to evaluate the effects of direct moxibustion on nocturia. Methods: Thirty-one nocturia patients were treated with direct moxibustion. Results: After the treatments, the numbers of nocturia incidences were reduced. Conclusions: Direct moxibustion might be effective as a treatment for nocturia.

Case Report of Combined Herbal Medicine Treatment for Nocturia and Perineal Discomfort Before Bladder Calculi Removal Surgery (방광 결석 제거 수술 대기 기간 중 호소한 야간뇨 및 회음부 불쾌감에 대한 복합 한약 치험 1례)

  • Background: Bladder calculi greater than 10 mm should be managed via surgical treatment, such as cystolithotomy or percutaneous removal. Because risk evaluation for underlying disease must precede operation, patients are medicated with uroselective alpha blockers or analgesics until the operation occurs. However, disagreement exists about the success of these treatments. Case report: We present a case of a male patient who was hospitalized for sequelae of internal cerebral hemorrhage with complaints of nocturia and perineal discomfort. Abdominal computed tomography showed large bladder calculi. However, because of suspected pheochromocytoma, immediate surgery could not be performed, so we treated him with Korean medicine for 27 days (Daesiho-tang for 27 days and Jakyakgamcho-tang was started at 14th day of treatment, treated till 27th day.) until a cystolithotomy could be performed. Nocturia and perineal discomfort improved after 9 days, and the patient stopped taking acetaminophen. Conclusion: The administration of Daesiho-tang and Jakyakgamcho-tang to a patient with bladder calculi successfully controlled symptoms of nocturia and perineal discomfort until cystolithotomy could be performed.

A Study of Electrolyte Excretion and Salt Intake in a Rural Community (일부 농촌지역 주민의 요중 전해질 측정을 통한 식염 섭취량 추정과 고혈압과의 관계 연구)

  • This study was carried out in order to examine the urinary excretion of electrolytes (Na, K) and their relationship with blood pressure, and to estimate the amount of daily salt intake in a rural community. From January to March in 1987, a mobile screeing team visited 40 villages, and carried out health screening of 537 adult volunteers whose age were over 30 years and collected 12-hours overnight urine. To determine the completeness of collection, the urinary creatinine was measured. If the creatinine excretion was beyond the range given to the age group, the sample was excluded from the analysis as an incomplete collection : 345 samples were remained for analysis. This study revealed the following results. 1. The mean excretion amounts of urinary electrolytes for 12 hours were Na 193.5 mEq, K 20.8 mEq, creatinine 1.0 g. The mean ratio of electrolytes were Na/K 9.84, Na/creatinine 0.44, K/creatinine 0.046. 2. Both the mean excretion amount of K and the mean ratio of K/creatinine were less in hypertensives than in normotensives. K excretion also showed a tendency towards a decrease in inverse proportion to systolic blood pressure when it exceeded 120 mmHg. There was no significant difference between the hypertensives and normotensives in Na excretion. The sodium to potassium ratio increased in poportion to systolic blood pressure. 3. The mean daily salt excretion amount was 22.4 g. Assuming that 90% of the intake was excreted, the estimated amount of daily salt intake was 24.9 g.

A Study on the Urinary Incontinence and Overactive Bladder Syndrome of Women in a Rural Region (일부 농촌여성에서 요실금 및 과민성방광증후군의 실태에 관한 조사)

  • Objectives: This investigation was carried out to understand the status of the urinary incontinence (UI) and overactive bladder (OABs) syndrome of women in a rural region. Methods: The authors conducted a questionnaire survey among the 322 females who voluntarily participated in a health examination, from 16 to 18 August 2001. Using by definition of UI and OABs, we calculated the proportion of UI, OABs I, and OABs II. The data collected was evaluated using the SPSS 12.0 statistical package, and the differences of symptoms and problems related to daily life between UI, OABs patients and the others were analyzed using a Chi-square test or Fisher's exact test. Results: The overall proportion of UI was 35.4%, and stress UI (32.9%) is more common than urgency UI (17.4%) and mixed UI (14.9%). The proportion of OABs I and OABs II were respectively 36.0%, 14.0%. Symptoms related to UI or OABs were nocturia (35.1%), frequency (23.9%), urgency (21.4%) etc. Of the incontinence cases, 27.2% had experienced UI for a period of one to three years. The proportion of OABs increased significantly by age (p<0.05), UI didn't. The most frequent symptoms in UI and OABs were respectively 'slow stream', 'urgency'. The most frequent problem of daily life in UI and OABs was 'seeking toilet firstly at stranger place'. Conclusions: The proportion of UI and OABs in our study were respectively 35.4%, 14.0%. UI and OABs must be very significant health problems in women, especially rural region. Systemic and profound interventions for UI and OABs need to administer to women in Korea.

Analysis of Changing Pattern of Noxious Gas Levels with Malodorous Substance Concentrations in Individual Stage of Pig Pens for 24 hrs to Improve Piggery Environment (돈사환경 개선을 위한 생육단계별 돈사내 악취물질 농도 및 유해가스의 1일 변화추세 분석)

  • Noxious gases with malodorous substance concentrations in each stages of pig buildings were determined at a typical 400sow-scale farm to improve piggery environment. Using IAQ-300 and pDR-1000AN, continuous records for the concentration of $NH_3$, CO, $CO_2$, $NO_2$, $SO_2$, $H_2S$, $O_2$, and along with temperature, humidity, dust concentrates from individual pig pens were collected to analyze every 6 hours' condition of indoor environment for 24 hours' period. In most pig houses, the air quality at noon was good, while at night (00:00~06:00), air composition became noxious in all buildings. The order of buildings' air quality for 24 hrs was pregnant > farrowing > nursery > growing > finishing. The cause of air quality differences was presumed to be the differences of stocking density, defecating amount and the length of exposure time of slurry in indoors. In conclusion, well-designed building structure, proper control of stocking density, quick removal of excreta from pig pens and continuous ventilation are prerequisites to improve pig housing environment.
